    Intrapulmonary Cystic Lymphangioma in a 2-month-old Infant

    Lymphangioma is an abnormal collection of lymphatics that are developmentally isolated from the normal lymphatic system. Lymphangioma rarely presents as a solitary pulmonary lesion. We report a rare case of intrapulmonary cystic lymphangioma involving the upper lobe of the right lung, which presented with dyspnea in a 2-month-old infant. High-resolution computed tomography (HRCT) of the chest demonstrated a well-circumscribed, multiseptate, cystic lesion in the upper lobe of the right lung, mimicking the feature of type I congenital cystic adenomatoid malformation. The tumor was removed by bilobectomy of the upper and middle lobes of the right lung, and its pathologic examination confirmed the diagnosis of an intrapulmonary cystic lymphangioma

    <p>Abstract</p> <p>Background</p> <p>Left main bronchus resection and reconstruction (LMBRR) is a complex surgical procedure indicated for management of inflammatory, benign and low grade malignant lesions. Its application provides maximal parenchymal sparing.</p> <p>Methods</p> <p>Out of 98 bronchoplastic procedures performed at the Authors' Institution in the 1995-2011 period, 4 were LMBRR. Indications were bronchial carcinoid in 2 cases, inflammatory pseudotumor in 1 case, TBC stricture in 1 case. All patients underwent preoperatively a rigid bronchoscopy to restore the airway lumen patency. At surgery a negative resection margin was confirmed by frozen section in the neoplastic patients. In all patients an end-to-end bronchial anastomosis was constructed according to Grillo.</p> <p>Results</p> <p>There were neither mortality nor major complications. Airway lumen was optimal in 3 patients, good in 1.</p> <p>Conclusion</p> <p>LMBRR is a valuable option for the thoracic surgeon. It maximizes the parenchyma-sparing philosophy, broadening the spectrum of potential candidates for cure. It remains a technically demanding procedure, to be carried out by an experienced surgical team. Correct surgical planning affords excellent results, both in the short and long term.</p

    Chance mechanisms affecting the burden of metastases

    BACKGROUND: The burden of cancer metastases within an individual is commonly used to clinically characterize a tumor's biological behavior. Assessments like these implicitly assume that spurious effects can be discounted. Here the influence of chance on the burden of metastasis is studied to determine whether or not this assumption is valid. METHODS: Monte Carlo simulations were performed to estimate tumor burdens sustained by individuals with cancer, based upon empirically derived and validated models for the number and size distributions of metastases. Factors related to the intrinsic metastatic potential of tumors and their host microenvironments were kept constant, to more clearly demonstrate the contribution from chance. RESULTS: Under otherwise identical conditions, both the simulated numbers and the sizes of metastases were highly variable. Comparable individuals could sustain anywhere from no metastases to scores of metastases, and the sizes of the metastases ranged from microscopic to macroscopic. Despite the marked variability in the number and sizes of the metastases, their respective growth times were rather more narrowly distributed. In such situations multiple occult metastases could develop into fully overt lesions within a comparatively short time period. CONCLUSION: Chance can have a major effect on the burden of metastases. Random variability can be so great as to make individual assessments of tumor biology unreliable, yet constrained enough to lead to the apparently simultaneous appearance of multiple overt metastases

    Effects of Adenosine on Histamine Release from Human Lung Fragments

    The actions of adenosine on histamine release of human lung fragments were investigated. Histamine release was stimulated either with the calcium ionophore A 23187 orwith concanavalin A. Adenosine and its analogue 5'-N-ethylcarboxamidoadenosine alone had no significant effect on basal release or on the release elicited by A 23187 or concanavalin A. However, in the presence of the adenosine receptor antagonist 8-[4-[[[[(2-aminoethyl)amino]-carbonyl] methyloxy]-phenyl]-1,3-dipropylaxanthine (XAC), which itself did not affect the release, adenosine increased the stimulated histamine release. On the other hand, in the presence of the nucleoside transport inhibitor S-(p-nitrobenzyl)-6-thioninosine (NBTI), adenosine caused a reduction in stimulated histamine release. NBTI itself caused a stimulation of release. Thus, a stimulatory effect of adenosine was seen in the presence ofXAC, whereas an inhibitory effect was unmasked by NBTI. From these data it is concluded that adenosine exerts two opposing effects on histamine release in the human lung which neutralize each other: it inhibits release via a si te antagonized by XAC, which presumably represents an A2 adenosine receptor, and it stimulates release via a mechanism that is blocked by NBTI, suggesting that adenosine needs to reach the interior of cells to exert this effect. The slight stimulatory effect of NBTI alone demonstrates that trapping intracellularly formed adenosine inside mast cells leads to sufficient concentrations of adenosine to stimulate histamine release. These findings suggest an important bimodal role of adenosine in regulating histamine release in the human lung

    The sacred in the profane: Sinai thinking in German Jewish literature

    How can readers identify a text as being "Jewish" if there is no Jewish language? How can they explain the term "Jewish literature" if there is no Jewish nation and if an author can declare himself Jewish while denying any relationship to Judaism? This study seeks to clarify a Jewish intellectual structure, as demonstrated by Jewish texts from the biblical era presenting themselves in modern texts. This research assumes this structure is the base of contemporary Jewish identity and offers a coherent answer to the question, "What is Jewish literature?"The term "Jewish" is not an epithet, affirms the philosopher Shmuel Trigano by showing in its writings that Judaism constitutes an epistemological order to the same degree, the same nature and the same truth, that Western thinking claims for itself. Trigano affirms that Judaism is neither reducible to religion, nor more particularistic, and is also not more bound to a specific identity than its Greco-Western sibling. Nor does it depend on ethnic origin, but refers to an order of the human thought, a noetic order that nourishes and is nourished up to the present day by its writings.This project puts Trigano's epistemology into practice by describing the role of the adjective "Jewish" in regard to German-language literature. This study describes mental structures of "Sinai thinking" in literary texts. This thought has been developed through a long tradition of hermeneutics, thanks to three fundamental Jewish scriptures: the Torah, the Talmud and the Kabala. The study contends that the literature of Jews written in German, their observance of Jewish law not withstanding, reflects this noetic order. This order will help the reader to understand that Jewish identity is above all a textual identity and that the Jewish text is a hermeneutical mega-structure of the self, a so to say "bibliognostic" ontology.The study analyzes some components of "Sinai-thinking" by means of an etymological study of basic Hebrew words and as well as by a method of Talmudic interpretation that calls upon four levels of reading (Pardes).This dissertation sees itself as a reaction to the intellectuals who pose the existence of a "Jewish literature" without ever defining it.
