    Assessment of the Impact of Sustainable Land Use Practices on Food Security in West Usambara Mountains, Tanzania

    This paper assessed the impact(s) of practicing sustainable land management (SLM) technology on food security in West Usambara Mountains. Primary data were collected through household questionnaires, focus group discussions, key informants interviews and personal observations while secondary data were collected from relevant local authority reports and records. A total of 160 households were interviewed. Research results suggest that, the area is still experiencing soil erosion problem reported by 61.9% of the respondents although at reduced scale. Multiple linear regression model to establish the contributing factors revealed that age of household head, farmland ownership and household income have significant and positive impact on improving household dietary diversity and hence improves food security while non adopters of SLM showed declined dietary diversity. The study conclude that in order to increase food security keeping soil health in place through practicing SLM is a necessary condition.The study concludes that, to increase your food security keep your soil in place by practicing SLM

    Policy Options to Enhance Markets for Nutrient-Dense Foods in Tanzania

    There is an urgent need to reduce alarming rates of undernutrition in Tanzania. This report analyses options for policies and interventions to improve the functioning of markets that deliver nutrient-dense foods. Currently, a set of constraints inhibit businesses from making nutrient-dense foods that reach the poor. These constraints affect businesses across the spectrum, from small enterprises to large corporations, and impact on a range of food products. This report examines the case for why government and development actors in Tanzania should act to overcome these constraints. It asks how these actors might intervene by looking at five broad strategies, including various forms of regulation and public–private partnership. While no single strategy can address all the constraints completely, different interventions can address some of them and benefit certain populations. For any strategy, the first steps should be to identify the primary constraints facing a particular market and assess whether a particular approach will overcome them. The report outlines specific options for each intervention strategy, aimed at national government, donors, private-sector and non-profit organisations working in Tanzania.     Narrowing the wood supply gap through on-farm wood production in Tanzania

    Based on recent estimates, the average demand for wood at 1.39 m3/year/capita surpasses the average annual allowable cut at 0.95 m3/year/capita. The annual wood supply deficit in Tanzania is estimated to be 19.5 million m3. Underlying causes for the wood supply gap include: weak value chains that undermine market access, unsustainable wood production, and harvesting practices. Policies targeted at strengthening the institutional capacity of the forestry sector at local and national levels are suggested to increase the share contribution of the sector to the national economy. To foster small-scale wood production, government needs to expedite efforts in tenure security, and make financial services, extension services, and market information accessible to farmers. Continuous collaboration between the government and actors in the wood value chain is crucial.     Prevalence and intensity of Schistosoma mansoni and hookworm infections among pre-school and school-aged children in Ilemela District, north-western Tanzania

    Background: World Health Organization have recently recommended the inclusion of pre-school children in the Mass Drug Administration (MDA) against schistosomiasis and soil-transmitted helminths in endemic areas. This study was conducted to determine the prevalence and intensity of Schistosoma mansoni and hookworm infections among pre- and school going children in Ilemela District, north-western Tanzania.Methods: This cross-sectional study included pre- and school going children aged 4-14 years. A single stool sample was collected from each child and processed using Kato Katz thick smears and examined microscopically for presence of S. mansoni and hookworm eggs. A questionnaire was used to collect demographic information of the study participants.Results: Overall, prevalence of S. mansoni was 80.0%; with pre-school children aged 4-6 years having the point prevalence of 60.6%. The overall prevalence of hookworm infection was 18.7%; with age group 4-6 years having the prevalence of 14.1%. The intensity of hookworm infection was light in all age groups.  The intensity of infection of S. mansoni increased with age. Using lake water for domestic purposes (OR=3.09, 95% CI: 1.93-4.95, p<0.001), for bathing (OR=2.65, 95% CI: 1.66-4.23, p<0.0001), and for washing purposes (OR=3.08, 95% CI: 1.90-4.97, p<0.0001) remained independently associated with S. mansoni infection. Children who reported to swim in the lake and involved in paddy farming had 1.84 and 1.95 times odds of being infected than those who did not, respectively.Conclusion: These findings indicate that S. mansoni and hookworm infections are common among pre-school children as well as in school going children. These findings call for the need to urgently include the pre-school age children the MDSA programme

    Markets for Nutrition: What Role for Business?

    Policymakers are increasingly seeking to use food systems to help reduce rates of chronic undernutrition and to use markets to deliver nutrient?rich foods to vulnerable populations. This article examines how this might be achieved, drawing lessons from three intervention types: ready?to?use therapeutic foods (RUTFs), mandatory fortification and voluntarily fortified products. We find that a common set of constraints tends to inhibit markets from delivering nutrition and makes it difficult to reach populations at the ‘bottom of the pyramid’. Overcoming these constraints requires a shift from working at the level of individual businesses to that of market and food systems. It also suggests a need for renewed focus on the effectiveness of products in reaching key groups, on the informal markets that serve the poor and on the inherent complexity of market systems. These findings suggest that food and nutrition policies and partnerships should be based on principles of experimentation and adaptive learning

