    Effects of hospital facilities on patient outcomes after cancer surgery: an international, prospective, observational study

    Background Early death after cancer surgery is higher in low-income and middle-income countries (LMICs) compared with in high-income countries, yet the impact of facility characteristics on early postoperative outcomes is unknown. The aim of this study was to examine the association between hospital infrastructure, resource availability, and processes on early outcomes after cancer surgery worldwide.Methods A multimethods analysis was performed as part of the GlobalSurg 3 study-a multicentre, international, prospective cohort study of patients who had surgery for breast, colorectal, or gastric cancer. The primary outcomes were 30-day mortality and 30-day major complication rates. Potentially beneficial hospital facilities were identified by variable selection to select those associated with 30-day mortality. Adjusted outcomes were determined using generalised estimating equations to account for patient characteristics and country-income group, with population stratification by hospital.Findings Between April 1, 2018, and April 23, 2019, facility-level data were collected for 9685 patients across 238 hospitals in 66 countries (91 hospitals in 20 high-income countries; 57 hospitals in 19 upper-middle-income countries; and 90 hospitals in 27 low-income to lower-middle-income countries). The availability of five hospital facilities was inversely associated with mortality: ultrasound, CT scanner, critical care unit, opioid analgesia, and oncologist. After adjustment for case-mix and country income group, hospitals with three or fewer of these facilities (62 hospitals, 1294 patients) had higher mortality compared with those with four or five (adjusted odds ratio [OR] 3.85 [95% CI 2.58-5.75]; p<0.0001), with excess mortality predominantly explained by a limited capacity to rescue following the development of major complications (63.0% vs 82.7%; OR 0.35 [0.23-0.53]; p<0.0001). Across LMICs, improvements in hospital facilities would prevent one to three deaths for every 100 patients undergoing surgery for cancer.Interpretation Hospitals with higher levels of infrastructure and resources have better outcomes after cancer surgery, independent of country income. Without urgent strengthening of hospital infrastructure and resources, the reductions in cancer-associated mortality associated with improved access will not be realised

    Shorted TEM Horn Antenna for GPR Applications

    In this study, a novel TEM horn antenna design is introduced for Ground Penetrating Radar (GPR) applications. TEM horn antennas are mostly used in GPR for the advantages of high gain and power capability and wide bandwidth. At the low frequency of the GPR applications, the antenna size must be big. However with using shorting plate with conventional TEM Horn antennas, antenna size can be reduced. A novel shorting plate structure is presented. The effect of this structure and the contributions to antenna performance is investigated. HFSS simulations and measurement results of a prototype is presented. It has been seen that they are consistent with each other

    The structural behaviour of composite beams with prefabricated reinforced concrete plate in positive moment zone

    In this study, it was investigated that the bearing behaviour of reinforced concrete beams and steel elements in the positive moment zone. The bearing behaviour and the bearing capacities of the reinforced concrete beams with steel elements agglutinated by the epoxy resin. Moreover, certain model researches are done theoretically to determine the distribution of the sliding and the normal strain throughout the coherence surface of the beams which are composed of the reinforced concrete plate and the steel elements agglutinated with the epoxy resin and equation are suggested for these kind of composite due to the results. Test results indicated that the bonding strength between steel and steel is 470% longer than that of concrete to steel. (C) 2014 Elsevier Ltd. All rights reserved

    Farklı Irklardan Tekelerde Başlıca Spermatolojik Parametreler

    Öz: Bu araştırmanın amacı, ülkemize ait yerli teke ırklarında başlıca spermatolojik parametreleri değerlendirmektir. Çalışmada her ırktan 2 olmak üzere 5 farklı ırk tekeden alınan spermalar muayene edildi. Toplam 100 ejakülatta başlıca spermatolojik parametreler değerlendirildi. Genel ortalama ejakülat miktarı 0.83±3.07 ml, kitle hareketi 3.34±1.10, spermatozoa motilitesi % 73.40±11.17, spermatozoa yoğunluğu 2.243±138.46X109/ml, anormal spermatozoa oranı % 9.38±4.02 ve spermanın pH değeri 7.20±2.65 kaydedildi. Başlıca spermatolojik parametreler yönüyle bireyler ve ırklar arasında önemli farklılıklar gözlendi (P<0.05, P< 0.01,  P<0.05). Araştırmada anormal spermatozoon tipleri ve oranları (%) akrozom, baş, orta kısım ve kuyruk için sırasıyla 1.3±0.1, 2.0±0.3, 2.1±0.2 ve 4.0±0.4 saptandı. İstatistik olarak bireyler ve ırklar arasında bulunan farklılıklar önemli kaydedildi. Sonuç olarak, farklı teke ırklarına ait başlıca spermatolojik parametreler ortaya konuldu, kimi spermatolojik özelliklerde bireyler ve ırklar arasındaki farklılıklar belirlendi. Elde edilen verilerin bundan sonra yapılacak özellikle teke spermasının dondurulması ve dölverimi ile ilgili çalışmalara ışık tutması düşünülmektedir.     Asthma is an inflammatory disease of the airways of the lungs, which is characterized by airflow obstruction and bronchospasms. Glabridin is a major flavonoid, especially found in root of Glycyrrhiza glabra, and has several pharmacological activities, including antioxidant and anti-inflammatory effects. The anti-asthmatic effect and possible mechanism of glabridin, however, have not been revealed so far. The aim of this study is to investigate the effects and possible mechanisms of glabridin against ovalbumin (OVA)-induced airway hyperresponsiveness (AHR) and inflammation in mice. In male BALB/c mice, asthma was induced by intraperitoneal (i.p) injection of OVA mixed with 2 mg aluminium hydroxide on days 0, 14 and boosted with OVA aerosol challenge on days 21, 22, and 23. Mice were either treated with dexamethasone (i.p, 1 mg/kg) or glabridin (10, 20, and 30 mg/kg) from days 18–23. Pulmonary function parameters such as peak inspiratory flow, peak expiratory flow, tidal volume, expiratory volume, the frequency of breathing, enhanced pause values were evaluated by using whole-body plethysmography. Measurements were performed at baseline and following methacholine (50 mg/mL) challenges. In addition, white blood cells (WBC) count, total protein, and IgE levels were measured in bronchial alveolar lavage fluid (BALF), lung, and serum, respectively. Glabridin (20 or 30 mg/kg) significantly attenuated (p < 0.05) OVA-induced alteration in respiratory parameters. Elevated counts of total WBC, differential WBC (neutrophils, lymphocytes, monocytes, and eosinophils) in BALF and the total protein in lungs and BALF were significantly decreased (p < 0.05) by glabridin (20 or 30 mg/kg). It also significantly attenuated the increased serum IgE levels (p < 0.05). As glabridin reduces the level of serum IgE, the total protein and the count of WBC and improves respiratory function, it may be a novel therapeutic agent in asthma. © 2020 Elsevier Lt
