Initial Experience with Radical Prostatectomy Following Holmium Laser Enucleation of the Prostate
BACKGROUND: Although an increasing number of prostate cancer (PCa) patients received holmium laser enucleation of the prostate (HoLEP) previously for benign prostatic obstruction (BPO), there is still no evidence regarding the outcomes of radical prostatectomy (RP) in this setting. OBJECTIVE: To assess functional and oncological results of RP in PCa patients who received HoLEP for BPO previously in a contemporary multi-institutional cohort. DESIGN, SETTING, AND PARTICIPANTS: A total of 95 patients who underwent RP between 2011 and 2019 and had a history of HoLEP were identified in two institutions. Functional as well as oncological follow-up was prospectively assessed and retrospectively analyzed. INTERVENTION: RP following HoLEP compared with RP without previous transurethral surgery. OUTCOME MEASUREMENTS AND STATISTICAL ANALYSIS: Patients with complete follow-up data were matched with individuals with no history of BPO surgery using propensity score matching. Complications were assessed using the Clavien-Dindo scale. RESULTS AND LIMITATIONS: The median follow-up was 50.5 mo. We found no significant impact of previous HoLEP on positive surgical margin rate (14.0% [HoLEP] vs 18.8% [no HoLEP], p = 0.06) and biochemical recurrence-free survival (hazard ratio 0.74, 95% confidence interval [CI] 0.32-1.70, p = 0.4). Patients with a history of HoLEP had increased 1-yr urinary incontinence rates after RP. After adjusting for confounders, no significant impact of previous HoLEP was found (odds ratio [OR] 0.87, 95% CI 0.74-1.01; p = 0.07). Previous HoLEP did not hamper 1-yr erectile function recovery (OR 1.22, 95% CI 1.05-1.43; p = 0.01). Limitations include retrospective design and small sample size. CONCLUSIONS: RP after previous HoLEP is surgically feasible, with low complication rates and no negative impact on biochemical recurrence-free survival. However, in a multivariable analysis, we observed significantly worse 1-yr continence rates in patients after previous HoLEP. PATIENT SUMMARY: In the current study, we assessed the oncological and functional outcomes of radical prostatectomy in patients who underwent holmium laser enucleation of the prostate (HoLEP) previously due to prostatic bladder outlet obstruction. Analysis of efficiency and profitability of franchise services
The present study analyses the relative efficiency of franchise services and
characterises the best companies, confirming the relationship between efficiency and
profit. These companies are from the trade and other services sector , the main
group of service-providing companies in the Spanish economy. The methodology
calls for first comparing the relative efficiency of franchisers and ownership
enterprises. Second, the focus turns to the most efficient franchise services, using a
super-efficiency model to rank them. The paper then goes on to cover the analysis of
the main characteristics of the best franchise enterprises, the number of own
establishments in a franchise business and the profitability of the company. This
paper presents arguments as to why companies from the trade and other services
sector are included. The main conclusion is that, whilst the number of
establishments is irrelevant in achieving greater efficiency, many of the most

Assessment of a novel smartglass-based point-of-care fusion approach for mixed reality-assisted targeted prostate biopsy: A pilot proof-of-concept study
PurposeWhile several biopsy techniques and platforms for magnetic resonance imaging (MRI)-guided targeted biopsy of the prostate have been established, none of them has proven definite superiority. Augmented and virtual reality (mixed reality) smartglasses have emerged as an innovative technology to support image-guidance and optimize accuracy during medical interventions. We aimed to investigate the benefits of smartglasses for MRI-guided mixed reality-assisted cognitive targeted biopsy of the prostate.MethodsFor prospectively collected patients with suspect prostate PIRADS lesions, multiparametric MRI was uploaded to a smartglass (Microsoft® Hololens I), and smartglass-assisted targeted biopsy (SMART TB) of the prostate was executed by generation of a cognitive fusion technology at the point-of-care. Detection rates of prostate cancer (PCA) were compared between SMART TB and 12-core systematic biopsy. Assessment of SMART-TB was executed by the two performing surgeons based on 10 domains on a 10-point scale ranging from bad (1) to excellent (10).ResultsSMART TB and systematic biopsy of