    Profile of the physiotherapy profession in New South Wales (1975–2002)

    This was the first study known to the authors exploring workforce data from the New South Wales Physiotherapists Registration Board over several decades. Labour force statistics were examined from various data sources over the years 1975 to 2002. The results indicate that the demographics and working patterns of physiotherapists in New South Wales have remained remarkably stable over time. The workforce continues to grow, however the growth rate has decreased markedly with only a 2.3% increase from 2001 to 2002. The proportion of men within the profession is steadily increasing; in 1975 men accounted for 5.2% of physiotherapists, in 2000 23.5% of physiotherapists were male. While the male workforce increases, the female workforce is ageing. The modal age for female physiotherapists has steadily increased from 25–29 years in 1975 to 40–44 years in 2001. Importantly, in relation to attrition, the degree of workforce participation has not undergone significant change since 1987. Although labour force analysis has demonstrated that the workforce is in shortage, attrition does not appear to be the major contributor to this situation. The proportion of the profession who are inactive has remained relatively stable since 1987. More pertinent to the current shortage is the slowing of the growth rate of the profession while demand for physiotherapy services continues to rise

    Interrelationships of light and space within the still-life painting and the figure drawing

    My Master of Fine Arts thesis is built around two genre, still-life and the human figure, in a representational style that is catagorically classical with an impressionistic skew. It implies, however, neither a singularly literal rendering nor a deliberately analytical diffusion of forms or colors. It is, rather, a very general composite of both resolutions employed with care not to pin-point either in such a way as to risk "freezing" the work in a kind of stylistic rigidity that does a direct disservice to both classicism and impressionism. The still-life presentation of oil paintings emanates from the investigation of two types of objects and surroundings propounding one theme. Themetically the paintings depict the nuances of natural daylight upon objects - most of them near white in color and in an environment of similar color. The two divisions of still-life subject matter circumscribed within this theme are as follows: 1. Objects and their settings carefully selected and arranged to form a unified grouping in a specific space and light situation in order to capture a particular spatial and luminary effect. 2. Objects (basically immovable or fixed) found within their preexistent environment and chosen because of the way in which they work with their already established space and light as a unit controlled only by the ways I have transferred their united image to the canvas. The arranged objects are, for the most part, organic, natural forms against a setting of manufactured forms put together in such a way as to allow the light not only to fuse these disparate elements, but also to give these immobile objects a sense of suspended animation

    Burnout, Depression, Anxiety, and Stress Among Resident Physicians 18 Months into the COVID-19 Pandemic: A Cross-Sectional Study

    INTRODUCTION. Burnout among resident physicians has been an area of concern that predates the COVID-19 pandemic. With the significant turmoil during the pandemic, this study examined resident physicians’ burnout, depression, anxiety, and stress as well as the benefits of engaging in activities related to  wellness, mindfulness, or mental wellbeing. METHODS. The authors conducted a cross-sectional survey of 298 residents from 13 residency programs sponsored by the University of Kansas School of Medicine-Wichita, in October and November 2021. Authors used a 31-item questionnaire to measure levels of burnout, depression, anxiety, and stress. A mixed method approach was used to collect, analyze, and interpret the data. Descriptive statistics, One-way ANOVA/Kruskal-Wallis tests, adjusted odds ratios (aOR), and immersion-crystallization methods were used to analyze the data. RESULTS. There was a 52% response rate, with 65.8% (n=102) of the respondents reporting manifestations of burnout. Those who reported at least one manifestation of burnout experienced a higher level of emotional exhaustion (aOR=6.73; 95% CI, 2.66-16.99; P<.01), depression (aOR=1.21; 95% CI, 1.04-1.41; P=.01), anxiety (aOR=1.14; 95% CI, 1.00-1.30; P=.04), and stress (aOR=1.36; 95% CI, 1.13-1.64; P<.01). Some wellness activities respondents engaged in included regular physical activities, meditation and yoga, support from family and friends, religious activities, time away from work, and counseling sessions. CONCLUSIONS. The findings suggest the COVID-19 pandemic continues to pose a significant and worsening prevalence of burnout and other negative mental health effects on residents. Appropriate wellness and mental health support initiatives are needed to help resident physicians thrive in the health care environment

    The genome and transcriptome of the enteric parasite Entamoeba invadens, a model for encystation

    BACKGROUND: Several eukaryotic parasites form cysts that transmit infection. The process is found in diverse organisms such as Toxoplasma, Giardia, and nematodes. In Entamoeba histolytica this process cannot be induced in vitro, making it difficult to study. In Entamoeba invadens, stage conversion can be induced, but its utility as a model system to study developmental biology has been limited by a lack of genomic resources. We carried out genome and transcriptome sequencing of E. invadens to identify molecular processes involved in stage conversion. RESULTS: We report the sequencing and assembly of the E. invadens genome and use whole transcriptome sequencing to characterize changes in gene expression during encystation and excystation. The E. invadens genome is larger than that of E. histolytica, apparently largely due to expansion of intergenic regions; overall gene number and the machinery for gene regulation are conserved between the species. Over half the genes are regulated during the switch between morphological forms and a key signaling molecule, phospholipase D, appears to regulate encystation. We provide evidence for the occurrence of meiosis during encystation, suggesting that stage conversion may play a key role in recombination between strains. CONCLUSIONS: Our analysis demonstrates that a number of core processes are common to encystation between distantly related parasites, including meiosis, lipid signaling and RNA modification. These data provide a foundation for understanding the developmental cascade in the important human pathogen E. histolytica and highlight conserved processes more widely relevant in enteric pathogens

