    Forming the Architecture of a Multi-Layered Model of Physical Data Storage for Complex Telemedicine Systems

    The relevance of this research is determined by the need to study the issues of improving data storage technologies for complex telemedicine systems. The objective is to create a multi-layered data storage model for complex telemedicine systems to ensure the most complete use of their capacity and the timely expansion of existing storage. The research is conducted on the basis of an analysis of existing opportunities and problems in the field of data storage technologies. An analysis of the main features of the development of data storage technologies revealed that the existing models have no detailed description of the recording and physical storage of data bits, which is necessary for describing the storage process. Different architectures are reviewed, and their strengths and weaknesses are discussed. Within the framework of a demonstration experiment using the Kohonen neural network apparatus as a tool for solving the problem of placing objects in accordance with the required parameters, it is shown that the proposed storage system resource management model is operable and allows solving the problem of rational use of physical resources. As a result, a multilevel model of data storage is proposed, which combines the levels of storage process organization and technology.     Occlusion of rectal arteries in the treatment of recurrent hemorrhoidal disease after transanal surgical interventions

    The article presents data on variant X-ray anatomy of the superior rectal artery and types of arterial architectonics of the rectum. The causes of recurrent hemorrhoidal disease after traditional transanal surgical interventions are highlighted. The possibilities of endovascular occlusion of rectal arteries in the treatment of relapses of chronic hemorrhoids are shown. Its technical variants are given depending on the number of hemorrhoidal arteries and types of arterial architectonics of the rectum.Over a 2-year period, clinical results of endovascular occlusion of hemorrhoidal arteries were evaluated in 11 patients with relapse of the disease after various transanal interventions: after latex ligation of internal hemorrhoids – in 5, transanal desarterization of internal hemorrhoids with mucopexia – in 4, hemorrhoidectomy – in 2. The average age of patients was 44.2 ± 12.5 years. Men – 5, women – 6. Superselective occlusion of the target hemorrhoidal branches of the upper rectal artery was performed with EmboGold (Merit Medical) microparticles, 500–700/300–500 mkm in size. Immediate results: cessation of bleeding on the first day – in 4 patients, after 3 days – in 5, after 7 days – in 2. Terms of hospitalization – 1 day, disability – 4.3 ± 0.7 days. Long-term results (from 1 to 2 years) were observed in all 11 patients. The course is relapse-free.As an illustration, a clinical case is presented – successful treatment of a patient with recurrent hemorrhoidal disease by endovascular occlusion of hemorrhoidal arteries after five traditional transanal surgical interventions: sclerosing, ligation, dearterization of internal hemorrhoids, dearterization of internal hemorrhoids with mucopexia, removal of external hemorrhoids. Within 3 months after endovascular occlusion – complete regression of all symptoms of hemorrhoidal disease. According to the questionnaire (after 1 and 2 years) there is no relapse of the disease, the patient is subjectively satisfied with the endovascular treatment

    A simple device for direct measurement of buoyancy driven fluid flows in a low-gravity environment is proposed. A system connecting spacecraft accelerometers data and results of thermal convection in enclosure measurements and numerical simulations is developed. This system will permit also to evaluate the low frequency microacceleration component.     Carrageenans as a New Source of Drugs with Metal Binding Properties

    Carrageenans are abundant and safe non-starch polysaccharides exerting their biological effects in living organisms. Apart from their known pro-inflammation properties and some pharmacological activity, carrageenans can also strongly bind and hold metal ions. This property can be used for creation of the new drugs for elimination of metals from the body or targeted delivery of these metal ions for healing purposes. Metal binding activity of different carrageenans in aqueous solutions containing Y3+ or Pb2+ ions was studied in a batch sorption system. The metal uptake by carrageenans is not affected by the change of the pH within the range from 2.0 to 6.0. The rates and binding capacities of carrageenans regarding metal ions were evaluated. The Langmuir, Freundlich and BET sorption models were applied to describe the isotherms and constants, and the sorption isothermal data could be explained well by the Langmuir equation. The results obtained through the study suggest that κ-, ι-, and λ-carrageenans are favorable sorbents. The largest amount of Y3+ and Pb2+ ions are bound by ι-carrageenan. Therefore, it can be concluded that this type of polysaccharide is the more appropriate substance for elaboration of the drugs with high selective metal binding properties

    Опыт применения фотодинамической терапии в лечении рака пищевода

    Photodynamic therapy (PDT) is a worthy alternative to surgical esophageal resection or endoscopic mucosal resection and dissection (EMR, ESD) in patients with superficial esophageal cancer with severe concomitant diseases as well as in patients with a common form of esophageal cancer with severe malignant dysphagia. Patients with superficial (7) and advanced (15) esophageal cancer received PDT as an independent method and as a supplement to complex treatment. Radachlorin was used as a photosensitizer at a dose of 0.6–0.8 mg/kg, administered intravenously 3 hours before irradiation. A PDT session was carried out using a laser with a wavelength of 662 nm. The light dose used was 150–300 J/cm2 . The use of PDT made it possible to achieve the full effect in 7 (100%) patients in the group of superficial (T1a-T1b) esophageal cancer where PDT was either the only method of treatment or in combination with radiation therapy. In the group of patients with stenotic cancer the use of PDT made it possible to achieve full recovery of food intake after recanalization for 20% of patients, and partial – for 66.7%. Thus, complete natural food intake was restored for 86.7% of patients which improved their quality of life. PDT is also a method of choice for cancer of the upper esophagus as esophageal stenting in this situation can cause unwanted subjective sensations. Фотодинамическая терапия (ФДТ) является достойной альтернативой хирургической резекции пищевода и эндоскопической резекции или диссекции слизистой (EMR, ESD) у пациентов с поверхностным раком пищевода при наличии у них тяжелых сопутствующих заболеваний, а также у пациентов с распространенной формой рака пищевода при выраженной злокачественной дисфагии. Больные поверхностным (7 человек) и стенозирующим (15 человек) раком пищевода получали ФДТ в качестве самостоятельного лечения, а также в качестве дополнения к комплексной терапии. В качестве фотосенсибилизатора применяли препарат радахлорин в дозе 0,6–0,8 мг/кг, который внутривенно вводили пациентам за 3 ч до начала облучения. Сеанс ФДТ проводили с использованием лазера с длиной волны 662 нм. Световая доза составила 150-300 Дж/см2 . Применение ФДТ позволило достичь полного эффекта у 7 (100%) пациентов в группе с поверхностным (Т1а-Т1b) раком пищевода, где ФДТ применяли как единственный метод лечения или в сочетании с лучевой терапией. В группе пациентов со стенозирующим раком с помощью ФДТ удалось добиться полного восстановления питания после реканализации у 20% больных, частичного – у 66,7%. Таким образом, у 86,7% пациентов было восстановлено полноценное естественное питание, что значительно улучшило качество их жизни. ФДТ является методом выбора при лечении злокачественных новообразований верхних отделов пищевода, так как стентирование пищевода в данной ситуации может вызвать нежелательные субъективные ощущения.
