    Pattern of tuberculosis in patients of a university hospital during the period (2004–2011)

    AbstractAim of the workTo identify the pattern of infection among diagnosed patients with tuberculosis presented to chest diseases clinic and/or admitted to the department of chest diseases in Bab El Shaaria (Al-Azhar) University hospital.Subjects and methodsBy retrospective analysis of available data, all patients who were diagnosed and registered in the period from January 2004 till December 2011 in the follow up records in TB clinic and in the chest diseases department in Bab El Shaaria University hospital were included. Data were collected from the patient files. These data included: full history, chest X-ray, sputum examination results, and all other data related to the patient’s condition.ResultsA total of 384 patients were registered in TB clinic in Bab El Shaaria University hospital with a diagnosis of tuberculosis (pulmonary or extra-pulmonary). Their age ranged from 3 to 80years, with mean age of 37.5+16.0years. As regards gender there were 178 male patients (46.4%), and 206 female patients (53.6%). As regards site affected there were 209 patients (54.4%) with pulmonary TB and 175 patients (45.6%) with extrapulmonary TB. The percentage of TB cases to the total number of chest clinic visitors per year was ranging between 0.08% and 0.8% with an average of 0.48%, while the percentage of pulmonary TB cases to the total number of TB cases per year was ranging between 27% and 65% of total TB cases per year with an average of 51.7%

    Methylphenidate improves executive functions in patients with traumatic brain injuries : a feasibility trial via the idiographic approach

    Background: Road traffic accidents are known to be the main cause of traumatic brain injury (TBI). TBI is also a leading cause of death and disability. This study, by means of the idiographic approach (single-case experimental designs using multiple-baseline designs), has examined whether methylphenidate (MPH - trade name Ritalin) had a differential effect on cognitive measures among patients with TBI with the sequel of acute and chronic post-concussion syndromes. The effect on gender was also explored. Methods: In comparison with healthy controls, patients with TBI (acute and chronic) and accompanying mild cognitive impairment (MCI) were screened for their integrity of executive functioning. Twenty-four patients exhibiting executive dysfunction (ED) were then instituted with the pharmacological intervention methylphenidate (MPH). The methylphenidate was administered using an uncontrolled, open label design. Results: The administration of methylphenidate impacted ED in the TBI group but had no effect on mood. Attenuation of ED was more apparent in the chronic phases of TBI. The effect on gender was not statistically significant with regard to the observed changes. Conclusions: To our knowledge, this is the first feasibility trial from the Arabian Gulf to report the performance of a TBI population with mild cognitive impairment according to the IQCODE Arabic version. This investigation confirms anecdotal observations of methylphenidate having the potential to attenuate cognitive impairment; particularly those functions that are critically involved in the integrity of executive functioning. The present feasibility trial should be followed by nomothetic studies such as those that adhere to the protocol of the randomized controlled trial. This evidence-based research is the foundation for intervention and future resource allocation by policy- or public health decision-makers

    Unlocking Tourism’s Potential: Pricing Strategies for the Post-COVID Renaissance

    In response to the challenges faced by tourism managers in devising effective pricing strategies to overcome the impacts of the COVID-19 pandemic, this study aims to make a valuable contribution to the existing literature on tourism management and marketing. Specifically, we investigate the pricing strategies that tourists are most likely to prefer in the aftermath of the pandemic. To achieve this, we conducted semi-structured interviews with a sample size of experienced tourists in Egypt. Our findings reveal that tourists exhibit a strong inclination towards hedonic and bundling pricing strategies. This suggests that tourism managers should prioritize the incorporation of these strategies into their post-pandemic business plans. However, it is crucial for managers to carefully consider tourists’ behavioral responses to dynamic and pay-what-you-want pricing strategies, as these are influenced by various constraints that necessitate careful attention. Furthermore, our study highlights potential negative consequences associated with the implementation of the dual-pricing strategy following the pandemic. This strategy has the potential to compromise tourists’ intentions to utilize certain services, thereby leading to unfavorable behavioral outcomes for tourism destinations. The implications of this study are of practical significance for tourism managers seeking to design more effective pricing strategies to capitalize on the opportunities presented by the post-pandemic recovery period. By understanding tourists’ preferences and considering the potential drawbacks associated with certain pricing strategies, managers can make informed decisions that promote the sustainable and successful revival of the tourism industry.     An Effective Satellite Remote Sensing Tool Combining Hardware and Software Solutions

