    Respiratory rate estimation from multi-channel signals using auto-regulated adaptive extended Kalman filter

    Background: Respiration rate (RR) is a major cause for false alarms in intensive care units (ICU) and is primarily impaired by the artifact prone signals from skin-attached electrodes. Catheter-integrated esophageal electrodes are an alternative source for multi-channel physiological signals from multiple organs such as the heart and the diaphragm. Nonlinear estimation and sensor fusion are promising techniques for extracting the respiratory activity from such multi-component signals, however, pathologic breathing patterns with rapid RR changes typically observed in patient populations such as premature infants, pose significant challenges. Methods: We developed an auto-regulated adaptive extended Kalman filter (AA-EKF), which iteratively adapts the system model and the noise parameters based on the respiratory pattern. AA-EKF was tested on neonatal esophageal observations (NEO), and also on simulated multi-components signals created using waveforms in CapnoBase and ETNA databases. Results: AA-EKF derived RR (RRAA-EKF) from NEO had lower median (inter-quartile range) error of 0.1 (10.6) breaths per minute (bpm) compared to contemporary neonatal ICU monitors (RRNICU): −3.8 (15.7) bpm (p <0.001). RRAA-EKF error of −0.2 (3.2) bpm was achieved for ETNA wave forms and a bias (95% LOA) of 0.1 (−5.6, 5.9) in breath count. Mean absolute error (MAE) of RRAA-EKF with Capnobase waveforms, as median (inter-quartile range), at 0.3 (0.2) bpm was comparable to the literature reported values. Discussion: The auto-regulated approach allows RR estimation on a broad set of clinical data without requiring extensive patient specific adjustments. Causality and fast response times of EKF based algorithms makes the AA-EKF suitable for bedside monitoring in the ICU setting

    A two-center pilot study on the effects of clinical ethics support on coercive measures in psychiatry.

    BACKGROUND The use of formal coercion such as seclusion, mechanical restraint, and forced medication is one of the most challenging and complex issues in mental health care, on the clinical, the legal, and the ethical level. Clinical ethics support aims at assisting healthcare practitioners in determining the morally most justifiable course of action in these situations. However, the effectiveness of clinical ethics support has hardly been studied so far. METHODS Monthly moral case deliberation (MCD) was implemented in two acute wards of two different psychiatric hospitals in Switzerland. Frequency and intensity of coercion was measured on ward level (npatients = 405), and the Moral Attentiveness Scale, Knowledge on Coercion Scale, and Staff Attitudes towards Coercion Scale were applied on healthcare practitioner level (nHP = 46). Pre-post-comparisons were conducted using multi-level modeling where appropriate. RESULTS After implementation of MCD, formal coercion was less frequent (particularly seclusion, small effect size; 9.6 vs. 16.7%, p = .034, Cramér's V = .105) and less intense (particularly mechanical restraint, large effect size; 86.8 ± 45.3 vs. 14.5 ± 12.1 h, exact p = .019, r = -.74), and approval for coercive measures among healthcare practitioners was lower when controlling for the number of MCD sessions attended. CONCLUSIONS Clinical ethics support such as MCD may be a hitherto underutilized service for the reduction of coercion, complementing existing strategies and programs. Implementing clinical ethics support may help improve quality of care for persons suffering from severe mental illness

    Torpor, arousal and activity of hibernating greater horseshoe bats (Rhinolophus ferrumequinum)

Patterns of torpor, arousal, and activity in free-living greater horseshoe bats, Rhinolophus ferrumequinum, were investigated during the hibernation period by using temperature-sensitive radio-transmitters. 2. Torpor bouts varied between 0.1 - 11.8 days, with individual means ranging from 1.3 - 7.4 days. Torpor bout duration decreased with increasing ambient temperature. 3. Activity duration varied from 37 minutes – 54 hours 24 minutes, with individual means ranging from 2:29 to 8:58 hours. Activity duration increased with ambient temperatures above approximately 10oC. 4. Ten of 11 bats synchronised their arousals with dusk. The circadian rhythm of one bat showed a free-running pattern over a period of about five weeks. Arousals were more highly synchronised, and closer to dusk, in individuals with lower body condition. 5. That bats forage in mild weather is supported by the strong synchronisation of arousals with dusk, especially in bats with low body condition. 6. Patterns of torpor and subsequent activity are consistent with predictions that torpor lasts until a critical metabolic or water imbalance is achieved. Because metabolism and water loss are temperature-dependent, torpor bout duration decreases with increasing temperature. The imbalance is corrected during subsequent activity, which is relatively constant in duration until a temperature threshold of 10oC, above which increasing levels of foraging lead to longer activity bouts

    Data accuracy, consistency and completeness of the national Swiss cystic fibrosis patient registry: Lessons from an ECFSPR data quality project.

