    Low Resources in a High Stakes Game: Identifying Viable Rural Community Partners

    Extension resources are shrinking, yet community leadership needs are great, and, the consequences of neglecting them are dire. It is difficult to respond to all the requests that are made of Extension faculty and even more difficult to decide which of the communities will benefit the most from programming. This article illuminates these issues by examining contributions from related research. First, a link is forged between community capital theory and community survival indicators. Next, 111 signs are provided that identify community viability. Finally, a guide is proposed for use in Extension to help determine where to concentrate scant resources

    La pratique dans un contexte pluriethnique : Démarche en vue de la création d’une approche de négociation entre le personnel des services de soutien à domicile et les proches-aidantes d’un parent âgé

    Cette étude qualitative visait à mettre à l’essai une démarche pour la création d’une approche de négociation entre le personnel des services de soutien à domicile et des proches-aidantes d’un parent âgé, en vue de formuler des recommandations pour une offre de services culturellement sensibles. L’approche repose sur un cadre conceptuel de négociation qui tient compte des perspectives paradigmatiques des parties concernées : les utilisateurs et prestataires de services. L’étude a été réalisée auprès d’aidantes d’origine haïtienne, en tant que cas traceurs, et auprès d’intervenants et d’intervenantes des services de soutien à domicile. Les données révèlent que les attentes et solutions concernant les services sont largement convergentes entre ces deux groupes d’acteurs et portent notamment sur les barrières à l’utilisation des services et les relations entre les aidantes et les intervenants ou intervenantes. Les principales recommandations issues des détenteurs d’enjeux concernent la formulation d’une politique de soutien des aidantes, la mise en place d’équipes ethnoculturelles de quartier, l’établissement de liens avec les organismes communautaires et la formation interculturelle des intervenants. Cette démarche fournit des pistes pour une pratique basée sur la négociation dans des contextes culturels divers.The aim of this qualitative study was to test an approach to negotiations between home support service personnel and the family caregivers of an elderly relative, with a view to formulating recommendations for a culturally adjusted service supply. Based on a conceptual model of negotiations, the approach takes into account the paradigmatic perspectives of the stakeholders, namely, service providers and service users. The study was conducted with caregivers of Haitian origin, which served as the tracer condition, and with home support service practitioners. Data indicate that expectations and solutions regarding services are widely convergent between the two groups of actors and have to do in particular with service utilization barriers and caregiver-practitioner relations. The main recommendations concern service supply and coordination, community organization and practitioner training. This approach provides leads for practice in different cultural contexts

    Toward a conceptual model of executive coaching practices in organizations in the United States: A Modified Delphi Forecasting study

    This study examined the phenomenon of executive coaching, a leadership development strategy, from the perspective of executive coaches practicing in the field. An extensive literature review provided insight to the identification of the various components utilized in an executive coaching program. The current literature on executive coaching fails to articulate a coherent model of executive coaching. A Modified Delphi Forecasting method incorporating a mixed-method design was conducted to gather information on the strategies, and processes of executive coaching. The dominant quantitative portion of the data collection was acquired through Likert-type scale item questions that asked respondents to rate items in these areas. The secondary qualitative analysis was conducted to identify common phases of the coaching process and to elaborate on key concepts of the study. Data analysis revealed group consensus in several key areas of executive coaching, including coaching techniques, the skills, and the relationship aspects essential to the clarification and further understanding of coaching practices in organizations in the United States. An eight-phase conceptual model emerged from expertise of executive coaches and the integration of concepts informed through the literature. Findings revealed group consensus in several key areas, including: specific coaching techniques, client outcomes, aspects of the coach and the client relationship, and the necessary components in the coaching process. Executive coaching is still intangible in its clarification of the definition, the techniques and the processes, yet the findings of this study provide direction for the field

    Enzalutamide for the treatment of androgen receptor-expressing triple-negative breast cancer

    Purpose Studies suggest that a subset of patients with triple-negative breast cancer (TNBC) have tumors that express the androgen receptor (AR) and may benefit from an AR inhibitor. This phase II study evaluated the antitumor activity and safety of enzalutamide in patients with locally advanced or metastatic AR-positive TNBC. Patients and Methods Tumors were tested for AR with an immunohistochemistry assay optimized for breast cancer; nuclear AR staining .0% was considered positive. Patients received enzalutamide 160 mg once per day until disease progression. The primary end point was clinical benefit rate (CBR) at 16 weeks. Secondary end points included CBR at 24 weeks, progression-free survival, and safety. End points were analyzed in all enrolled patients (the intent-to-treat [ITT] population) and in patients with one or more postbaseline assessment whose tumor expressed $ 10% nuclear AR (the evaluable subgroup). Results Of 118 patients enrolled, 78 were evaluable. CBR at 16 weeks was 25% (95% CI, 17% to 33%) in the ITT population and 33% (95% CI, 23% to 45%) in the evaluable subgroup. Median progression-free survival was 2.9 months (95% CI, 1.9 to 3.7 months) in the ITT population and 3.3 months (95% CI, 1.9 to 4.1 months) in the evaluable subgroup. Median overall survival was 12.7 months (95% CI, 8.5 months to not yet reached) in the ITT population and 17.6 months (95% CI, 11.6 months to not yet reached) in the evaluable subgroup. Fatigue was the only treatment-related grade 3 or higher adverse event with an incidence of .2%. Conclusion Enzalutamide demonstrated clinical activity and was well tolerated in patients with advanced AR-positive TNBC. Adverse events related to enzalutamide were consistent with its known safety profile.
