    Discussion on complexity and TCAS indicators for coherent safety net transitions

    Transition between Separation Management in ATM and Collision Avoidance constitutes a source of potential risks due to non-coherent detection and resolution clearances between them. To explore an operational integration between these two safety nets, a complexity metric tailored for both Separation Management and Collision Avoidance, based on the intrinsic complexity, is proposed. To establish the framework to compare the complexity metric with current Collision Avoidance detection metrics, a basic pair-wise encounter model has been considered. Then, main indicators for horizontal detection of TCAS, i.e. tau and taumod, have been contrasted with the complexity metric. A simple method for determining the range locus for specific TCAS tau values, depending on relative speeds and encounter angles, was defined. In addition, range values when detection thresholds were infringed have been found to be similar, as well as its sensitivity to relative angles. Further work should be conducted for establishing a framework for the evaluation and validation of this complexity metric.     Cartilage adaptation after anterior cruciate ligament injury and reconstruction: implications for clinical management and research? A systematic review of longitudinal MRI studies

    SummaryObjectiveTo summarize the current evidence of magnetic resonance imaging (MRI)-measured cartilage adaptations following anterior cruciate ligament (ACL) reconstruction and of the potential factors that might influence these changes, including the effect of treatment on the course of cartilage change (i.e., surgical vs non-surgical treatment).MethodsA literature search was conducted in seven electronic databases extracting 12 full-text articles. These articles reported on in vivo MRI-related cartilage longitudinal follow-up after ACL injury and reconstruction in “young” adults. Eligibility and methodological quality was rated by two independent reviewers. A best-evidence synthesis was performed for reported factors influencing cartilage changes.ResultsMethodological quality was heterogenous amongst articles (i.e., score range: 31.6–78.9%). Macroscopic changes were detectable as from 2 years follow-up next to or preceded by ultra-structural and functional (i.e., contact-deformation) changes, both in the lateral and medial compartment. Moderate-to-strong evidence was presented for meniscal lesion or meniscectomy, presence of bone marrow lesions (BMLs), time from injury, and persisting altered biomechanics, possibly affecting cartilage change after ACL reconstruction. First-year morphological change was more aggravated in ACL reconstruction compared to non-surgical treatment.ConclusionIn view of osteoarthritis (OA) prevention after ACL reconstruction, careful attention should be paid to the rehabilitation process and to the decision on when to allow return to sports. These decisions should also consider cartilage fragility and functional adaptations after surgery. In this respect, the first years following surgery are of paramount importance for prevention or treatment strategies that aim at impediment of further matrix deterioration.     What is gender awareness in health? A scoping review of the concept, its operationalization, and its relation to health outcomes

    Gender awareness emerged in the 1990s and aimed to provide awareness and sympathy toward the needs of women, measuring health-care providers’ attitudes toward them and understand if providers possessed the knowledge for appropriate care. According to Miller et al.’s seminal model, gender awareness incorporates three sub-dimensions: gender sensitivity, gender ideology, and knowledge. Gender awareness has the potential to minimize gender bias in health care, improving the ecological validity of research. This scoping review provides an analysis of how gender awareness has been conceptualized, operationalized, and investigated in its relationship with health-related outcomes. A search was conducted on PubMed, PsycINFO, and ERIC. The relevance of 2.589 articles was assessed and 14 empirical studies were selected and included. Difficulties conceptualizing gender awareness were found and gender awareness and gender sensitivity were often presented as interchangeable. Most papers aimed to measure and compare levels of gender awareness among health professionals and the relationship between gender awareness and relevant health-related outcomes was not studied. Drawing upon a critical analysis of our findings, a proposal for a revised gender awareness conceptualization and operationalization is put forth as to inform novel research on its association with gender bias in health and health care.info:eu-repo/semantics/acceptedVersio

    The early identification of risk factors on the pathway to school dropout in the SIODO study:A sequential mixed-methods study

    BACKGROUND: School dropout is a persisting problem with major socioeconomic consequences. Although poor health probably contributes to pathways leading to school dropout and health is likely negatively affected by dropout, these issues are relatively absent on the public health agenda. This emphasises the importance of integrative research aimed at identifying children at risk for school dropout at an early stage, discovering how socioeconomic status and gender affect health-related pathways that lead to dropout and developing a prevention tool that can be used in public health services for youth. METHODS/DESIGN: The SIODO study is a sequential mixed-methods study. A case–control study will be conducted among 18 to 24 year olds in the south of the Netherlands (n = 580). Data are currently being collected from compulsory education departments at municipalities (dropout data), regional public health services (developmental data from birth onwards) and an additional questionnaire has been sent to participants (e.g. personality data). Advanced analyses, including cluster and factor analyses, will be used to identify children at risk at an early stage. Using the quantitative data, we have planned individual interviews with participants and focus groups with important stakeholders such as parents, teachers and public health professionals. A thematic content analysis will be used to analyse the qualitative data. DISCUSSION: The SIODO study will use a life-course perspective, the ICF-CY model to group the determinants and a mixed-methods design. In this respect, the SIODO study is innovative because it both broadens and deepens the study of health-related determinants of school dropout. It examines how these determinants contribute to socioeconomic and gender differences in health and contributes to the development of a tool that can be used in public health practice to tackle the problem of school dropout at its roots

