Posoperatorio inmediato de trasplante pulmonar: mapeo de intervenciones de enfermería

OBJETIVOS: conocer las principales intervenciones/acciones de enfermería prescritas para el posoperatorio inmediato de los receptores de trasplante pulmonar, registrados en las fichas médicas y mapear esas prescripciones con la taxonomía Nursing Interventions Classification (NIC). MÉTODO: investigación documental retrospectiva compuesta por 183 fichas médicas de pacientes sometidos a trasplante pulmonar (2007/2012). Los datos del perfil de los pacientes fueron agrupados de acuerdo con las variables investigadas y sometidos al análisis descriptivo. Las intervenciones de enfermería prescritas fueron analizadas por el método del mapeo cruzado con las intervenciones relacionadas en la NIC. Se excluyeron las fichas médicas sin prescripciones de enfermería. RESULTADOS: la mayoría de los pacientes era hombre, con diagnóstico médico de fibrosis pulmonar, sometidos a trasplante pulmonar con donador fallecido. Se encontraron 26 intervenciones/acciones más citadas. La mayoría (91,6%) pertenecía a los dominios fisiológicos complejo y básico de la NIC. No fue posible mapear dos acciones prescritas por los enfermeros. CONCLUSIONES: se identificó que las principales prescripciones contenían cuidados generales de posoperatorio de cirugías de gran porte, y no individualizadas para paciente en posoperatorio de trasplante pulmonar. Los cuidados relacionados al dolor fueron subestimados en las prescripciones. El mapeo con la taxonomía puede contribuir para la elaboración del plan de cuidados y la utilización de sistemas computarizados en esta compleja modalidad terapéutica.OBJETIVOS: conhecer as principais intervenções/ações de enfermagem prescritas no pós-operatório imediato dos receptores de transplante pulmonar, registradas nos prontuários, e mapear com a taxonomia Nursing Interventions Classification. Os dados do perfil dos pacientes foram agrupados de acordo com as variáveis investigadas e submetidos às análises descritivas. As intervenções de enfermagem prescritas foram analisadas pelo método de mapeamento cruzado com as intervenções relacionadas na Nursing Interventions Classification. Excluíram-se prontuários sem prescrições de enfermagem. RESULTADOS: a maioria dos pacientes era composta por homens, com diagnóstico médico de fibrose pulmonar e submetidos a transplante pulmonar com doador falecido. Encontraram-se 26 intervenções/ações mais citadas. A maioria (91,6%) alocou-se nos domínios fisiológico complexo e básico da Classification Nursing Information. Não foi possível mapear duas ações prescritas pelos enfermeiros. CONCLUSÕES: identificou-se que as principais prescrições continham cuidados gerais de pós-operatório de cirurgias de grande porte, e não individualizadas ao paciente em pós-operatório de transplante pulmonar. Cuidados relacionados à dor foram subestimados nas prescrições. O mapeamento com a taxonomia pode contribuir para a elaboração do plano de cuidados e utilização de sistemas informatizados nessa complexa modalidade terapêutica.OBJECTIVES: to investigate the principle nursing interventions/actions, prescribed in the immediate post-operative period for patients who receive lung transplantation, recorded in the medical records, and to map these using the Nursing Interventions Classification (NIC) taxonomy. METHOD: retrospective documental research using 183 medical records of patients who received lung transplantation (2007/2012). The data of the patients' profile were grouped in accordance with the variables investigated, and submitted to descriptive analysis. The nursing interventions prescribed were analyzed using the method of cross-mapping with the related interventions in the NIC. Medical records which did not contain nursing prescriptions were excluded. RESULTS: the majority of the patients were male, with medical diagnoses of pulmonary fibrosis, and underwent lung transplantation from a deceased donor. A total of 26 most frequently-cited interventions/actions were found. The majority (91.6%) were in the complex and basic physiological domains of the NIC. It was not possible to map two actions prescribed by the nurses. CONCLUSIONS: it was identified that the main prescriptions contained general care for the postoperative period of major surgery, rather than prescriptions individualized to the patient in the postoperative period following lung transplantation. Care measures related to pain were underestimated in the prescriptions. The mapping with the taxonomy can contribute to the elaboration of the care plan and to the use of computerized systems in this complex mode of therapy

