    Psychometric properties of the person-centred coordinated care experience questionnaire (P3CEQ) in a Norwegian radiotherapy setting

    The number of older adults with cancer is increasing. Radiotherapy is an important treatment modality for cancer and may cause side effects and distress. Patient-reported experience measures aim to measure patients’ experiences with health care. This can help healthcare services to improve in line with patients’ needs. To assess how Norwegian patients receiving radiotherapy experience their care, a valid and reliable tool is required. We selected the person-centred coordinated care experience questionnaire as a tool. Objective The aim of the study is to validate the Norwegian version of the person-centred coordinated care experience questionnaire in a radiotherapy setting. Methods A feasibility study of the person-centred coordinated care experience questionnaire and a cross-sectional study—testing psychometric properties of the questionnaire in a Norwegian radiotherapy setting—were conducted. Participants were recruited from two different hospitals in Norway. Patient characteristics and item scores are described using descriptive statistics. We performed an exploratory factor analysis and applied principal component analysis with a varimax rotation. Cronbach’s α was used to assess internal consistency. Results In total, 24 patients participated in the feasibility test, and 176 were included in the cross-sectional study where we explored the psychometric properties of the person-centred coordinated care experience questionnaire. Three factors were identified. Internal consistency was established for the 10-item scale, with Cronbach’s α = 0.698. Conclusions Conclusions must consider the Norwegian setting and healthcare context. We found that the Norwegian version of the person-centred coordinated care experience questionnaire is a relevant, valid and reliable tool to provide insight into different areas of patients’ experiences upon receiving radiotherapy.     Distributions of Beryllium 7 and Lead 210, and Soluble Aerosol-Associated Ionic Species Over the Western Pacific: PEM West B, February - March 1994

    Aerosol sampling for the determination of the concentrations of soluble ionic species and the natural radionuclides Be-7 and Pb-210 was conducted from the NASA DC-8 over the western Pacific as part of GTE/PEM-West B during February - March 1994. Concentrations of most soluble ionic species in the free troposphere were higher in samples collected on flights originating from Hong Kong and Japan than those collected further east over the open ocean. In both regions the measured concentrations were higher than those found during PEM-West A (fall 1991). Activities of Pb-210, a tracer of air masses influenced by sources on the Asian continent, showed the same patterns. These data indicate the effect of stronger continental outflow from Asia over the western Pacific during the spring compared to fall season. For readily scavenged aerosol-associated species and soluble acidic gases the strongest indications of Asian outflow were restricted to altitudes below 6 km. The distribution of the continental tracer Pb-210 was also compared to those of a large number of gas phase species measured on the DC-8. Relatively strong correlations were found with O3, and peroxyacetylnitrate (PAN), but only during the flights over the remote Pacific. During PEM-West A, similar correlations were seen, but they were stronger near Asia. We believe that correlations are a signature of continental air that has been processed by deep wet convection over land before being advected over the ocean. One flight over the Sea of Japan provided the opportunity to sample upper troposphere/lower stratosphere air in and around a tropopause fold. Concentrations of Be-7 reached 7 pCi/cu m STP, and peak O3, mixing ratios of 480 ppb were encountered at 10.7 km. The Be-7 data are used to estimate the fraction of stratospheric air mixed down into the troposphere by circulation in the fold

    Antimicrobial susceptibility testing of Clostridium difficile using EUCAST epidemiological cut-off values and disk diffusion correlates

    AbstractWith the emergence of reduced susceptibility of Clostridium difficile to metronidazole and vancomycin the value of antimicrobial susceptibility testing has increased. The aim of our study was to evaluate disk diffusion for susceptibility testing of C. difficile by comparing disk diffusion results with MICs from gradient tests and to propose zone diameter breakpoint correlates for the EUCAST epidemiological cut-off values (ECOFFs) recently published. We tested 211 clinical isolates of C. difficile, from patients with diarrhoea hospitalized at Aarhus and Odense University Hospitals, Denmark. Furthermore, ten clinical isolates of C. difficile from the Anaerobe Reference Laboratory, University Hospital of Wales, with known reduced susceptibility to either metronidazole or vancomycin, were included. Isolates were tested with Etest gradient strips and disk diffusion towards metronidazole, vancomycin and moxifloxacin on Brucella Blood Agar supplemented with hemin and vitamin K. We found an excellent agreement between inhibition zone diameter and MICs. For each MIC value, the inhibition zones varied from 0 to 8 mm, with 93% of values within 6 mm for metronidazole, 95% of values within 4 mm for vancomycin, and 98% of values within 4 mm for moxifloxacin. With proposed zone diameter breakpoints for metronidazole, vancomycin and moxifloxacin of WT ≥ 23 mm, WT ≥ 19 and WT ≥ 20 mm, respectively, we found no very major errors and only major errors below 2%. In conclusion, we suggest that disk diffusion is an option for antimicrobial susceptibility testing of C. difficile

