    Maintaining real-time precise point positioning during outages of orbit and clock corrections

    The precise point positioning (PPP) is a popular positioning technique that is dependent on the use of precise orbits and clock corrections. One serious problem for real-time PPP applications such as natural hazard early warning systems and hydrographic surveying is when a sudden communication break takes place resulting in a discontinuity in receiving these orbit and clock corrections for a period that may extend from a few minutes to hours. A method is presented to maintain real-time PPP with 3D accuracy less than a decimeter when such a break takes place. We focus on the open-access International GNSS Service (IGS) real-time service (RTS) products and propose predicting the precise orbit and clock corrections as time series. For a short corrections outage of a few minutes, we predict the IGS-RTS orbits using a high-order polynomial, and for longer outages up to 3 h, the most recent IGS ultra-rapid orbits are used. The IGS-RTS clock corrections are predicted using a second-order polynomial and sinusoidal terms. The model parameters are estimated sequentially using a sliding time window such that they are available when needed. The prediction model of the clock correction is built based on the analysis of their properties, including their temporal behavior and stability. Evaluation of the proposed method in static and kinematic testing shows that positioning precision of less than 10 cm can be maintained for up to 2 h after the break. When PPP re-initialization is needed during the break, the solution convergence time increases; however, positioning precision remains less than a decimeter after convergence

    Resident physician and hospital pharmacist familiarity with patient discharge medication costs

    Objective Cost-related medication non-adherence is associated with increased health-care resource utilization and poor patient outcomes. Physicians-in-training generally receive little education regarding costs of prescribed therapy and may rely on hospital pharmacists for this information. However, little is documented regarding either of these health care providers’ familiarity with out-of pocket medication expenses borne by patients in the community. The purpose of this study was to evaluate and compare resident physician and hospital pharmacist familiarity with what patients pay for medications prescribed once discharged. Setting A major tertiary patient care and medical teaching centre in Canada. Method Internal medicine residents and hospital pharmacists within a specific health care organization were invited to participate in an online survey. Eight patient case scenarios and associated discharge therapeutic regimens were outlined and respondents asked to identify the costs patients would incur when having the prescription filled once discharged. Main Outcome Measure Total number and proportion of estimates above and below actual cost were calculated and compared between the groups using χ2 tests. Responses ±10% of the true cost were considered correct. Mean absolute values and standard deviation estimated costs, as well as cost increments above and below 10%, were calculated to assess the magnitude of the discrepancy between the respondent estimates and the actual total cost. Results Forty-four percent of physician residents and 26% of hospital pharmacists accessed the survey. Overall 39% and 47% of medication costs were under-estimated, 32% and 33% were overestimated, and 29% and 21% were correctly estimated by residents and pharmacists, respectively (P = NS). Incorrect estimates were evident across all therapeutic classes and medical indications presented in the survey. The greatest absolute cost discrepancy for both groups was under-estimation of linezolid (800and800 and 400) and over-estimation of clopidogrel (80)andbisoprololtherapy(80) and bisoprolol therapy (22) by residents and pharmacists, respectively.     The Role of Hydrogeography and Climate in the Landscape Epidemiology of West Nile Virus in New York State from 2000 to 2010

    The epidemiology and ecology of West Nile virus (WNV) have not yet been completely described. In particular, the specific roles of climate and water in the landscape in the occurrence of human WNV cases remain unknown. This study used Poisson regression to describe the relationships between WNV cases and temperature, precipitation, and the hydrogeography of the landscape in New York State from 2000 to 2010. Fully adjusted models showed that hydrogeographic area was significantly inversely associated with WNV cases (incidence rate ratio (IRR) = 0.99; 95% C.I. = 0.98–0.997, p = 0.04), such that each one square kilometer increase in hydrogeographic area was associated with a 1% decrease in WNV incidence. This association was independent of both temperature, which was also associated with WNV incidence (IRR = 2.06; 95% C.I. = 1.84–2.31, p<0.001), and precipitation, which was not (IRR = 1.0; 95% C.I. = 0.99–1.01, p = 0.16). While the results are only suggestive due to the county-level aggregated data, these findings do identify a potentially important surveillance signal in the landscape epidemiology of WNV infection

