    Staphylococcal cassette chromosome mec typing and meca sequencing in methicillin-resistant staphylococci from Algeria: A highly diversified element with new mutations in mecA

    Genetic mechanisms of methicillin resistance are still relevant in staphylococci. The aims of this study are to assess the possible exchanges of staphylococcal cassette chromosome mec (SCCmec) among isolates of methicillin-resistant staphylococci (MRS) and to check for known or new mutations in mecA DNA. A total of 35 MRS non-repetitive isolates were recovered, including 20 Staphylococcus haemolyticus, 7 Staphylococcus aureus, 4 Staphylococcus sciuri, 2 Staphylococcus saprophyticus and 1 isolate each of Staphylococcus xylosus and Staphylococcus lentus. Only 16 of the 35 strains were assigned to known SCCmec types: 7 SCCmec VII, 6 SCCmec IV and 3 SCCmec III, with possible horizontal transfer of the SCCmec VII from methicillin-resistant S. haemolyticus to methicillin-susceptible S. aureus. mecA gene sequencing in ten selected isolates allowed description of nine punctual mutations, seven of which were reported for the first time. The most frequent mutation was G246E, identified in isolates of methicillin-resistant S. aureus, S. sciuri, S. saprophyticus and S. lentus. These results emphasized the high degree of genetic diversity of SCCmec element in MRS and describe new missense mutations in mecA, which might be important in understanding the evolution of methicillin and new b-lactam resistance

    Superoxide reductase from Desulfoarculus baarsii: reaction mechanism and role of glutamate 47 and lysine 48 in catalysis.

    International audienceSuperoxide reductase (SOR) is a small metalloenzyme that catalyzes reduction of O(2)(*)(-) to H(2)O(2) and thus provides an antioxidant mechanism against superoxide radicals. Its active site contains an unusual mononuclear ferrous center, which is very efficient during electron transfer to O(2)(*)(-) [Lombard, M., Fontecave, M., Touati, D., and Nivière, V. (2000) J. Biol. Chem. 275, 115-121]. The reaction of the enzyme from Desulfoarculus baarsii with superoxide was studied by pulse radiolysis methods. The first step is an extremely fast bimolecular reaction of superoxide reductase with superoxide, with a rate constant of (1.1 +/- 0.3) x 10(9) M(-1) s(-1). A first intermediate is formed which is converted to a second one at a much slower rate constant of 500 +/- 50 s(-1). Decay of the second intermediate occurs with a rate constant of 25 +/- 5 s(-1). These intermediates are suggested to be iron-superoxide and iron-peroxide species. Furthermore, the role of glutamate 47 and lysine 48, which are the closest charged residues to the vacant sixth iron coordination site, has been investigated by site-directed mutagenesis. Mutation of glutamate 47 into alanine has no effect on the rates of the reaction. On the contrary, mutation of lysine 48 into an isoleucine led to a 20-30-fold decrease of the rate constant of the bimolecular reaction, suggesting that lysine 48 plays an important role during guiding and binding of superoxide to the iron center II. In addition, we report that expression of the lysine 48 sor mutant gene hardly restored to a superoxide dismutase-deficient Escherichia coli mutant the ability to grow under aerobic conditions

    Descriptive Epidemiology of Nasal Carriage of Staphylococcus aureus and Methicillin-Resistant Staphylococcus aureus Among Patients Admitted to Two Healthcare Facilities in Algeria

    Aim: To evaluate nasal carriage rate and variables associated with Staphylococcus aureus and methicillin-resistant Staphylococcus aureus (MRSA) in patients admitted in two healthcare facilities. Results: S. aureus was isolated from 159 (26%) of the enrolled patients. Methicillin-susceptible S. aureus was isolated from 150 (24.5%) patients, and MRSA was isolated from 9 (1.5%). Cancer and previous hospitalization were associated with a significantly higher frequency of nasal S. aureus carriage among the patients admitted to the general hospital and the nephrology department, respectively. MRSA isolates were heterogeneous with respect to their staphylococcal cassette chromosome mec element (SCCmec) type, sequence type (ST), and toxin genes (pvl and tst1) content. Four isolates were attributed with the ST80-MRSA-IV clone, which is known to be predominant in Algeria. Conclusions: This is the first assessment of S. aureus and MRSA nasal carriage and associated variables in Algeria. Our findings provide also a picture of the MRSA strains circulating in the community in this geographic area. They can be useful as a guide for implementing screening and control procedures against S. aureus/MRSA in the Algerian healthcare facilities

