    Marfan Syndrome : Correct diagnosis can save lives

    Marfan syndrome is a heritable disorder of the connective tissue that affects many systems of the body. However, the most serious complication in patients with Marfan syndrome is progressive enlargement of the aortic root, which may lead to aortic dissection, rupture, or aortic regurgitation. Prevention of these life threatening complications is very important in the management of this condition. A 39-year-old Omani man presented with progressive shortness of breath and eventually underwent major but successful cardiac surgery. It is very important to recognise Marfan syndrome early as preventive actions are possible if the condition is diagnosed before complications occur

    Subacute Tension Hemopneumothorax with Novel Electrocardiogram Findings

    This case report describes a patient with a subacute right-sided tension hemopneumothorax following an occult stab. The patient’s electrocardiogram (ECG), performed as part of a standardized triage process, demonstrated significant abnormalities that misguided initial resuscitation, but resolved following evacuation of the tension hemopneumothorax. Tension pneumothorax is typically regarded as an immediately life-threatening condition that requires emergent management with needle or tube thoracostomy. However, we believe that subacute tension pneumothorax may be a rarely observed clinical phenomenon and may lead to unique ECG findings. We believe that the ECG changes we observed provided an early clue to the eventual diagnosis of a subacute tension pneumothorax and have not been previously described in this setting.

    Community participation in bureaucratic organizations: Principles and strategies

    The present paper points out that there are tasks that primary groups perform better than bureaucracies and those that bureaucracies perform better than primary groups. Both types of tasks are very interdependent so that sometimes primary group tasks must be performed within the boundaries of the bureaucratic organization. The argument is made that when primary groups intervene in bureaucracies, they can do so directly in non-expert tasks without lowering the effectiveness of the bureaucratic organization. When they intervene in expert aspects, they should do so indirectly through an expert advocate. However, in all intervention the primary group must take into account that its structure is contradictory to that of the bureaucracy and, therefore, it must keep as much distance as possible—consistent with its ability to intervene. From this analysis we derive a series of hypotheses suggesting when the community might ideally use the bureaucracy's own experts, when the community must hire its own experts, when the community should use mass media, strikes, indigenous workers, etc. It is suggested that the multitudinous possibilities for linkages can all be derived from a few basic underlying dimensions of the situation. Cet article montre qu'il existe certaines tâches que les groupes primaires accomplissent mieux que les bureaucraties, et certaines autres qu'accomplissent mieux les bureaucraties. Les deux sortes de tâches sont interdépendentes; donc parfois les tâches des groupes primaires doivent s'accomplir parmi les organisations bureaucratiques. Les auteurs soutiennent que les groupes primaires peuvent intervenir dans les bureaucraties en complétant les tâches non-expertes sans réduire l'efficacité de l'organisation bureaucratique. Quands ils participent à certains sujets experts, ils devraient se munir d'un agent expert. Mais dans toute intervention le groupe primaire doit se rendre compte que sa structure contredit celle de la bureaucratie et que, par conséquent, il doit se tenir autant à distance que possible par rapport à sa compétence d'intervention.     Finding Success and Stability in Long Lasting Marriages

    Many people worry about the divorce rate in America, according to the National Vital Statistics Reports of Births, Marriages, Divorces, and Deaths on July, 29, 2009 over 1 million people got divorced in 2008. Given such high divorce rates, what can we do to make marriage stronger? In order to investigate the properties of long lasting marriage, I interviewed 10 couples who have been married thirty or more years. The subjects were asked several questions about the beliefs and values that are important in their relationship. My results focus on two key findings that couples suggested contributed to their happiness and marital success: a) having a stay at home mother within the family, and b) their perspectives on raising children. Additionally, I discuss their current views on divorce and marriage today. If these findings are correct the fate of marriage today may be in more trouble than we once thought. This means that families today face more difficult obstacles in creating and keeping the marriage and family situation healthy and successful

    A case of a canine transmissible venereal tumor, the contagious cancer

    History, signalment, and tumor location are important factors used to diagnose canine transmissible venereal tumor (TVT). There should be a high index of suspicion of this disease in any intact, sexually active dog, with a recent travel history that presents with a genital mass. Clinical signs, signalment, and cytological features are often sufficient for making a diagnosis of TVT, however, biopsy and histological examination may be necessary in cases with an atypical presentation. This case report outlines the workup and treatment of a dog diagnosed with TVT, a rarely encountered tumor at the Cornell University Hospital for Animals

    Policing and internal security in Northern Ireland 1920-1939

    In 1920, at the height of the Anglo-Irish War, the British Government made a lastditch attempt to stave off a blood bath between Catholics and Protestants by partitioning the island. Six ofthe nine counties of Ulster were to form the province of Northern Ireland, an area rougWy the size of Yorkshire, where Protestants were in the majority. Its affairs were to be run by a home rule government based in Belfast, while , a similar administration in Dublin would become responsible for the other twenty-six counties ofwhat was soon to be known as the Irish Free State.Although there were a number of fairly large cities within the new province it was, for the most part' of a ,rural nature with a complex border that weaved from the north Donegal coast, running ro~gWy south and then cut its way along the northern boundaries of Counties Monaghan, Cavan' and Louth to the Irish Sea. The creation of this border and of a Northern Irish state threw up problems which needed solving rapidly in order to stabilise the province and to establish the authority of the new Northern Government.
