    In-Space Transportation for NASA's Evolvable Mars Campaign

    As the nation embarks on a new and bold journey to Mars, significant work is being done to determine what that mission and those architectural elements will look like. The Evolvable Mars Campaign, or EMC, is being evaluated as a potential approach to getting humans to Mars. Built on the premise of leveraging current technology investments and maximizing element commonality to reduce cost and development schedule, the EMC transportation architecture is focused on developing the elements required to move crew and equipment to Mars as efficiently and effectively as possible both from a performance and a programmatic standpoint. Over the last 18 months the team has been evaluating potential options for those transportation elements. One of the key aspects of the EMC is leveraging investments being made today in missions like the Asteroid Redirect Mission (ARM) mission using derived versions of the Solar Electric Propulsion (SEP) propulsion systems and coupling them with other chemical propulsion elements that maximize commonality across the architecture between both transportation and Mars operations elements. This paper outlines the broad trade space being evaluated including the different technologies being assessed for transportation elements and how those elements are assembled into an architecture. Impacts to potential operational scenarios at Mars are also investigated. Trades are being made on the size and power level of the SEP vehicle for delivering cargo as well as the size of the chemical propulsion systems and various mission aspects including Inspace assembly and sequencing. Maximizing payload delivery to Mars with the SEP vehicle will better support the operational scenarios at Mars by enabling the delivery of landers and habitation elements that are appropriately sized for the mission. The purpose of this investigation is not to find the solution but rather a suite of solutions with potential application to the challenge of sending cargo and crew to Mars. The goal is that, by building an architecture intelligently with all aspects considered, the sustainable Mars program wisely invests limited resources enabling a long-term human Mars exploration program

    BackgroundInter-professional collaborative care (ICC) for cancer leads to multiple system, organizational, professional, and patient benefits, but is limited by numerous challenges. Empirical research on interventions that promote or enable ICC is sparse so guidance on how to achieve ICC is lacking. Research shows that ICC for diagnosis could be improved. Diagnostic assessment programs (DAPs) appear to be a promising model for enabling ICC. The purpose of this study was to explore how DAP structure and function enable ICC, and whether that may be associated with organizational and clinical outcomes.MethodsA case study approach will be used to explore ICC among eight DAPs that vary by type of cancer (lung, breast), academic status, and geographic region. To describe DAP function and outcomes, and gather information that will enable costing, recommendations expressed in DAP standards and clinical guidelines will be assessed through retrospective observational study. Data will be acquired from databases maintained by participating DAPs and the provincial cancer agency, and confirmed by and supplemented with review of medical records. We will conduct a pilot study to explore the feasibility of estimating the incremental cost-effectiveness ratio using person-level data from medical records and other sources. Interviews will be conducted with health professionals, staff, and referring physicians from each DAP to learn about barriers and facilitators of ICC. Qualitative methods based on a grounded approach will be used to guide sampling, data collection and analysis.DiscussionFindings may reveal opportunities for unique structures, interventions or tools that enable ICC that could be developed, implemented, and evaluated through future research. This information will serve as a formative needs assessment to identify the nature of ongoing or required improvements, which can be directly used by our decision maker collaborators, and as a framework by policy makers, cancer system managers, and DAP managers elsewhere to strategically plan for and implement diagnostic cancer services

    Alcohol marketing in the Americas and Spain during the 2014 FIFA World Cup Tournament

    Background and aims To identify the nature of visual alcohol references in alcohol advertisements during televised broadcasts of the 2014 FIFA World Cup Tournament matches and to evaluate cross-national differences according to alcohol marketing policy restrictiveness. Design Content analysis using the Delphi method and identification of in-game sponsorships. Setting Television broadcasts of the 2014 FIFA World Cup in Argentina, Brazil, Canada, Finland, France, Mexico, Spain and the United States. Cases Eighty-seven alcohol advertisements20 matches. Measurements Quantitative rating scales, combined with the Delphi rating technique, were used to determine compliance of the alcohol advertisements with the International Alliance for Responsible Drinking's (IARD) Guiding Principles. Recordings of five matches from four countries were also used to identify the number of in-and out-of-game alcohol brand appearances. Findings A total of 86.2% of all unique alcohol advertisements contained at least one violation of IARD's Guiding Principles, with violation rates ranging from 72.7% (Mexico) to 100% (Brazil). Countries with the least restrictive marketing policies had a higher prevalence of violations in guidelines designed to protect minors. There were 2.76 in-game alcohol brand appearances and 0.83 out-of-game alcohol brand appearances per minute. Brand appearances did not differ across countries or according to a country's marketing policy restrictiveness. Conclusions Self-regulation and statutory policies were ineffective at limiting alcohol advertising during the 2014 FIFA World Cup Tournament television broadcasts.     Evaluation of the effects of biochar on diet digestibility and methane production from growing and finishing steers