    Pleuropulmonary blastoma (PPB) is the most frequent pediatric lung tumor and often the first indication of a pleiotropic cancer predisposition, DICER1 syndrome, comprising a range of other individually rare, benign and malignant tumors of childhood and early adulthood. The genetics of DICER1-associated tumorigenesis are unusual in that tumors typically bear neomorphic missense mutations at one of five specific “hotspot” codons within the RNase IIIb domain of DICER 1, combined with complete loss of function (LOF) in the other allele. We analyzed a cohort of 124 PPB children for predisposing DICER1 mutations and sought correlations with clinical phenotypes. Over 70% have inherited or de novo germline LOF mutations, most of which truncate the DICER1 open reading frame. We identified a minority of patients who have no germline mutation, but are instead mosaic for predisposing DICER1 mutations. Mosaicism for RNase IIIb domain hotspot mutations defines a special category of DICER1 syndrome patients, clinically distinguished from those with germline or mosaic LOF mutations by earlier onsets and numerous discrete foci of neoplastic disease involving multiple syndromic organ sites. A final category of patients lack predisposing germline or mosaic mutations and have disease limited to a single PPB tumor bearing tumor-specific RNase IIIb and LOF mutations. We propose that acquisition of a neomorphic RNase IIIb domain mutation is the rate limiting event in DICER1-associated tumorigenesis, and that distinct clinical phenotypes associated with mutational categories reflect the temporal order in which LOF and RNase IIIb domain mutations are acquired during development

    The genetic risk of acute seizures in African children with falciparum malaria.

    PURPOSE: It is unclear why some children with falciparum malaria develop acute seizures and what determines the phenotype of seizures. We sought to determine if polymorphisms of malaria candidate genes are associated with acute seizures. METHODS: Logistic regression was used to investigate genetic associations with malaria-associated seizures (MAS) and complex MAS (repetitive, prolonged, or focal seizures) in four MalariaGEN African sites, namely: Blantyre, Malawi; Kilifi, Kenya; Kumasi, Ghana; and Muheza, Tanzania. The analysis was repeated for five inheritance models (dominant, heterozygous, recessive, additive, and general) and adjusted for potential confounders and multiple testing. KEY FINDINGS: Complex phenotypes of seizures constituted 71% of all admissions with MAS across the sites. MAS were strongly associated with cluster of differentiation-ligand-rs3092945 in females in Kilifi (p = 0.00068) and interleukin (IL)-17 receptor E-rs708567 in the pooled analysis across the sites (p = 0.00709). Complex MAS were strongly associated with epidermal growth factor module-containing mucin-like hormone receptor (EMR)1-rs373533 in Kumasi (p = 0.00033), but none in the pooled analysis. Focal MAS were strongly associated with IL-20 receptor A-rs1555498 in Muheza (p = 0.00016), but none in the pooled analysis. Prolonged MAS were strongly associated with complement receptor 1-rs17047660 in Kilifi (p = 0.00121) and glucose-6-phosphate dehydrogenase-rs1050828 in females in the pooled analysis (p = 0.00155). Repetitive MAS were strongly associated with EMR1-rs373533 in Kumasi (p = 0.00003) and cystic fibrosis transmembrane conductance receptor-rs17140229 in the pooled analysis (p = 0.00543). MAS with coma/cerebral malaria were strongly associated with EMR1-rs373533 in Kumasi (p = 0.00019) and IL10-rs3024500 in the pooled analysis across the sites (p = 0.00064). SIGNIFICANCE: We have identified a number of genetic associations that may explain the risk of seizures in >2,000 cases admitted to hospitals with MAS across four sites in Africa.     Developing an expanded vector control toolbox for malaria elimination

    Vector control using long-lasting insecticidal nets (LLINs) and indoor residual spraying (IRS) accounts for most of the malaria burden reductions achieved recently in low and middle-income countries (LMICs). LLINs and IRS are highly effective, but are insufficient to eliminate malaria transmission in many settings because of operational constraints, growing resistance to available insecticides and mosquitoes that behaviourally avoid contact with these interventions. However, a number of substantive opportunities now exist for rapidly developing and implementing more diverse, effective and sustainable malaria vector control strategies for LMICs. For example, mosquito control in high-income countries is predominantly achieved with a combination of mosquito-proofed housing and environmental management, supplemented with large-scale insecticide applications to larval habitats and outdoor spaces that kill off vector populations en masse, but all these interventions remain underused in LMICs. Programmatic development and evaluation of decentralised, locally managed systems for delivering these proactive mosquito population abatement practices in LMICs could therefore enable broader scale-up. Furthermore, a diverse range of emerging or repurposed technologies are becoming available for targeting mosquitoes when they enter houses, feed outdoors, attack livestock, feed on sugar or aggregate into mating swarms. Global policy must now be realigned to mobilise the political and financial support necessary to exploit these opportunities over the decade ahead, so that national malaria control and elimination programmes can access a much broader, more effective set of vector control interventions
