Patterns, Presentations and Prognosis of Nasal Polyps
Few studies have documented the characteristic
features of nasal polyps in the developing countries. In this
study, we described the patterns, presentations and prognosis
of nasal polyps seen in clinical setting, with a view to
improve our understanding of its clinical and epidemiological
characteristics. The study was a 10-year retrospective
analysis of histologically-confirmed nasal polyps
seen between January 2006 and December 2015. Records
of patients with intranasal masses were retrieved from our
hospital’s records department, clinics, wards and theatre
suites. Those with nasal polyps were recruited into the
study. The results were descriptively analyzed using SPSS
statistical soft ware package version 10. There were 84
patients with intranasal masses seen within the reviewed
period. Of this, 52 (61.9%) were histologically-confirmed
nasal polyps. There were 22 males and 30 females. Their
age ranges from 16 to 69 years. The most frequent symptom
is nasal obstruction occurring in 76.9% of the cases.
None of the patients had epistaxis. Thirty-one (59.6%)
were associated with various complications either singly or
multiple (Table 1). All (100%) were treated with conventional
forceps excision. Eleven (21.2%) of them had
recurrence between 3 and 5 years after surgery. None of
the polyps or their recurrence exhibited malignant transformation.
Nasal polyp is the most common intranasal
mass seen in clinical practice. Its rarity in children and
propensity for recurrence are reaffirmed. Although, recurrence
is a major prognostic challenge, nasal polyp does not
exhibit malignant transformation
Gender Differences in Students-Staff Violence in Urban and Rural Secondary Schools of Osun State, South Western Nigeria
Background: School violence is of public health importance. One important but often overlooked dimension is student-staff violence. The aim of the study was to assess the gender differences in the pattern of students-staff violence in urban and rural areas of Osun state with the hypothesis that male students and staff perpetrate violence more than female students and staff respectively. Methodology: A cross sectional study conducted among 800 secondary school students from JSS 2 to SSS 3 (400 in urban and 400 in rural areas) selected by multi-stage sampling technique. Results: The mean age for all the respondents was 14.3 years ±2.0, with the mean age for males as 14.2±2.0 and females as 14.3±2.0. Male respondents were 51.5% in urban schools and 51% in rural schools. Males were the main perpetrators in both urban and rural areas but more females in the rural areas perpetrated violence than their urban counterparts. Out of those who verbally abused school staff in urban areas, males constituted 61.5% compared to 38.5% of females. Respondents also experienced violence in the hands of academic staff (male and female). Conclusion: All forms of violence were perpetrated against school staff with prevalence higher in rural than urban areas. Males perpetrated most forms of violence than females in both urban and rural schools, though this was marked in urban schools. Female respondents in rural areas experienced significantly higher perpetration of most forms of school-related violence than urban females.Journal of Community Medicine & Primary Health vol 23 (1-2) 2011
Pattern of presentation of ear , nose, throat, head and neck injury in a developing country
Objective: Ear, nose, throat, head and neck injuries are a common otorhinolaryngology disorder
worldwide. This study aimed at determining the prevalence, sociodemographic features, aetiology ,
clinical presentation management and outcome of injuries to the ear, nose, throat, head and neck region.
Methods: This was a prospective study of patients with otorhinolaryngolology , head and neck injuries
that presented at our tertiary health institution. Consented patients were studied between October 2015
and September 2017. Analysis of obtained data was done with SPSS version 16.0.
Results: The prevalence of ear, nose, throat, head and neck injury was 9.4%. There were 63.5% males
36.5% females with male to female ratio of 1.5:1. Foreign bodies' impaction was the commonest cause of
injury in 32.3% followed by road traffic accidents in 19.8%. Commonest anatomical region were ear and
nose in 49.7% and 28.5% respectively . Common clinical features among the patients were pain in 46.5%,
bleeding in 37.8% and foreign bodies' impaction in 32.3%. Presentations for otorhinolaryngology care
among the patients were common in 95.1% acute injury than 4.9% chronic injury (≥13 weeks).
Commonest associated complications of the injuries were otitis media in 18.8% others were 14.9% otitis
externa, 9.4% perforated tympanic membrane and 6.3% epistaxis. Pre-hospital treatment in the patients
was 67.4%. Major treatment offered to the patients was conservative/medical therapy in 28.8%.
Conclusion: Ear , nose, throat, head and neck injuries are common in the otorhinolaryngology practice.