    Long-Term Use of Aldosterone-Receptor Antagonists in Uncontrolled Hypertension: A Retrospective Analysis

    Background. The long-term efficacy of aldosterone-receptor antagonists (ARAs) as add-on treatment in uncontrolled hypertension has not yet been reported. Methods. Data from 123 patients (21 with primary aldosteronism, 102 with essential hypertension) with difficult-to-treat hypertension who received an ARA between May 2005 and September 2009 were analyzed retrospectively for their blood pressure (BP) and biochemical response at first followup after start with ARA and the last follow-up available. Results. Systolic BP decreased by 22 ± 20 and diastolic BP by 9.4 ± 12 mmHg after a median treatment duration of 25 months. In patients that received treatment >5 years, SBP was 33 ± 20 and DBP was 16 ± 13 mmHg lower than at baseline. Multivariate analysis revealed that baseline BP and follow-up duration were positively correlated with BP response. Conclusion. Add-on ARA treatment in difficult-to-treat hypertension results in a profound and sustained BP reduction

    Application of inulin-type fructans in animal feed and pet food

    The inulin-type fructans are non-digestible oligosaccharides that are fermented in the gastrointestinal tract of farm animals and pets. This review focuses on the various effects of inulin-type fructans in pigs, poultry, calves and companion animals. Effects of the inulin-type fructans on gut microflora, digestion and availability of nutrients, gut morphology, fermentation characteristics and animal performance are discussed. Inulin-type fructans can support animal performance and health by affecting nutrient digestion, gut microflora and gut morphology, although results vary depending on composition of the basal diet, inclusion level, type of fructan, adaptation period and experimental hygienic conditions

    How caring work of older women gets disappeared: the gendered dynamics of changing everyday occupations in an older German couple

    Background: In older couples, everyday occupations need to be adjusted when one of the partners experiences a health decline. Gender dynamics will play a role in this process, yet there is little understanding about how gender can influence and shape changes in couples’ occupations. Aim: To understand the changing occupations of old, independent-living couples when becoming caregivers and care receivers, through a gender lens. Methods: A narrative-in-action methodology with a case-study design has been chosen to enable an in-depth analysis of one couple. Besides joint and individual interviews with both partners, observations were made of their everyday occupations. The analytic process was interpretative, using gender theory as a lens for understanding. Results: In the couple’s narrative there was a difference in the time spent on, and meaning given to, occupations performed by each partner to sustain everyday life. The wife was heavily enrolled in taking care of her husband, a gendered pattern that was rooted in their spousal history. While her efforts were taken-for-granted, his efforts were acknowledged as special. Conclusion: The adjustment of the everyday occupations of this older couple were gendered, and led to a disappearing of the woman’s occupations and the care she was giving.     Natural language processing for aviation safety: Extracting knowledge from publicly-available loss of separation reports

    Background: The air traffic management (ATM) system has historically coped with a global increase in traffic demand ultimately leading to increased operational complexity. When dealing with the impact of this increasing complexity on system safety it is crucial to automatically analyse the losses of separation (LoSs) using tools able to extract meaningful and actionable information from safety reports. Current research in this field mainly exploits natural language processing (NLP) to categorise the reports,with the limitations that the considered categories need to be manually annotated by experts and that general taxonomies are seldom exploited. Methods: To address the current gaps,authors propose to perform exploratory data analysis on safety reports combining state-of-the-art techniques like topic modelling and clustering and then to develop an algorithm able to extract the Toolkit for ATM Occurrence Investigation (TOKAI) taxonomy factors from the free-text safety reports based on syntactic analysis. TOKAI is a tool for investigation developed by EUROCONTROL and its taxonomy is intended to become a standard and harmonised approach to future investigations. Results: Leveraging on the LoS events reported in the public databases of the Comisión de Estudio y Análisis de Notificaciones de Incidentes de Tránsito Aéreo and the United Kingdom Airprox Board,authors show how their proposal is able to automatically extract meaningful and actionable information from safety reports,other than to classify their content according to the TOKAI taxonomy. The quality of the approach is also indirectly validated by checking the connection between the identified factors and the main contributor of the incidents. Conclusions: Authors' results are a promising first step toward the full automation of a general analysis of LoS reports supported by results on real-world data coming from two different sources. In the future,authors' proposal could be extended to other taxonomies or tailored to identify factors to be included in the safety taxonomies
