Sacral Neuromodulation: Foray into Chronic Pelvic Pain in End Stage Endometriosis

The infiltrating nature of the disease as well as extensive surgery may, however, cause chronic pain that cannot be relieved by either surgery or hormonal treatment. As a pilot treatment, we tested the effect of sacral neuromodulation (SNM) for four endometriosis patients suffering chronic pelvic pain and pelvic organ dysfunction after radical surgical treatment. Three out of four patients reported improvement in their symptoms during the neuromodulation testing period and a permanent pulse generator was installed. After 2.5 years, all three patients report better quality of life and want to continue with SNM
Biomolecular profiles of Arctic sea-ice diatoms highlight the role of under-ice light in cellular energy allocation.
Arctic sea-ice diatoms fuel polar marine food webs as they emerge from winter darkness into spring. Through their photosynthetic activity they manufacture the nutrients and energy that underpin secondary production. Sea-ice diatom abundance and biomolecular composition vary in space and time. With climate change causing short-term extremes and long-term shifts in environmental conditions, understanding how and in what way diatoms adjust biomolecular stores with environmental perturbation is important to gain insight into future ecosystem energy production and nutrient transfer. Using synchrotron-based Fourier transform infrared microspectroscopy, we examined the biomolecular composition of five dominant sea-ice diatom taxa from landfast ice communities covering a range of under-ice light conditions during spring, in Svalbard, Norway. In all five taxa, we saw a doubling of lipid and fatty acid content when light transmitted to the ice-water interface was >5% but <15% (85%-95% attenuation through snow and ice). We determined a threshold around 15% light transmittance after which biomolecular synthesis plateaued, likely because of photoinhibitory effects, except for Navicula spp., which continued to accumulate lipids. Increasing under-ice light availability led to increased energy allocation towards carbohydrates, but this was secondary to lipid synthesis, whereas protein content remained stable. It is predicted that under-ice light availability will change in the Arctic, increasing because of sea-ice thinning and potentially decreasing with higher snowfall. Our findings show that the nutritional content of sea-ice diatoms is taxon-specific and linked to these changes, highlighting potential implications for future energy and nutrient supply for the polar marine food web
Studies on the Diversity and Species Composition of Odonata (Arthropoda: Insecta) from Chandgad Taluk of Kolhapur District, Maharashtra, India
In present study, a total of 66 odonate species belonging to 41 genera representing 10 families were recorded from Chandgad taluka of Kolhapur district in Northern Western Ghats. Dragonflies (Anisoptera) with 38 species (58%) were dominating the study area followed by 28 species (42%) of Damselflies (Zygoptera). Among the genera, Orthertum were in maximum number (6) of species followed by Pseudagrin were of 5 species. Libellulidae family members are recorded in the highest number (30) followed by Coenagrionidae (15). Among the study sites highest numbers of odonates were recorded from slow running stream Kitwade with 33 species and weedy pond Tawarewadi with 32 species. The fast-running stream Tilari represents the least number (3) of species. The slow running stream and weedy pond habitat support the odonate diversity compared to fast running stream, semi forest and lake side habitat
Extralevator versus standard abdominoperineal excision in locally advanced rectal cancer: a retrospective study with long-term follow-up
To analyze the results of abdominoperineal excisions (APE) for locally advanced rectal cancer at our institution before and after the adoption of extralevator abdominoperineal excision (ELAPE) with a special reference to long-term survival.A retrospective cohort study conducted in a tertiary referral center. All consecutive patients operated for locally advanced (TNM classification T3-4) rectal cancer with APE in 2004-2009 were compared to patients with similar tumors operated with ELAPE in 2009-2016.Forty-two ELAPE and 27 APE patients were included. Circumferential resection margin (CRM) was less than 1 mm (R1-resection) in 10 (24%) of ELAPE patients and 11 (41%) of APE patients (p = 0.1358). Intraoperative perforation (IOP) occurred in 4 (10%) patients and 6 (22%) patients in ELAPE and APE groups, respectively (p = 0.1336). There were 3 (7%) local recurrences (LRs) in ELAPE group and 5 (19%) in APE (p = 0.2473). There were no statistical differences in adverse events, overall survival, or disease-free survival between ELAPE and APE groups.We found a non-significant tendency to lower rates of IOP and positive CRM as well as lower rate of LR in the ELAPE group. Long-term survival and adverse events did not differ between the groups. ELAPE is beneficial for the surgeon in offering better vicinity to the perineal area and better work ergonomics. These technical aspects and the clinically very important tendency to lower rate of LR support the use of ELAPE technique in spite of the lack of survival benefit
Feasibility of a transmucosal sublingual fentanyl tablet as a procedural pain treatment in colonoscopy patients: a prospective placebo-controlled randomized study
Since patients often experience pain and unpleasantness during a colonoscopy, the present study aimed to evaluate the efficacy and safety of sublingually administered fentanyl tablets for pain treatment. Furthermore, since the use of intravenous drugs significantly increases colonoscopy costs, sublingual tablets could be a cost-effective alternative to intravenous sedation. We conducted a prospective placebo-controlled randomized study of 158 patients to evaluate the analgesic effect of a 100 µg dose of sublingual fentanyl administered before a colonoscopy. Pain, sedation, nausea, and satisfaction were assessed during the colonoscopy by the patients as well as the endoscopists and nurses. Respiratory rate and peripheral arteriolar oxygen saturation were monitored throughout the procedure. There were no differences between the fentanyl and placebo groups in any of the measured variables. The median pain intensity values, as measured using a numerical rating scale, were 4.5 in the fentanyl group and 5 in the placebo group. The sedation and oxygen saturation levels and the respiratory rate did not differ between the groups. The majority of the colonoscopies were completed.Our results indicate that a 100 µg dose of sublingual fentanyl is not beneficial compared to the placebo in the treatment of procedural pain during a colonoscopy.</p
