Improving Care of Assisted Living Residents with Dementia Through a Competency-Based Behavioral Training Curriculum for Assisted Living Caregivers

    Get PDF
Behavioral and psychological symptoms of dementia (BPSD) are common among residents of assisted living facilities. Inadequately managed symptoms of BPSD can lead to problematic behaviors with significant consequences for patients and caregivers. Non-pharmacologic approaches to the management of problematic behaviors in individuals with dementia are the preferred management option. Caregiver training and education to enhance knowledge and skills in managing behavioral and psychological symptoms for assisted living caregivers is indicated to provide optimum care for patients with dementia. The purpose of this doctor of nursing practice (DNP) project was to develop and implement a caregiver-training workshop utilizing a competency-based behavioral training curriculum to improve the care of dementia residents and ultimately enhance the physical and mental well-being of both the residents and caregivers. Fifteen assisted living caregivers attended a two-hour training workshop, individualized onsite monitoring, and follow-up. Overall implementation of this quality improvement project resulted in a high level of satisfaction among participating staff. Upon completion of the training workshop, a considerable increase was evident in self-perceived knowledge of dementia and related behaviors (87%), behavior management skills (94%), inter-professional communication skills (73%), and comfort level in managing behavior problems (74%), relative to self-reported pre-training levels. In contrast, an overall reduction appeared in perceived competencies regarding the same four components six weeks after training. These results suggest that this may be an effective model for a training intervention for assisted living caregivers of dementia residents with behavioral symptoms. Additional follow-up concept review and staff support may be required to sustain the changed practice behavior and maintain the benefits of the training

© 2018 Elsevier B.V. The aim of this work is exploration of kinetic, thermodynamic and mechanistic aspects of triethylamine-mediated selenocyclofunctionalization of 4-pentenoic acid by means of phenylselenyl halides (PhSeCl and PhSeBr). The kinetics and mechanism for these reactions have been investigated by the UV–Vis spectrophotometry. The rate constants, as well as thermodynamic parameters (ΔH≠, ΔS≠) have been determined using UV–Vis method at three different temperatures (288, 298 and 308 K) in THF as a solvent. All studied reactions gave negative values for entropy of activation, which is in agreement with the SN2 mechanism of substitution. In addition, obtained values for the rate constants have indicated the higher reactivity of PhSeCl as reagent for cyclization

© 2019, Springer-Verlag GmbH Germany, part of Springer Nature. The reaction of the Δ4-alkenols with PhSeX can follow three possible reaction pathways: two pathways lead to the formation of two regioisomeric cyclic ether products through the process of intramolecular cyclization, while the third represents the addition of the reagent to the double bond of an alkenol. As there are relatively few literature data on the kinetics of these reactions, we have chosen 6-methyl-hept-5-en-2-ol as a substrate of interest in order to obtain valuable results that will enable better understanding of the mechanism of phenylselenoetherification reactions. 6-Methyl-hept-5-en-2-ol is a particularly interesting model-substrate due to its substitution pattern of functional groups involved in the cyclization process. In this research, through synthetic and kinetic studies, we aimed to resolve key questions concerning the influence on kinetics, chemo- and regioselectivity of the reagent’s counter ion, steric hindrances in substrate functional groups and the presence of additives

The Effects of Trade, Aid, and Investment on China's Image in Developing Countries

One goal of China’s Go Out policy is to create goodwill in countries around the world. At the same time, China’s growing economic engagement has provoked much criticism. This paper is the first to study whether these activities change the attitudes of individuals in developing countries towards China at both the national and subnational level. Using repeated cross-sectional survey data from the Latinobarómetro, we analyze whether and how growing amounts of exports, foreign aid, and foreign direct investment from China to Latin America affect opinions on China within 18 Latin American countries over the 2002-2013 period. We run instrumental-variables regressions by exploiting exogenous variation in the supply of Chinese exports, aid, and investment proxied by China’s market penetration of developing countries outside Latin America. In contrast to the widespread criticism, we do not find evidence that China’s growing economic activities in the respective countries deteriorate average attitudes towards China — neither at the national nor the provincial level. However, our results show that the young, educated, and economically privileged population develops more positive views of China. We interpret this as evidence that China’s economic engagement creates winners and losers

