    EAGLE ISS - A modular twin-channel integral-field near-IR spectrograph

    The ISS (Integral-field Spectrograph System) has been designed as part of the EAGLE Phase A Instrument Study for the E-ELT. It consists of two input channels of 1.65x1.65 arcsec field-of-view, each reconfigured spatially by an image-slicing integral-field unit to feed a single near-IR spectrograph using cryogenic volume-phase-holographic (VPH) gratings to disperse the image spectrally. A 4k x 4k array detector array records the dispersed images. The optical design employs anamorphic magnification, image slicing, VPH gratings scanned with a novel cryo-mechanism and a three-lens camera.     Antimicrobial Stewardship Strategies Including Point-of-Care Testing (POCT) for Pediatric Patients with Upper-Respiratory-Tract Infections in Primary Care: A Systematic Review of Economic Evaluations

    Upper-respiratory-tract infections (URTIs) are among the main causes of antibiotic prescriptions in pediatric patients. Over one-third of all antibiotic prescriptions for URTIs in children are estimated to be inappropriate, as the majority of URTIs are caused by viral agents. Several strategies, including clinical scoring algorithms and different point-of-care tests (POCTs) have been developed to help discriminate bacterial from viral URTIs in the outpatient clinical setting. A systematic review of the literature was conducted following PRISMA guidelines with the objective of summarizing evidence from health-economic evaluations on the use of POCT for URTIs in pediatric outpatients. A total of 3375 records identified from four databases and other sources were screened, of which 8 met the inclusion criteria. Four studies were classified as being of high reporting quality, and three were of medium quality. Five out of eight studies concluded in favor of strategies that included POCTs, with an additional study finding several POCTs to be cost-effective compared to usual care but over an acceptable WTP threshold. This review found POCT could be a valuable tool for antimicrobial stewardship strategies targeted towards childhood URTIs in primary care

    Shortage of Albendazole and Its Consequences for Patients with Cystic Echinococcosis Treated at a Referral Center in Italy

    Albendazole (ABZ) is the best drug available to treat cystic echinococcosis (CE), a neglected tropical disease. Cystic echinococcosis patients often receive a continuous course of the drug for 6-12 months. In Italy, ABZ shortages occur almost on a yearly basis. We searched clinical records at the World Health Organization Collaborating Center for the Clinical Management of CE in Pavia, Italy, to estimate the amount of ABZ prescribed to patients between January 2012 and February 2017. The cost of ABZ was estimated at €2.25 per tablet based on the current market price in Italy. Patients to whom ABZ had been prescribed were contacted to determine if they had experienced difficulties in purchasing the drug and to assess how such problems affected their treatment. Of 348 identified CE patients, 127 (36.5%) were treated with ABZ for a total of 20,576 days. This led to an estimated cost of €92,592. Seventy-five patients were available for follow-up, 42 (56%) reported difficulties in obtaining ABZ. Of these patients, four (9.5%) had to search out of their region and 10 (23.8%) had to go out of the country. A total of 27 patients (64%) had to visit more than five pharmacies to locate the drug and 10 patients (23.8%) interrupted treatment because of ABZ nonavailability. Shortages in ABZ distribution can disrupt CE treatment schedules and jeopardize patient health

    Recombinant Anti-Human Melanoma Antibodies Are Versatile Molecules

    The low cost, high versatility, and reliable production of bacterially produced recombinant antibody fragments speeds up the development of tumor-targeting agents. High-quality recombinant anti-melanoma antibodies are much sought after in the scientific community. We cloned the murine antibody 225.28S, currently used in radioimmunoimaging of human melanoma lesions, in single-chain Fv configuration (scFv) for soluble expression in bacteria. The recombinant antibody fragment conserved the binding specificity of the parental antibody. In order to arm the scFv(225.28S) with biologically useful effector functions, we developed vectors for soluble expression of scFv(225.28S) in bacteria that allow both covalent and noncovalent chemical antibody modification at positions that do not interfere with antigen binding. An expression vector was developed that appends a cysteine residue at the C-terminal extremity of the recombinant antibody, thus allowing reaction with thiol-specific reagents, including 99mTc labeling, at a position that does not interfere with antigen binding. The scFv(225.28S) was also successfully expressed with a casein kinase II substrate tag that enables efficient and stable 32P labeling. For noncovalent antibody modification, we developed an expression vector that appends the human calmodulin gene at the C-terminal extremity of scFv(225.28S). The calmodulin domain is poorly immunogenic and can be targeted with chemically modified high-affinity calmodulin ligands. The recombinant anti-human melanoma antibodies described in this article should prove useful “building blocks” for the development of anti-melanoma diagnostic and therapeutic strategies

