    Associations between age and sleep apnea risk among newborn infants

    ObjectiveAmong older children, sleep‐disordered breathing (SDB) is associated with measurable neurocognitive consequences. However, diagnostic SDB thresholds are lacking for infants < 12 months. We sought to evaluate the relationship between SDB indices, gestational age (GA), and postmenstrual age (PMA) for infants who underwent clinically‐indicated polysomnograms at a tertiary care center.MethodsEvery infant < 3‐months chronological age whose first clinically‐indicated polysomnogram was between 2/2012 and 2/2017 was included. Linear regression was used to evaluate associations between apnea‐hypopnea index (AHI), obstructive‐apnea index (OAI), and GA and PMA for infants with and without obvious clinical risk factors for SDB (eg, micrognathia and cleft palate).ResultsFor 53 infants without obvious SDB risk factors (GA 35.6 ± 4.5 weeks; PMA 41.2 ± 4.0 weeks), mean AHI was 27 ± 18 and OAI 2.9 ± 4.5. There was a weak inverse relationship between AHI and PMA (r2 = 0.12, P = 0.01), but AHI was not predicted by GA (r2 = 0.04, P = 0.13). Conversely, OAI was more strongly associated with GA (r2 = 0.33, P < 0.0001) than PMA (r2 = 0.08, P = 0.036). For 28 infants with congenital structural anomalies that predispose to SDB (GA 38.0 ± 3.1 weeks, PMA 43.1 ± 3.3 weeks, AHI 37.7 ± 30, OAI 8.2 ± 11.8), neither AHI nor OAI were related to PMA or GA.ConclusionsAmong infants who received clinically‐indicated polysomnograms but did not have obvious structural risk for SDB, AHI declined with advancing PMA, but obstructive‐apnea was best predicted by prematurity.     Placenta-specific methylation of the vitamin D 24-hydroxylase gene: implications for feedback autoregulation of active vitamin D levels at the fetomaternal interface

    Plasma concentrations of biologically active vitamin D (1,25- (OH)2D) are tightly controlled via feedback regulation of renal 1-hydroxylase (CYP27B1; positive) and 24-hydroxylase (CYP24A1; catabolic) enzymes. In pregnancy, this regulation is uncoupled, and 1,25-(OH)2D levels are significantly elevated, suggesting a role in pregnancy progression. Epigenetic regulation of CYP27B1 and CYP24A1 has previously been described in cell and animal models, and despite emerging evidence for a critical role of epigenetics in placentation generally, little is known about the regulation of enzymes modulating vitamin D homeostasis at the fetomaternal interface. In this study, we investigated the methylation status of genes regulating vitamin D bioavailability and activity in the placenta. No methylation of the VDR (vitamin D receptor) and CYP27B1 genes was found in any placental tissues. In contrast, the CYP24A1 gene is methylated in human placenta, purified cytotrophoblasts, and primary and cultured chorionic villus sampling tissue. No methylation was detected in any somatic human tissue tested. Methylation was also evident in marmoset and mouse placental tissue. All three genes were hypermethylated in choriocarcinoma cell lines, highlighting the role of vitaminDderegulation in this cancer. Gene expression analysis confirmed a reduced capacity for CYP24A1 induction with promoter methylation in primary cells and in vitro reporter analysis demonstrated that promoter methylation directly down-regulates basal promoter activity and abolishes vitamin D-mediated feedback activation. This study strongly suggests that epigenetic decoupling of vitamin D feedback catabolism plays an important role in maximizing active vitamin D bioavailability at the fetomaternal interface

    Human papillomavirus infections in women seeking cervical Papanicolaou cytology of Durango, Mexico: prevalence and genotypes

    BACKGROUND: HPV infection in women from developing countries is an important public health problem. Therefore, we sought to determine the prevalences of HPV infection and HPV genotypes in a female population of Durango City, Mexico. Also to determine whether any socio-demographic characteristic from the women associated with HPV infection exists. METHODS: Four hundred and ninety eight women seeking cervical Papanicolaou examination in three public Health Centers were examined for HPV infection. All women were tested for HPV DNA PCR by using HPV universal primers. In addition, all positive HPV DNA PCR samples were further analyzed for genotyping of HPV genotype 16, 18 and 33. Socio-demographic characteristics from each participant were also obtained. RESULTS: Twenty-four out of four hundred and ninety-eight (4.8%) women were found infected by HPV. HPV genotype 16 was found in 18 out of the 24 (75%) infected women. Two of them were also coinfected by HPV genotype 18 (8.3%). In the rest 6 PCR positive women, genotyping for HPV genotypes 16, 18 and 33 were negative. CONCLUSION: The prevalence of HPV in women of Durango City is low; however, most infected women have high risk HPV genotype. The women who were studied showed low frequency of risk factors for HPV infection and this may explain the low prevalence of HPV infection. The high frequency of high risk HPV genotypes observed might explain the high rate of mortality for cervical cancer in our region