    Liquid-based cytology for primary cervical cancer screening: a multi-centre study

    The aim of this six-centre, split-sample study was to compare ThinPrep fluid-based cytology to the conventional Papanicolaou smear. Six cytopathology laboratories and 35 gynaecologists participated. 5428 patients met the inclusion criteria (age > 18 years old, intact cervix, informed consent). Each cervical sample was used first to prepare a conventional Pap smear, then the sampling device was rinsed into a PreservCyt vial, and a ThinPrep slide was made. Screening of slide pairs was blinded (n = 5428). All non-negative concordant cases (n = 101), all non-concordant cases (n = 206), and a 5% random sample of concordant negative cases (n = 272) underwent review by one independent pathologist then by the panel of 6 investigators. Initial (blinded) screening results for ThinPrep and conventional smears were correlated. Initial diagnoses were correlated with consensus cytological diagnoses. Differences in disease detection were evaluated using McNemar's test. On initial screening, 29% more ASCUS cases and 39% more low-grade squamous intraepithelial lesions (LSIL) and more severe lesions (LSIL+) were detected on the ThinPrep slides than on the conventional smears (P = 0.001), including 50% more LSIL and 18% more high-grade SIL (HSIL). The ASCUS:SIL ratio was lower for the ThinPrep method (115:132 = 0.87:1) than for the conventional smear method (89:94 = 0.95:1). The same trend was observed for the ASCUS/AGUS:LSIL ratio. Independent and consensus review confirmed 145 LSIL+ diagnoses; of these, 18% more had been detected initially on the ThinPrep slides than on the conventional smears (P = 0.041). The ThinPrep Pap Test is more accurate than the conventional Pap test and has the potential to optimize the effectiveness of primary cervical cancer screening. © 2001 Cancer Research Campaign http://www.bjcancer.co

    Descriptive epidemiology of cleft lip and cleft palate in Western Australia

    BACKGROUND: Most national and provincial commissions on healthcare services in Canada over the past decade have recommended that primary care services be strengthened in order to guarantee each citizen access to a family physician. Despite these recommendations, finding a family physician continues to be problematic. The issue of enrollment with a family physician is worrying in Canada, where nearly 21% of the country's population reported not having a family physician in the last Commonwealth Fund survey.To respond to this important need, centralized waiting lists have been implemented in four Canadian provinces to help 'orphan,' or unaffiliated, patients find a family physician. These organizational mechanisms are intended to better coordinate the demand for and supply of family physicians. The objectives of this study are: to assess the effects of centralized waiting lists for orphan patients (GACOs) implemented in the province of Quebec and to explain the variation among their effects by analyzing factors influencing implementation process. METHODS: This study is based on two complementary and sequential research strategies. The first (objective 1) is a quantitative longitudinal design to assess the effects of all the GACOs (n = 93) in Quebec using clinical-administrative data. The second (objective 2) involves using four case studies to explain variations in effects through in-depth analysis of the various factors contributing to the observed effects. The primary source of data will be key actors involved in the GACOs. We expect to conduct around 40 semi-structured interviews. DISCUSSION: This will be the first study in Canada to evaluate the implementation of this innovation. It will provide an exhaustive picture of the effects of GACO implementation in Quebec and to assess their potential for generalization elsewhere in Canada. At the theoretical level, this study will produce new knowledge on the factors having the greatest influence on the implementation of primary care innovations in professional environments

    From nanosized precursors to high performance ceramics: the case of Bi2Ca2Co1.7Ox