    Building Clinical Clerkships Capacity in a Resource-limited Setting: The Case of the Kilimanjaro Christian Medical University College in Tanzania

    Background: The shortage of medical doctors in sub-Saharan Africa (SSA) has resulted in increased enrolment in medical schools, which has not been matched with increased faculty size or physical infrastructure. This process has led to overcrowding and possibly reduced quality of training. To reduce overcrowding at its teaching hospital, the Kilimanjaro Christian Medical University College introduced eight-week peripheral clerkship rotations in 2012. We explore students’ perceptions and attitudes towards peripheral hospital placements. Method: The clerkship rotations were conducted in eight hospitals operating in the northern Tanzania, after evaluating each hospitals’ capabilities and establishing the optimum number of students per hospital. Paper-based surveys were conducted after student rotations from 2014 to 2016. Results: Overall student satisfaction was moderate (strength of consensus measure (sCns), 77%). The three cohorts exhibited improving trends over three years with respect to satisfaction with clinical skills and attitude towards placements. student-preceptor interaction was rated highly (sCns 81–84%). The first cohort students expressed concerns about limited laboratory support, and poor access to Internet and learning resources. Specific interventions were undertaken to address these concerns. Conclusions: Student experiences in peripheral rotations were positive with adequate satisfaction levels. Opportunities exist for medical schools in SSA to enhance clinical training and relieve overcrowding through peripheral clerkship rotations.<p

    "Half plate of rice to a male casual sexual partner, full plate belongs to the husband": Findings from a qualitative study on sexual behaviour in relation to HIV and AIDS in northern Tanzania

    A thorough understanding of the contexts of sexual behaviour of the people who are vulnerable to HIV infection is an important component in the battle against AIDS epidemic. We conducted a qualitative study to investigate perceptions, attitudes and practices of sexually active people in three districts of northern Tanzania with the view of collecting data to inform the formulation of appropriate complementary interventions against HIV and AIDS in the study communities. We conducted 96 semi-structured interviews and 48 focus group discussions with sexually active participants (18-60 years of age) who were selected purposively in two fishing and one non-fishing communities. The study revealed a number of socio-economic and cultural factors which act as structural drivers of HIV epidemic. Mobility and migration were mentioned to be associated with the risk of HIV acquisition and transmission. Sexual promiscuous behaviour was common in all study communities. Chomolea, (a quick transactional sex) was reported to exist in fishing communities, whereas extramarital sex in the bush was reported in non-fishing community which was predominantly Christian and polygamous. Traditional practices such as Kusomboka (death cleansing through unprotected sex) was reported to exist. Other risky sexual behaviour and traditional practices together with their socio-economic and cultural contexts are presented in details and discussed. Knowledge of condom was low as some people mistook them for balloons to play with and as decorations for their living rooms. Acute scarcity of condoms in some remote areas such as vizingani (fishing islands) push some people to make their own condoms locally known as kondomu za pepsi using polythene bags. HIV prevention efforts can succeed by addressing sexual behaviour and its socio-economic and cultural contexts. More innovative, interdisciplinary and productive structural approaches to HIV prevention need to be developed in close collaboration with affected communities and be closely related to policy-making and implementation; to go beyond the limited success of traditional behavioural and biomedical interventions to particularly address the underlying social and structural drivers of HIV risk and vulnerability in the study communities

    Identification of field caught Anopheles gambiae s.s. and Anopheles arabiensis by TaqMan single nucleotide polymorphism genotyping

    BACKGROUND: Identification of Anopheles gambiae s.s. and Anopheles arabiensis from field-collected Anopheles gambiae s.l. is often necessary in basic and applied research, and in operational control programmes. The currently accepted method involves use of standard polymerase chain reaction amplification of ribosomal DNA (rDNA) from the 3' 28S to 5' intergenic spacer region of the genome, and visual confirmation of amplicons of predicted size on agarose gels, after electrophoresis. This report describes development and evaluation of an automated, quantitative PCR method based upon TaqManâ„¢ single nucleotide polymorphism (SNP) genotyping. METHODS: Standard PCR, and TaqMan SNP genotyping with newly designed primers and fluorophore-labeled probes hybridizing to sequences of complementary rDNA specific for either An. gambiae s.s. or An. arabiensis, were conducted in three experiments involving field-collected An. gambiae s.l. from western Kenya, and defined laboratory strains. DNA extraction was from a single leg, sonicated for five minutes in buffer in wells of 96-well PCR plates. RESULTS: TaqMan SNP genotyping showed a reaction success rate, sensitivity, and species specificity comparable to that of standard PCR. In an extensive field study, only 29 of 3,041 (0.95%) were determined to be hybrids by TaqMan (i.e., having rDNA sequences from both species), however, all but one were An. arabiensis by standard PCR, suggesting an acceptably low (ca. 1%) error rate for TaqMan genotyping in mistakenly identifying species hybrids. CONCLUSION: TaqMan SNP genotyping proved to be a sensitive and rapid method for identification of An. gambiae s.l. and An. arabiensis, with a high success rate, specific results, and congruence with the standard PCR method