    Deformabilidad en hormigones con agregados reciclados

    Diversos trabajos han demostrado la factibilidad de elaborar hormigones con agregados reciclados, sin embargo sobre algunos aspectos poco explorados aún existen informaciones contrapuestas. Entre ellos se destacan el comportamiento diferido del hormigón (contracción y fluencia) y la capacidad de deformación en tracción (extensibilidad). Estas propiedades afectan directamente el grado de fisuración que puede tener una estructura de hormigón, lo que adquiere una significativa relevancia en la práctica, al considerar su vida en servicio. En este trabajo se presenta un estudio de la deformabilidad de hormigones que contienen 50 o 100 % de agregado grueso obtenido a partir de la trituración de losas de pavimento. Los resultados se comparan con los de otros dos hormigones elaborados con idénticas proporciones de materiales componentes variando el tipo de agregado grueso, piedra partida granítica o piedra partida cuarcítica. Se evaluaron la resistencia a tracción, la extensibilidad en flexión bajo cargas rápidas, y la fluencia en compresión. Bajo cargas de corta duración se encontró una deformabilidad creciente en el hormigón con mayor contenido de agregados reciclados, tanto en flexotracción como en compresión; también se midieron mayores valores de contracción libre y de fluencia. A partir de los resultados surge que es posible estimar la deformabilidad del hormigón con agregados reciclados siguiendo criterios similares a los aplicados en hormigones con agregados naturales; las diferencias de deformabilidad se pueden justificar considerando la menor rigidez del agregado reciclado.Different works have demonstrated the feasibility of elaborating concrete with recycled aggregates, nevertheless there is still opposite information about some aspects that have not been widely studied. The differed behaviour (shrinkage and creep) and the deformation capacity in tension (extensibility) of concrete are among them. These properties have a direct effect over the degree of cracking that can have a concrete structure so, considering its service life, they acquire a significant relevance in practice. This paper presents a study on the deformability of concretes that contain 50 or 100 % of coarse aggregate obtained from crushed pavement slabs. The results are compared with those obtained on other two concretes prepared with the same mixture proportions varying only the type of coarse aggregate, granitic crushed stone or quartzitic crushed stone. The tensile strength, extensibility in flexure under rapid rate of loading and creep in compression were evaluated. It was found that under short term loads the deformability of concrete increases with the content of recycled aggregates, both in flexure and in compression, grater values of free shrinkage and creep were also measured. From the obtained results it appears that the deformability of concrete with recycled aggregates can be estimated following the same criteria applied to concrete with natural aggregate, the differences in deformability can be justified considering the lower stiffness of the recycled aggregate

    Performance and parasitosis in heifers grazing mixed with sows

    Summary The aim of the study was to investigate the effect of mixed grazing with first season heifers and pregnant sows on animal performance, gastro-intestinal helminths, pasture quality and sward structure during three grazing seasons. This presentation will focus on results from 1999, primarily regarding performance and parasitosis in heifers. There have been no earlier reports on such mixed grazing systems. Three grazing systems were studied in replicate: 1) Heifers grazing alone; 2) sows grazing alone; 3) heifers grazing together with sows. The heifers were inoculated with low doses of infective O.ostertagi larvae at turn-out. Continuous grazing was practised in paddocks regulated in size according to herbage allowance. Individual weight gain, faecal egg output and serum pepsinogen concentrations -as indicator of O.ostertagi infection -were measured fortnightly. The sward structure and quality were greatly influenced by the applied grazing system. The average daily gain of the heifers was significantly higher (P=0.0006) when grazing together with sows (1,121±45 g/day, n=16) than when grazing alone (869±48 g/day, n=14). The mean pepsinogen concentrations were elevated in the heifers grazing alone. It is concluded, that weight gains were significantly better and infection levels with O.ostertagi were significantly reduced in heifers grazing together with sows. When sows with nose-ring graze alone, experience shows that the sward gets dirty and stemmy, and the sows destroy the sward in spots.     A Quantum Mechanical Model of the Reissner-Nordstrom Black Hole