Commonest causes are self inflicting foreign bodies' impaction and road traffic accident. Pre-hospital
treatment among the patients was very high
Sedimentation, depositional environments, and hydrocarbon potential of the Maastrichtian-Paleocene Araromi Formation, eastern Dahomey (Benin) Basin, southwestern Nigeria
The Cretaceous sediments in southwestern Nigeria are host to one of the largest bitumen deposits in the world. In the current paper, an integrated study on sedimentology, palynology, and applied petroleum geochemistry of the Maastrichtian-Paleocene Araromi Formation was used to determine the depositional environments and hydrocarbon potentials of the formation on the eastern Dahomey Basin. Four sedimentary lithofacies were identified from core samples, namely, lower limestone (F); medium to coarse-grained sandstone (F); lower loosely consolidated sandstone (F); and shale and siltstone (F). Sedimentation in the eastern Dahomey Basin occurred mainly in fluvial and shallow-marine (shelf) environments. The palynological assemblages of the Araromi Formation reflect deposition in coastal through brackish water to shallow shelf environment with periods of localized wind-induced storms. The shale and siltstone samples of the Araromi Formation are characterized by total organic carbon (TOC) values of up to 2.50 wt % and S (hydrocarbon-generating potential) values ranging from 0.26 to 0.70 mgHC/g rock, indicating poor source rocks. Shales show poor quality and thermally immature organic matter at shallow depth and could neither have generated liquid hydrocarbon nor contributed to the heavy oil occurrence on the bitumen and tar-sand belt of eastern Dahomey (Benin) Basin
Butylated caffeic acid: An efficient novel antioxidant
A novel antioxidant, butylated caffeic acid (BCA) was rationally designed by adding a tert-butyl group to caffeic acid, which was synthesized at a high yield (36.2%) from 2-methoxy-4-methylphenol by a four-step reaction including Friedel-Crafts alkylation, bromine oxidation, ether bond hydrolysis and Knoevenagel condensation. Its antioxidant capacity was much stronger than common commercial antioxidant tert-butyl hydroquinone (TBHQ) and its mother compound, caffeic acid, in both rancimat and deep frying tests. When investigated via the DPPH method, the antioxidant capacity of BCA was almost equal to TBHQ, but lower than caffeic acid. BCA could be a potentially strong antioxidant, especially for food processing at high temperatures such as deep frying and baking
A reliability and validity study of the Palliative Performance Scale
<p>Abstract</p> <p>Background</p> <p>The Palliative Performance Scale (PPS) was first introduced in1996 as a new tool for measurement of performance status in palliative care. PPS has been used in many countries and has been translated into other languages.</p> <p>Methods</p> <p>This study evaluated the reliability and validity of PPS. A web-based, case scenarios study with a test-retest format was used to determine reliability. Fifty-three participants were recruited and randomly divided into two groups, each evaluating 11 cases at two time points. The validity study was based on the content validation of 15 palliative care experts conducted over telephone interviews, with discussion on five themes: PPS as clinical assessment tool, the usefulness of PPS, PPS scores affecting decision making, the problems in using PPS, and the adequacy of PPS instruction.</p> <p>Results</p> <p>The intraclass correlation coefficients for absolute agreement were 0.959 and 0.964 for Group 1, at Time-1 and Time-2; 0.951 and 0.931 for Group 2, at Time-1 and Time-2 respectively. Results showed that the participants were consistent in their scoring over the two times, with a mean Cohen's kappa of 0.67 for Group 1 and 0.71 for Group 2. In the validity study, all experts agreed that PPS is a valuable clinical assessment tool in palliative care. Many of them have already incorporated PPS as part of their practice standard.</p> <p>Conclusion</p> <p>The results of the reliability study demonstrated that PPS is a reliable tool. The validity study found that most experts did not feel a need to further modify PPS and, only two experts requested that some performance status measures be defined more clearly. Areas of PPS use include prognostication, disease monitoring, care planning, hospital resource allocation, clinical teaching and research. PPS is also a good communication tool between palliative care workers.</p
Pomegranate inhibits neuroinflammation and amyloidogenesis in IL-1β stimulated SK-N-SH cells
Purpose: Pomegranate fruit, Punica granatum L. (Punicaceae) and its constituents have been shown to inhibit inflammation. In this study we aimed to assess the effects of freeze-dried pomegranate (PWE) on PGE2 production in IL-1β stimulated SK-N-SH cells.