    Potential Benefits of Sequential Inhibitor-Mutagen Treatments of RNA Virus Infections

    Lethal mutagenesis is an antiviral strategy consisting of virus extinction associated with enhanced mutagenesis. The use of non-mutagenic antiviral inhibitors has faced the problem of selection of inhibitor-resistant virus mutants. Quasispecies dynamics predicts, and clinical results have confirmed, that combination therapy has an advantage over monotherapy to delay or prevent selection of inhibitor-escape mutants. Using ribavirin-mediated mutagenesis of foot-and-mouth disease virus (FMDV), here we show that, contrary to expectations, sequential administration of the antiviral inhibitor guanidine (GU) first, followed by ribavirin, is more effective than combination therapy with the two drugs, or than either drug used individually. Coelectroporation experiments suggest that limited inhibition of replication of interfering mutants by GU may contribute to the benefits of the sequential treatment. In lethal mutagenesis, a sequential inhibitor-mutagen treatment can be more effective than the corresponding combination treatment to drive a virus towards extinction. Such an advantage is also supported by a theoretical model for the evolution of a viral population under the action of increased mutagenesis in the presence of an inhibitor of viral replication. The model suggests that benefits of the sequential treatment are due to the involvement of a mutagenic agent, and to competition for susceptible cells exerted by the mutant spectrum. The results may impact lethal mutagenesis-based protocols, as well as current antiviral therapies involving ribavirin

    El propósito fue describir las principales circunstancias que generan tentación al uso de drogas en pacientes que reciben tratamiento, relación con el tiempo de tratamiento y uso actual de drogas. El estudio fue descriptivo correlacional, el muestreo fue no probabilístico convencional por cuota, muestra de 61 participantes. El intrumento Escala de Tentaciones de Uso de Drogas con consistencia interna de .93. Las principales circunstancias que generan tentación a usar drogas fueron; que estén en lugares donde todos usen drogas, estar con alguien que use drogas, ir a fiestas con los amigos, observar a alguien usar y disfrutar drogas. A menor tiempo de tratamiento mayor es la percepción de tentación a usar drogas. Aquellos que refirieron consumir algún tipo de droga reportaron medias mas altas de tentación que aquellos que no consumen. Los que indicaron como droga de impacto, mariguana y otras drogas ilícitas, mostraron mayor tentación al uso de drogas.<br>O objetivo foi investigar as principais circunstancias que geram tentações para o uso de drogas em pacientes que receberam tratamento, a relação com o tempo de tratametno e o uso atual de drogas. Este é um estudo descritivo correlacional, a amostra foi não probabilistica convencional por cota, com 61 participantes. O intrumento Escala de Tentaciones de Uso de Drogas com consistência interna de .93. As principais circunstâncias que geraram tentação para o uso de drogas foi estar em locais onde todos usam drogas, estar com alguém que usa drogas, ir às festas com os amigos, observar alguien usando e disfrutam as drogas. Para o menor tempo de tratamento foi percebido as maiores tentación para o uso de drogas. Aqueles que refiriram consumir algum tipo de droga relataram médias mais altas de tentação que aqueles que não consomem. Os que indicaram como droga de impacto a maconha e outras drogas ilícitas, mostraram maior tentação ao uso de drogas.<br>This study aimed to describe the main circumstances that tempt patients under treatment to use drugs, their relation with treatment time and current drugs use. A descriptive and correlational study was carried out with conventional non probabilistic quota sampling. The sample consisted of 61 participants. The Temptation to Use Drugs Scale revealed an internal consistency of .93. The main circumstances tempting towards drugs use were: being in places where everybody uses drugs, being with somebody who uses drugs, going to parties with friends, seeing somebody who uses and has fun with drugs. The shorter the treatment time, the greater the perception of temptations to use drugs. Those persons who mentioned they use some kind of drugs reported on higher temptation averages than those who do not use these substances. Those mentioning marihuana and other illicit substances as impact drugs demonstrated greater temptation towards drugs use
