    En tolkning – och omtolkning – av ett benföremĂ„l med djurhuvud frĂ„n mĂ„nberget i Åbo

    Hösten 1959 hittades ett mĂ€rkligt spetsigt benföremĂ„l av lokala invĂ„nare pĂ„ MĂ„nberget i Åbo. UtgĂ„ende frĂ„n paralleller till tre benföremĂ„l hittade i Lund, kom föremĂ„let att bli katalogiserat som ett skrivverktyg, och mer precist; en medeltida griffel (stylus). En AMS-datering hösten 2013 av nĂ€mnda föremĂ„l visade dock att det Ă€r betydligt yngre; 150 +-30 före nutid. Det hĂ€r innebĂ€r att föremĂ„let kan dateras till en period frĂ„n sent 1600-tal till tidigt 1900-tal. Det hĂ€r resultatet pĂ„verkar tolkningen av föremĂ„let och dess antagna funktion betydligt. Den hĂ€r artikeln tar upp forskningsprocessen kring MĂ„nbergsfyndet och analyserar det utgĂ„ende frĂ„n nya jĂ€mförbara fynd, scanner- och printerteknik samt resultat frĂ„n C14-dateringar. Med grund i den hĂ€r analysen presenteras en kritik mot att dessa benföremĂ„l tolkats som grifflar. IstĂ€llet mĂ„ste nya tolkningar sökas. Det Ă€r plausibelt att MĂ„nbergsfyndet och dess paralleller var de facto nĂ„lliknande verktyg som anvĂ€nts i nĂ„got numera bortglömt hantverk, möjligen med maskulin och/eller maritim prĂ€gel, som fallit utanför vĂ„r nuvarande arkeologiska och etnologiska expertis i studiet av materiell kultur

    UriSed 3 PRO automated microscope in screening bacteriuria at region-wide laboratory organization

    Background and aims: We assessed the possibility to rule out negative urine cultures by counting with UriSed 3 PRO (77 Elektmnika, Hungary) at Helsinki and Uusimaa Hospital District. Materials and methods: Bacteria counting of the UriSed 3 PRO automated microscope was verified with reference phase contrast microscopy against growth in culture. After acceptance into routine, results of bacteria and leukocyte counting from 56 426 specimens with eight UriSed 3 PRO instruments were compared against results from parallel samples cultured on chromogenic agar. Laboratory data including preanalytical details were accessed through the regional database of the Helsinki and Uusimaa Hospital District. Results: A combined sensitivity of 87-92% and a negative predictive value of 90-96% with a specificity of 54-50% was reached, depending on criteria. Preanalytical data (incubation time in bladder) combined with the way of urine collection would improve these figures if reliable. Conclusions: Complex patient populations, regional logistics and data interfases, and economics related to increased costs of additional particle counts against costs of screening cultures of all samples, did not support adaptation of a screening process of urine cultures. We present the Euro-GASP results from 2019 (26 countries), linked to patient epidemiological data, and compared with data from previous years. Methods: Agar dilution and minimum inhibitory concentration (MIC) gradient strip methodologies were used to determine the antimicrobial susceptibility (using EUCAST clinical breakpoints, where available) of 3239 N. gonorrhoeae isolates from 26 countries across the EU/EEA. Significance of differences compared with Euro-GASP results in previous years was analysed using Z-test and the Pearson's χ2 test was used to assess significance of odds ratios for associations between patient epidemiological data and antimicrobial resistance. Results: European N. gonorrhoeae isolates collected between 2016 and 2019 displayed shifting MIC distributions for; ceftriaxone, with highly susceptible isolates increasing over time and occasional resistant isolates each year; cefixime, with highly-susceptible isolates becoming increasingly common; azithromycin, with a shift away from lower MICs towards higher MICs above the EUCAST epidemiological cut-off (ECOFF); and ciprofloxacin which is displaying a similar shift in MICs as observed for azithromycin. In 2019, two isolates displayed ceftriaxone resistance, but both isolates had MICs below the azithromycin ECOFF. Cefixime resistance (0.8%) was associated with patient sex, with resistance higher in females compared with male heterosexuals and men-who-have-sex-with-men (MSM). The number of countries reporting isolates with azithromycin MICs above the ECOFF increased from 76.9% (20/26) in 2016 to 92.3% (24/26) in 2019. Isolates with azithromycin MICs above the ECOFF (9.0%) were associated with pharyngeal infection sites. Following multivariable analysis, ciprofloxacin resistance remained associated with isolates from MSM and heterosexual males compared with females, the absence of a concurrent chlamydial infection, pharyngeal infection sites and patients ≄ 25 years of age. Conclusions: Resistance to ceftriaxone and cefixime remained uncommon in EU/EEA countries in 2019 with a significant decrease in cefixime resistance observed between 2016 and 2019. The significant increase in azithromycin "resistance" (azithromycin MICs above the ECOFF) threatens the effectiveness of the dual therapy (ceftriaxone + azithromycin), i.e., for ceftriaxone-resistant cases, currently recommended in many countries internationally and requires close monitoring.The study was funded by the European Centre for Disease Prevention and Control (Framework Contract No. ECDC/2017/004).info:eu-repo/semantics/publishedVersio

    Efficiency of Finnish Upper Secondary Schools: An Application of Stochastic Frontier Analysis with Panel Data

    In this study the efficiency of Finnish upper secondary schools is evaluated with stochastic frontier analysis. Different stochastic frontier models for panel data are used to estimate education production functions. The results in matriculation examination are explained with comprehensive school grade point average, parents' socioeconomic background, resources, length of studies and decentralization of test taking on matriculation examination. Controls for schools with specialized curriculum are also included. The heterogeneity across schools is allowed by estimating both true random and true fixed effects models. The results show that the effect of teaching resources on examination results is even negative when the heterogeneity across schools is taken into account. Length of studies and decentralization of test taking affected negatively on student achievement. The inefficiency and the rankings of schools based on inefficiency score varied quite considerably depending on the type of stochastic frontier model. The lowest estimates for inefficiency were obtained with true random and true fixed effects models that separate time constant random or fixed effects from inefficiency.Tutkimuksessa tarkasteltiin lukioiden tehokkuutta estimoimalla erilaisia tuotantofunktioita stokastisella rintama-analyysilla kÀyttÀen paneeli-aineistoa. Ylioppilaskirjoitusten pakollisten aineiden puoltoÀÀnten mÀÀrÀÀ selitettiin peruskoulun pÀÀttötodistuksen keskiarvolla, vanhempien sosio-ekonomisella taustalla, resursseilla, opiskelun kestolla ja ylioppilastutkinnon hajauttamisella. Erikoislukiot otettiin huomioon malleissa omilla muuttujillaan. Koulujen vÀlinen ajassa pysyvÀ heterogeenisuus kontrolloitiin estimoimalla varsinaisia satunnaisten ja kiinteiden vaikutusten malleja. Tulosten mukaan resurssien vaikutus on jopa negatiivinen kun malleissa otetaan huomioon koulujen vÀlinen heterogeenisuus. Opiskelun kestolla ja ylioppilastutkinnon hajauttamisella oli negatiivinen vaikutus suoritustasoon. Tehottomuus ja koulujen tehottomuuden mukainen jÀrjestys vaihteli melko paljon stokastisesta rintamamallista riippuen. KeskimÀÀrÀinen tehottomuus oli alhaisinta varsinaisten satunnaisten ja kiinteiden vaikutusten malleissa, jotka ottivat huomioon koulujen vÀlisen ajassa pysyvÀn heterogeenisuuden