    Bi2Ca2Co1.7Ox thermoelectric ceramics were prepared by four different synthesis routes: coprecipitation with ammonium carbonate or oxalic acid, attrition milling, and solid state (as reference). Microstructure showed that coprecipitation and attrition milling produced sintered materials with less porosity and smaller particle sizes than the solid state ones. Thermoelectric properties reflect the microstructure, leading to materials with lower electrical resistivity and higher Seebeck coefficient, when compared with the solid state ones. In spite of an increase in thermal conductivity in these samples due to their lower porosity, the maximum estimated Figure-of-Merit is higher than in sintered materials

    Effect of exogenous circulating anti-bPL antibodies on bovine placental lactogen measurements in foetal samples

    BACKGROUND: The involvement of placental lactogen (PL) in the regulation of foetal growth has been investigated in different species by in vivo immunomodulation techniques. However, when circulating antibodies are present together with the hormone, the procedure for hormonal measurement becomes considerably complex. The aim of this study was the immunoneutralization of bovine placental lactogen (bPL) concentrations in bovine foetal circulation by direct infusion of rabbit anti-bPL purified immunoglobulins (IgG) via a foetal catheter (in vivo study). The ability of a RIA based on guinea pig anti-bPL antiserum, for the measurement of bPL concentrations in samples containing exogenous rabbit anti-bPL immunoglobulins, was also analyzed in in vitro and in vivo conditions. METHODS: Six bovine foetuses were chronic cannulated on the aorta via the medial tarsal artery. Infusion of rabbit anti-bPL IgG was performed during late gestation. Pooled rabbit anti-bPL antisera had a maximal neutralization capacity of 25 microg bPL/mL of immunoglobulin. Interference of rabbit anti-bPL immunoglobulin with radioimmunoassay measurement using guinea pig anti-bPL as primary antibody was first evaluated in vitro. Polyclonal anti-bPL antibodies raised in rabbit were added in foetal sera to produce 100 samples with known antibodies titers (dilutions ranging from 1:2,500 till 1:1,280,000). RESULT(S): Assessment of the interference of rabbit anti-bPL antibody showed that bPL concentrations were significantly lower (P < 0.05) in samples added with dilutions of rabbit antiserum lower than 1:80,000 (one foetus) or 1:10,000 (four foetuses). It was also shown that the recovery of added bPL (12 ng/mL) was markedly reduced in those samples in which exogenous rabbit anti-bPL were added at dilutions lower than 1:20,000. Concentrations of foetal bPL were determined in samples from cannulated foetuses. In foetuses 1 and 6, bPL concentrations remained almost unchanged (<5 ng/mL) during the whole experimental period. In Foetus 3, bPL concentrations decreased immediately after IgG infusion and thereafter, they increased until parturition. CONCLUSION(S): The use of a bPL RIA using a guinea pig anti-bPL as primary antiserum allowed for the measurement of bPL concentrations in foetal plasma in presence of rabbit anti-bPL IgG into the foetal circulation.     Significant enhancement of the thermoelectric performance in Ca3Co4O9 thermoelectric materials through combined strontium substitution and hot-pressing process

    This work explores the possibilities for a further enhancement of the thermoelectric properties of Ca3Co4O9 by Sr-doping combined with hot-pressing. Modified hot-pressing process resulted in highly-textured and dense ceramics. Sr-doping significantly improves electrical properties, resulting in extremely large power factor (1.2 mW/K2m at 800 °C) due to simultaneous electrical resistivity decrease and Seebeck coefficient increase. The main effect on cumulative electrical performance is provided by the Seebeck coefficient, reaching 270μV/K at 800 °C. XPS revealed relatively high average cobalt oxidation state at room temperature (+3.3), compared to materials produced by conventional sintering. The results of combined XPS and Auger electron spectroscopy emphasize the importance of high densification in Ca3Co4O9-based ceramics for preventing phase decomposition and interaction with CO2 and moisture. Still, despite the exceptional electrical performance, the calculated figure-of-merit (estimated as 0.29 at 800 °C) is around the best reported in the literature due to a high thermal conductivity (4.4 W/K m at room temperature).publishe
