    « Collaborer et réussir » ou « Se confronter et échouer », tel est le choix qui en résumé s’offre aux différents acteurs d’un projet routier. La réflexion menée ici commence par s’interroger sur les causes de la problématique des conflits existants. Après l’examen de la procédure, le jeu des différents acteurs est analysé. Tout ceci aboutit à une proposi-tion d’idées simples, mais efficaces, pour mener un projet d’étude d’une manière concertée entre tous les intervenants. En conclusion, le débat est entamé sur les limites de cette méthode concertative, sans que ses défauts ne viennent finalement à refuser son usage

    The present communication is based on the research undertaken by the author and whose principal results are detailed in its thesis entitled « Choice of alternatives of road infrastructures : multicriteria methods » like on the prospects offered by this one. The problems of the choice between various alternatives are permanent and crucial in the projects of systems of transport and road infrastructures. The designer who works out a technical solution must have objective and global methods in order to propose to the decision makers, who are generally a political entity, an optimal alternative. The complexity of the many fields affected by the transport infrastructure as well as the diversity of the various intervening actors directly or indirectly in the study process are as much elements to integrate in this choice. Only methods of multicriteria decision analysis, associated with a dialogue integrated into all the stages of the study process, make it possible to the designer as well as possible to take account of this complexity. The use of such methods also makes it possible to bind the objective aspects of the choices, based in particular on technical evaluation of the performance indicators of the alternatives for each criterion, with its subjective aspects which are the relative consideration of importance of each criterion, also called weighting. These methods of multicriteria decision analysis used in parallel with dialogue methods are clearly at the basis of a study process ensuring the realization of a sustainability road. They indeed make it possible to bind social, by the dialogue of the various actors, economic and environmental dimensions by the consideration of adequate criteria. Moreover, the consideration of the needs for the future generations, by the taking into account of the cycle of life of the infrastructure, is easy to realize. The author carried out an actualization of methodology of the road project process while basing him on the integration of these principles in the procedure. An evaluation of the various methods of multicriteria decision analysis was carried out and tested on a real case in Switzerland. It results the following main clauses from them : The use of the method of partial aggregation Electre III makes it possible to better moderate the judgments and use easy indicators; The honest separation in the stages of the process of the phase of weighting, which is to be realized before the alternatives generation, this one preceding the phase of the evaluation of the criteria; The objective judgment is to be realized by the designer and subjective weighting by the decision makers

    Acute and late toxicity in prostate cancer patients treated by dose escalated intensity modulated radiation therapy and organ tracking

    BACKGROUND: To report acute and late toxicity in prostate cancer patients treated by dose escalated intensity-modulated radiation therapy (IMRT) and organ tracking. METHODS: From 06/2004 to 12/2005 39 men were treated by 80 Gy IMRT along with organ tracking. Median age was 69 years, risk of recurrence was low 18%, intermediate 21% and high in 61% patients. Hormone therapy (HT) was received by 74% of patients. Toxicity was scored according to the CTC scale version 3.0. Median follow-up (FU) was 29 months. RESULTS: Acute and maximal late grade 2 gastrointestinal (GI) toxicity was 3% and 8%, late grade 2 GI toxicity dropped to 0% at the end of FU. No acute or late grade 3 GI toxicity was observed. Grade 2 and 3 pre-treatment genitourinary (GU) morbidity (PGUM) was 20% and 5%. Acute and maximal late grade 2 GU toxicity was 56% and 28% and late grade 2 GU toxicity decreased to 15% of patients at the end of FU. Acute and maximal late grade 3 GU toxicity was 8% and 3%, respectively. Decreased late > or = grade 2 GU toxicity free survival was associated with higher age (P = .025), absence of HT (P = .016) and higher PGUM (P < .001). DISCUSSION: GI toxicity rates after IMRT and organ tracking are excellent, GU toxicity rates are strongly related to PGUM

    Interactions entre les réseaux de la faune et des voies de circulation

    L’ingénieur qui développe les réseaux d’infrastructure de transports en raison d’une demande croissante en mobilité (essor de l’économie, population et niveau de vie en augmentation, etc.) doit faire en sorte que ceux-ci soient efficaces, sûrs, économiques et respectueux de l’environnement. Cette tâche ardue ne peut s’accomplir sans un travail interdisciplinaire faisant notamment intervenir un écologue pour tout ce qui concerne la protection de la faune. La collaboration « ingénieur – spécialiste de la faune » doit débuter au stade de l’étude de planification du projet, les documents situant les réseaux écologiques (corridors et zones-réservoir) et les milieux protégés devant être dans les mains de l’ingénieur lors du choix du tracé de la voie de circulation. Les impacts d’une infrastructure de transport sur la faune portent atteinte à la capacité d’accueil du milieu, au taux de mortalité des espèces animales et à la fonctionnalité du réseau écologique. Les solutions passent par l’identification des réseaux écologiques existants, l’analyse de la perméabilité effective de la voie de circulation et l’élaboration d’un concept de réseau écologique futur qui doit tenir compte de l’évolution prévisible du paysage et de l’aspect régional de la problématique. Le plan de mesures pour la faune est élaboré conjointement par l’ingénieur et l’écologue sur la base de ce concept de réseau écologique futur. Il doit viser la sécurité pour les usagers de la voie de circulation, la protection de la faune et un rapport « efficacité/coût » élevé pour les mesures projetées. Ce dernier s’obtient entre autres en aménageant pour la faune les ouvrages de franchissement non spécifiques et en intégrant les passages à faune dans le paysage à l’aide d’aménagements s’étendant au-delà de l’emprise de la voie de circulation. La gestion des mesures pour la faune est une nécessité pour garantir leur efficacité à long terme. Elle doit être planifiée lors du projet et doit fixer notamment le financement et les intervenants prenant part à l’entretien, à la surveillance et aux contrôles d’efficacité

    Orbital inflammation and colitis in pediatric IgG4-related disease: A case report and review of the literature.

