    Examining the influence of contextual factors on risky sexual behavior among young women in Zomba district, Malawi: A multilevel analysis

    The objective of this study was to examine the association between risky sexual behavior and contextual factors related to economic resources, woman’s empowerment, and health facility characteristics among young women in Zomba district, Malawi. Secondary analyses of the Schooling, Income, and Health Risk (SIHR) study were undertaken. Four outcomes related to risky sexual behavior were examined: if participants had ever had sex, consistent condom use, and two scores measuring risk related to partner history and age during sexual activity. Regression models with cluster-robust standard errors and multilevel regression models were used to estimate associations; analyses were stratified by school enrolment status at baseline of the SIHR study and utilized weights to account for SIHR sampling design. For participants in school at baseline, the percent of girls enrolled in school at the community level was associated with ever having sex and consistent condom use. Belief in the right to refuse sex was protective against ever having sex, lower household education was associated with higher odds of ever having sex, and near rural and far rural residence was associated with decreased odds of condom. For participants not in school at baseline, lower individual education was associated with riskier scores related to age and partner history and lower household education was associated with lower age during sexual activity. Private or non-governmental health facilities were associated with decreased odds of condom use and higher age during sexual activity. In both strata, increasing age and near rural residence (within 16 kilometers of urban center) increased odds of ever having sex; lower educational achievement was associated with lower age during sexual activity. A history of pregnancy was associated with lower odds of condom use and riskier partner history. Risky sexual behavior is multifaceted and complex. While various factors related to women’s empowerment played a role, the most consistent variables associated with risky sexual behavior were those related to education. Interventions and programs seeking to reduce risky sexual behavior among young women, thereby reducing their risk of HIV infection, should continue to focus on improving access to education at multiple levels

    Reviews and syntheses: Spatial and temporal patterns in seagrass metabolic fluxes

    Seagrass meadow metabolism has been measured for decades to gain insight into ecosystem energy, biomass production, food web dynamics, and, more recently, to inform its potential in ameliorating ocean acidification (OA). This extensive body of literature can be used to infer trends and drivers of seagrass meadow metabolism. Here, we synthesize the results from 56 studies reporting in situ rates of seagrass gross primary productivity, respiration, and/or net community productivity to highlight spatial and temporal variability in oxygen (O2) fluxes. We illustrate that daytime net community production (NCP) is positive overall and similar across seasons and geographies. Full-day NCP rates, which illustrate the potential cumulative effect of seagrass beds on seawater biogeochemistry integrated over day and night, were also positive overall but were higher in summer months in both tropical and temperate ecosystems. Although our analyses suggest seagrass meadows are generally autotrophic, the effects on seawater oxygen are relatively small in magnitude. We also find positive correlations between gross primary production and temperature, although this effect may vary between temperate and tropical geographies and may change under future climate scenarios if seagrasses approach thermal tolerance thresholds. In addition, we illustrate that periods when full-day NCP is highest could be associated with lower nighttime O2 and increased diurnal variability in seawater O2. These results can serve as first-order estimates of when and where OA amelioration by seagrasses may be likely. However, improved understanding of variations in NCPdic:NCPO2 ratios and increased work directly measuring metabolically driven alterations in seawater pH will further inform the potential for seagrass meadows to serve in this context

    Effects of a Large Fishing Closure on Benthic Communitites in the Western Gulf of Maine: Recovery from the Effects of Gillnets and Otter Trawls

    The recovery of benthic communities inside the western Gulf of Maine fishing closure area was evaluated by comparing invertebrate assemblages at sites inside and outside of the closure four to six years after the closure was established. The major restriction imposed by the closure was a year-round prohibition of bottom gillnets and otter trawls. A total of 163 seafloor sites (~half inside and half outside the closure) within a 515-km2 study area were sampled with some combination of Shipek grab, Wildco box corer, or underwater video. Bottom types ranged from mud (silt and clay) to boulders, and the effects of the closure on univariate measures (total density, biomass, taxonomic richness) of benthos varied widely among sediment types. For sites with predominantly mud sediments, there were mixed effects on inside and outside infauna and no effect on epifauna. For sites with mainly sand sediments, there were higher density, biomass, and taxonomic richness for infauna inside the closure, but no significant effects on epifauna. For sites dominated by gravel (which included boulders in some areas), there were no effects on infauna but strong effects on epifaunal density and taxonomic richness. For fishing gear, the data indicated that infauna recovered in sand from the impacts of otter trawls operated inside the closure but that they did not recover in mud, and that epifauna recovered on gravel bottoms from the impact of gillnets used inside the closure. The magnitudes of impact and recovery, however, cannot be inferred directly from our data because of a confounding factor of different fishing intensities outside the closure for a direct comparison of preclosure and postclosure data. The overall negative impact of trawls is likely underestimated by our data, whereas the negative impact of gillnets is likely overestimated

    Research Knowledge of Advanced Standing and Traditional Students: Implications for BSW Education

    The advanced standing model of social work education, which affords graduate credit to qualified BSW students who pursue their MSW, has not been without issue or controversy, including questions of potential differences in performance on various educational outcomes. Specifically related to research curriculum, the importance of which is often not wholly embraced by students, this article reports the results of a secondary data analysis comparing research knowledge among advanced standing and traditional MSW students as well as among the various undergraduate majors (i.e., BSW, psychology, and sociology). Results suggest that research knowledge is similar and low across student subgroups. Important differences in research knowledge were found among student groups based on undergraduate major, with BSW undergraduates without advanced standing, on average, scoring lower than any other group.     Rapid Saline Permafrost Thaw Below a Shallow Thermokarst Lake in Arctic Alaska