    BACKGROUND Good data quality is essential when rare disease registries are used as a data source for pharmacovigilance studies. This study investigated data quality of the Swiss cystic fibrosis (CF) registry in the frame of a European Cystic Fibrosis Society Patient Registry (ECFSPR) project aiming to implement measures to increase data reliability for registry-based research. METHODS All 20 pediatric and adult Swiss CF centers participated in a data quality audit between 2018 and 2020, and in a re-audit in 2022. Accuracy, consistency and completeness of variables and definitions were evaluated, and missing source data and informed consents (ICs) were assessed. RESULTS The first audit included 601 out of 997 Swiss people with CF (60.3 %). Data quality, as defined by data correctness ≥95 %, was high for most of the variables. Inconsistencies of specific variables were observed because of an incorrect application of the variable definition. The proportion of missing data was low with 5 % of missing documents). After providing feedback to the centers, availability of genetic source data and ICs improved. CONCLUSIONS Data audits demonstrated an overall good data quality in the Swiss CF registry. Specific measures such as support of the participating sites, training of data managers and centralized data collection should be implemented in rare disease registries to optimize data quality and provide robust data for registry-based scientific research

    Accelerating root system phenotyping of seedlings through a computer-assisted processing pipeline

    Background: There are numerous systems and techniques to measure the growth of plant roots. However, phenotyping large numbers of plant roots for breeding and genetic analyses remains challenging. One major difficulty is to achieve high throughput and resolution at a reasonable cost per plant sample. Here we describe a cost-effective root phenotyping pipeline, on which we perform time and accuracy benchmarking to identify bottlenecks in such pipelines and strategies for their acceleration. Results: Our root phenotyping pipeline was assembled with custom software and low cost material and equipment. Results show that sample preparation and handling of samples during screening are the most time consuming task in root phenotyping. Algorithms can be used to speed up the extraction of root traits from image data, but when applied to large numbers of images, there is a trade-off between time of processing the data and errors contained in the database. Conclusions: Scaling-up root phenotyping to large numbers of genotypes will require not only automation of sample preparation and sample handling, but also efficient algorithms for error detection for more reliable replacement of manual interventions

    Comparison of clinical characteristics and healthcare resource use of pediatric chronic and non-chronic critically ill patients in intensive care units: a retrospective national registry study

    IntroductionChronic critically ill patients (CCI) in pediatric intensive care unit (PICU) are at risk of negative health outcomes, and account for a considerable amount of ICU resources. This study aimed to (a) describe the prevalence of CCI children, (b) compare their clinical characteristics and ICU resources use with non-CCI children, and (c) identify associated risk factors of CCI.MethodsA retrospective national registry study including 2015–2017 data from the eight Swiss PICUs of five tertiary and three regional hospitals, admitting a broad case-mix of medical and surgical patients, including pre- and full-term infants. CCI patients were identified using an adapted definition: PICU length of stay (LOS) ≥8 days and dependence on ≥1 PICU technology.ResultsOut of the 12,375 PICU admissions, 982 (8%) were CCI children and compared to non-CCI children, they were younger (2.8 vs. 6.7 months), had more cardiac conditions (24% vs. 12%), and higher mortality rate (7% vs. 2%) (p &lt; 0.001). Nursing workload was higher in the CCI compared to the non-CCI group (22 [17–27]; 21 [16–26] respectively p &lt; 0.001). Factors associated with CCI were cardiac (aOR = 2.241) and neurological diagnosis (aOR = 2.062), surgery (aORs between 1.662 and 2.391), ventilation support (aOR = 2.278), high mortality risk (aOR = 1.074) and agitation (aOR = 1.867).Conclusionthe results confirm the clinical vulnerability and the complexity of care of CCI children as they were defined in our study. Early identification and adequate staffing is required to provide appropriate and good quality care

    Supportive and palliative care for people with respiratory problems and pre-existing serious mental illness

    PURPOSE OF REVIEW: People living with serious mental illness are at a higher risk of developing respiratory problems that can lead to increased morbidity and early mortality. This review aimed to identify recent advances in care provision for people with respiratory problems and preexisting serious mental illness to ease symptom burden and reduce the risk of premature mortality. RECENT FINDINGS: Intervention-based studies in this area are scarce. The evidence reviewed originated from observational studies. Concluding comments from the synthesis suggest there are specific needs for proactive screening of respiratory function as part of routine physical health checks across care settings for people living with serious mental illness, more stringent monitoring of comorbid chronic lung conditions and increased attention in reducing the frequency respiratory infections. Integrated services across care settings are needed to support people with serious mental illness to limit the impact of modifiable lifestyle factors known to be detrimental to respiratory health, such as smoking. SUMMARY: Key priorities are identified to improve accessibility and inclusivity of respiratory care pathways for people living with serious mental illness to support early detection and proactive monitoring of respiratory problems to help reduce the risk of early mortality
