The adaptive optics lucky imager (AOLI): presentation, commissioning, and AIV innovations

Here we present the Adaptive Optics Lucky Imager (AOLI), a state-of-the-art instrument which makes use of two well proved techniques, Lucky Imaging (LI) and Adaptive Optics (AO), to deliver diffraction limited imaging at visible wavelengths, 20 mas, from ground-based telescopes. Thanks to its revolutionary TP3-WFS, AOLI shall have the capability of using faint reference stars. In the extremely-big telescopes era, the combination of techniques and the development of new WFS systems seems the clue key for success. We give details of the integration and verification phases explaining the defiance that we have faced and the innovative and versatile solutions for each of its subsystems that we have developed, providing also very fresh results after its first fully-working observing run at the William Herschel Telescope (WHT).Comment: 6 pages, 6 figures, conference. arXiv admin note: text overlap with arXiv:1703.09354, arXiv:1608.0480

Among the variables associated with occupational stress, two of the most studied are the adverse perceptions of psychosocial risk factors in the workplace and burnout. With the rise of positive psychology, other variables of the individual type, such as hardy personality have also been the subject of study. No studies have been found that jointly examine these variables related to mental health in police officers. The aim of this research was to analyze which variables were associated with mental health in police officers. A total of 223 policemen (202 men and 21 women) participated in a cross-sectional study. Of all the variables, emotional exhaustion and perception of problems as challenges were the only factors introduced in the regression model. These factors must be considered to improve both human resource interventions and occupational health practices in this professional group

Introduction: Phenomena such as fetal monitoring and specific T cell repertoire against the allergen Poa p9 have been evaluated through the Zipf/Mandelbrot law. Objective: To establish a methodology for the characterization of the dynamics of dengue in Palmira, Valle del Cauca, Colombia, based on the Zipf/Mandelbrot law during the period 2001-2004.Materials and methods: Using the Zipf-Mandelbrot law, changes by age groups (1 year, 1-4, 5-14, 15-44, 45-59 and 60) were evaluated, as well as the complexity of the system versus the number of patients infected with dengue in Palmira, each year between 2001 and 2004. Results were compared to establish variations in the complexity, and simulations of possible behaviors that might arise were performed. Results: Fractal dimensions for each year were among 0.5329 and 0.8703, and for age groups between 0.4694 and 0.6689.The simulations of the dynamics per year presented fractal dimensions between 0.4512 and 0.6316, and age ranges between 0.455 and 0.6095.Conclusion: The dynamic of dengue onset in Palmira is caused by a statistical fractal behavior with finite and limited degrees of complexity, useful in decision-making in public health

Five Patients with Disorders of Calcium Metabolism Presented with GCM2 Gene Variants

The GCM2 gene encodes a transcription factor predominantly expressed in parathyroid cells that is known to be critical for development, proliferation and maintenance of the parathyroid cells. A cohort of 127 Spanish patients with a disorder of calcium metabolism were screened for mutations by Next-Generation Sequencing (NGS). A targeted panel for disorders of calcium and phosphorus metabolism was designed to include 65 genes associated with these disorders. We observed two variants of uncertain significance (p.(Ser487Phe) and p.Asn315Asp), one likely pathogenic (p.Val382Met) and one benign variant (p.Ala393_Gln395dup) in the GCM2 gene in the heterozygous state in five families (two index cases had hypocalcemia and hypoparathyroidism, respectively, and three index cases had primary hyperparathyroidism). Our study shows the utility of NGS in unravelling the genetic origin of some disorders of the calcium and phosphorus metabolism, and confirms the GCM2 gene as an important element for the maintenance of calcium homeostasis. Importantly, a novel variant in the GCM2 gene (p.(Ser487Phe)) has been found in a patient with hypocalcemia. This study was supported by three grants from the Department of Health (2017111014, 2018111097 and 2019111052) and one grant from the Department of Education (IT1281-19) of the Basque Government. Identification of the Immunological Changes Appearing in the CSF During the Early Immunosenescence Process Occurring in Multiple Sclerosis

