    Time dependence of breakdown in a global fiber-bundle model with continuous damage

    A time-dependent global fiber-bundle model of fracture with continuous damage is formulated in terms of a set of coupled non-linear differential equations. A first integral of this set is analytically obtained. The time evolution of the system is studied by applying a discrete probabilistic method. Several results are discussed emphasizing their differences with the standard time-dependent model. The results obtained show that with this simple model a variety of experimental observations can be qualitatively reproduced.Comment: APS style, two columns, 4 figures. To appear in Phys. Rev.

    Interdependent network reciprocity in evolutionary games

    Besides the structure of interactions within networks, also the interactions between networks are of the outmost importance. We therefore study the outcome of the public goods game on two interdependent networks that are connected by means of a utility function, which determines how payoffs on both networks jointly influence the success of players in each individual network. We show that an unbiased coupling allows the spontaneous emergence of interdependent network reciprocity, which is capable to maintain healthy levels of public cooperation even in extremely adverse conditions. The mechanism, however, requires simultaneous formation of correlated cooperator clusters on both networks. If this does not emerge or if the coordination process is disturbed, network reciprocity fails, resulting in the total collapse of cooperation. Network interdependence can thus be exploited effectively to promote cooperation past the limits imposed by isolated networks, but only if the coordination between the interdependent networks is not disturbe

    Aperiodicity in one-way Markov cycles and repeat times of large earthquakes in faults

    A common use of Markov Chains is the simulation of the seismic cycle in a fault, i.e. as a renewal model for the repetition of its characteristic earthquakes. This representation is consistent with Reid's elastic rebound theory. Here it is proved that in {\it any} one-way Markov cycle, the aperiodicity of the corresponding distribution of cycle lengths is always lower than one. This fact concurs with observations of large earthquakes in faults all over the world

    Do you have what it takes? An analysis of the factors associated with the intention to choose a surgical career among medical undergraduates

    Resumen Introducción Los cambios en la práctica profesional han transformado la intención de los estudiantes de medicina de optar por una carrera en cirugía. Objetivo Evaluar los factores asociados con esta intención en un contexto de acelerada dinámica educativa y sanitaria. Materiales y Método Evaluamos la intención de los estudiantes de diez facultades de medicina en Colombia, y los factores asociados, mediante un cuestionario (escala de Likert) desarrollado a partir de la revisión de la literatura. Identificamos la asociación entre los factores y la intención mediante un análisis de regresión lineal. Los resultados de la regresión se presentan mediante coeficientes ? (p < 0,05). Resultados Un total de 252 estudiantes (65,87% mujeres) fueron incluidos en el análisis. El promedio de la intención fue 3,15 ± 1,34 (1-5). Los factores significativamente asociados fueron: identificación con el estilo de vida quirúrgico (? = 0,25); identificación con modelos (? = 0,18); atributos personales (? = 0,25) e identificación con la práctica profesional del cirujano (? = 0,16). No encontramos asociación significativa con el género (mujeres en la especialidad), prestigio, dinámica del posgrado ni recompensa futura. Conclusión Existe una intención moderada de elegir una carrera de cirugía entre estudiantes de medicina colombianos. Optar por esta carrera requiere atributos personales, modelos e identificación con el estilo de vida y práctica profesional del cirujano. Se requieren estudios cualitativos que exploren en profundidad estos resultados e intervenciones focalizadas a estimular esta intención entre los estudiantes.Introduction The changes in the professional practice have transformed the intention of medical students to choose a surgical career. Aim Evaluate the factors associated with this intention in a context of accelerated educational and health dynamics. Materials and Method We evaluated the intention of the students of ten medical schools in Colombia, and the associated factors, through a questionnaire (Likert scale) settled from a literature review. We identify the association between the factors and the intention by using linear regression analysis. The results of the regression are presented by ? coefficients (p < 0.05). Results A total of 252 students (65.87% women) were included in the analysis. The average of the intention was 3.15 ± 1.34 (1-5). The factors significantly associated were: identification with the surgical lifestyle (? = 0.25); identification with models (? = 0.18); personal attributes (? = 0.25) and identification with the professional practice of the surgeon (? = 0.16). We did not find significant associations with gender (women in the specialty), prestige, postgraduate dynamics and future rewards. Conclusion There is a moderate intention to choose a career in surgery among Colombian medical undergraduates. Choosing this career requires personal attributes, models and identification with the lifestyle and professional practice of the surgeon. Further qualitative studies are necessary to explore these results in depth, as well as interventions to stimulate this intention among students

    Complicaciones de la colocación de malla sintética vaginal para la incontinencia urinaria de esfuerzo

