Direct observation of precipitation along twin boundaries and dissolution in a magnesium alloy annealing at high temperature
Precipitation along twin boundaries and dissolution in a cold-rolled Mg-Y-Nd alloy was directly observed for the first time during annealing at 490 Β°C. Precipitation occurred concurrently with recrystallization and the combined effect of precipitation and solute segregated to twin boundaries modified the recrystallization behaviour. Precipitates later dissolved into the matrix at the point where full recrystallization was nearly complete. The precipitates and higher solute concentration along original twin boundaries hindered grain growth of newly formed recrystallized grains. Even where twin boundaries had been consumed by recrystallization, the size of recrystallized grains were still controlled by the pre-existing twin boundaries
Individual effect of recrystallisation nucleation sites on texture weakening in a magnesium alloy: Part 1- double twins
Recrystallised grain nucleation, grain growth and corresponding texture evolution in a cold-rolled rare
earth containing WE43 Mg alloy during annealing at 490 οΏ½C was fully tracked using a quasi-in-situ
electron backscatter diffraction method. The results show nucleation sites, such as double twins, can
weaken the deformed texture and for the first time provide direct evidence that recrystallised grains
originating from double twins can form the rare earth texture during annealing. Precipitation and
recrystallisation occurred concurrently during most of the annealing period, with precipitates forming
preferentially along prior grain and twin boundaries. These precipitates effectively retard the recrystallisation
due to particle pinning leading to an excessively long time for the completion of recrystallisation.
A large portion of recrystallised grains were observed to have β©0001βͺ poles tilted 20e45οΏ½ away
from the normal direction. The RE texture emerges during the nucleation of recrystallised grains and is
maintained during subsequent uniform grain growth, which results in a stable RE texture being developed
as recrystallisation progresses. The uniform grain growth could be attributed to solute drag suppressing
the grain boundary mobility of those grains that had recrystallised with a basal texture and
precipitate pinning restricting potential orientated grain growth

The systematic guideline review: method, rationale, and test on chronic heart failure
Background: Evidence-based guidelines have the potential to improve healthcare. However, their de-novo-development requires substantial resources-especially for complex conditions, and adaptation may be biased by contextually influenced recommendations in source guidelines. In this paper we describe a new approach to guideline development-the systematic guideline review method (SGR), and its application in the development of an evidence-based guideline for family physicians on chronic heart failure (CHF).
Methods: A systematic search for guidelines was carried out. Evidence-based guidelines on CHF management in adults in ambulatory care published in English or German between the years 2000 and 2004 were included. Guidelines on acute or right heart failure were excluded. Eligibility was assessed by two reviewers, methodological quality of selected guidelines was appraised using the AGREE instrument, and a framework of relevant clinical questions for diagnostics and treatment was derived. Data were extracted into evidence tables, systematically compared by means of a consistency analysis and synthesized in a preliminary draft. Most relevant primary sources were re-assessed to verify the cited evidence. Evidence and recommendations were summarized in a draft guideline.
Results: Of 16 included guidelines five were of good quality. A total of 35 recommendations were systematically compared: 25/35 were consistent, 9/35 inconsistent, and 1/35 un-rateable (derived from a single guideline). Of the 25 consistencies, 14 were based on consensus, seven on evidence and four differed in grading. Major inconsistencies were found in 3/9 of the inconsistent recommendations. We re-evaluated the evidence for 17 recommendations (evidence-based, differing evidence levels and minor inconsistencies) - the majority was congruent. Incongruity was found where the stated evidence could not be verified in the cited primary sources, or where the evaluation in the source guidelines focused on treatment benefits and underestimated the risks. The draft guideline was completed in 8.5 man-months. The main limitation to this study was the lack of a second reviewer.