    We consider a Hamiltonian quantum theory of spherically symmetric, asymptotically flat electrovacuum spacetimes. The physical phase space of such spacetimes is spanned by the mass and the charge parameters MM and QQ of the Reissner-Nordstr\"{o}m black hole, together with the corresponding canonical momenta. In this four-dimensional phase space, we perform a canonical transformation such that the resulting configuration variables describe the dynamical properties of Reissner-Nordstr\"{o}m black holes in a natural manner. The classical Hamiltonian written in terms of these variables and their conjugate momenta is replaced by the corresponding self-adjoint Hamiltonian operator, and an eigenvalue equation for the ADM mass of the hole, from the point of view of a distant observer at rest, is obtained. Our eigenvalue equation implies that the ADM mass and the electric charge spectra of the hole are discrete, and the mass spectrum is bounded below. Moreover, the spectrum of the quantity M2−Q2M^2-Q^2 is strictly positive when an appropriate self-adjoint extension is chosen. The WKB analysis yields the result that the large eigenvalues of the quantity M2−Q2\sqrt{M^2-Q^2} are of the form 2n\sqrt{2n}, where nn is an integer.     The reaction pp -> pf (eta pi pi) ps has been studied at 450 GeV/c. There is clear evidence for an a2(1320)pi decay mode of the eta2(1645) and eta2(1870). In addition, there is evidence for an a0(980)pi$ decay mode of both resonances and an f2(1270)eta decay mode of the eta2(1870).     The Transcription co-Repressors MTG8 and MTG16 Regulate Exit of Intestinal Stem Cells From Their Niche and Differentiation into Enterocyte vs Secretory Lineages

    BACKGROUND & AIMS: Notch signaling maintains intestinal stem cells (ISCs). When ISCs exit the niche, Notch signaling among early progenitor cells at position +4/5 regulates their specification toward secretory vs enterocyte lineages (binary fate). The transcription factor ATOH1 is repressed by Notch in ISCs; its de-repression, when Notch is inactivated, drives progenitor cells to differentiate along the secretory lineage. However, it is not clear what promotes transition of ISCs to progenitors and how this fate decision is established. METHODS: We sorted cells from Lgr5-Gfp knock-in intestines from mice and characterized gene expression patterns. We analyzed Notch regulation by examining expression profiles (by quantitative reverse transcription PCR and RNAscope) of small intestinal organoids incubated with the Notch inhibitor DAPT, intestine tissues from mice given injections of the γ-secretase inhibitor dibenzazepine, and mice with intestine-specific disruption of Rbpj. We analyzed intestine tissues from mice with disruption of the RUNX1 translocation partner 1 gene (Runx1t1, also called Mtg8) or CBFA2/RUNX1 partner transcriptional co-repressor 3 (Cbfa2t3, also called Mtg16), and derived their organoids, by histology, immunohistochemistry, and RNA sequencing. We performed chromatin immunoprecipitation and sequencing analyses of intestinal crypts to identify genes regulated by MTG16. RESULTS: The transcription co-repressors MTG8 and MTG16 were highly expressed by +4/5 early progenitors, compared with other cells along crypt-villus axis. Expression of MTG8 and MTG16 were repressed by Notch signaling via ATOH1 in organoids and intestine tissues from mice. MTG8- and MTG16-knockout intestines had increased crypt hyperproliferation and expansion of ISCs, but enterocyte differentiation was impaired, based on loss of enterocyte markers and functions. Chromatin immunoprecipitation and sequencing analyses showed that MTG16 bound to promoters of genes that are specifically expressed by stem cells (such as Lgr5 and Ascl2) and repressed their transcription. MTG16 also bound to previously reported enhancer regions of genes regulated by ATOH1, including genes that encode delta-like canonical Notch ligand and other secretory-specific transcription factors. CONCLUSIONS: In intestine tissues of mice and human intestinal organoids, MTG8 and MTG16 repress transcription in the earliest progenitor cells to promote exit of ISCs from their niche (niche exit) and control the binary fate decision (secretory vs enterocyte lineage) by repressing genes regulated by ATOH1

    A study of the f0(1370), f0(1500), f0(2000) and f2(1950) observed in the centrally produced 4pi final states

    The production and decay properties of the f0(1370), f0(1500), f0(2000) and f2(1950) have been studied in central pp interactions at 450 GeV/c. The dPT, phi and |t| distributions of these resonances are presented. For the J = 0 states, the f0(1370) and f0(2000) have similar dPT and phi dependences. These are different to the dPT and phi dependences of the f0(980), f0(1500) and f0(1710). For the J = 2 states the f2(1950) has different dependences to the f2(1270) and f2'(1520).
