    Influence of Cardiorespiratory Clinical Placements on the Specialty Interest of Physiotherapy Students

    Clinical placements are an important part of health students’ training. Whilst much value is placed on the clinical environment as a place to learn, there is a paucity of direct evidence about its effectiveness. The aim of this study was to compare the competence, importance, and interest in cardiorespiratory physiotherapy of students before and after one month of clinical practice. A preand post-placement questionnaire about students’ interest in different physiotherapy subspecialties was used. The students with a cardiorespiratory clinical placement showed a significant change in their perception about the importance of the cardiorespiratory specialty (0.348 1.01; p < 0.001), while no significant change was observed in the students without cardiorespiratory placement (-0.014 0.825; p = 0.883). The presence or absence of clinical placements seems to have a definitive impact on students’ choice of a specialty. This implies the need for developing a set of clinical placements in all the subareas of physiotherapy in order to give undergraduate students the opportunity to make a better decision

    Impact of Previous Physical Activity Levels on Symptomatology, Functionality, and Strength during an Acute Exacerbation in COPD Patients

    The main objective of this study is to determine the relationship between physical activity (PA) level prior to hospitalization and the pulmonary symptomatology, functionality, exercise capacity, and strength of acute exacerbated chronic obstructive pulmonary disease (COPD) patients. In this observational study, all data were taken during the patient’s first day in hospital. Patients were divided into two groups (a PA group, and a physical inactivity (PI) group), according to the PA level evaluated by the Baecke questionnaire. Cough status was evaluated by the Leicester Cough Questionnaire (LCQ), and dyspnea was assessed using the modified Medical Research Council dyspnea scale (mMRC). Functionality was measured by the Functional Independence Measure (FIM) and the London Chest Activity of Daily Living scale (LCADL). Exercise capacity was evaluated by the two-minute step-in-place (2MSP) test, and strength assessed by dynamometry. A total of 151 patients were included in this observational study. Patients in the PI group obtained worse results compared to the PA group, and significant differences (p < 0.05) were found in all of the variables. Those COPD patients who regularly perform PA have less dyspnea and cough, as well as better functionality, exercise capacity and strength during an exacerbation, without relationship to the severity of the pathology.The author JRT has received financial support through a FPU (“Formación Profesorado Universitario”) grant (FPU:16/01531) of the Spanish Ministry of Education.     Actualización del Sistema de Gestión de Seguridad y Salud en el Trabajo de la empresa Básico Proyecto S.A.S.

    La organización Básico Proyectos S.A.S., para el año 2016, inicio con el diseño e implementación del Sistema de Gestión de Seguridad y Salud en el Trabajo (SG-SST), bajo la norma establecida en el Decreto 1072 de 2015 y la Resolución 1111 de 2017, la cual está derogada por la Resolución 0312 de 2019. En la actualidad la empresa cuenta con un SG-SST desactualizado desde el año 2019. Durante la investigación se procuró el mejoramiento del SGSST a través de la evaluación inicial de la situación actual del SGSST y de la actualización de la matriz de riesgos donde se identificaron nuevos peligros y se valoraron los riesgos debido a los cambios organizacionales, de tal manera que se establecieron directrices para la prevención de los posibles incidentes y accidentes de trabajo a partir de probables controles de ingeniería y/o administrativos.The organization Básico Proyectos SAS, for the year 2016, began with the design and implementation of the Occupational Health and Safety Management System (SG-SST), under the norm established in Decree 1072 of 2015 and Resolution 1111 of 2017, which is repealed by Resolution 0312 of 2019.     TGFβ Governs the Pleiotropic Activity of NDRG1 in Triple-Negative Breast Cancer Progression