the prostate were performed for 10 patients with a total of 17 suspect PIRADS lesions (PIRADS 3, n = 6; PIRADS 4, n = 6; PIRADS 5, n = 5). PCA detection rate per core was significant (p < 0.05) higher for SMART TB (47%) than for systematic biopsy (19%). Likelihood for PCA according to each core of a PIRADS lesion (17%, PIRADS 3; 58%, PIRADS 4; 67%, PIRADS 5) demonstrated convenient accuracy. Feasibility scores for SMART TB were high for practicality (10), multitasking (10), execution speed (9), comfort (8), improvement of surgery (8) and image quality (8), medium for physical stress (6) and device handling (6) and low for device weight (5) and battery autonomy (4).ConclusionSMART TB has the potential to increase accuracy for PCA detection and might enhance cognitive MRI-guided targeted prostate biopsy in the future

Factors Influencing Households’ Intention to Adopt Solar PV : A Systematic Review
Rising energy needs, concerns of energy security, mitigating greenhouse gas emissions, climate change phenomenon and a push to utilize indigenous sources for energy generation purposes has encouraged the use of solar photovoltaics (PV). The technological advancements of the recent past, improvement in technologies’ performance, reduction in the prices, policy and regulatory support, and its applicability at household level has made solar energy as a preferred form of energy generation. However, despite its rapid diffusion, it is widely believed that its current application is insignificant compared to its potential. This leads us to ask why solar PV has not been adopted to the level it should have. The existing literature has highlighted a number of factors affecting solar PV adoption. This paper systematically reviews the literature to identify the factors that have been instrumental to solar PV adoption. By exploring the Scopus database, this research identifies 39 articles matching the study objectives. Findings of this research will help academics, technology companies and policymakers in understanding the factors influencing the process and proposing solutions to address these.©2020 Springer. Evaluando el progreso de la eficiencia con tecnología en una cadena de hoteles española
This paper analyzes the changes in the total factor productivity index of a Spanish hotel chain in the
period from 2007 to 2010 with the purpose of identifying efficiency patterns for the chain in a period of
financial crisis. The data envelopment analysis (DEA) Malmquist productivity index was used to estimate
productivity change in 38 hotels of the AC chain. Results reveal AC hotels’ efficiency trends and,
therefore, their competitiveness in the recession period; they also show the changes experienced in
these hotels’ total productivity and its components: technological and efficiency changes. Positive
efficiency changes were due to positive technical efficiency rather than technological efficiency. The
recession period certainly influenced the performance of AC Hotels, which focused on organizational
changes rather than investing in technology.Este artigo analisa as mudanças no fator total de produtividade de uma cadeia de hotéis na Espanha,
no período de 2007-2010, com o propósito de identificar os padrões da cadeia em um período
de crise financeira. O índice data envelopment analysis (DEA) Malmquist de produtividade foi usado
para estimar a mudança da produtividade nos 38 hotéis da AC Cadeia de Hotéis. Os resultados revelaram
as tendências de eficiência e competitividade da AC Hotéis em um período de recessão, bem
como as mudanças vivenciadas na produtividade total e, consequentemente, em seus componentes
de eficiência e tecnológicos. O período de recessão influenciou, sem dúvida, o comportamento da AC
Hotéis, que buscou mais mudanças organizacionais do que tecnológicas.Este artículo analiza los cambios del índice de productividad del factor total de una cadena de hoteles
españoles en el periodo de 2007 hasta 2010, con el propósito de identificar patrones de eficiencia
para la cadena en un periodo de crisis financiera. El índice de productividad data envelopment analysis
(DEA) Malmquist fue utilizado para estimar el cambio de productividad en 38 hoteles de la cadena
AC. Los resultados revelan las tendencias de la eficiencia de los hoteles AC y, por lo tanto, su competitividad
en el periodo de recisión; ellos también demuestran los cambios experimentados en la
productividad total de eses hoteles y sus componentes: cambios de eficiencia y tecnológicos. Cambios
de eficiencia positivos se debieron más bien a eficiencias técnicas positivas que a eficiencias tecnológicas.