LA PREVENCIÓN DESDE LA FAMILIA PARA EVITAR UNA INADECUADA DIETA NUTRICIONAL DE LOS NIÑOS EN LA PRIMERA INFANCIA

La familia juega un significativo papel en la alimentación de sus niños, de ahí que la educación nutricional sea un factor importante a considerar para su crecimiento y desarrollo. El presente artículo tiene como objetivo proponer variadas actividades dirigidas a la preparación de las familias para que conozcan acerca de los alimentos y su incidencia en la evolución del organismo humano, a tales efectos se realizó un diagnóstico con la utilización de métodos teóricos como el analítico-sintético e inductivo-deductivo;  otros de carácter  empíricos, la observación,  entrevista y encuesta, los que proporcionaron todo el sustento teórico y práctico para fundamentar la necesidad de una alimentación adecuada en el hogar  orientada desde la institución educativa. EL TRABAJO PREVENTIVO DE LA FAMILIA EN INTERACCIÓN CON LA INSTITUCIÓN

El trabajo preventivo de la familia desde su labor educativa con los hijos es uno de los grandes retos a los que se enfrenta la institución educativa, la que requiere de vías, métodos y alternativas que sean efectivas para lograr alcanzar un desarrollo integral de los niños y adolescentes. De ahí que la preparación recíproca de ambas instituciones sea una prioridad en la educación cubana. En el presente artículo se exponen algunos fundamentos teóricos que sustentan los principios formativos de nuestro sistema educativo como eje esencial desde la propia actividad del educador, el que debe contemplar la atención a la familia desde la diversidad y el contexto en que se desarrollan estos niños y adolescentes. PREVENTIVE WORK OF THE FAMILY IN INTERACTION WITH THE INSTITUTION ABSTRACT The preventive work of the family from its educational work with the children is one from the big challenges to those that faces the educational institution, the one that requires of roads, methods and alternative that are effective to be able to reach an integral development of the children and adolescents. With the result that the reciprocal preparation of both institutions is a priority in the Cuban education. Presently article some theoretical foundations are exposed that sustain the formative principles of our educational system as essential axis from the educator's own activity, the one that should contemplate the attention to the family from the diversity and the context in that these children and adolescents are developed. EDUCACIÓN PARA LA SALUD EN ADOLESCENTES Y JÓVENES DESDE EL CONTEXTO COMUNITARIO

HEALTH EDUCATION IN ADOLESCENTS AND YOUNG PEOPLE FROM THE COMMUNITY CONTEXT RESUMEN En más de una oportunidad se ha expresado el daño que provoca el mal hábito de fumar e ingerir bebidas alcohólicas, así como la necesidad de que la juventud conozca los perjuicios que pueden evitarse al no consumirlos. El trabajo conforma una de las aristas que pueden servir de pautas para la educación de la salud, vistas desde el contexto comunitario, referidas a estas prácticas. Ofrecemos algunos elementos que han podido constituir factor de riesgo, se han utilizaron algunos métodos que permitió la interpretación, selección y adopción de posiciones relativas al tema y el enriquecimiento del diagnóstico, Es el contexto comunitario dado sus potencialidades, el mejor espacio para desde ahí ejercer influencia y brindar un grupo de sugerencias. ABSTRACT On more than one occasion has been expressed the damage caused by bad habit of smoking and ingesting alcoholic beverages, as well as the need for youth to know the damages that can be avoided by not consuming them. The work is one of the aspects that can serve as guidelines for health education, seen from the community context, referring to these practices. We offer some elements that may have been a risk factor, some methods have been used that allowed the Interpretation, selection and adoption of positions related to the topic and the enrichment of the diagnosis. It is the community context given its potentialities, the best space from which to exert influence and offer a group of suggestions. Single Multiplex Polymerase Chain Reaction To Detect Diverse Loci Associated with Diarrheagenic Escherichia coli

We developed and tested a single multiplex polymerase chain reaction (PCR) that detects enterotoxigenic, enteropathogenic, enteroinvasive, and Shiga-toxin–producing Escherichia coli. This PCR is specific, sensitive, and rapid in detecting target isolates in stool and food. La sociedad ha puesto empeño en la difícil tarea de prevenir, minimizar o impedir las manifestaciones de intento suicida, fenómeno que posee un nexo inseparable con los procesos sociales, de ahí que la preparación integral desde la formación inicial del maestro primario para el desarrollo de este trabajo en sus diferentes contextos de actuación, son retos que el sector educacional deberá enfrentar para eliminar los obstáculos que interfieren en la prevención desde el proceso educativo de cualquier manifestación con un desenlace fatal.