    In triple-negative breast cancer (TNBC), the pleiotropic NDRG1 (N-Myc downstream regulated gene 1) promotes progression and worse survival, yet contradictory results were documented, and the mechanisms remain unknown. Phosphorylation and localization could drive NDRG1 pleiotropy, nonetheless, their role in TNBC progression and clinical outcome was not investigated. We found enhanced p-NDRG1 (Thr346) by TGFβ1 and explored whether it drives NDRG1 pleiotropy and TNBC progression. In tissue microarrays of 81 TNBC patients, we identified that staining and localization of NDRG1 and p-NDRG1 (Thr346) are biomarkers and risk factors associated with shorter overall survival. We found that TGFβ1 leads NDRG1, downstream of GSK3β, and upstream of NF-κB, to differentially regulate migration, invasion, epithelial-mesenchymal transition, tumor initiation, and maintenance of different populations of cancer stem cells (CSCs), depending on the progression stage of tumor cells, and the combination of TGFβ and GSK3β inhibitors impaired CSCs. The present study revealed the striking importance to assess both total NDRG1 and p-NDRG1 (Thr346) positiveness and subcellular localization to evaluate patient prognosis and their stratification.     Recicland, demostration on the management of organic and inorganic waste from agriculture

    La agricultura protegida que se desarrolla principalmente en el litoral oriental andaluz es una actividad de gran importancia económica y social para Andalucía. Sin embargo, el crecimiento exponencial de la superficie de invernadero ha dado lugar a un incremento del volumen de residuos agrarios. Además, este sistema agrícola está evolucionando hacia nuevas técnicas de cultivo que implican, no sólo la generación de residuos, sino también una mayor diversificación que hace más complicada su gestión. El principal problema para la gestión de estos residuos agrarios no está sólo en su dispersión geográfica, sino también en su estacionalidad, ya que se genera gran cantidad en determinados periodos anuales, coincidiendo con la finalización de las campañas agrícolas. Por este motivo, la capacidad máxima de recogida y almacenamiento y el proceso de gestión están condicionados por el volumen generado al finalizar los ciclos de cultivo. En este contexto, surge el proyecto RECICLAND que pretende palear dichos problemas y contribuir a la gestión sostenible de los residuos generados en los procesos productivos. Concretamente, el objetivo general de este proyecto es desarrollar actividades demostrativas piloto, informando de modo práctico sobre diferentes técnicas para la gestión específica de residuos inorgánicos y restos vegetales. Para ello, se ha establecido un área de reciclaje, junto con actividades de transferencia (jornadas prácticas, visitas, vídeos demostrativos y publicaciones) en el Centro IFAPA La Mojonera (Almería).Protected agriculture, which takes place mainly on the eastern coast of Andalusia, constitutes the major economic and social activity for this region. However, greenhouse area exponential growth has led to an increase in the volume of agricultural waste. In addition, this agricultural system is evolving towards new cultivation techniques that not only involve the generation of waste, but also a greater diversification that makes its management hard to tackle. The main issue regarding management of this agricultural waste is not only its geographical dispersion, but also its seasonality, as a large quantity is generated during certain periods of the year, coinciding with the end of agricultural campaigns. Indeed, the maximum collection and storage capacity and the management process itself are conditioned by the volume generated at the end of cropping cycles. In this context, the RECICLAND project arises, which aims to alleviate these problems and contribute to the sustainable management of the waste generated in the production processes. Specifically, the general objective of project is to develop pilot demonstration activities, providing practical information on different techniques for the specific management of inorganic waste and plant debris.     Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Search for eccentric black hole coalescences during the third observing run of LIGO and Virgo

    Despite the growing number of confident binary black hole coalescences observed through gravitational waves so far, the astrophysical origin of these binaries remains uncertain. Orbital eccentricity is one of the clearest tracers of binary formation channels. Identifying binary eccentricity, however, remains challenging due to the limited availability of gravitational waveforms that include effects of eccentricity. Here, we present observational results for a waveform-independent search sensitive to eccentric black hole coalescences, covering the third observing run (O3) of the LIGO and Virgo detectors. We identified no new high-significance candidates beyond those that were already identified with searches focusing on quasi-circular binaries. We determine the sensitivity of our search to high-mass (total mass M&gt;70 M⊙) binaries covering eccentricities up to 0.3 at 15 Hz orbital frequency, and use this to compare model predictions to search results. Assuming all detections are indeed quasi-circular, for our fiducial population model, we place an upper limit for the merger rate density of high-mass binaries with eccentricities 0&lt;e≤0.3 at 0.33 Gpc−3 yr−1 at 90\% confidence level
