    Tuberculosis in pediatric patients treated with anti-TNFα drugs: a cohort study

    Background: Adult patients receiving anti-TNFα drugs are at increased risk of tuberculosis (TB), but studies in pediatric populations are limited, and the best strategy for latent tuberculosis infection (LTBI) screening in this population remains controversial. We describe the prevalence of LTBI prior to anti-TNFα therapy and the long-term follow-up after biological treatment initiation in a cohort of children and adolescents. Methods: Cohort observational study in children and adolescents receiving anti-TNFα agents in a tertiary-care pediatric hospital. LTBI was ruled out prior to the implementation of anti-TNFα drugs by tuberculin skin test (TST), and, from March 2012 on, QuantiFERON Gold-In Tube® test (QTF-G). During anti-TNFα treatment, patients were evaluated every 6 months for TB with history and physical examination. TST/QTF-G were not repeated unless signs or symptoms consistent with TB arose or there was proven TB contact. Results: The final cohort consisted of 221 patients (56.1 % female; 261 treatments), of whom 51.7 %/30.0 %/17.3 % were treated with etanercept/adalimumab/infliximab, respectively, for a variety of rheumatic diseases (75.6 %), inflammatory bowel disease (20.8 %), and inflammatory eye diseases (3.6 %). The median (IQR) age at diagnosis of the primary condition was 6.8 years (2.7-11.0) and the duration of the disease before implementing the anti-TNFα agent was 1.8 years (0.6-4.2). LTBI was diagnosed in 3 adolescent girls (prevalence rate: 1.4 %; 95 % CI: 0.4-4.2) affected with juvenile idiopathic arthritis: TST tested positive in only 1, while QTF-G was positive in all cases (including 2 patients already on etanercept). They all received antiTB chemoprophylaxis and were later (re)treated with etanercept for 24-29 months, without incidences. No incident cases of TB disease were observed during the follow-up period under anti-TNFα treatment of 641 patients-year, with a median (IQR) time per patient of 2.3 years (1.4-4.3). Conclusions: In our study, the prevalence of LTBI (1.4 %) was similar to that reported in population screening studies in Spain; no incident cases of TB disease were observed. In low-burden TB settings, initial screening for TB in children prior to anti-TNFα treatment should include both TST and an IGRA test, but systematic repetition of LTBI immunodiagnostic tests seems unnecessary in the absence of symptoms or known TB contact

    Influencia de la prueba de detección rápida del virus de la gripe en el manejo del niño febril en Urgencias pediátricas

    Objetivos: Determinar las características clínicas de los pacientes diagnosticados de gripe con una prueba de detección rápida y constatar las diferencias de actuación en Urgencias con respecto a los pacientes con resultado negativo. Métodos: Revisión de las historias clínicas de aquellos pacientes a los que se solicitó una prueba de detección rápida para virus Influenza en aspirado nasofaríngeo. Resultados: Se solicitó el test en 142 pacientes (mediana de edad: 2,4 meses). Se obtuvo un resultado positivo en 63 (44,4%). La indicación principal fue fiebre asociada a clínica respiratoria (62,7%), seguida de fiebre sin foco (33,8%). De todos los pacientes con gripe sólo uno presentó un urocultivo positivo; los hemocultivos realizados y los cultivos de LCR fueron negativos. Al comparar los pacientes diagnosticados de gripe con aquéllos sin gripe no se detectaron diferencias en cuanto al sexo, edad, presencia de fiebre alta o de síntomas respiratorios. Por el contrario, los pacientes con gripe tenían menos dificultad respiratoria (1,6% vs 17,7%) y existía con más frecuencia ambiente epidémico en el domicilio (54% vs 31,6%) (p<0,01). A los pacientes con test de diagnóstico rápido positivo se les practicaron menos radiografías de tórax (30,2% vs 51,9%) (p<0,01), ingresaron menos (19% vs 57%) (p<0,001) y recibieron menos antibiótico (14,3% vs 40,5%) (p<0,001). En el subgrupo de 25 pacientes menores de 1 mes no se encontraron diferencias en ninguno de los parámetros estudiados. Conclusiones: Los pacientes con test positivo para Influenza presentan escasas características clínicas diferenciales. Disponer de técnicas de diagnóstico rápido para la gripe en Urgencias cambia el manejo diagnóstico- terapéutico en los niños de 1 a 36 meses con fiebre

    Streptococcus agalactiae, or group B streptococci (GBS), is the main aetiological agent of early neonatal sepsis in developed countries. This microorganism belongs to the gastrointestinal tract microbiota wherefrom it can colonize the vagina and be vertically transmitted to the child either before or at birth, and subsequently cause infection in the newborn. Approximately, 50% of newborns born to women with GBS become colonized, with 1–2% developing early neonatal infection if no preventive intervention is performed. The aim of this study was to characterize and compare serotypes, virulence factors and antimicrobial resistance of GBS isolates collected from pregnant women and newborns in several hospitals in Catalonia. Methods 242 GBS strains were analyzed including 95 colonizers and 68 pathogenic strains isolated from pregnant women, and 79 strains isolated from neonates with sepsis in order to determine serotype, virulence and antimicrobial resistance. Results Serotype distribution was different among the three groups, with serotypes Ia and II being significantly more frequent among colonizing strains (p = 0.001 and 0.012, respectively). Virulence factors bca and scpB were significantly more frequent among neonatal strains than pathogenic or colonizing strains (p = 0.0001 and 0.002, respectively). Pathogenic strains were significantly more resistant to erythromycin, clindamycin and azithromycin than their non-pathogenic counterparts. Conclusions Taking into account that neonatal sepsis represents a significant problem on a global scale, epidemiological surveillance, antimicrobial resistance and GBS virulence at the local level could provide important knowledge about these microorganisms as well as help to improve treatment and prevent invasive infection caused by this microorganism
