Roma Mentor Projekt: a Barát-modell
A Roma Mentor Projekt című pedagógiai kísérletnek kevés a szakirodalma Magyarországon, pedig olyan programról van szó, amely méltán kaphatna nagyobb figyelmet is. A kísérlet elemei rendkívüli módon hasonlítanak a Tanoda-modellre, de azzal az eltéréssel, hogy amíg – ideális esetben – a Tanoda-modell egy oktatási intézményen kívüli inter- és multikulturális közeg, amelyben infrastrukturális keretek között teremtik meg a halmozottan hátrányos helyzetű gyermekek számára az értelmiségi családi hátteret, addig a Roma Mentor Projekt az oktatási intézményen belüli – a nevelő-oktató munkára közvetlenebbül is kiható, de szintén infrastrukturális keretek között megvalósított – értelmiségi barát-modell, amelynek keretében egy – általában országosan is közismert – roma entellektüel (helyi nem roma pedagógustársával, iskolai kapcsolattartójával közösen) szocializálja csoportmentorként – származástól függetlenül – a halmozottan hátrányos helyzetű gyermekeket, s ez utóbbi megállapítás akkor is érvényes, ha egyébként a csoport kultúrából meríti

Absztrakt:
A szulfonamidszármazékok („sulpha drugs”) széles körben és változatos
indikációval alkalmazott gyógyszerek hatóanyagát képezik. Az elmúlt néhány
évtizedben számos közlemény jelent meg a fenti szerek, különösen az acetazolamid
általános alkalmazása mellett, szemészeti idioszinkráziás hatásként jelentkező
sugártest- és érhártyaleválásról, amit minden publikált esetben tranzitórikus
myopia és/vagy akut szemnyomás-emelkedés megjelenése kapcsán diagnosztizáltak.
Jelen közleményben két olyan esetet ismertetünk, ahol a gyógyszermellékhatásként
jelentkező chorioidealeválás véletlen lelet volt, és ahhoz sem a refrakció
változása, sem a szemnyomás kiugrása nem társult. Tudomásunk szerint ez az első
közlemény, amely az acetazolamid okozta chorioideaablatio ezen speciális, „néma”
formájáról számol be. Tekintve, hogy az érhártya érintettsége változó mértékű
lehet, és – eseteink alapján – nem feltétlenül jár együtt akut glaucomával és
rövidlátással, feltételezzük, hogy az acetazolamid szemészeti mellékhatásainak
egy része felfedezetlen marad. Cikkünkkel szeretnénk felhívni a társszakmák
figyelmét a szemészeti vizsgálat fontosságára szulfonamidtartalmú gyógyszert

Abstract:
Sulpha drugs are widely employed in medicine for various diseases and disorders.
During the last several decades, numerous papers had been published on supra
ciliary and posterior choroidal effusion likely presenting as an idiosyncratic
effect of these drugs especially of acetazolamide. In each publication, the
effusion was associated with either an acute angle-closure glaucoma or
transitory myopia or both of these as leading symptoms. In the current
publication, authors report on two cases where the acetazolamide-induced
choroidal effusion was an accidental finding without either a myopic shift in
refraction or an acute elevation in intraocular pressure. To our best knowledge,
ours is the first report in the literature describing this unusual, “silent”
form of a sulpha drug-induced choroidal effusion. Since the choroidal
involvement may vary in size and location, and is not necessarily associated
with acute glaucoma and myopia, one can assume that a considerable amount of
acetazolamide-related ocular side-effects will not be discovered. The above case
report aims to draw the attention of other specialities to the need for
ophthalmic examination for their patients taking sulpha drugs with acute visual

Tüdőreszekciót követő tüdővérzés ritka esete = Pulmonary hemorrhage after lung resection. A case report
Absztrakt:
Tüdőműtéteket követően gyakori, hogy a beteg légúti váladékának mennyisége megnő,
ami légzési nehezítettséget, radiológiai eltérést okoz. Ugyanakkor a beteg
anamnézisétől függően eltérő mértékű cardialis terheléshez is vezethet, mely
súlyos esetben ritmuszavar vagy akár pulmonalis pangás formájában nyilvánulhat
meg. A ritmuszavar kiváltó oka viszont egy kezdődő gyulladásos folyamat is
lehet, amely magas proinflammatorikus citokinszinttel társul. Citokinkiáramlást
azonban az erek falának megfeszülése is képes okozni, aminek a perioperatív
időszakban lehetséges gyakori oka a thrombus okozta érfalérintettség. Orv Hetil.

