    Towards the MSSM from F-theory

    We study the MSSM in F-theory. Its group is the commutant to a structure group SU(5) x U(1)_Y of a gauge bundle in E_8. The spectrum contains three generations of quarks and leptons plus vectorlike electroweak and colored Higgses.     Early detection of cerebral ischemia due to pericardium traction using cerebral oximetry in pediatric minimally invasive cardiac surgery: a case report

    Background Minimally invasive cardiac surgery (MICS) for simple congenital heart defects has become popular, and monitoring of regional cerebral oxygen saturation (rSO2) is crucial for preventing cerebral ischemia during pediatric MICS. We describe a pediatric case with a sudden decrease in rSO2 during MICS. Case presentation An 8-month-old male underwent minimally invasive ventricular septal defect closure. He developed a sudden decrease in rSO2 and right radial artery blood pressure (RRBP) without changes in other parameters following pericardium traction. The rSO2 and RRBP immediately recovered after removal of pericardium fixation. Obstruction of the right innominate artery secondary to the pericardium traction would have been responsible for it. Conclusions Pericardium traction, one of the common procedures during MICS, triggered rSO2 depression alerting us to the risk of cerebral ischemia. We should be aware that pericardium traction during MICS can lead to cerebral ischemia, which is preventable by cautious observation of the patient

    Glucagon-like peptide 1 receptor agonists (GLP-1 RAs) have been attracted attention for various beneficial effects. Among them, oral semaglutide (Rybelsus) was developed using the absorption enhancer sodium N-(8-[2-hydroxybenzoyl] amino) caprylate (SNAC). The relationship between the bioavailability and post-dose fasting time period was analyzed. The results showed that i) its bioavailability with drinking 50mL and 120mL of water together was almost similar, ii) bioavailability for 240mL intake was about 2/3 of 50-120mL, iii) general availability would be about 1.4% for 50-120ml water, and 0.8% for 240mL of water. Tolerability and safety of Rybelsus showed similar results for healthy subjects, patients with renal and/or hepatic impairment. From mentioned above, Rybelsus has been one of the topic agents with characteristic mechanism of SNAC and clinical benefit of GLP-1RA. It will be expected to be applied widely in actual diabetic practice