    Necessary connections: ‘Feelings photographs’ in criminal justice research

    Visual representations of prisons and their inmates are common in the news and social media, with stories about riots, squalor, drugs, self-harm and suicide hitting the headlines. Prisoners’ families are left to worry about the implications of such events on their kin, while those incarcerated and less able to understand social cues, norms and rules, are vulnerable to deteriorating mental health at best, to death at worst. As part of the life-story method in my research with offenders who are on the autism spectrum, have mental health problems and/or have learning difficulties, and prisoner’s mothers, I asked participants to take photographs, reflecting upon their experiences. Photographs in this case, were primarily used to help respondents consider and articulate their feelings in follow-up interviews. Notably, seeing (and imagining) is often how we make a connection to something (object or feeling), or someone (relationships), such that images in fiction, news/social media, drama, art, film and photographs can shape the way people think and behave – indeed feel about things and people. Images and representations ought to be taken seriously in researching social life, as how we interpret photographs, paintings, stories and television shows is based on our own imaginings, biography, culture and history. Therefore, we look at and process an image before words escape, by ‘seeing’ and imagining. How my participants and I ‘collaborate’ in doing visual methods and then how we make meaning of the photographs in storying their feelings, is insightful. As it is, I wanted to enable my participants to make and create their own stories via their photographs and narratives, whilst connecting to them, along with my own interpretation and subjectivities

    A global analysis of the comparability of winter chill models for fruit and nut trees

    Many fruit and nut trees must fulfill a chilling requirement to break their winter dormancy and resume normal growth in spring. Several models exist for quantifying winter chill, and growers and researchers often tacitly assume that the choice of model is not important and estimates of species chilling requirements are valid across growing regions. To test this assumption, Safe Winter Chill (the amount of winter chill that is exceeded in 90% of years) was calculated for 5,078 weather stations around the world, using the Dynamic Model [in Chill Portions (CP)], the Chilling Hours (CH) Model and the Utah Model [Utah Chill Units (UCU)]. Distributions of the ratios between different winter chill metrics were mapped on a global scale. These ratios should be constant if the models were strictly proportional. Ratios between winter chill metrics varied substantially, with the CH/CP ratio ranging between 0 and 34, the UCU/CP ratio between −155 and +20 and the UCU/CH ratio between −10 and +5. The models are thus not proportional, and chilling requirements determined in a given location may not be valid elsewhere. The Utah Model produced negative winter chill totals in many Subtropical regions, where it does not seem to be useful. Mean annual temperature and daily temperature range influenced all winter chill ratios, but explained only between 12 and 27% of the variation. Data on chilling requirements should always be amended with information on the location and experimental conditions of the study in which they were determined, ideally including site-specific conversion factors between winter chill models. This would greatly facilitate the transfer of such information across growing regions, and help prepare growers for the impact of climate change

    Decreased Numbers of Blood Dendritic Cells and Defective Function of Regulatory T Cells in Antineutrophil Cytoplasmic Antibody-Associated Vasculitis

    BACKGROUND: Dendritic cells (DC) and regulatory cells (Treg) play pivotal roles in controlling both normal and autoimmune adaptive immune responses. DC are the main antigen-presenting cells to T cells, and they also control Treg functions. In this study, we examined the frequency and phenotype of DC subsets, and the frequency and function of Treg from patients with ANCA-associated vasculitis (AAV). METHODOLOGY/PRINCIPAL FINDINGS: Blood samples from 19 untreated patients with AAV during flares and before any immunosuppressive treatment were analyzed, along with 15 AAV patients in remission and 18 age-matched healthy controls. DC and Treg numbers, and phenotypes were assessed by flow cytometry, and in vitro suppressive function of Treg was determined by co-culture assay. When compared to healthy volunteers, absolute numbers of conventional and plasmacytoid DC were decreased in AAV patients. During the acute phase this decrease was significantly more pronounced and was associated with an increased DC expression of CD62L. Absolute numbers of Treg (CD4(+)CD25(high)CD127(low/-) Tcells) were moderately decreased in patients. FOXP3 and CD39 were expressed at similar levels on Treg from patients as compared to controls. The suppressive function of Treg from AAV patients was dramatically decreased as compared to controls, and this defect was more pronounced during flares than remission. This Treg functional deficiency occurred in the absence of obvious Th17 deviation. CONCLUSION: In conclusion, these data show that AAV flares are associated with both a decrease number and altered phenotype of circulating DC and point to a role for Treg functional deficiency in the pathogenesis of AAV
