    The influence of 'significant others' on persistent back pain and work participation: a qualitative exploration of illness perceptions

    Background Individual illness perceptions have been highlighted as important influences on clinical outcomes for back pain. However, the illness perceptions of 'significant others' (spouse/partner/close family member) are rarely explored, particularly in relation to persistent back pain and work participation. The aim of this study was to initiate qualitative research in this area in order to further understand these wider influences on outcome. Methods Semi-structured interviews based on the chronic pain version of the Illness Perceptions Questionnaire-Revised were conducted with a convenience sample of UK disability benefit claimants, along with their significant others (n=5 dyads). Data were analysed using template analysis. Results Significant others shared, and perhaps further reinforced, claimants' unhelpful illness beliefs including fear of pain/re-injury associated with certain types of work and activity, and pessimism about the likelihood of return to work. In some cases, significant others appeared more resigned to the permanence and negative inevitable consequences of the claimant's back pain condition on work participation, and were more sceptical about the availability of suitable work and sympathy from employers. In their pursuit of authenticity, claimants were keen to stress their desire to work whilst emphasising how the severity and physical limitations of their condition prevented them from doing so. In this vein, and seemingly based on their perceptions of what makes a 'good' significant other, significant others acted as a 'witness to pain', supporting claimants' self-limiting behaviour and statements of incapacity, often responding with empathy and assistance. The beliefs and responses of significant others may also have been influenced by their own experience of chronic illness, thus participants lives were often intertwined and defined by illness. Conclusions The findings from this exploratory study reveal how others and wider social circumstances might contribute both to the propensity of persistent back pain and to its consequences. This is an area that has received little attention to date, and wider support of these findings may usefully inform the design of future intervention programmes aimed at restoring work participation

    Nonlinear Coupling of Tearing Modes with Self-Consistent Resistivity Evolution in Tokamaks

    The nonlinear interaction of tearing modes of different helicity is studied for realistic values of the tokamak parameters of resistivity and parallel heat conduction. The self-consistent evolution of the resistivity is taken into account through the electron heat conduction equation. For equilibrium q profiles inferred from electron temperature profile measured before a tokamak disruption, the essential result is that the (m = 2; n = 1) model nonlinearly destabilizes other modes on a rapid time scale. Because of the development of magnetic islands of different helicity, the toroidal current density is severely deformed. These islands overlap and field lines become stochastic in a sizable plasma volume, flattening the temperature profile in this region through parallel heat transport. The deformation of the toroidal current produces a rapid decrease in the self-inductance of the plasma, and the voltage at the limiter decreases, becoming increasingly negative. An extensive survey of equilibria and initial conditions has been conducted, and a simple prescription for their nonlinear stability properties is given

    Identification of the factors associated with outcomes in a condition management programme

    <p>Background: A requirement of the Government’s Pathways to Work (PtW) agenda was to introduce a Condition Management Programme (CMP). The aim of the present study was to identify the differences between those who engaged and made progress in this telephone-based biopsychosocial intervention, in terms of their health, and those who did not and to determine the client and practitioner characteristics and programme elements associated with success in a programme aimed at improving health.</p> <p>Methods: Data were obtained from the CMP electronic spreadsheets and clients paper-based case records. CMP standard practice was that questionnaires were administered during the pre- and post-assessment phases over the telephone. Each client’s record contains their socio-demographic data, their primary health condition, as well as the pre- and post-intervention scores of the health assessment tool administered. Univariate and multivariate statistical analysis was used to investigate the relationships between the database variables. Clients were included in the study if their records were available for analysis from July 2006 to December 2007.</p> <p> Results: On average there were 112 referrals per month, totalling 2016 referrals during the evaluation period. The majority (62.8%) of clients had a mental-health condition. Successful completion of the programme was 28.5% (575 “completers”; 144 “discharges”). Several factors, such as age, health condition, mode of contact, and practitioner characteristics, were significant determinants of participation and completion of the programme. The results showed that completion of the CMP was associated with a better mental-health status, by reducing the number of clients that were either anxious, depressed or both, before undertaking the programme, from 74% to 32.5%.</p> <p>Conclusions: Our findings showed that an individual's characteristics are associated with success in the programme, defined as completing the intervention and demonstrating an improved health status. This study provides some evidence that the systematic evaluation of such programmes and interventions could identify ways in which they could be improved.</p&gt