Validation of the Low Anterior Resection Syndrome Score in Finnish Patients: Preliminary Results on Quality of Life in Different Lars Severity Groups
Background and Aims: Low anterior resection syndrome is common after anterior resection for rectal cancer. Its severity can be tested with the low anterior resection syndrome score. We have translated the low anterior resection syndrome score to Finnish, and the aim of this study is to validate the translation.Materials and Methods: The translated Finnish low anterior resection syndrome score and European Organisation for Research and Treatment of Cancer quality-of-life questionnaire-C30 and QLQ-CR29 questionnaires were sent to 159 surviving patients operated with anterior resection for rectal adenocarcinoma between 2007 and 2014 in a tertiary referral center. Psychometric properties of the translation were evaluated in comparison to quality-of-life scales and in different risk factor groups. Results: In the study, 104 (65%) patients returned the questionnaires. Of these, 56 (54%) had major low anterior resection syndrome, 26 (25%) had minor low anterior resection syndrome, and 22 (21%) had no low anterior resection syndrome. Patients with major low anterior resection syndrome had a significantly lower quality of life and more defecatory symptoms as assessed with the European Organisation for Research and Treatment of Cancer questionnaires compared with those with no low anterior resection syndrome. Patients operated with total mesorectal excision had significantly higher low anterior resection syndrome scores compared with those operated with partial mesorectal excision (median/interquartile range 32/15 and 29/11, respectively,p = 0.037). The test-retest validity of the translation was good with an intraclass correlation coefficient of 0.77 (95% confidence interval 0.51-0.90). Conclusion: The Finnish low anterior resection syndrome score is a valid test in the assessment of postoperative bowel function and its impact on the quality of life. It can be implemented to use during regular follow-up visits of Finnish-speaking rectal cancer patients.</div
Foraging tactics in dynamic sea-ice habitats affect individual state in a long-ranging seabird
Individual heterogeneity in diet and foraging behaviour is common in wild animal populations, and can be a strong determinant of how populations respond to environmental changes. Within populations, variation in foraging behaviour and the occurrence of individual tactics in relation to resources distribution can help explain differences in individual fitness, and ultimately identify important factors affecting population dynamics. We examined how foraging behaviour and habitat during the breeding period related to the physiological state of a long-ranging seabird adapted to sea ice, the Antarctic petrel Thalassoica antarctica. Firstly, using GPS tracking and state-switching movement modelling (hidden Markov models) on 124 individual birds, we tested for the occurrence of distinct foraging tactics within our study population. Our results highlight a large variation in the movement and foraging behaviour of a very mobile seabird, and delineate distinct foraging tactics along a gradient from foraging in dense pack ice to foraging in open water. Secondly, we investigated the effects of these foraging tactics on individual state at return from a foraging trip. We combined movement data with morphometric and physiological measurements of a suite of plasma metabolites that provided a general picture of a bird's individual state. Foraging in denser sea ice was associated with lower gain in body mass during brooding, as well as lower level of energy acquisition (plasma triacylglycerol) during both brooding and incubation. We found no clear relationship between the foraging tactic in relation to sea ice and the energetic stress (changes in plasma corticosterone), energetic balance (β-hydroxybutyrate) or trophic level (δ15N). However, a shorter foraging range was related to both the energetic balance (positively) and the trophic level (negatively). Our results highlight a diverse range of foraging tactics in relation to sea ice in Antarctic petrels. While the various foraging tactics do not seem to strongly alter energetic balance, they may affect other aspects of Antarctic petrels' physiology. Future changes in sea-ice habitats can thus be expected to have an impact on the individual state of seabirds such as Antarctic petrels, which could ultimately affect their population dynamics. Nonetheless, strong individual heterogeneity in the use of sea-ice habitats by a typical pagophilic species might strengthen its resilience to environmental changes and in particular to forecasted sea-ice loss. Developing common protocols to measure tundra herbivory across spatial scales
Understanding and predicting large-scale ecological responses to global environmental change requires comparative studies across geographic scales with coordinated efforts and standardized methodologies. We designed, applied, and assessed standardized protocols to measure tundra herbivory at three spatial scales: plot, site (habitat), and study area (landscape). The plot- and site-level protocols were tested in the field during summers 2014–2015 at 11 sites, nine of them consisting of warming experimental plots included in the International Tundra Experiment (ITEX). The study area protocols were assessed during 2014–2018 at 24 study areas across the Arctic. Our protocols provide comparable and easy to implement methods for assessing the intensity of invertebrate herbivory within ITEX plots and for characterizing vertebrate herbivore communities at larger spatial scales. We discuss methodological constraints and make recommendations for how these protocols can be used and how sampling effort can be optimized to obtain comparable estimates of herbivory, both at ITEX sites and at large landscape scales. The application of these protocols across the tundra biome will allow characterizing and comparing herbivore communities across tundra sites and at ecologically relevant spatial scales, providing an important step towards a better
