Clinical Presentation, Cholangiographic Features, Natural History, and Outcome: A Series of 16 Cases
Secondary sclerosing cholangitis in critically ill patients (SSC-CIP) is an
important differential diagnosis in patients presenting with cholestasis and
PSC-like cholangiographic changes in endoscopic retrograde cholangiography
(ERC). As a relatively newly described entity, SSC-CIP is still
underdiagnosed, and the diagnosis is often delayed. The present study aims to
improve the early detection of SSC-CIP and the identification of its
complications. A total of 2633 records of patients who underwent or were
listed for orthotopic liver transplantation at the University Hospital
Charité, Berlin, were analyzed retrospectively. The clinical presentation and
outcome (mean follow-up 62.7 months) of the 16 identified SSC-CIP cases were
reviewed. Cholestasis was the first sign of SSC-CIP. GGT was the predominant
enzyme of cholestasis. Hypercholesterolemia occurred in at least 75% of the
patients. SSC-CIP provoked a profound weight loss (mean 18 kg) in 94% of our
patients. SSC-CIP was diagnosed by ERC in all patients. The 3 different
cholangiographic features detected correspond roughly to the following stages:
(I) evidence of biliary casts, (II) progressive destruction of intrahepatic
bile ducts, and (III) picture of pruned tree. Biliary cast formation is a
hallmark of SSC-CIP and was seen in 87% of our cases. In 75% of the patients,
the clinical course was complicated by cholangiosepsis, cholangitic liver
abscesses, acalculous cholecystitis, or gallbladder perforation. SSC-CIP was
associated with worse prognosis; transplant-free survival was ∼40 months
(mean). Because of its high rate of serious complications and unfavorable
prognosis, it is imperative to diagnose SSC-CIP early and to differentiate
SSC-CIP from other types of sclerosing cholangitis. Specific characteristics
enable identification of SSC-CIP. Early cooperation with a transplant center
and special attention to biliary complications are required after diagnosis of
SSC-CIP
An outbreak of carbapenem-resistant OXA-48 – producing Klebsiella pneumonia associated to duodenoscopy
Background Carbapenemase-producing Enterobacteriaceae (CPE) have become a
major problem for healthcare systems worldwide. While the first reports from
European hospitals described the introduction of CPE from endemic countries,
there is now a growing number of reports describing outbreaks of CPE in
European hospitals. Here we report an outbreak of Carbapenem-resistant K.
pneumoniae in a German University hospital which was in part associated to
duodenoscopy. Findings Between December 6, 2012 and January 10, 2013,
carbapenem-resistant K. pneumoniae (CRKP) was cultured from 12 patients
staying on 4 different wards. The amplification of carbapenemase genes by
multiplex PCR showed presence of the bla OXA-48 gene. Molecular typing
confirmed the identity of all 12 isolates. Reviewing the medical records of
CRKP cases revealed that there was a spatial relationship between 6 of the
cases which were located on the same wards. The remaining 6 cases were all
related to endoscopic retrograde cholangiopancreatography (ERCP) which was
performed with the same duodenoscope. The outbreak ended after the endoscope
was sent to the manufacturer for maintenance. Conclusions Though the outbreak
strain was also disseminated to patients who did not undergo ERCP and
environmental sources or medical personnel also contributed to the outbreak,
the gut of colonized patients is the main source for CPE. Therefore, accurate
and stringent reprocessing of endoscopic instruments is extremely important,
which is especially true for more complex instruments like the duodenoscope
(TJF Q180V series) involved in the outbreak described here
Expression Analysis of Fibronectin Type III Domain-Containing (FNDC) Genes in Inflammatory Bowel Disease and Colorectal Cancer