Abstract:
Enormous airway secretion after lung resection is common and can lead to dyspnea
and radiological anomalies. Depending on the patient’s medical history, it can
lead to heart failure, which appears in arrythmia or pulmonary edema. An
inflammatory event with high level of proinflammatory cytokines can also lead to
arrythmia. The stretch of the wall of vessels can also cause a cytokine release,
and the thromboembolic event is a possible common cause of this stretch in the

Interaction of polyamines, abscisic acid and proline under osmotic stress in the leaves of wheat plants
The exact relationship between polyamine, abscisic acid and proline metabolisms is still poorly understood. In the present study, the effects of putrescine and abscisic acid treatments alone or in combination with polyethylene glycol-induced osmotic stress were investigated in young wheat plants. It was observed that abscisic acid plays a role in the coordinated regulation of the proline and polyamine biosynthetic pathways, which compounds are related to each other through a common precursor. Abscisic acid pre-treatment induced similar alteration of polyamine contents as the osmotic stress, namely increased the putrescine, but decreased the spermidine contents in the leaves. These changes were mainly related to the polyamine cycle, as both the synthesis and peroxisomal oxidation of polyamines have been induced at gene expression level. Although abscisic acid and osmotic stress influenced the proline metabolism differently, the highest proline accumulation was observed in the case of abscisic acid treatments. The proline metabolism was partly regulated independently and not in an antagonistic manner from polyamine synthesis. Results suggest that the connection, which exists between polyamine metabolism and abscisic acid signalling leads to the controlled regulation and maintenance of polyamine and proline levels under osmotic stress conditions in wheat seedlings
Radiofrequency Ablation at Low Irrigation Flow Rates Using a Novel 12-Hole Gold Open-Irrigation Catheter
BACKGROUND: High irrigation rates during radiofrequency (RF) ablation may cause fluid overload and limit lesion size. This in vivo animal study assessed the safety and efficacy of RF ablation at low irrigation rates using a novel 12-hole gold catheter. METHODS: A total of 103 lesions, created on the thigh of five mongrel dogs, were analyzed. Lesions were created using a 12-hole irrigated gold-tip (Au) and a six-hole irrigated platinum-iridium (PtIr) catheter (both 7F/3.5-mm electrode; BIOTRONIK SE & CO, KG, Berlin, Germany) in parallel and perpendicular orientation. RF current was delivered for 60 seconds at 30 W using 8 mL/min and 15 mL/min irrigation. Electrode temperature, steam pops, lesion dimensions, and coagulum formation were recorded. RESULTS: Electrode temperatures were lower for Au compared to PtIr in parallel (8 mL/min: 38.1 +/- 1.7 degrees C vs 48.0 +/- 4.8 degrees C, P < 0.0001; 15 mL/min: 36.0 +/- 1.5 degrees C vs 46.9 +/- 5.4 degrees C, P < 0.0001) and perpendicular position (15 mL/min: 35.5 +/- 1.2 degrees C vs 38.4 +/- 2.5 degrees C, P = 0.003). The number of steam pops between Au and PtIr was comparable for parallel (8 mL/min: 14% vs 27%, P = 0.65; 15 mL/min: 14% vs 43%, P = 0.21) and perpendicular orientation (8 mL/min: 25% vs 17%, P = 1.00; 15 mL/min: 18% vs 0%, P = 0.48). Au created larger volumes than PtIr at 8 mL/min irrigation (861 +/- 251 mm3 vs 504 +/- 212 mm3 , P = 0.004); however, for 15 mL/min, volumes were comparable (624 +/- 269 mm3 vs 768 +/- 466 mm3 , P = 0.46). No coagulum formation was observed for any of the catheters on the surface and catheter tip. CONCLUSION: RF ablation at low flow rate using a novel 12-hole irrigation Au catheter is safe and results in larger lesions than with a PtIr electrode