    Spectroscopic survey of the Galaxy with Gaia I. Design and performance of the Radial Velocity Spectrometer

    The definition and optimisation studies for the Gaia satellite spectrograph, the Radial Velocity Spectrometer (RVS), converged in late 2002 with the adoption of the instrument baseline. This paper reviews the characteristics of the selected configuration and presents its expected performance. The RVS is a 2.0 by 1.6 degree integral field spectrograph, dispersing the light of all sources entering its field of view with a resolving power R=11 500 over the wavelength range [848, 874] nm. The RVS will continuously and repeatedly scan the sky during the 5 years of the Gaia mission. On average, each source will be observed 102 times over this period. The RVS will collect the spectra of about 100-150 million stars up to magnitude V~17-18. At the end of the mission, the RVS will provide radial velocities with precisions of ~2 km/s at V=15 and \~15-20 km/s at V=17, for a solar metallicity G5 dwarf. The RVS will also provide rotational velocities, with precisions (at the end of the mission) for late type stars of sigma_vsini ~5 km/s at V~15 as well as atmospheric parameters up to V~14-15. The individual abundances of elements such as Silicon and Magnesium, vital for the understanding of Galactic evolution, will be obtained up to V~12-13. Finally, the presence of the 862.0 nm Diffuse Interstellar Band (DIB) in the RVS wavelength range will make it possible to derive the three dimensional structure of the interstellar reddening.Comment: 17 pages, 9 figures, accepted for publication in MNRAS.     Unlocking the secrets of Al-tobermorite in Roman seawater concrete

    Ancient Roman syntheses of Al-tobermorite in a 2000-year-old concrete block submerged in the Bay of Pozzuoli (Baianus Sinus), near Naples, have unique aluminum-rich and silica-poor compositions relative to hydrothermal geological occurrences. In relict lime clasts, the crystals have calcium contents that are similar to ideal tobermorite, 33 to 35 wt%, but the low-silica contents, 39 to 40 wt%, reflect Al3+ substitution for Si4+ in Q(2)(1Al), Q(3)(1Al), and Q(3)(2Al) tetrahedral chain and branching sites. The Al-tobermorite has a double silicate chain structure with long chain lengths in the b [020] crystallographic direction, and wide interlayer spacing, 11.49 angstrom. Na+ and K+ partially balance Al3+ substitution for Si4+. Poorly crystalline calcium-aluminum-silicate-hydrate (C-A-S-H) cementitious binder in the dissolved perimeter of relict lime clasts has Ca/(Si+Al) = 0.79, nearly identical to the Al-tobermorite, but nanoscale heterogeneities with aluminum in both tetrahedral and octahedral coordination. The concrete is about 45 vol% glassy zeolitic tuff and 55 vol% hydrated lime-volcanic ash mortar; lime formed <10 wt% of the mix. Trace element studies confirm that the pyroclastic rock comes from Flegrean Fields volcanic district, as described in ancient Roman texts. An adiabatic thermal model of the 10 m(2) by 5.7 m thick Baianus Sinus breakwater from heat evolved through hydration of lime and formation of C-A-S-H suggests maximum temperatures of 85 to 97 degrees C. Cooling to seawater temperatures occurred in two years. These elevated temperatures and the mineralizing effects of seawater and alkali- and alumina-rich volcanic ash appear to be critical to Al-tobermorite crystallization. The long-term stability of the Al-tobermorite provides a valuable context to improve future syntheses in innovative concretes with advanced properties using volcanic pozzolans