    WALLABY pre-pilot survey: Two dark clouds in the vicinity of NGC 1395

    We present the Australian Square Kilometre Array Pathfinder (ASKAP) WALLABY pre-pilot observations of two ‘dark’ HI sources (with HI masses of a few times 108 M⊙ and no known stellar counterpart) that reside within 363 kpc of NGC 1395, the most massive early-type galaxy in the Eridanus group of galaxies. We investigate whether these ‘dark’ HI sources have resulted from past tidal interactions or whether they are an extreme class of low surface brightness galaxies. Our results suggest that both scenarios are possible, and not mutually exclusive. The two ‘dark’ HI sources are compact, reside in relative isolation and are more than 159 kpc away from their nearest HI-rich galaxy neighbour. Regardless of origin, the HI sizes and masses of both ‘dark’ HI sources are consistent with the HI size–mass relationship that is found in nearby lowmass galaxies, supporting the possibility that these HI sources are an extreme class of low surface brightness galaxies. We identified three analogues of candidate primordial ‘dark’ HI galaxies within the TNG100 cosmological, hydrodynamic simulation. All three model analogues are dark matter-dominated, have assembled most of their mass 12–13 Gyr ago, and have not experienced much evolution until cluster infall 1–2 Gyr ago. Our WALLABY pre-pilot science results suggest that the upcoming large area HI surveys will have a significant impact on our understanding of low surface brightness galaxies and the physical processes that shape them

    This study provides the proof of principle that probing the host and the microbe transcriptomes simultaneously is a valuable means to accessing unique information on host pathogen interactions. Our results also underline the extraordinary plasticity of host cell and pathogen responses to infection, and provide a solid framework to further understand the complex mechanisms involved in immunity to M. tuberculosis and in mycobacterial adaptation to different intracellular environments

    Subacute and chronic, non-specific back and neck pain: cognitive-behavioural rehabilitation versus primary care. A randomized controlled trial

    <p>Abstract</p> <p>Background</p> <p>In the industrial world, non-specific back and neck pain (BNP) is the largest diagnostic group underlying sick-listing. For patients with subacute and chronic (= full-time sick-listed for 43 – 84 and 85 – 730 days, respectively) BNP, cognitive-behavioural rehabilitation was compared with primary care. The specific aim was to answer the question: within an 18-month follow-up, will the outcomes differ in respect of sick-listing and number of health-care visits?</p> <p>Methods</p> <p>After stratification by age (≤ 44/≥ 45 years) and subacute/chronic BNP, 125 Swedish primary-care patients were randomly allocated to cognitive-behavioural rehabilitation (rehabilitation group) or continued primary care (primary-care group). Outcome measures were <it>Return-to-work share </it>(percentage) and <it>Return-to-work chance </it>(hazard ratios) over 18 months, <it>Net days </it>(crude sick-listing days × degree), and the number of <it>Visits </it>(to physicians, physiotherapists etc.) over 18 months and the three component six-month periods. Descriptive statistics, Cox regression and mixed-linear models were used.</p> <p>Results</p> <p>All patients: <it>Return-to-work share </it>and <it>Return-to-work chance </it>were equivalent between the groups. <it>Net days </it>and <it>Visits </it>were equivalent over 18 months but decreased significantly more rapidly for the rehabilitation group over the six-month periods (<it>p </it>< .05). Subacute patients: <it>Return-to-work share </it>was equivalent. <it>Return-to-work chance </it>was significantly greater for the rehabilitation group (hazard ratio 3.5 [95%CI1.001 – 12.2]). <it>Net days </it>were equivalent over 18 months but decreased significantly more rapidly for the rehabilitation group over the six-month periods and there were 31 days fewer in the third period. <it>Visits </it>showed similar though non-significant differences and there were half as many in the third period. Chronic patients: <it>Return-to-work share, Return-to-work chance </it>and <it>Net days </it>were equivalent. <it>Visits </it>were equivalent over 18 months but tended to decrease more rapidly for the rehabilitation group and there were half as many in the third period (non-significant).</p> <p>Conclusion</p> <p>The results were equivalent over 18 months. However, there were indications that cognitive-behavioural rehabilitation in the longer run might be superior to primary care. For subacute BNP, it might be superior in terms of sick-listing and health-care visits; for chronic BNP, in terms of health-care visits only. More conclusive results concerning this possible long-term effect might require a longer follow-up.</p> <p>Trial registration</p> <p>NCT00488735.</p

    The Discovery of a Second Field Methane Brown Dwarf from Sloan Digital Sky Survey Commissioning Data

    We report the discovery of a second field methane brown dwarf from the commissioning data of the Sloan Digital Sky Survey (SDSS). The object, SDSS J134646.45-003150.4 (SDSS 1346-00), was selected because of its very red color and stellar appearance. Its spectrum between 0.8-2.5 mic is dominated by strong absorption bands of H_2O and CH_4 and closely mimics those of Gliese 229B and SDSS 162414.37+002915.6 (SDSS 1624+00), two other known methane brown dwarfs. SDSS 1346-00 is approximately 1.5 mag fainter than Gliese 229B, suggesting that it lies about 11 pc from the sun. The ratio of flux at 2.1 mic to that at 1.27 mic is larger for SDSS 1346-00 than for Gliese 229B and SDSS 1624+00, which suggests that SDSS 1346-00 has a slightly higher effective temperature than the others. Based on a search area of 130 sq. deg. and a detection limit of z* = 19.8, we estimate a space density of 0.05 pc^-3 for methane brown dwarfs with T_eff ~ 1000 K in the 40 pc^3 volume of our search.
