    Complaints in the foot and ankle unit

    Complaints in the NHS have significant legal and financial implications. We performed an investigation to identify the causes, incidence and outcome of complaints in the Foot and Ankle unit of a tertiary referral centre. We found the complaint rate in the foot and ankle unit is low. Good communication and a professional attitude will significantly reduce complaints. The majority of complaints have local resolution, but there is potential for significant cost with every complaint. </jats:p

    Absence of force suppression in rabbit bladder correlates with low expression of heat shock protein 20

    BACKGROUND: Nitroglycerin can induce relaxation of swine carotid artery without sustained reductions in [Ca(2+)](i )or myosin regulatory light chain (MRLC) phosphorylation. This has been termed force suppression and been found to correlate with ser(16)-phosphorylation of heat shock protein 20 (HSP20). We tested for the existence of this mechanism in a smooth muscle that is not responsive to nitric oxide. METHODS: Isometrically mounted mucosa free rabbit bladder strips were contracted with carbachol and relaxed with 8-Br-cGMP, forskolin, or isoprenaline. RESULTS: Contraction was associated with a highly cooperative relation between MRLC phosphorylation and force such that very small increases in MRLC phosphorylation induced large increases in force. Relaxation induced by 8-Br-cGMP, forskolin, or isoprenaline did not shift the MRLC phosphorylation-force relation from that observed with carbachol alone, i.e. there was no force suppression. HSP20 content was negligible (approximately two hundred-fold less than swine carotid). CONCLUSION: The lack of force suppression in the absence of HSP20 is consistent with the hypothesized role for HSP20 in the force suppression observed in tonic smooth muscles

    peer-reviewedThe gut hormone glucagon-like peptide (GLP)-1 and its analogues represent a new generation of anti-diabetic drugs, which have also demonstrated propensity to modulate host lipid metabolism. Despite this, drugs of this nature are currently limited to intramuscular administration routes due to intestinal degradation. The aim of this study was to design a recombinant microbial delivery vector for a GLP-1 analogue and assess the efficacy of the therapeutic in improving host glucose, lipid and cholesterol metabolism in diet induced obese rodents. Diet-induced obese animals received either Lactobacillus paracasei NFBC 338 transformed to express a long-acting analogue of GLP-1 or the isogenic control microbe which solely harbored the pNZ44 plasmid. Short-term GLP-1 microbe intervention in rats reduced serum low-density lipoprotein cholesterol, triglycerides and triglyceride-rich lipoprotein cholesterol substantially. Conversely, extended GLP-1 microbe intervention improved glucose-dependent insulin secretion, glucose metabolism and cholesterol metabolism, compared to the high-fat control group. Interestingly, the microbe significantly attenuated the adiposity associated with the model and altered the serum lipidome, independently of GLP-1 secretion. These data indicate that recombinant incretin-secreting microbes may offer a novel and safe means of managing cholesterol metabolism and diet induced dyslipidaemia, as well as insulin sensitivity in metabolic dysfunction

    The Kepler Smear Campaign: Light curves for 102 Very Bright Stars

    We present the first data release of the Kepler Smear Campaign, using collateral 'smear' data obtained in the Kepler four-year mission to reconstruct light curves of 102 stars too bright to have been otherwise targeted. We describe the pipeline developed to extract and calibrate these light curves, and show that we attain photometric precision comparable to stars analyzed by the standard pipeline in the nominal Kepler mission. In this paper, aside from publishing the light curves of these stars, we focus on 66 red giants for which we detect solar-like oscillations, characterizing 33 of these in detail with spectroscopic chemical abundances and asteroseismic masses as benchmark stars. We also classify the whole sample, finding nearly all to be variable, with classical pulsations and binary effects. All source code, light curves, TRES spectra, and asteroseismic and stellar parameters are publicly available as a Kepler legacy sample.Comment: 35 pages, accepted ApJ

    Finding binaries from phase modulation of pulsating stars with Kepler: V. Orbital parameters, with eccentricity and mass-ratio distributions of 341 new binaries

    The orbital parameters of binaries at intermediate periods (102–103 d) are difficult to measure with conventional methods and are very incomplete. We have undertaken a new survey, applying our pulsation timing method to Kepler light curves of 2224 main-sequence A/F stars and found 341 non-eclipsing binaries. We calculate the orbital parameters for 317 PB1 systems (single-pulsator binaries) and 24 PB2s (double-pulsators), tripling the number of intermediate-mass binaries with full orbital solutions. The method reaches down to small mass ratios q ≈ 0.02 and yields a highly homogeneous sample. We parametrize the mass- ratio distribution using both inversion and Markov-Chain Monte Carlo forward-modelling techniques, and find it to be skewed towards low-mass companions, peaking at q ≈ 0.2. While solar-type primaries exhibit a brown dwarf desert across short and intermediate periods, we find a small but statistically significant (2.6σ ) population of extreme-mass-ratio companions (q 0.1, we measure the binary fraction of current A/F primaries to be 15.4 per cent ± 1.4 per cent, though we find that a large fraction of the companions (21 per cent ± 6 per cent) are white dwarfs in post-mass-transfer systems with primaries that are now blue stragglers, some of which are the progenitors of Type Ia supernovae, barium stars, symbiotics, and related phenomena. Excluding these white dwarfs, we determine the binary fraction of original A/F primaries to be 13.9 per cent ± 2.1 per cent over the same parameter space. Combining our measurements with those in the literature, we find the binary fraction across these periods is a constant 5 per cent for primaries M1 < 0.8 M , but then increases linearly with log M1, demonstrating that natal discs around more massive protostars M1 2: 1M become increasingly more prone to fragmentation. Finally, we find the eccentricity distribution of the main-sequence pairs to be much less eccentric than the thermal distribution

    SPYGLASS. III. The Fornax-Horologium Association and its Traceback History within the Austral Complex

    The study of young associations is essential for building a complete record of local star formation processes. The Fornax-Horologium association (FH), including the χ1\chi^1 Fornacis cluster, represents one of the nearest young stellar populations to the Sun. This association has recently been linked to the Tuc-Hor, Carina, and Columba associations, building an extensive "Austral Complex" almost entirely within 150 pc. Using Gaia astrometry and photometry in addition to new spectroscopic observations, we perform the deepest survey of FH to date, identifying over 300 candidate members, nearly doubling the known population. By combining this sample with literature surveys of the other constituent populations, we produce a contiguous stellar population covering the entire Austral Complex, allowing the definitions of sub-populations to be re-assessed along with connections to external populations. This analysis recovers new definitions for FH, Tuc-Hor, Columba, and Carina, while also revealing a connection between the Austral complex and the Sco-Cen-affiliated Platais 8 cluster. This suggests that the Austral complex may be just a small component of a much larger and more diverse star formation event. Computing ages and tracing stellar populations back to formation reveals two distinct nodes of cospatial and continuous formation in the Austral Complex, one containing Tuc-Hor, and the other containing FH, Carina, and Columba. This mirrors recent work showing similar structure elsewhere, suggesting that these nodes, which only emerge through the use of traceback, may represent the clearest discrete unit of local star formation, and a key building block needed to reconstruct larger star-forming events.Comment: Accepted to ApJ; 29 pages, 10 figures, 5 tables in two-column AASTEX63 forma