    The objectives of these studies were to evaluate the effects of biochar (0%, 0.8%, or 3% of diet dry matter) on diet digestibility and methane and carbon dioxide production from cattle on growing and finishing diets. The growing diet consisted of 21% brome hay, 20% wheat straw, 30% corn silage, 22% wet distillers grains plus solubles, and 7% supplement. The finishing diet consisted of 53% dry-rolled corn, 15% corn silage, 25% wet distillers grains plus solubles, and 7% supplement. In both trials biochar replaced fine ground corn in the supplement. Six crossbred steers (initial body weight [BW] 529 kg; SD = 16 kg) were used in both the growing and finishing trial. The growing diets were evaluated over 6 periods followed by the finishing trial with 3 periods. Digestibility measures were taken over 4 d after at least 8 d of adaptation to diets followed by 2 d of gas emission measurements using headbox calorimeters. Dry matter intake (DMI) was not affected (P ≥ 0.43; 7.91 kg/d) by biochar inclusion in the growing study and increased quadratically (P = 0.07) in the finishing study with 0.8% biochar inclusion having the greatest DMI (12.9 kg/d). Organic matter (OM) and neutral detergent fiber (NDF) digestibility increased quadratically (P = 0.10) in the growing study whereas OM digestibility tended to linearly decrease (P = 0.13) and NDF digestibility was not affected (P ≥ 0.39) by biochar inclusion in the finishing diet. Digestible energy intake (Mcal/d) was not affected (P ≥ 0.25) by biochar inclusion in the growing or finishing study. Methane production (g/d) tended to decrease quadratically (P = 0.14) in the growing study and was decreased 10.7% for the 0.8% biochar treatment relative to the control. There were no statistical differences in methane production (g/d) in the finishing study (P ≥ 0.32) but cattle on the 0.8% biochar treatment produced numerically less (9.6%) methane than the control. Methane production as g/kg DMI of the 0.8% biochar treatment relative to the control was numerically reduced 9.5% and 18.4% in the growing and finishing studies, respectively (P ≥ 0.13). Carbon dioxide production (g/d and g/kg of intake) quadratically decreased (P ≤ 0.06) in the growing study but was not affected by treatment in the finishing study (P ≥ 0.34). Although biochar is not a U.S. Food and Drug Administration -approved feed for cattle, the initial research shows potential as a methane mitigation strategy in both growing and finishing diets

    Communicating choice: an exploration of mothers' experiences of birth

    YesObjective: This article gives an in-depth insight into the ways in which communication between midwives and the birthing woman shape the birth experience. Background: Birth is a significant life event for many women that can have profound, long-lasting effects on how they see themselves as women and mothers. Within the literature the importance of control over the birth experience and the support that the birthing woman receives from midwives is stressed. Methods: Six women who had recently given birth participated in one-to-one semi-structured interviews designed to explore the kinds of support they received before, during and after their birth. An inductive thematic analysis was employed in order to identify and explore key issues which ran throughout the interviews. Results: Within the interviews the importance of being an active mother, someone who made decisions in relation to her labour, was stressed. The analysis explores the ways in which communication style and compassionate care either enabled or prevented women from adopting the position of ‘active’ mother. Conclusion: It is argued that a personal connection with midwives and clear and open communication which places the birthing woman in a position of control are key to positive birth experiences

    ObjectiveTo evaluate serum estradiol (E2) concentrations during use of 90-day contraceptive vaginal rings releasing E2 75, 100, or 200 mcg/day and segesterone acetate (SA) 200 mcg/day to identify a dose that avoids hypoestrogenism.Study designWe conducted a multicenter dose-finding study in healthy, reproductive-aged women with regular cycles with sequential enrollment to increasing E2 dose groups. We evaluated serum E2 concentrations twice weekly for the primary outcome of median E2 concentrations throughout initial 30-day use (target ≥40 pg/mL). In an optional 2-cycle extension substudy, we randomized participants to 2- or 4-day ring-free intervals per 30-day cycle to evaluate bleeding and spotting based on daily diary information.ResultsSixty-five participants enrolled in E2 75 (n = 22), 100 (n = 21), and 200 (n = 22) mcg/day groups; 35 participated in the substudy. Median serum E2 concentrations in 75 and 100 mcg/day groups were <40 pg/mL. In the 200 mcg/day group, median E2 concentrations peaked on days 4-5 of CVR use at 194 pg/mL (range 114-312 pg/mL) and remained >40 pg/mL throughout 30 days; E2 concentrations were 37 pg/mL (range 28-62 pg/mL) on days 88-90 (n = 11). Among the E2 200 mcg/day substudy participants, all had withdrawal bleeding following ring removal. The 2-day ring-free interval group reported zero median unscheduled bleeding and two (range 0-16) and three (range 0-19) unscheduled spotting days in extension cycles 1 and 2, respectively. The 4-day ring-free interval group reported zero median unscheduled bleeding or spotting days.ConclusionsEstradiol concentrations with rings releasing E2 200 mcg/day and SA 200 mcg/day avoid hypoestrogenism over 30-day use.ImplicationsA 90-day contraceptive vaginal ring releasing estradiol 200 mcg/day and segesterone acetate 200 mcg/day achieves estradiol concentrations that should avoid hypoestrogenism and effectively suppresses ovulation
