    The asymmetric unit of the title compound, C21H18ClN3O, contains two crystallographically independent mol­ecules which differ slightly in the orientations of chloro­benz­yloxy units. In one of the mol­ecules, the phenyl and chloro­phenyl rings are oriented at dihedral angles of 38.09 (6) and 42.15 (6)°, respectively, with respect to the benzotriazole ring [43.23 (6) and 29.80 (6)° in the other mol­ecule]. The dihedral angle between the phenyl and chloro­phenyl rings is 77.63 (6)° in one of the mol­ecules and 72.97 (6)° in the other. The crystal structure is stabilized by weak C—H⋯π inter­actions

    The Role of the Mediterranean Diet in the Prevention of Deep Vein Thrombosis

    Since anticoagulation performed by affecting hematological parameters has serious complications such as bleeding, studies are also conducted to benefit from diets instead of prophylactic anticoagulants in order to reduce the risk of developing venous thromboembolism (VTE). Our aim is to review the benefits of the Mediterranean diet in the prophylaxis and treatment of deep vein thrombosis (DVT), although the current studies have yet to be conclusive. The Mediterranean diet is characterized by the consumption of plant-based foods such as vegetables, fruits, nuts, legumes and unprocessed cereals, and seafood such as fish. The consumption of meat and meat products is low in this diet. The consumption of dairy products is also low. Fruits, vegetables and juices in the Mediterranean diet contain a significant amount of flavonoids. In addition, olive oil, which is one of the basic elements of the Mediterranean diet, has antioxidant properties due to its essential polyphenolic compounds. It has been reported that the low consumption of vegetables contributes to the increase of VTE incidence in Thai society. On the contrary, a diet with plenty of herbal food, fish and a small amount of red meat (Mediterranean diet) has been reported to reduce the risk of VTE. We think that the Mediterranean diet, which is beneficial for our health, may also decrease the risk of DVT. However, medical therapy is required for the treatment of patients with DVT.     Real-world efficacy and safety of Ledipasvir plus Sofosbuvir and Ombitasvir/Paritaprevir/Ritonavir +/- Dasabuvir combination therapies for chronic hepatitis C: A Turkish experience

    Background/Aims: This study aimed to evaluate the real-life efficacy and tolerability of direct-acting antiviral treatments for patients with chronic hepatitis C (CHC) with/without cirrhosis in the Turkish population.Material and Methods: A total of 4,352 patients with CHC from 36 different institutions in Turkey were enrolled. They received ledipasvir (LDV) and sofosbuvir (SOF)+/- ribavirin (RBV) ombitasvir/paritaprevir/ritonavir +/- dasabuvir (PrOD)+/- RBV for 12 or 24 weeks. Sustained virologic response (SVR) rates, factors affecting SVR, safety profile, and hepatocellular cancer (HCC) occurrence were analyzed.Results: SVR12 was achieved in 92.8% of the patients (4,040/4,352) according to intention-to-treat and in 98.3% of the patients (4,040/4,108) according to per-protocol analysis. The SVR12 rates were similar between the treatment regimens (97.2%-100%) and genotypes (95.6%-100%). Patients achieving SVR showed a significant decrease in the mean serum alanine transaminase (ALT) levels (50.90 +/- 54.60 U/L to 17.00 +/- 14.50 U/L) and model for end-stage liver disease (MELD) scores (7.51 +/- 4.54 to 7.32 +/- 3.40) (p<0.05). Of the patients, 2 were diagnosed with HCC during the treatment and 14 were diagnosed with HCC 37.0 +/- 16.0 weeks post-treatment. Higher initial MELD score (odds ratio [OR]: 1.92, 95% confidence interval [CI]: 1.22-2.38; p=0.023]), higher hepatitis C virus (HCV) RNA levels (OR: 1.44, 95% CI: 1.31-2.28; p=0.038), and higher serum ALT levels (OR: 1.38, 95% CI: 1.21-1.83; p=0.042) were associated with poor SVR12. The most common adverse events were fatigue (12.6%), pruritis (7.3%), increased serum ALT (4.7%) and bilirubin (3.8%) levels, and anemia (3.1%).Conclusion: LDV/SOF or PrOD +/- RBV were effective and tolerable treatments for patients with CHC and with or without advanced liver disease before and after liver transplantation.     The aims of this study were to investigate the role of endothelin-1 in FK506-induced hypertension and vascular dysfunction of rats treated with the drug for 8 (short-term) or 30 (long-term) days and to measure malondialdehyde levels in the kidneys. Kidney and mesentery of rats were perfused. In the short-term treated groups, there was no significant change in systolic blood pressure. The response to noradrenaline only in renal vascular beds was significantly increased by FK506 and this increase was prevented by Bosentan. FK506 had no significant effect on sodium nitroprusside-induced vasodilation in comparison with solvent in both vascular beds. Bosentan failed to prevent these responses. In the long-term treated groups, at the end of the treatment with FK506, there was a significant increase in blood pressure, but no change in the response to noradrenaline in either kidneys or mesentery. The increase in blood pressure was prevented by bosentan treatment. FK506 increased malondialdehyde levels in the kidneys of the rats from only the long-term treated groups. Bosentan did not change this increase. Our results indicated that endothelin-1 plays a key role in the FK506-induced change in vascular reactivity to noradrenaline in renal vascular beds and drug-induced hypertension in the rats.     Effects of Quadriceps Muscle Strength and Kinesiophobia on Health Related Quality of Life in Elderly Women and Men with Chronic Obstructive Pulmonary Disease

Methods: After recording the socio -demographic information of individuals, the Visual Analogue Scale (VAS) for pain status, the modified Medical Research Council Scale (mMRCS) for dyspnea, TAMPA Scale for Kinesiophobia (TSK), Saint George Respiratory Questionnaire (SGRQ) for health-related quality of life, and manual dynamometer were used to measure for muscle strength of quadriceps. Results: Seventeen (37.8%) of the 45 participants were female. The mean age and body mass index of females were 71.59 +/- 4.84 years and 28.75 +/- 6.71 kg/m(2), respectively. Kinesiophobia averages were 47.47 +/- 7.40 and 41.89 +/- 7.65 in women and men, respectively, and SGRQ((Symptom)) scores were 75.97 +/- 15.87 and 63.89 +/- 18.54 in women and men, respectively. Statistical analysis showed that there was a significant difference between men and women between kinesiophobia and SGRQ((symptom)) (p0.05). Conclusion: In elderly individuals with COPD, kinesiophobia is an important factor affecting the quality of life and is recommended to increase physical activity, and monitor the physiological, emotional well-being in order to improve quality of life
