    Wrong-Site Procedures: Preventable Never Events that Continue to Happen

    A comprehensive discussion of ā€œnever eventsā€ or preventable and grievously shocking medical errors that may result in serious morbidity and mortality is incomplete without a thorough analysis of wrongā€site procedures (WSP). These occurrences are often due to multiple, simultaneous failures in team processes and communication. Despite being relatively rare, wrongā€site surgery can be devastating to all parties involved, from patients and families to healthcare workers and hospitals. This chapter provides a general overview of the topic in the context of clinical vignettes discussing specific examples of WSP. The goal of this work is to educate the reader about risk factors and preventive strategies pertinent to WSP, with the hope of propagating the knowledge required to eliminate these ā€œnever events.ā€ To that end, the chapter discusses pitfalls in current surgical practice that may contribute to critical safety breakdowns and emphasizes the need for multiple overlapping measures designed to improve patient safety. Furthermore, updated definitions regarding WSP are included in order to better characterize the different types of WSP. Most importantly, this chapter presents evidenceā€based support for the current strategies to prevent wrongā€site events. A summary of selected recent wrongā€site occurrences is also provided as a reference for researchers in this important area of patient safety

    Self identified research needs of New York organic farmers

    A survey of organic farmers in New York State identified problems in need of university research. Weed management was the most frequently mentioned problem by far, identified as significant by two-thirds of the organic farmers. Only a few other problems were listed as significant, including insufficient time for farm work, lack of markets, low prices, and lack of appropriate tools. These were cited by more than a third of the farmers. Drought, insect management, and a lack of a dependable supply of labor were cited by about one-third of the respondents. The survey also examined organic farmers' information sources. They do not use conventional sources of agricultural information, such as the extension service and conventional agricultural media, as much as books, magazines, and newsletters on organic f arming, other organic f armers, and on-farm experiments. Many respondents noted that local extension agents did not know very much about non-chemical solutions to organic production problems. They considered University Extension to be accessible, but not very useful in solving problems specific to organic farming, and had many suggestions to improve Land Grant research in organic agriculture

Well before any written historical record, chronic wounds of all shapes and sizes have plagued patients and created a significant burden on their caretakers. It has long been noticed that some patient factors are more likely to be associated with better wound healing . Likewise, certain wound types have been noted to be associated with a better prognosis than others. Until recently, there has been little scientific evidence regarding the risk factors and characteristics, both positive and negative, responsible for wound healing behaviors .     A surgical perspective report on melanoma management

    Management of melanoma includes wide excision with adequate margins and lymph node biopsy depending on the depth of the lesion, with subsequent completion lymphadenectomy for positive sentinel node. Locally advanced disease can be approached in several different ways depending on a variety of patient and disease-specific factors. These include surgical resection, isolated limb perfusion and infusion and intralesional injection therapy such as talimogene laherparepvec, IL-2 and Bacille Calmetteā€“Guerin. Ongoing controversy exists regarding the utility of completion lymphadenectomy, and trials such as MSLT-2 will attempt to shed light on this issue. The future of melanoma management will likely focus on expanding the use of immunotherapy, allowing for narrower surgical margins, particularly in sensitive anatomic areas, and limiting the number of completion lymphadenectomies

    Proximalization through one incision of a wrist arteriovenous fistula and distal revascularization with interval ligation

    In the setting of ischemic steal syndrome with tissue loss, patients with radiocephalic fistulas have limited options to preserve their conduit and treat their ischemic symptoms. To address this, we have proposed the technique of proximalization through one incision of a wrist arteriovenous fistula (POWR) with distal revascularization with interval ligation (DRIL) procedure. In the present retrospective, single-center, case series, we evaluated the outcomes of three patients with radiocephalic fistulas who had undergone POWR DRIL from 2017 to 2021. Their ischemic symptoms were monitored for regression. All three patients showed signs of regressing ischemia. The POWR DRIL represents an efficient procedure to address tissue loss and preserve the autogenous conduit

    Acellular Dermal Matrix Versus Inferior Deepithelialized Flap Breast Reconstruction: Equivalent Outcomes, with Increased Cost.

    BACKGROUND: Approximately 250,000 new cases of breast cancer are diagnosed yearly in the U.S. resulting in more postmastectomy breast reconstructions (PMBRs). The acellular dermal matrix (ADM) expander-implant method became popular in the mid-2000s, but newer techniques such as the inferior deepithelialized flap (IDF) has more recently been described. We hypothesize that ADMs and IDFs provide comparable aesthetic outcomes, with no difference in complication rates and operative characteristics. METHODS: A retrospective, single-institution study was performed between July 1, 2012, and June 30, 2014, examining all PMBR\u27s (ADM and IDF). Outcomes were categorized as clinical (e.g., complications requiring surgical intervention) or aesthetic. RESULTS: A total of 65 patients (41 ADM; 24 IDF; mean age, 53.4ā€‰Ā±ā€‰10.7 years) were included, with 101 PMBR\u27s evaluated (63 ADM and 38 IDF). Patients who underwent IDFs had higher body mass index (32 versus 25; CONCLUSIONS: The IDF procedure took 30 minutes longer for each reconstructed side, without significant differences in complications or aesthetic outcomes between the 2 PMBRs. IDF reconstructions may be more suitable for patients with grade 3 breast ptosis and higher body mass index. Further studies should focus on long-term outcomes and value-based approaches to PMBR

    Micropatterned Structural Control Suppresses Mechanotaxis of Endothelial Cells

    Vascular endothelial cell migration is critical in many physiological processes including wound healing and stent endothelialization. To determine how preexisting cell morphology influences cell migration under fluid shear stress, endothelial cells were preset in an elongated morphology on micropatterned substrates, and unidirectional shear stress was applied either parallel or perpendicular to the cell elongation axis. On micropatterned 20-Ī¼m lines, cells exhibited an elongated morphology with stress fibers and focal adhesion sites aligned parallel to the lines. On 115-Ī¼m lines, cell morphology varied as a function of distance from the line edge. Unidirectional shear stress caused unpatterned cells in a confluent monolayer to exhibit triphasic mechanotaxis behavior. During the first 3 h, cell migration speed increased in a direction antiparallel to the shear stress direction. Migration speed then slowed and direction became spatially heterogeneous. Starting 11ā€“12 h after the onset of shear stress, the unpatterned cells migrated primarily in the downstream direction, and migration speed increased significantly. In contrast, mechanotaxis was suppressed after the onset of shear stress in cells on micropatterned lines during the same time period, for the cases of both parallel and perpendicular flow. The directional persistence time was much longer for cells on the micropatterned lines, and it decreased significantly after flow onset. Migration trajectories were highly correlated among micropatterned cells within a three-cell neighborhood, and shear stress disrupted this spatially correlated migration behavior. Thus, presetting structural morphology may interfere with mechanisms of sensing local physical cues, which are critical for establishing mechanotaxis in response to hemodynamic shear stress
