Correlation of these molecular changes with clinical events has been explored. However, consistently useful markers have not been identified yet, although many promising developments are in progress. International Consensus Statement on Rhinology and Allergy: Rhinosinusitis
Background: The 5 years since the publication of the first International Consensus Statement on Allergy and Rhinology: Rhinosinusitis (ICARâRS) has witnessed foundational progress in our understanding and treatment of rhinologic disease. These advances are reflected within the more than 40 new topics covered within the ICARâRSâ2021 as well as updates to the original 140 topics. This executive summary consolidates the evidenceâbased findings of the document. Methods: ICARâRS presents over 180 topics in the forms of evidenceâbased reviews with recommendations (EBRRs), evidenceâbased reviews, and literature reviews. The highest grade structured recommendations of the EBRR sections are summarized in this executive summary. Results: ICARâRSâ2021 covers 22 topics regarding the medical management of RS, which are grade A/B and are presented in the executive summary. Additionally, 4 topics regarding the surgical management of RS are grade A/B and are presented in the executive summary. Finally, a comprehensive evidenceâbased management algorithm is provided. Conclusion: This ICARâRSâ2021 executive summary provides a compilation of the evidenceâbased recommendations for medical and surgical treatment of the most common forms of RS
Letales Mittelliniengranulom - Ein Fallbericht
Einleitung: Das sehr seltene extranodale Nk/T-Zell-Lymphom vom nasalen Typ, auch "letales Mittelliniengranulom", gehört in die Gruppe der Non-Hodgkin-Lymphome, ist mit einer Epstein-Barr-Virus (EBV)-Infektion assoziiert und durch einen sehr aggressiven Verlauf charakterisiert. Die mediane Ăberlebenszeit betrĂ€gt 6 Monate, die 5-Jahres-Ăberlebensrate 22%.Methoden: Es wird der Erkrankungsverlauf eines 72-jĂ€hrigen Patienten geschildert, der im MĂ€rz 2012 an den Nasennebenhöhlen operiert worden war. Danach kam es zu einer zunehmenden endonasalen Borkenbildung und EntzĂŒndung. Ergebnisse: Histologisch ergab sich der Verdacht auf einen M. Wegener, der laborchemisch nicht zu bestĂ€tigen war. Es wurde trotzdem eine Therapie mit Cotrimoxazol und Prednisolon begonnen. Darunter kam es zu einer Remission, die mit Azathioprin sowie Kortison in absteigender Dosierung weiterbehandelt worden war. Ca. 4 Wochen spĂ€ter, unter 10 mg Kortison kam es zu einer massiven Progression des endonasalen Befundes sowie zur Auftreibung der Oberlippe sowie der Wangen. Histologisch war nun ein EBV-assoziiertes, angiozentrisches bzw. angiodestruktives, peripheres T-Zell-Lymphoms vom nasalen Typ nachweisbar. Im durchgefĂŒhrten Ganzkörper-CT sowie einer Knochenmarkspunktion lieĂ sich keine weitere Ausbreitung des Lymphoms feststellen.Es wurde daraufhin mit einer Chemotherapie nach dem Chop-Protokoll (Cyclophosphamid, Hydroxydaunorubicin, OncovinÂź, Prednisolon) begonnen. Da es darunter ebenfalls nicht zu einer Besserung kam wurde auf Cytarabin (AraC) umgestellt. Darunter war der Lokalbefund langsam rĂŒcklĂ€ufig, und es wurde zusĂ€tzlich noch eine lokale Radiatio angeschlossen.Schlussfolgerungen: Bislang existieren keine definierten Therapieprotokolle fĂŒr diese sehr aggressive Erkrankung. Diskutiert werden eine sofortige hochdosierte Strahlentherapie versus primĂ€rer oder adjuvanter Chemotherapie.Der Erstautor gibt keinen Interessenkonflikt an
Brauchen wir eine spezielle postoperative Schmerztherapie in der HNO-Heilkunde?
Zielsetzung: Mit der vorlegenden Observations- Studie sollte die QualitĂ€t der postoperativen Schmerztherapie vor und nach Implementierung eines Algorithmus in der Hals-Nasen-Ohrenchirurgie ĂŒberprĂŒft werden.Methode: Nach Genehmigung durch die Ethik-Kommission, AufklĂ€rung und schriftlicher Einwilligung wurden 258 erwachsene HNO-Patienten in die Studie eingeschlossen. Die Observation fand in Phase1 (= herkömmliche Schmerztherapie) und Phase2 (= nach EinfĂŒhrung eines neuen Algorithmus) statt. Die QualitĂ€t der postoperativen Schmerztherapie wurde mit der Hilfe einer numerischen Analogskala (NRS) stĂŒndlich dokumentiert. Eine zufriedenstellende Analgesie wurde bei einem Durchschnittswert <3 auf der NRS angenommen. Die HĂ€ufigkeit von potenziellen Nebenwirkungen der Behandlung mit Analgetika, wurde in einem standardisierten Protokoll am zweiten Tag nach OP dokumentiert. Ergebnisse: Insgesamt konnte 168 Patienten ausgewertet werden. 90 Patienten wurden nicht eingeschlossen. In der Phase 1 wurde eine zufrieden stellende postoperative Analgesie bei folgenden OP- Gruppen dokumentiert: NNH- Operationen (n=35), Mittelgesichtsfrakturen (n=11), Ohr-Operationen (n=15), Septumplastik / Nasenmuschelplastik (n=42), Parotis- Chirurgie (n=20). Nach Tonsillektomie (n= 45) lag die durchschnittliche postoperative SchmerzintensitĂ€t zwischen 2,8 + 2,2 und 5,1 + 2,8. Schlussfolgerung: In der Phase 1 unserer Observationsstudie zeigte sich, dass ein Handlungsbedarf zur Verbesserung der postoperativen Schmerztherapie nach HNO-Operationen nur fĂŒr Tonsillektomien besteht. Von 113 Messzeitpunkten lag die durchschnittliche SchmerzintensitĂ€t in dieser Gruppe nur zu 13 Zeitpunkten im Zielbereich von NRS<3.Der Erstautor gibt keinen Interessenkonflikt an