Background. Fibronectin type III domain-containing (FNDC) proteins fulfill manifold functions in tissue development and regulation of cellular metabolism. FNDC4 was described as anti-inflammatory factor, upregulated in inflammatory bowel disease (IBD). FNDC signaling includes direct cell-cell interaction as well as release of bioactive peptides, like shown for FNDC4 or FNDC5. The G-protein-coupled receptor 116 (GPR116) was found as a putative FNDC4 receptor. We here aim to comprehensively analyze the mRNA expression of FNDC1, FNDC3A, FNDC3B, FNDC4, FNDC5, and GPR116 in nonaffected and affected mucosal samples of patients with IBD or colorectal cancer (CRC). Methods. Mucosa samples were obtained from 30 patients undergoing diagnostic colonoscopy or from surgical resection of IBD or CRC. Gene expression was determined by quantitative real-time PCR. In addition, FNDC expression data from publicly available Gene Expression Omnibus (GEO) data sets (GDS4296, GDS4515, and GDS5232) were analyzed. Results. Basal mucosal expression revealed higher expression of FNDC3A and FNDC5 in the ileum compared to colonic segments. FNDC1 and FNDC4 were significantly upregulated in IBD. None of the investigated FNDCs was differentially expressed in CRC, just FNDC3A trended to be upregulated. The GEO data set analysis revealed significantly downregulated FNDC4 and upregulated GPR116 in microsatellite unstable (MSI) CRCs. The expression of FNDCs and GPR116 was independent of age and sex. Conclusions. FNDC1 and FNDC4 may play a relevant role in the pathobiology of IBD, but none of the investigated FNDCs is regulated in CRC. GPR116 may be upregulated in advanced or MSI CRC. Further studies should validate the altered FNDC expression results on protein levels and examine the corresponding functional consequences
Effizienz der Ligaturtherapie in der Behandlung der akuten Ösophagusvarianzblutung
Die bisherigen Erfahrungen mit der Ligaturtechnik in der Behandlung der aktiven Ösophagusvarianzblutung sind gering. In den Studien der letzten sieben Jahre wurden insgesamt lediglich 133 Behandlungsfälle publiziert. In der vorgelegten Studie wurde unter Notfallbedingungen in einem unselektionierten Krankengut von 54 Patienten durch die Anwendung der Ligatur eine Hämostaserate von 74,1 % erzielt. Damit wird die hohe Effizienz der Ligaturtechnik als Notfallmethode bestätigt. Das Ergebnis liegt im unteren Bereich der bisher veröffentlichten Hömostaseraten. Dies ist durch die ungünstige Zusammensetzung des Patientenkollektives mit einem Anteil von 57,4% Child C Patienten zu erklären. Auch die bisher vorgelegten Veröffentlichungen führen zu besseren Ergebnissen, die aber nicht der Ligaturtherapie zuzuschreiben sind. Bei 14 patienten oder 25,9% der Fälle war die Ligatur als primäre Hämostasetherapie nicht erfolgreich. Das Scheitern der Ligatur unter Notfallbedingungen kann auf zwei Ursachen reduziert werden: Zum einen führt das durch den Ligaturaufsatz stark eingeschränkte endospkopische Gesichtsfeld zur mangelhaften Lokalisation der Blutungsquelle, so dass eine Therapie nicht möglich ist, zum anderen kann das Anbringen einer Ligatur zur Hämostase an der rigiden Wandstarre des Ösophagus scheitern, insbesondere da bei diesen Patienten meistens eine umfangreiche Sklerotherapie vorangegangen war. Die im Vergleich zu anderen Behandlungsmethoden niedrige Rezidivblutungsrate in der Akutphase konnte auch in dieser Studie bestätigt werden. Es fanden sich bis zu 10% frühe Rezidivblutungen. Die Ligaturtechnik ist bisher im Notfall schwierig anzuwenden, da das endoskopische Operationsfeld durch das Instrumentarium und die Blutmassen eingeengt wird. Durch Verkürzung der Ligaturaufsätze in Kombination mit einem während der Untersuchung ein- und ausfahrbaren Abzugszylinder wird das endoskopisch nutzbare Blickfeld ist verdreifacht. Neue Notfallligaturzylinder in Kombination mit anderen technischen Verbesserungen des Ligaturinstrumentariums, wie sie z.B. die Mehrfachligatursysteme darstellen,, lassen eine signifikante Verbesserung der Hämostaserate erwarten. // Für die Behandlung der aktiven Varianzblutung stehen eine Vielzahl von Behandlungsmethoden von annähernd gleicher Effizienz zur Verfügung. Falls vorhanden, sollte der Ligaturtechnik aus folgenden Gründen der Vorzug gegeben werden: * sie hat als lokale Therapie deutlich weniger Komplikationen als zum Beispiel die Sklerotherapie ; * insbesondere bei Patienten mit defekter Gerinnung ist sie wegen der geringen Gewebstraumatisierung von Vorteil ; * nach Notfallligatur kann bereits sehr früh (nach 24 Stunden) weiter behandelt werden, dadurch können frühe Rezidivblutungen reduziert werden. Nach Sklerotherapie sollte im Allgemeinen eine Therapiepause von einer Woche eingelegt werden, um ulzerative Komplikationen zu vermeiden. ; * durch das frühe Einleiten der weiteren Eradikationstherapie mittels Ligatur wir der Krankenhausaufenthalt verkürzt, da weniger Therapiesitzungen erforderlich sind, um eine Varizenradikation zu erreichen. // Wegen der hohen Effizienz in Kombination mit niedrigen Komplikations- und Rezidivblutungsraten, kann die Ligaturtechnik als Methode der Wahl auch bei der Behandlung von blutenden Ösophagusvarizen gelten. Sie versagt bei umfangreich mit der Sklerotherapie vorbehandelten Patienten