Methods: An enzyme immuno assay (EIA) was used to measure prostaglandin E2 (PGE2) production from supernatants of IL-1β stimulated SK-N-SH cells. Expression of COX-2, phospho-IκB and phospho-IKK proteins were evaluated, while NF-κB reporter gene assay was carried out in TNFα-stimulated HEK293 cells to determine the effect of PWE on NF-κB transactivation. Levels of BACE-1 and Aβ in SK-N-SH
cells stimulated with IL-1β were measured with an in cell ELISA.
Results: PWE (25-200 µg/ml) dose dependently reduced COX-2 dependent PGE2 production in SK-N-SH cells stimulated with IL-1β. Phosphorylation of IκB and IKK
were significantly (p<0.001) inhibited by PWE (50- 200 µg/ml). Our studies also show that PWE (50-200 µg/ml) significantly (p<0.01) inhibited NF-κB transactivation in TNFα-stimulated HEK293 cells. Furthermore PWE inhibited BACE-1 and Aβ expression in SK-N-SH cells treated with IL-1β.
Conclusions: Taken together, our study demonstrates that pomegranate inhibits inflammation, as well as amyloidogenesis in IL-1β-stimulated SK-N-SH cells. We
propose that pomegranate is a potential nutritional strategy in slowing the progression of neurodegenerative disorders like Alzheimer’s disease
Charting achievements: a two-year retrospective of the society for environmental geochemistry and health (SEGH) and the evolving strategies
Emerging from the shadow of the COVID-19 pandemic, it is time to ground ourselves and retrospectively assess the recent achievements of SEGH over the past years. This editorial serves as a comprehensive report on the progress made in comparison to the aspirations and goals set by the society's board in 2019 (Watts et al., Environ Geochem Health 42:343–347, 2019) (Fig. 1) and reflects on the state of the SEGH community as it reached its 50th anniversary at the close of 2021 (Watts et al. Environ Geochem Health 45:1165–1171, 2023). The focus lies on how the SEGH community navigated through the extraordinary challenges posed by the COVID-19 pandemic since early 2020, and to what extent the 2023 targets have been met
Antimalarial drug artemether inhibits neuroinflammation in BV2 microglia through Nrf2-dependent mechanisms
Artemether, a lipid-soluble derivative of artemisinin has been reported to possess anti-inflammatory properties. In this study, we have investigated the molecular mechanisms involved in the inhibition of neuroinflammation by the drug. The effects of artemether on neuroinflammation-mediated HT22 neuronal toxicity were also investigated in a BV2 microglia/HT22 neuron co-culture. To investigate effects on neuroinflammation, we used LPS-stimulated BV2 microglia treated with artemether (5-40µM) for 24 hours. ELISAs and western blotting were used to detect pro inflammatory cytokines, nitric oxide, PGE2, iNOS, COX-2 and mPGES-1. BACE-1 activity and Aβ levels were measured with ELISA kits. Protein levels of targets in NF-kappaB and p38 MAPK signalling, as well as HO-1, NQO1 and Nrf2 were also measured with western blot. NF-kappaB binding to the DNA was investigated using EMSA. MTT, DNA fragmentation and ROS assays in BV2-HT22 neuronal co-culture were used to evaluate the effects of artemether on neuroinflammation-induced neuronal death. The role of Nrf2 in the anti-inflammatory activity of artemether was investigated in BV2 cells transfected with Nrf2 siRNA. Artemether significantly suppressed pro-inflammatory mediators (NO/iNOS, PGE2/COX-2/mPGES-1, TNFα, and IL-6), Aβ and BACE-1 in BV2 cells following LPS stimulation. These effects of artemether were shown to be mediated through inhibition of NF-kappaB and p38MAPK signalling. Artemether produced increased levels of HO-1, NQO1 and GSH in BV2 microglia. The drug activated Nrf2 activity by increasing nuclear translocation of Nrf2 and its binding to antioxidant response elements in BV2 cells. Transfection of BV2 microglia with Nrf2 siRNA resulted in the loss of both anti-inflammatory and neuroprotective activities of artemether. We conclude that artemether induces Nrf2 expression and suggest that Nrf2 mediates the anti-inflammatory effect of artemether in BV2 microglia. Our results suggest that this drug has a therapeutic potential in neurodegenerative disorders
