    Continuously varying skin potentials elicited by sinusoidally varying electric shock potentials

    An investigation was carried out to determine whether a form of quasi-linear systems analysis can be applied to electrodormal responses to yield new insights into the nature of the response mechanisms and their interrelationships. The response investigated was the electrodermal response (galvanic skin potential, GSP) as elicited by an electric shock stimulus applied to the skin. The response subsequent to this stimulation was examined and its characteristics measured. A series of experimental runs on three Ss was accomplished, using sinusoidal modulation envelopes of frequencies. Results showed that it was possible to drive the GSP and to achieve relatively high coherence between the driving frequency and the response itself. The analysis was limited to Fourier analysis of the response in order to determine the relative energies at the driving frequency and at successive harmonics of that driving frequency, and correlational analysis in order to determine the degree of linear relationship between the driving frequency and the driven response

    Agents for fluorescence-guided glioma surgery: a systematic review of preclinical and clinical results

    Background: Fluorescence-guided surgery (FGS) is a technique used to enhance visualization of tumor margins in order to increase the extent of tumor resection in glioma surgery. In this paper, we systematically review all clinically tested fluorescent agents for application in FGS for glioma and all preclinically tested agents with the potential for FGS for glioma. Methods: We searched the PubMed and Embase databases for all potentially relevant studies through March 2016. We assessed fluorescent agents by the following outcomes: rate of gross total resection (GTR), overall and progression-free survival, sensitivity and specificity in discriminating tumor and healthy brain tissue, tumor-to-normal ratio of fluorescent signal, and incidence of adverse events. Results: The search strategy resulted in 2155 articles that were screened by titles and abstracts. After full-text screening, 105 articles fulfilled the inclusion criteria evaluating the following fluorescent agents: 5-aminolevulinic acid (5-ALA) (44 studies, including three randomized control trials), fluorescein (11), indocyanine green (five), hypericin (two), 5-aminofluorescein-human serum albumin (one), endogenous fluorophores (nine) and fluorescent agents in a pre-clinical testing phase (30). Three meta-analyses were also identified. Conclusions: 5-ALA is the only fluorescent agent that has been tested in a randomized controlled trial and results in an improvement of GTR and progression-free survival in high-grade gliomas. Observational cohort studies and case series suggest similar outcomes for FGS using fluorescein. Molecular targeting agents (e.g., fluorophore/nanoparticle labeled with anti-EGFR antibodies) are still in the pre-clinical phase, but offer promising results and may be valuable future alternatives

    Mindfulness-based stress reduction for people with multiple sclerosis ? a feasibility randomised controlled trial

    Background: Multiple sclerosis (MS) is a stressful condition. Mental health comorbidity is common. Stress can increase the risk of depression, reduce quality of life (QOL), and possibly exacerbate disease activity in MS. Mindfulness-Based Stress Reduction (MBSR) may help, but has been little studied in MS, particularly among more disabled individuals. Methods: The objective of this study was to test the feasibility and likely effectiveness of a standard MBSR course for people with MS. Participant eligibility included: age > 18, any type of MS, an Expanded Disability Status Scale (EDSS) </= 7.0. Participants received either MBSR or wait-list control. Outcome measures were collected at baseline, post-intervention, and three-months later. Primary outcomes were perceived stress and QOL. Secondary outcomes were common MS symptoms, mindfulness, and self-compassion. Results: Fifty participants were recruited and randomised (25 per group). Trial retention and outcome measure completion rates were 90% at post-intervention, and 88% at 3 months. Sixty percent of participants completed the course. Immediately post-MBSR, perceived stress improved with a large effect size (ES 0.93; p < 0.01), compared to very small beneficial effects on QOL (ES 0.17; p = 0.48). Depression (ES 1.35; p < 0.05), positive affect (ES 0.87; p = 0.13), anxiety (ES 0.85; p = 0.05), and self-compassion (ES 0.80; p < 0.01) also improved with large effect sizes. At three-months post-MBSR (study endpoint) improvements in perceived stress were diminished to a small effect size (ES 0.26; p = 0.39), were negligible for QOL (ES 0.08; p = 0.71), but were large for mindfulness (ES 1.13; p < 0.001), positive affect (ES 0.90; p = 0.54), self-compassion (ES 0.83; p < 0.05), anxiety (ES 0.82; p = 0.15), and prospective memory (ES 0.81; p < 0.05). Conclusions: Recruitment, retention, and data collection demonstrate that a RCT of MBSR is feasible for people with MS. Trends towards improved outcomes suggest that a larger definitive RCT may be warranted. However, optimisation changes may be required to render more stable the beneficial treatment effects on stress and depression.     Hybrid PET- and MR-driven attenuation correction for enhanced ยนโธF-NaF and ยนโธF-FDG quantification in cardiovascular PET/MR imaging

