    Objective Structured Clinical Examination’s adaptation for final evaluation of the curricular component “Semiological Bases”

    BACKGROUND: The combination of both physiological knowledge of a variety of sistems and the clinical findings it is of extreme relevance to the medical student learning process. The proposal of a simulated enviroment, with clinical data collection and regular physical examination, is crucial to this process. The Semiological bases of Escola Bahiana de Medicina e Saúde Pública module, used by the OSCE’s 4th semester medical students, capacitate them for such means. SUMMARY OF WORK: The evaluation logistic was divided into two segments: the first one was made through a theorical exam with close questioning, exhibiting images and videos, but also containing an open-ended question requesting a construction of a fictional clinical case; in the second one it was applied a practical exam segmented by different fields of medical knowledge (neurological, cardiovascular and respiratory/abdomen), with the evaluation of 120 students and followed by the teacher’s feedbacks at the conclusion of each field exam. SUMMARY OF RESULTS: The experience was important, however, it is notable the need for a complex management and understanding of the evaluational process. Discussion between students and teachers happened in behalf of the development of such proposal. CONCLUSIONS: The application of theorical exams throughout the semester combined with a practical exam (the OSCE) at the conclusion of the curricular component Bases Semiológicas had significant weight to the whole process

    Molecular epidemiological investigation of Mayaro virus in febrile patients from Goiania City, 2017-2018.

    Mayaro virus (MAYV) has historically been associated with sylvatic transmission; however, urban outbreaks have been reported in Brazil, including cases of co-detection with dengue virus (DENV). Therefore, we performed a molecular survey to investigate MAYV circulation and cocirculation with DENV within Goiania, a major city in Central-West Brazil. Among 375 subjects with arbovirus-like symptoms, 259 were positive for DENV and 26 for MAYV. Of these, 17 were coinfected with DENV-2, suggesting co-transmission of the viruses. The most common complaints at the time of inclusion were myalgia, headache, fever, arthralgia, retro-orbital pain, and skin rash. No specific symptoms were associated with MAYV when either detected alone or co-detected with DENV, compared to that when DENV was detected alone. Most MAYV-infected subjects were women with no recent travel history to rural/sylvatic areas. Phylogenetic reconstruction indicated that the MAYV identified in this study is closely related with a lineage observed in Peru, belonging to genotype D. Our results corroborate the growing circulation of MAYV in urban environments in Brazil and reinforce the need to implement laboratory diagnosis in the Unified Health System, considering that the clinical manifestations of Mayaro fever are similar to those of other arboviruses, particularly dengue. Furthermore, most cases occurred in association with DENV-2. Further phylogenetic studies are needed to evaluate MAYV, which has not been widely examined

    As patologias neurodegenerativas cursam com depleção progressiva e irreversível dos neurônios existentes em regiões específicas do cérebro. A Doença de Parkinson (DP) é um protótipo, na qual o extravio neuronal do hipocampo e do córtex resulta em déficit de memória e disfunção cognitiva. O seguinte artigo objetivou descrever de modo narrativo as considerações clínicas da doença de Parkinson que justifiquem a ação farmacológica dos fármacos empregados em sua terapêutica. Atualmente, a intervenção farmacológica e a cirúrgica não são capazes de reverter o quadro clínico, mas evitam a progressão da morbimortalidade da DP. O tratamento é individual, baseado na reação específica, o quadro clínico, resposta farmacológica e aspectos socioeconômicos, ocupacionais e emocionais. A finalidade se baseia em perpetuar a autonomia e funcionalidade, o máximo de tempo possível. A escolha dos fármacos mais apropriados para cada paciente e o início do tratamento e o acompanhamento ao longo da evolução são etapas difíceis. Devido a cronicidade, o tratamento deve continuar por toda a vida, considerando que os fármacos e suas doses mudam com o tempo, o surgimento de efeitos adversos

    A rinossinusite é um processo inflamatório da mucosa dos seios paranasais e da cavidade nasal. O sistema nasossinusal é responsável pelo balanço adequado entre a fabricação e o clearence de muco nas cavidades paranasais. A fisiologia deste é de vital importância para a proteção das vias aéreas superiores. No advém, determinados fatores podem acarretar um desbalanço nesse complexo, consequentemente um processo inflamatório. Qualquer fator que altere a drenagem, seja por obstrução, maior produção ou espessamento do muco, como processo infecciosos ou alérgicos, haverá uma impactação de secreções e a facilitação de colonização bacteriana, dando início ao processo infeccioso. A identificação da inflamação do nariz e seios paranasais é basicamente clínica. A suspeição desta ocorre através da manifestação de dois ou mais sintomatologias. As quais são o bloqueio ou obstrução nasal, a descarga nasal, pressão ou dor facial e redução ou perda do olfato. De modo geral, é essencial à prevenção básica das rinossinusites agudas é barrar a infecção viral. O suporte inclui medidas gerais de higiene, alimentação e hidratação, imunização para o combate de vírus respiratórios , administração de fármacos para turbinar o sistema imune se necessário

    Guidelines for the management of neuroendocrine tumours by the Brazilian gastrointestinal tumour group

    Neuroendocrine tumours are a heterogeneous group of diseases with a significant variety of diagnostic tests and treatment modalities. Guidelines were developed by North American and European groups to recommend their best management. However, local particularities and relativisms found worldwide led us to create Brazilian guidelines. Our consensus considered the best feasible strategies in an environment involving more limited resources.     Breast and ovarian cancer are the most common cancers in women. Available cancer treatments, in general, have limited efficacy and frequent, undesirable side effects. Recently, scientists have focused on searching for new epigenetic modulators such as inhibitors of DNA methyltransferases and histone deacetylases (HDACs), with novel properties and selectivity. We report the synthesis of seven new analogs of Santacruzamate A. Molecular modeling showed that compounds 3?9 presented the best binding energies (kcal/mol) against HDAC4 compared to that of crystallographic ligand. The compounds were evaluated against MCF-7 and MDA-MB-231 (breast cancer), TOV-21G (ovarian adenocarcinoma), and WI-26VA4 (non-tumor lung fibroblasts) cells. Compound 5, the most potent and selective of the series, exhibited remarkably enhanced anticancer potency, with IC50 values for the tumor cells of 24.3?44.93??M, compared with that of etoposide (12?18.57??M) and doxorubicin (2.1?4.37??M). Further investigation showed that compound 5 could promote DNA damage, increase the activity of caspases-3 and -9, and upregulate mRNA levels of p21, TP53, and BAK, suggesting apoptotic cell death of the tumor cells via the intrinsic pathway. This study demonstrated that synthetic analogs of santacruzamate A with zinc-linked groups are effective for improving both HDAC inhibition and antitumor activity