Abstract In order to determine the species of entomopathogenic nematodes in Continental Portugal, a survey was conducted in the country. Nematodes were recovered from soil samples collected from Alentejo, Algarve, center and the north of Portugal from 2006-2009. Isolates were identified based on morphology and sequence analysis. Phylogenetic analysis was based on sequences of partial 28S (D2D3), internal transcribed spacer (ITS) and cytochrome oxidase c subunit I gene (COX I). Isolates 59F, 15G, 20F and 2B were characterized in detail. Isolate 2B shows a morphology identical to Steinernema intermedium which is a member of the affine/intermedium-group and is characterized by the presence of strongly curved and robust spicules with a distinct rostrum, in the male, and dorsal tail depression in third-stage infective junveniles (IJ). However, the phylogenies based on the three molecular markers revealed that isolate 2B is more closely related to S. affine than with S. intermedium. The morphological results for isolate 20F identify it as S. kraussei which is a member of kraussei/feltiae–group, characterized by IJs with a straight body of medium length (mean = 700–950 μm), lateral field mostly with eight ridges, rather broad, flatly rounded and continuous cephalic region, excretory pore at level of mid-pharynx; males with mucronate tail, yellowish spicules ca. 50 μm long and wide manubria; females with short conoid tail with pointed non-mucronate tip. Based on morphology and sequence analysis, isolates 59F and 15G were considered conspecific and identified as a species belonging to the glaseri–group. More detailed studies are necessary to solve if these isolates represent a new species

Structure of the full-length TRPV2 channel by cryo-EM.

Transient receptor potential (TRP) proteins form a superfamily Ca(2+)-permeable cation channels regulated by a range of chemical and physical stimuli. Structural analysis of a 'minimal' TRP vanilloid subtype 1 (TRPV1) elucidated a mechanism of channel activation by agonists through changes in its outer pore region. Though homologous to TRPV1, other TRPV channels (TRPV2-6) are insensitive to TRPV1 activators including heat and vanilloids. To further understand the structural basis of TRPV channel function, we determined the structure of full-length TRPV2 at ∼5 Å resolution by cryo-electron microscopy. Like TRPV1, TRPV2 contains two constrictions, one each in the pore-forming upper and lower gates. The agonist-free full-length TRPV2 has wider upper and lower gates compared with closed and agonist-activated TRPV1. We propose these newly revealed TRPV2 structural features contribute to diversity of TRPV channels

A local characterisation for static charged black holes

We obtain a purely local characterisation that singles out the Majumdar-Papapetrou class, the near-horizon Bertotti-Robinson geometry and the Reissner-Nordstr\"om exterior solution, together with its plane and hyperbolic counterparts, among the static electrovacuum spacetimes. These five classes are found to form the whole set of static Einstein-Maxwell fields without sources and conformally flat space of orbits, this is, the conformastat electrovacuum spacetimes. The main part of the proof consists in showing that a functional relationship between the gravitational and electromagnetic potentials must always exist. The classification procedure provides also an improved characterisation of Majumdar-Papapetrou, by only requiring a conformally flat space of orbits with a vanishing Ricci scalar of the usual conveniently rescaled 3-metric. A simple global consideration allows us to state that the asymptotically flat subset of the Majumdar-Papapetrou class and the Reissner-Nordstr\"om exterior solution are the only asymptotically flat conformastat electrovacuum spacetimes.Comment: LaTeX; 31 pages. Get PDF
The pH along the midgut of M. funereus larvae had different values, being acidic in the anterior section and basic in the middle and posterior sections. Elastase- and chymotrypsin-like activities were highest in the middle, low in the anterior, and negligible in the posterior section of the midgut. Trypsin-like activities were detected along the whole midgut, with more than 90% of activity in the anterior section. The level of elastase- and chymotrypsin-like activity was very low compared to trypsin-like activity. In the anterior section of the midgut, two isoforms of trypsin-like enzymes were found, both being basic and almost completely inhibited by benzamidine