    ResumenAntecedentesLas mallas para la incontinencia urinaria (IU) se han utilizado a pesar de la falta de evidencia de nivel i en la literatura para evaluar su seguridad y eficacia a largo plazo. Las complicaciones relacionadas con la malla provocaron una advertencia emitida por la FDA en 2008 y dejaron de ser consideradas eventos raros. Las complicaciones más comunes son la retención aguda de orina, sangrado, recurrencia o persistencia de la IU, erosión de uretra e infección de la malla.ObjetivoConocer cuáles son las complicaciones derivadas del tratamiento quirúrgico de la IU femenina con la colocación de mallas suburetrales.Material y métodoSe analizaron los pacientes con diagnóstico de IU operadas con cirugía antiincontinencia y colocación de malla vaginal del año 2007 al 2014. Se analizó las complicaciones derivadas de la colocación de malla y la recuperación de continencia.ResultadosA 58 pacientes se les colocó sling (cabestrillo) con malla sintética; edad promedio de 56 años. El 51% de las pacientes tenían IU de esfuerzo y el 49% IU mixta. Las complicaciones tempranas fueron: 4 pacientes con retención aguda de orina. Las complicaciones tardías: dolor uretral o vaginal (12.2%), dispareunia (20%) y extrusión de malla (10%) manejado con retiro de la misma, 2 de ellas requiriendo 2 procedimientos quirúrgicos. El 12% presentaron urgencia de novo, el 90.2% evolucionaron sin incontinencia, el 9.7% con IU postoperatoria y 2 pacientes sin mejoría.DiscusiónNuestra serie, aunque pequeña, muestra una baja tasa de complicaciones a largo plazo relacionadas con la malla en comparación con la mayoría de las series que muestran una tasa de reoperación de hasta>70% y con múltiples casos de retiro de malla, por lo que en nuestra experiencia los slings suburetrales continúan siendo una opción adecuada en el manejo de la IU.ConclusionesLos slings suburetrales son una opción segura y efectiva en el manejo de la IU.AbstractBackgroundSurgical mesh has been used for treating urinary incontinence (UI) despite the lack of level i evidence in the literature evaluating its long-term safety and efficacy. Mesh-related complications were responsible for a warning issued by the FDA in 2008 and they stopped being considered rare events. The most common complications are acute urinary retention, bleeding, recurrence or persistence of UI, erosion of the urethra, and mesh infection.AimsTo determine the complications derived from surgical UI treatment with the placement of suburethral mesh in women.Material and methodPatients diagnosed with UI that underwent anti-incontinence surgery with the placement of vaginal mesh within the time frame of 2007 and 2014 were analyzed. Mesh placement complications and the recovery of continence were evaluated.ResultsA synthetic mesh sling was placed in 58 patients with a mean age of 56 years. A total of 51% of the patients had stress urinary incontinence and 49% had mixed urinary incontinence. Early complications were: 4 patients with acute urine retention. Late complications were: urethral or vaginal pain (12.2%), dyspareunia (20%), and mesh extrusion (10%) that was managed through mesh removal; 2 of those patients required surgical procedures. Twelve percent of the patients presented with de novo urgency, 90.2% progressed with no incontinence, 9.7% presented with postoperative UI, and 2 patients had no improvement.DiscussionAlthough small, our case series showed a low long-term complication rate with the use of surgical mesh, compared with the majority of case series that demonstrate a re-operation rate>70%, as well as numerous cases of mesh removal. Thus, it is our experience that suburethral slings continue to be an adequate UI management option.ConclusionsSuburethral slings are a safe and effective option in UI management

    Electromagnetic-field quantization and spontaneous decay in left-handed media

    We present a quantization scheme for the electromagnetic field interacting with atomic systems in the presence of dispersing and absorbing magnetodielectric media, including left-handed material having negative real part of the refractive index. The theory is applied to the spontaneous decay of a two-level atom at the center of a spherical free-space cavity surrounded by magnetodielectric matter of overlapping band-gap zones.     Differential leukocyte expression of IFITM1 and IFITM3 in patients with severe pandemic influenza A(H1N1) and COVID-19

    Interferon-induced transmembrane (IFITM) proteins mediate protection against enveloped viruses by blocking membrane fusion at endosomes. IFITM1 and IFITM3 are crucial for protection against influenza, and various single nucleotide polymorphisms altering their function have been linked to disease susceptibility. However, bulk IFITM1 and IFITM3 mRNA expression dynamics and their correlation with clinical outcomes have not been extensively addressed in patients with respiratory infections. In this study, we evaluated the expression of IFITM1 and IFITM3 in peripheral leukocytes from healthy controls and individuals with severe pandemic influenza A(H1N1) or coronavirus disease 2019 (COVID-19). Comparisons between participants grouped according to their clinical characteristics, underlying disease, and outcomes showed that the downregulation of IFITM1 was a distinctive characteristic of severe pandemic influenza A(H1N1) that correlated with outcomes, including mortality. Conversely, increased IFITM3 expression was a common feature of severe pandemic influenza A(H1N1) and COVID-19. Using a high-dose murine model of infection, we confirmed not only the downregulation of IFITM1 but also of IFITM3 in the lungs of mice with severe influenza, as opposed to humans. Analyses in the comparative cohort also indicate the possible participation of IFITM3 in COVID-19.     Cooperation and its evolution in growing systems with cultural reproduction

    We explore the evolution of cooperation in the framework of the evolutionary game theory using the prisoner's dilemma as metaphor of the problem. We present a minimal model taking into account the growing process of the systems and individuals with imitation capacity. We consider the topological structure and the evolution of strategies decoupled instead of a coevolutionary dynamic. We show conditions to build up a cooperative system with real topological structures for any natural selection intensity. When the system starts to grow, cooperation is unstable but becomes stable as soon as the system reaches a small core of cooperators whose size increase when the intensity of natural selection decreases. Thus, we reduce the emergence of cooperative systems with cultural reproduction to justify a small initial cooperative structure that we call cooperative seed. Otherwise, given that the system grows principally as cooperator whose cooperators inhabit the most linked parts of the system, the benefit-cost ratio required for cooperation evolve is drastically reduced compared to the found in static networks.