    Permafrost warming and degradation is well documented across the Arctic. However, observation- and model-based studies typically consider thaw to occur at 0°C, neglecting the widespread occurrence of saline permafrost in coastal plain regions. In this study, we document rapid saline permafrost thaw below a shallow arctic lake. Over the 15-year period, the lakebed subsided by 0.6 m as ice-rich, saline permafrost thawed. Repeat transient electromagnetic measurements show that near-surface bulk sediment electrical conductivity increased by 198% between 2016 and 2022. Analysis of wintertime Synthetic Aperture Radar satellite imagery indicates a transition from a bedfast to a floating ice lake with brackish water due to saline permafrost thaw. The regime shift likely contributed to the 65% increase in thermokarst lake lateral expansion rates. Our results indicate that thawing saline permafrost may be contributing to an increase in landscape change rates in the Arctic faster than anticipated

    Association Between Race/Ethnicity and Survival of Melanoma Patients in the United States Over 3 Decades

    Melanoma is a treatable and preventable skin cancer. It is responsible for 75% of deaths among all skin cancers. Previous studies have found that race/ethnicity may play a role in survival among melanoma patients. However, there are no studies that cover 30 years and take race into account for the U.S. population. This study is a secondary analysis of the National Cancer Institute\u27s Surveillance, Epidemiology, and End Result (SEER) Program. Adults with primary cutaneous melanoma from 1982 to 2011 were included; the final sample size was 185,219. The outcome was survival; both cause-specific and all-cause mortality were examined. The main exposure was race/ethnicity. Kaplan?Meier survival analysis was used to estimate overall survival. Cox proportional hazards regression was used to estimate unadjusted and adjusted hazard ratios (HRs). A P-value less than 0.05 was considered statistically significant. More than 50% of patients in all races/ethnicities were diagnosed at the in situ or localized stage. Non-Hispanic White patients were more frequently diagnosed at the in situ stage. Overall, more men were diagnosed than women. The majority of cases among all races were men. Non-Hispanic Black females represented the smallest percentage of melanoma cases among all races. The smallest number of diagnoses across all races/ethnicities was made from 1982 to 1991. Median follow-up was 81 months and no collinearity was observed in the adjusted models. When examining cause-specific mortality and controlling for site and stage at diagnosis, gender, age and decade of diagnosis, the HR for non-Hispanic Black patients was lower than that for non-Hispanic White patients (HR 0.7; 95% confidence interval (CI): 0.6?0.8). However, when examining all-cause mortality, this difference disappeared (HR 1.1; 95% CI: 1.0?1.2). Stage at diagnosis impacted HR; patients diagnosed with distant metastases had significantly worse survival. When taking cause-specific mortality into consideration and after controlling for stage and site at diagnosis, gender, and age and decade of diagnosis, non-Hispanic Black patients had a lower HR compared to non-Hispanic White patients. However, this difference disappeared when examining all-cause mortality. Further research is needed to explore this finding and to determine what factors may be associated with late-stage melanoma diagnosis

    Provider Perceptions of Barriers to HIV Care Among Women with HIV in Miami-Dade County, Florida, and Possible Solutions: A Qualitative Study

    In Miami-Dade County, women with HIV (WWH) enrolled in Ryan White Program (RWP) services belong to groups that have historically faced structural barriers to care. To examine provider perceptions of WWH\u27s barriers to care and elicit possible solutions, we conducted semi-structured interviews (n = 20) with medical case managers and human immunodeficiency virus (HIV) healthcare providers from medical case management sites serving WWH enrolled in the Miami-Dade RWP. Verbatim transcripts were analyzed thematically by two coders through an iterative process; disagreements were resolved through consensus. Barriers included lack of disclosure and stigma, additional psychosocial barriers to care, structural and logistical barriers, and negative interactions with health care providers. Participant suggestions to address these barriers included strategies that support women and foster individualized services that are responsive to their lived experiences and needs. Other solutions, such as those related to transportation, housing, and general funding for the RWP, will require advocacy and policy change

    Epidemiology of antimicrobial-resistant Escherichia coli carriage in sympatric humans and livestock in a rapidly urbanizing city

    There are substantial limitations in understanding of the distribution of antimicrobial resistance (AMR) in humans and livestock in developing countries. This papers present the results of an epidemiological study examining patterns of AMR in Escherichia coli isolates circulating in sympatric human (n = 321) and livestock (n = 633) samples from 99 households across Nairobi, Kenya. E. coli isolates were tested for susceptibility to 13 antimicrobial drugs representing nine antibiotic classes. High rates of AMR were detected, with 47.6% and 21.1% of isolates displaying resistance to three or more and five or more antibiotic classes, respectively. Human isolates showed higher levels of resistance to sulfonamides, trimethoprim, aminoglycosides and penicillins compared with livestock (P0.05). These findings revealed a high prevalence of AMR E. coli circulating in healthy humans and livestock in Nairobi, with no evidence to suggest that keeping livestock, when treated as a single risk factor, contributed significantly to the burden of AMR in humans, although the presence of livestock waste was significant. These results provide an understanding of the broader epidemiology of AMR in complex and interconnected urban environments