    In this paper we propose a new effective remote sensing tool combining hardware and software solutions as an extension of our previous work. In greater detail the tool consists of a low cost receiver subsystem for public weather satellites and a signal and image processing module for several tasks such as signal and image enhancement, image reconstruction and cloud detection. Our solution allows to manage data from satellites effectively with low cost components and portable software solutions. We aim at sampling and processing of the modulated signal entirely in software enabled by Software Defined Radios (SDR) and CPU computational speed overcoming hardware limitation such as high receiver noise and low ADC resolution. Since we want to extend our previous method to demodulate signals coming from various meteorological satellites, we propose a new high frequency receiving system designed to receive and demodulate signals transmitted at 1.7 GHz. The signals coming from satellites are demodulated, synchronized and enhanced by using low level image processing techniques, then cloud detection is performed by using the well known K-means clustering algorithm. The hardware and software architecture extensions make our solution able to receive and demodulate high frequency and bandwidth meteorological satellite signals, such as those transmitted by NOAA POES, NOAA GOES, EUMETSAT Metop, Meteor-M and FengYun

    An investigation into the roles of chlorides and sulphate salts on the performance of low salinity injection in sandstone reservoirs : experimental approach

    Numerous studies have been carried out to ascertain the mechanisms of low salinity and smart water flooding technique for improved oil recovery. Focus were often on brine composition and, specifically the cationic content in sandstone reservoirs. Given the importance of the salt composition and concentration, tweaking the active ions which are responsible for the fluids-rock equilibrium will bring into effect numerous mechanisms of displacement which have been extensively debated. This experimental study, however, was carried out to evaluate the extent of the roles of chloride and sulphate-based brines in improved oil recovery. To carry this out, 70,000 ppm sulphates and chloride-based brines were prepared to simulate formation water and 5,000ppm brines of the same species as low salinity displacement fluids. Core flooding process was used to simulate the displacement of oil by using four (4) native sandstones core samples, obtained from Burgan oil field in Kuwait, at operating conditions of 1500 psig and 50oC. The core samples were injected with 70,000 ppm chloride and sulphates and subsequently flooded with the 5,000 ppm counterparts in a forced imbibition process. Separate evaluations of chloride and sulphate-based brines were carried out to investigate the displacement efficiencies of each brine species. The results showed that the in both high and low salinity displacement tests, the SO4 brine presented better recovery of up to 89% of the initial oil saturation (Soi). Several mechanisms of displacement were observed to be responsible for improved recovery during SO4 brine displacement. IFT measurement experiments also confirmed that there was reduction in IFT at test conditions between SO4 brine and oil and visual inspection of the effluent showed a degree emulsification of oil and brines. Changes in pH were observed in the low salinity flooding and negligible changes were noticed in the high salinity floods. These results provide an insight into the roles of chloride and sulphate ions in the design of smart “designer” water and low salinity injection scenarios

    Claudin 7 expression and localization in the normal murine mammary gland and murine mammary tumors