Conclusion: The systematic guideline review including framework development, consistency analysis and validation is an effective, valid, and resource saving-approach to the development of evidence-based guidelines
Would you be surprised if this patient died?: Preliminary exploration of first and second year residents' approach to care decisions in critically ill patients
BACKGROUND: How physicians approach decision-making when caring for critically ill patients is poorly understood. This study aims to explore how residents think about prognosis and approach care decisions when caring for seriously ill, hospitalized patients. METHODS: Qualitative study where we conducted structured discussions with first and second year internal medicine residents (n = 8) caring for critically ill patients during Medical Intensive Care Unit Ethics and Discharge Planning Rounds. Residents were asked to respond to questions beginning with "Would you be surprised if this patient died?" RESULTS: An equal number of residents responded that they would (n = 4) or would not (n = 4) be surprised if their patient died. Reasons for being surprised included the rapid onset of an acute illness, reversible disease, improving clinical course and the patient's prior survival under similar circumstances. Residents reported no surprise with worsening clinical course. Based on the realization that their patient might die, residents cited potential changes in management that included clarifying treatment goals, improving communication with families, spending more time with patients and ordering fewer laboratory tests. Perceived or implied barriers to changes in management included limited time, competing clinical priorities, "not knowing" a patient, limited knowledge and experience, presence of diagnostic or prognostic uncertainty and unclear treatment goals. CONCLUSIONS: These junior-level residents appear to rely on clinical course, among other factors, when assessing prognosis and the possibility for death in severely ill patients. Further investigation is needed to understand how these factors impact decision-making and whether perceived barriers to changes in patient management influence approaches to care
Balancing equity and efficiency in the Dutch basic benefits package using the principle of proportional shortfall
Economic evaluations are increasingly used to inform decisions regarding the allocation of scarce health care resources. To systematically incorporate societal preferences into these evaluations, quality-adjusted life year gains could be weighted according to some equity principle, the most suitable of which is a matter of frequent debate. While many countries still struggle with equity concerns for priority setting in health care, the Netherlands has reached a broad consensus to use the concept of proportional shortfall. Our study evaluates the concept and its support in the Dutch health care context. We discuss arguments in the Netherlands for using proportional shortfall and difficulties in transitioning from principle to practice. In doing so, we address universal issues leading to a systematic consideration of equity concerns for priority setting in health care. The article thus has relevance to all countries struggling with the formalization of equity concerns for priority setting
Multiple-Locus Variable Number Tandem Repeat Analysis of Staphylococcus Aureus: Comparison with Pulsed-Field Gel Electrophoresis and spa-Typing
(MRSA) is required to study the routes and rates of transmission of this pathogen. Currently available typing techniques are either resource-intensive or have limited discriminatory ability. Multiple-locus variable number tandem repeat analysis (MLVA) may provide an alternative high throughput molecular typing tool with high epidemiological resolution.-sequence typing and PFGE, at the MLVA complex level with group separation values of 95.1% and 89.2%. MLVA could not discriminate between pig-related MRSA strains isolated from humans and pigs, corroborating the high degree of relationship. MLVA was also superior in the grouping of MRSA isolates previously assigned to temporal-spatial clusters with indistinguishable SpaTypes, demonstrating its enhanced epidemiological usefulness. that yields discrete and unambiguous data that can be used to assign biological meaningful genotypes and complexes and can be used for interlaboratory comparisons in network accessible databases. Results suggest that MLVA offsets the disadvantages of other high discriminatory typing approaches and represents a promising tool for hospital, national and international molecular epidemiology
Efficient Genetic Method for Establishing Drosophila Cell Lines Unlocks the Potential to Create Lines of Specific Genotypes
Analysis of cells in culture has made substantial contributions to biological research. The versatility and scale of in vitro manipulation and new applications such as high-throughput gene silencing screens ensure the continued importance of cell-culture studies. In comparison to mammalian systems, Drosophila cell culture is underdeveloped, primarily because there is no general genetic method for deriving new cell lines. Here we found expression of the conserved oncogene RasV12 (a constitutively activated form of Ras) profoundly influences the development of primary cultures derived from embryos. The cultures become confluent in about three weeks and can be passaged with great success. The lines have undergone more than 90 population doublings and therefore constitute continuous cell lines. Most lines are composed of spindle-shaped cells of mesodermal type. We tested the use of the method for deriving Drosophila cell lines of a specific genotype by establishing cultures from embryos in which the warts (wts) tumor suppressor gene was targeted. We successfully created several cell lines and found that these differ from controls because they are primarily polyploid. This phenotype likely reflects the known role for the mammalian wts counterparts in the tetraploidy checkpoint. We conclude that expression of RasV12 is a powerful genetic mechanism to promote proliferation in Drosophila primary culture cells and serves as an efficient means to generate continuous cell lines of a given genotype
A Quantitative Analytical Method to Test for Salt Effects on Giant Unilamellar Vesicles
Today, free-standing membranes, i.e. liposomes and vesicles, are used in a multitude of
applications, e.g. as drug delivery devices and artificial cell models. Because current
laboratory techniques do not allow handling of large sample sizes, systematic and
quantitative studies on the impact of different effectors, e.g. electrolytes, are limited.
In this work, we evaluated the Hofmeister effects of ten alkali metal halides on giant
unilamellar vesicles made of palmitoyloleoylphosphatidylcholine for a large sample size by
combining the highly parallel water-in-oil emulsion transfer vesicle preparation method with
automatic haemocytometry. We found that this new quantitative screening method is highly
reliable and consistent with previously reported results. Thus, this method may provide a
significant methodological advance in analysis of effects on free-standing model
membranes