Patients with multiple sclerosis (MS) suffer with age an early immunosenescence process, which influence the treatment response and increase the risk of infections. We explored whether lipid-specific oligoclonal IgM bands (LS-OCMB) associated with highly inflammatory MS modify the immunological profile induced by age in MS. This cross-sectional study included 263 MS patients who were classified according to the presence (M+, n=72) and absence (M-, n=191) of LS-OCMB. CSF cellular subsets and molecules implicated in immunosenescence were explored. In M- patients, aging induced remarkable decreases in absolute CSF counts of CD4+ and CD8+ T lymphocytes, including Th1 and Th17 cells, and of B cells, including those secreting TNF-alpha. It also increased serum anti-CMV IgG antibody titers (indicative of immunosenescence) and CSF CHI3L1 levels (related to astrocyte activation). In contrast, M+ patients showed an age-associated increase of TIM-3 (a biomarker of T cell exhaustion) and increased values of CHI3L1, independently of age. Finally, in both groups, age induced an increase in CSF levels of PD-L1 (an inductor of T cell tolerance) and activin A (part of the senescence-associated secretome and related to inflammaging). These changes were independent of the disease duration. Finally, this resulted in augmented disability. In summary, all MS patients experience with age a modest induction of T-cell tolerance and an activation of the innate immunity, resulting in increased disability. Additionally, M- patients show clear decreases in CSF lymphocyte numbers, which could increase the risk of infections. Thus, age and immunological status are important for tailoring effective therapies in MS.This work was supported by grants FIS-PI15/00513, FIS-PI18/00572 and RD16/0015/0001 from the Instituto de Salud Carlos III. [Background and purpose] Prognosis of myasthenia gravis (MG) in patients with thymoma is not well established. Moreover, it is not clear whether thymoma recurrence or unresectable lesions entail a worse prognosis of MG.[Methods] This multicenter study was based on data from a Spanish neurologist-driven MG registry. All patients were aged >18 years at onset and had anti-acetylcholine receptor antibodies. We compared the clinical data of thymomatous and nonthymomatous patients. Prognosis of patients with recurrent or nonresectable thymomas was assessed.[Results] We included 964 patients from 15 hospitals; 148 (15.4%) had thymoma-associated MG. Median follow-up time was 4.6 years. At onset, thymoma-associated MG patients were younger (52.0 vs. 60.4 years, p < 0.001), had more generalized symptoms (odds ratio [OR]: 3.02, 95% confidence interval [CI]: 1.95–4.68, p < 0.001) and more severe clinical forms according to the Myasthenia Gravis Foundation of America (MGFA) scale (OR: 1.6, 95% CI: 1.15–2.21, p = 0.005). Disease severity based on MGFA postintervention status (MGFA-PIS) was higher in thymomatous patients at 1 year, 5 years, and the end of follow-up. Treatment refractoriness and mortality were also higher (OR: 2.28, 95% CI: 1.43–3.63, p = 0.001; hazard ratio: 2.46, 95% CI: 1.47–4.14, p = 0.001). Myasthenic symptoms worsened in 13 of 27 patients with recurrences, but differences in long-term severity were not significant. Fifteen thymomatous patients had nonresectable thymomas with worse MGFA-PIS and higher mortality at the end of follow-up.[Conclusions] Thymoma-associated MG patients had more severe myasthenic symptoms and worse prognosis. Thymoma recurrence was frequently associated with transient worsening of MG, but long-term prognosis did not differ from nonrecurrent thymoma. Patients with nonresectable thymoma tended to present severe forms of MG.This work is supported by Fondo de Investigaciones Sanitarias (FIS) grant FIS19/01774, Instituto de Salud Carlos III and cofunded by the European Union (ERDF/ESF, A Way to Make Europe/Investing in Your Future).
