    Effect of a combination of Xiaochaihu decoction and teprenone on peripheral blood T lymphocytes in chronic atrophic gastritis, and on expression of COX-2 in gastric mucosa

    Purpose: To study the effects of the combined use of Xiaochaihu decoction and teprenone on peripheral blood T lymphocytes in chronic atrophic gastritis (CAG) and gastric mucosal expression of cyclooxygenase-2 (COX-2).Methods: Patients with CAG who were treated at Traditional Chinese Medicine Hospital of Jiaxing from January 2017 to January 2018 were used as subjects. They consisted of observation and control groups (99 patients per group). Both groups were treated with teprenone (50 mg, thrice daily), but patients in the observation group received 200 mL of Xiaochaihu decoction, in addition to teprenone. The treatments were given orally, and lasted for 3 weeks. Comparisons were made between the two groups with respect to the effects of the treatments on peripheral blood T lymphocytes, CAG, and quality of life.Results: Peripheral T lymphocytes, chronic atrophic gastritis and quality of life in patients in the observation group were significantly improved, relative to control group patients. The combined treatment led to a significant decrease in the expression of COX-2. After treatment, there was upregulation in CD3+, CD4+ and CD4+/CD8+ levels in both groups, relative to the corresponding levels prior to drug exposure (p < 0.05). However, the observation group levels of CD3+, CD4+ and CD4+/CD8+ were higher than corresponding control values (t = -14.45, p < 0.001; t = -12.47, p < 0.001; t = -3.49, p < 0.001, respectively). Moreover, results from histopathological studies showed marked improvement in the observation group.Conclusion: A combination treatment of Xiaochaihu decoction and teprenone improves the condition of CAG patients via changes in peripheral blood lymphocytes and COX-2 expression in gastric mucosa.Keywords: Xiaochaihu decoction, Teprenone, Chronic atrophic gastritis, T lymphocytes, Cyclooxygenase-

    The relation of body mass index, demographic and health-related variables to length of stay for patients at an acute rehabilitation hospital after total hip arthroplasty

    Obesity is a growing public health concern world-wide. At the same time, knee and hip replacements are becoming an increasingly regular treatment for osteoarthritis. There are conflicting reports in the literature as to what extent body mass affects the length of stay (LOS) of patients undergoing total hip replacements. This chart review aims at examining the ­relationship between body mass index (BMI), demographic and health-related variables, and LOS. The retrospective chart review of one acute inpatient rehabilitation facility involved n = 119 patients. There was no significant association between LOS and body mass index (BMI), confirming earlier results. Insurance payer type (Medicare vs private) was statistically significant related to LOS. Moreover, there was a trend for the potential influence of race/ethnic patient background on LOS with Caucasians having shorter hospital stays. © 2010 Greenberg and Kroll, publisher and licensee Dove Medical Press Ltd.     Practical aspects of modelling parameter uncertainty for risk capital calculation

The final authenticated version is available online at: http://dx.doi.org/10.1007/s12297-019-00428-xWe assume that an insurance undertaking models its risk by a random variable X=X(¿0) with a fixed parameter (vector) ¿0. If the undertaking does not know ¿0 and can only estimate it from historical data, it faces parameter uncertainty. Neglecting parameter uncertainty can lead to an underestimation of the true risk capital requirement (see e.g. Gerrard and Tsanakas 2011; Fröhlich and Weng 2015). In this contribution we address some practical questions. To illustrate the relevance of the parameter risk we determine the probability of solvency for a risk capital model not taking parameter uncertainty into account for different distributions and samples sizes. We then follow the “inversion method” introduced in Fröhlich and Weng (2015) known to model an appropriate risk capital requirement respecting parameter uncertainty for a wide class of distributions and common estimation methods.