    IgG4-related disease (IgG4-RD) is an inflammatory disorder characterized by tumor-like swelling in one or more organs, elevated serum IgG4 levels, and histological alterations with infiltration of IgG4-positive plasma cells. IgG4-RD is rare and likely underdiagnosed in children. We report a case of a 16-year-old girl with IgG4-positive colitis that developed weeks after IgG4-related ophthalmic disease and discuss diagnosis and treatment in the context of the literature available. Since the pathophysiology of IgG4-RD is unknown, treatment options are empiric and, for the most part, untargeted. Systemic corticosteroid treatment is the basis of anti-inflammatory treatment in IgG4-RD and induced early remission in our patient. During corticosteroid taper, the patient developed weight loss and intestinal inflammation. Histopathological assessment of the intestinal walls confirmed IgG4-positive colitis. Immune-modulating treatment with non-biologic (e.g., methotrexate (MTX) and mycophenolate mofetil) or biologic (rituximab) disease-modifying antirheumatic drugs has been reported in treatment refractory or corticosteroid-dependent patients. The patient responded to treatment with anti-inflammatory therapy with food rich in TGF-β2 (modulen) and MTX. This is one of the first pediatric patients reported with IgG4-related colitis extending the phenotype of pediatric IgG4-RD. International collaboration to prospectively document clinical presentation and treatment responses may help to further establish the phenotype and treatment options and to raise awareness for IgG4-RD

    Experimental evaluation of biological regeneration of arable soil : the effects of grass-clover leys and arbuscular mycorrhizal inoculants on wheat growth, yield, and shoot pathology

    Wheat yields have plateaued in the UK over the last 25 years, during which time most arable land has been annually cropped continuously with short rotations dominated by cereals. Arable intensification has depleted soil organic matter and biology, including mycorrhizas, which are affected by tillage, herbicides, and crop genotype. Here, we test whether winter wheat yields, mycorrhization, and shoot health can be improved simply by adopting less intensive tillage and adding commercial mycorrhizal inoculum to long-term arable fields, or if 3-year grass-clover leys followed direct drilling is more effective for biological regeneration of soil with reduced N fertiliser. We report a trial of mycorrhization, ear pathology, and yield performance of the parents and four double haploid lines from the Avalon x Cadenza winter wheat population in a long-term arable field that is divided into replicated treatment plots. These plots comprised wheat lines grown using ploughing or disc cultivation for 3 years, half of which received annual additions of commercial arbuscular mycorrhizal (AM) inoculum, compared to 3-year mown grass-clover ley plots treated with glyphosate and direct-drilled. All plots annually received 35 kg of N ha−1 fertiliser without fungicides. The wheat lines did not differ in mycorrhization, which averaged only 34% and 40% of root length colonised (RLC) in the ploughed and disc-cultivated plots, respectively, and decreased with inoculation. In the ley, RLC increased to 52%. Two wheat lines were very susceptible to a sooty ear mould, which was lowest in the ley, and highest with disc cultivation. AM inoculation reduced ear infections by >50% in the susceptible lines. In the ley, yields ranged from 7.2 to 8.3 t ha−1, achieving 92 to 106% of UK average wheat yield in 2018 (7.8 t ha−1) but using only 25% of average N fertiliser. Yields with ploughing and disc cultivation averaged only 3.9 and 3.4 t ha−1, respectively, with AM inoculum reducing yields from 4.3 to 3.5 t ha−1 in ploughed plots, with no effect of disc cultivation. The findings reveal multiple benefits of reintegrating legume-rich leys into arable rotations as part of a strategy to regenerate soil quality and wheat crop health, reduce dependence on nitrogen fertilisers, enhance mycorrhization, and achieve good yields

High-throughput sequencing techniques like 454 or Solexa-Illumina promise new possibilities as they are able to produce huge amounts of data in much shorter time and with less efforts and costs than the traditional Sanger technique. But the data produced comes in even shorter reads (35-100 basepairs with Illumina, 100-500 basepairs with 454-sequencing). CARMA is a new software pipeline for the characterisation of species composition and the genetic potential of microbial samples using short, unassembled reads. Results In this paper, we introduce WebCARMA, a refined version of CARMA available as a web application for the taxonomic and functional classification of unassembled (ultra-)short reads from metagenomic communities. In addition, we have analysed the applicability of ultra-short reads in metagenomics. Conclusions We show that unassembled reads as short as 35 bp can be used for the taxonomic classification of a metagenome.