    INTRODUCTION: Claudins, membrane-associated tetraspanin proteins, are normally associated with the tight junctions of epithelial cells where they confer a variety of permeability properties to the transepithelial barrier. One member of this family, claudin 7, has been shown to be expressed in the human mammary epithelium and some breast tumors. To set the stage for functional experiments on this molecule, we examined the developmental expression and localization of claudin 7 in the murine mammary epithelium and in a selection of murine mammary tumors. METHOD: We used real-time polymerase chain reaction, in situ mRNA localization, and immunohistochemistry (IHC) to examine the expression and localization of claudin 7. Frozen sections were examined by digital confocal microscopy for colocalization with the tight-junction protein ZO1. RESULTS: Claudin 7 was expressed constitutively in the mammary epithelium at all developmental stages, and the ratio of its mRNA to that of keratin 19 was nearly constant through development. By IHC, claudin 7 was located in the basolateral part of the cell where it seemed to be localized to discrete vesicles. Scant colocalization with the tight-junction scaffolding protein ZO1 was observed. Similar results were obtained from IHC of the airway epithelium and some renal tubules; however, claudin 7 did partly colocalize with ZO1 in EPH4 cells, a normal murine mammary cell line, and in the epididymis. The molecule was localized in the cytoplasm of MMTV-neu and the transplantable murine tumor cell lines TM4, TM10, and TM40A, in which its ratio to cytokeratin was higher than in the normal mammary epithelium. CONCLUSION: Claudin 7 is expressed constitutively in the mammary epithelium at approximately equal levels throughout development as well as in the murine tumors examined. Although it is capable of localizing to tight junctions, in the epithelia of mammary gland, airway, and kidney it is mostly or entirely confined to punctate cytoplasmic structures, often near the basolateral surfaces of the cells and possibly associated with basolateral membranes. These observations suggest that claudin 7 might be involved in vesicle trafficking to the basolateral membrane, possibly stabilizing cytoplasmic vesicles or participating in cell–matrix interactions

Modifying nanomaterials produced by green-methods is safe, inexpensive, and easy. Therefore, the control and modification of nanoparticles and their properties were also discussed

    The Effect of a DNA Repair Gene on Cellular Invasiveness: Xrcc3 Over-Expression in Breast Cancer Cells

    Over-expression of DNA repair genes has been associated with resistance to radiation and DNA-damage induced by chemotherapeutic agents such as cisplatin. More recently, based on the analysis of genome expression profiling, it was proposed that over-expression of DNA repair genes enhances the invasive behaviour of tumour cells. In this study we present experimental evidence utilizing functional assays to test this hypothesis. We assessed the effect of the DNA repair proteins known as X-ray complementing protein 3 (XRCC3) and RAD51, to the invasive behavior of the MCF-7 luminal epithelial-like and BT20 basal-like triple negative human breast cancer cell lines. We report that stable or transient over-expression of XRCC3 but not RAD51 increased invasiveness in both cell lines in vitro. Moreover, XRCC3 over-expressing MCF-7 cells also showed a higher tumorigenesis in vivo and this phenotype was associated with increased activity of the metalloproteinase MMP-9 and the expression of known modulators of cell-cell adhesion and metastasis such as CD44, ID-1, DDR1 and TFF1. Our results suggest that in addition to its' role in facilitating repair of DNA damage, XRCC3 affects invasiveness of breast cancer cell lines and the expression of genes associated with cell adhesion and invasion

    Claudin-7 Is Frequently Overexpressed in Ovarian Cancer and Promotes Invasion

    Background: Claudins are tight junction proteins that are involved in tight junction formation and function. Previous studies have shown that claudin-7 is frequently upregulated in epithelial ovarian cancer (EOC) along with claudin-3 and claudin-4. Here, we investigate in detail the expression patterns of claudin-7, as well as its possible functions in EOC. Methodology/Principal Findings: A total of 95 ovarian tissue samples (7 normal ovarian tissues, 65 serous carcinomas, 11 clear cell carcinomas, 8 endometrioid carcinomas and 4 mucinous carcinomas) were studied for claudin-7 expression. In real-time RT-PCR analysis, the gene for claudin-7, CLDN7, was found to be upregulated in all the tumor tissue samples studied. Similarly, immunohistochemical analysis and western blotting showed that claudin-7 protein was significantly overexpressed in the vast majority of EOCs. Small interfering RNA-mediated knockdown of claudin-7 in ovarian cancer cells led to significant changes in gene expression as measured by microarrays and validated by RT-PCR and immunoblotting. Analyses of the genes differentially expressed revealed that the genes altered in response to claudin-7 knockdown were associated with pathways implicated in various molecular and cellular functions such as cell cycle, cellular growth and proliferation, cell death, development, and cell movement. Through functional experiments in vitro, we found that both migration and invasion were altered in cells where CLDN7 had been knocked down or overexpressed. Interestingly, claudin-7 expression was associated with a net increase in invasion, but also with a decrease in migration