    Background: The standard MR Dixon-based attenuation correction (AC) method in positron emission tomography/magnetic resonance (PET/MR) imaging segments only the air, lung, fat and soft-tissues (4-class), thus neglecting the highly attenuating bone tissues and affecting quantification in bones and adjacent vessels. We sought to address this limitation by utilizing the distinctively high bone uptake rate constant Ki expected from ยนโธF-Sodium Fluoride (ยนโธF-NaF) to segment bones from PET data and support 5-class hybrid PET/MR-driven AC for ยนโธF-NaF and ยนโธF-Fluorodeoxyglucose (ยนโธF-FDG) PET/MR cardiovascular imaging. Methods: We introduce 5-class Ki/MR-AC for (i) ยนโธF-NaF studies where the bones are segmented from Patlak Ki images and added as the 5th tissue class to the MR Dixon 4-class AC map. Furthermore, we propose two alternative dual-tracer protocols to permit 5-class Ki/MR-AC for (ii) ยนโธF-FDG-only data, with a streamlined simultaneous administration of ยนโธF-FDG and ยนโธF-NaF at 4:1 ratio (R4:1), or (iii) for ยนโธF-FDG-only or both ยนโธF-FDG and ยนโธF-NaF dual-tracer data, by administering ยนโธF-NaF 90 minutes after an equal ยนโธF-FDG dosage (R1:1). The Ki-driven bone segmentation was validated against computed tomography (CT)-based segmentation in rabbits, followed by PET/MR validation on 108 vertebral bone and carotid wall regions in 16 human volunteers with and without prior indication of carotid atherosclerosis disease (CAD). Results: In rabbits, we observed similar (< 1.2% mean difference) vertebral bone ยนโธF-NaF SUVmean scores when applying 5-class AC with Ki-segmented bone (5-class Ki/CT-AC) vs CT-segmented bone (5-class CT-AC) tissue. Considering the PET data corrected with continuous CT-AC maps as gold-standard, the percentage SUVmean bias was reduced by 17.6% (ยนโธF-NaF) and 15.4% (R4:1) with 5-class Ki/CT-AC vs 4-class CT-AC. In humans without prior CAD indication, we reported 17.7% and 20% higher ยนโธF-NaF target-to-background ratio (TBR) at carotid bifurcations wall and vertebral bones, respectively, with 5- vs 4-class AC. In the R4:1 human cohort, the mean ยนโธF-FDG:ยนโธF-NaF TBR increased by 12.2% at carotid bifurcations wall and 19.9% at vertebral bones. For the R1:1 cohort of subjects without CAD indication, mean TBR increased by 15.3% (ยนโธF-FDG) and 15.5% (ยนโธF-NaF) at carotid bifurcations and 21.6% (ยนโธF-FDG) and 22.5% (ยนโธF-NaF) at vertebral bones. Similar TBR enhancements were observed when applying the proposed AC method to human subjects with prior CAD indication. Conclusions: Ki-driven bone segmentation and 5-class hybrid PET/MR-driven AC is feasible and can significantly enhance ยนโธF-NaF and ยนโธF-FDG contrast and quantification in bone tissues and carotid walls

    A Numerical Study on the Behavior of Offshore Suction Bucket Foundations under Cyclic Lateral Loading

    Offshore wind turbines are exposed to high cyclic lateral loads induced by wind and waves which play a special role in the design of foundations. It's essential to clarify the bucket behavior under cyclic lateral loading to fulfil the design requirements of these foundations in both ultimate and serviceability limit states. In this study the behavior of monopods embedded in saturated sandy soil subjected to cyclic lateral loads were investigated using a sophisticated finite element model. The accumulation of permanent soil deformations and excess pore water pressure in saturated sandy soil due to cyclic lateral loading are taken into account by a fully coupled two-phase model combined with a hypoplastic constitutive model. Porosity-permeability dependence is modelled with the Kozeny-Carman relationship. The main focus was dedicated to loads levels that led to progressive failure, attenuation, and shakedown behavior under cyclic lateral loading. The main finding of these investigations are presented which make an effective contribution to the safe and economic design of bucket foundations
