    Clinical and MRI responses to etanercept in early non-radiographic axial spondyloarthritis : 48-week results from the EMBARK study

    Objective: To evaluate the efficacy and safety of etanercept (ETN) after 48 weeks in patients with early active non-radiographic axial spondyloarthritis (nr-axSpA). Methods: Patients meeting Assessment of SpondyloArthritis international Society (ASAS) classification criteria for axSpA, but not modified New York radiographic criteria, received double-blind ETN 50 mg/week or placebo (PBO) for 12 weeks, then open-label ETN (ETN/ETN or PBO/ETN). Clinical, health, productivity, MRI and safety outcomes were assessed and the 48-week data are presented here. Results: 208/225 patients (92%) entered the open-label phase at week 12 (ETN, n=102; PBO, n=106). The percentage of patients achieving ASAS40 increased from 33% to 52% between weeks 12 and 48 for ETN/ETN and from 15% to 53% for PBO/ETN (within-group p value <0.001 for both). For ETN/ETN and PBO/ETN, the EuroQol 5 Dimensions utility score improved by 0.14 and 0.08, respectively, between baseline and week 12 and by 0.23 and 0.22 between baseline and week 48. Between weeks 12 and 48, MRI Spondyloarthritis Research Consortium of Canada sacroiliac joint (SIJ) scores decreased by -1.1 for ETN/ETN and by -3.0 for PBO/ETN, p<0.001 for both. Decreases in MRI SIJ inflammation and C-reactive protein correlated with several clinical outcomes at weeks 12 and 48. Conclusions: Patients with early active nr-axSpA demonstrated improvement from week 12 in clinical, health, productivity and MRI outcomes that was sustained to 48 weeks

    The Decoupled Kinematics of High- z QSO Host Galaxies and Their Ly α Halos

This is an open access article distributed under the Creative Commons Attribution License, to view a copy of the license, see: https://creativecommons.org/licenses/by/4.0/We present a comparison of the interstellar medium traced by [C ii] (Atacama Large Millimeter/submillimeter Array), and ionized halo gas traced by Lyα (Multi Unit Spectroscopic Explorer), in and around QSO host galaxies at z ∼ 6. To date, 18 QSOs at this redshift have been studied with both MUSE and high-resolution ALMA imaging; of these, 8 objects display a Lyα halo. Using data cubes matched in velocity resolution, we compare and contrast the spatial and kinematic information of the Lyα halos and the host galaxies’ [C ii] (and dust-continuum) emission. We find that the Lyα halos extend typically 3−30 times beyond the interstellar medium of the host galaxies. The majority of the Lyα halos do not show ordered motion in their velocity fields, whereas most of the [C ii] velocity fields do. In those cases where a velocity gradient can be measured in Lyα, the kinematics do not align with those derived from the [C ii] emission. This implies that the Lyα emission is not tracing the outskirts of a large rotating disk, which is a simple extension of the central galaxy seen in [C ii] emission. It rather suggests that the kinematics of the halo gas are decoupled from those of the central galaxy. Given the scattering nature of Lyα, these results need to be confirmed with James Webb Space Telescope Integral Field Unit observations that can constrain the halo kinematics further using the nonresonant Hα line.Peer reviewe

    The Importance of Turbulent Fluxes in the Surface Energy Balance of a Debris-Covered Glacier in the Himalayas

    Surface energy balance models are common tools to estimate melt rates of debris-covered glaciers. In the Himalayas, radiative fluxes are occasionally measured, but very limited observations of turbulent fluxes on debris-covered tongues exist to date. We present measurements collected between 26 September and 12 October 2016 from an eddy correlation system installed on the debris-covered Lirung Glacier in Nepal during the transition between monsoon and post-monsoon. Our observations suggest that surface energy losses through turbulent fluxes reduce the positive net radiative fluxes during daylight hours between 10 and 100%, and even lead to a net negative surface energy balance after noon. During clear days, turbulent flux losses increase to over 250 W m−2 mainly due to high sensible heat fluxes. During overcast days the latent heat flux dominates the turbulent losses and together they reach just above 100 W m−2. Subsequently, we validate the performance of three bulk approaches in reproducing the observations from the eddy correlation system. Large differences exist between the approaches, and accurate estimates of surface temperature, wind speed, and surface roughness are necessary for their performance to be reasonable. Moreover, the tested bulk approaches generally overestimate turbulent latent heat fluxes by a factor 3 on clear days, because the debris-covered surface dries out rapidly, while the bulk equations assume surface saturation. Improvements to bulk surface energy models should therefore include the drying process of the surface. A sensitivity analysis suggests that, in order to be useful in distributed melt models, an accurate extrapolation of wind speed, surface temperature and surface roughness in space is a prerequisite. By applying the best performing bulk model over a complete melt period, we show that turbulent fluxes reduce the available energy for melt at the debris surface by 17% even at very low wind speeds. Overall, we conclude that turbulent fluxes play an essential role in the surface energy balance of debris-covered glaciers and that it is essential to include them in melt models

    Sustained efficacy, safety and patient-reported outcomes of certolizumab pegol in axial spondyloarthritis: 4-year outcomes from RAPID-axSpA.

    Objective: The aim was to assess the long-term safety and efficacy of certolizumab pegol over 4 years of continuous treatment in patients with axial spondyloarthritis (axSpA), including both AS and non-radiographic (nr-) axSpA. Methods: RAPID-axSpA was a phase 3 randomized trial, double blind and placebo controlled to week 24, dose blind to week 48 and open label to week 204. Patients had a clinical diagnosis of axSpA, meeting Assessment of SpondyloArthritis international Society (ASAS) criteria, and had active disease. The assessed outcomes included ASAS20, ASAS40, AS DAS (ASDAS), BASDAI, BASFI and BASMI scores, along with selected measures of remission. Further patient-reported outcomes, peripheral arthritis, enthesitis, uveitis and quality-of-life measures are also reported. Results: Two hundred and eighteen of 325 patients randomized (AS: 121; nr-axSpA: 97) received certolizumab pegol from week 0. Of these, 65% remained in the study at week 204 (AS: 67%; nr-axSpA: 63%). Across all outcomes, for AS and nr-axSpA, sustained improvements were observed to week 204 [week 204 overall axSpA: ASAS20: 54.1% (non-responder imputation); 83.7% (observed case, OC); ASAS40: 44.0% (non-responder imputation); 68.1% (OC); ASDAS inactive disease: 32.1% (last observation carried forward); 31.4% (OC)]. In the safety set (n = 315), there were 292.8 adverse events and 10.4 serious adverse events per 100 patient-years. No deaths were reported. Conclusion: In the first study to evaluate the efficacy of an anti-TNF across both axSpA subpopulations, improvements in clinical and patient-reported outcomes at 24 and 96 weeks were sustained through 4 years of treatment, with no new safety signals.
