A vesetranszplantáció sikerességének pszichológiai tényezői. A szorongásszint és a szerv intrapszichés integrációjának hatása a gyógyulási folyamatra
Järnvägsstationer är att betrakta som offentliga platser och regleras av transportpolitiska målsättningar om jämställdhet och tillgänglighet för alla (Prop. 2008/09:93). Trots det saknas forskning om hur genus påverkar resenärer vid deras vardagliga vistelser på järnvägsstationer. Det övergripande syftet med denna avhandling är därför att ur ett genusperspektiv undersöka hur manliga och kvinnliga resenärer i sin vardag använder och upplever järnvägsstationer som fysiska platser och sociala rum. Det empiriska materialet baseras på resedagböcker, intervjuer med resenärer, deltagande observationer och intervjuer med planerare och förvaltare av järnvägsstationer. Kimstad pendeltågsstation, Norrköpings järnvägsstation och Stockholms Centralstation ingår i studien. I avhandlingen kombineras olika teorier som gör det möjligt att betrakta genus som rytm. Genom detta teoretiska ramverk undersöks hur genusmaktordningen återverkar i tid, rum och mobilitet. Resultaten av studien visar att resenärer är påverkade av genusmaktordningen, som återfinns såväl i kollektiva föreställningar som i materialiserade objekt som möter resenärer när de vistas på järnvägsstationerna. Sammanfattningsvis visar studien att såväl manliga som kvinnliga resenärer påverkas av denna maktordning. Även om denna ordning påverkar alla tycks kvinnor vara de som påverkas mest negativt, eftersom de genom en manlig normerad blick betraktas som antingen ärbara eller sexuellt tillgängliga objekt, därutöver att de är tvungna att förhålla sig till risken att utsättas för sexualiserat våld från män. Av dessa anledningar tenderar kvinnors livsrum att inskränkas, oavsett ålder och plats.Railway stations are perceived as public spaces and are regulated by national transport goals of equality and accessibility for all (Prop. 2008/09:93). However, hardly any research has looked at how gender affects commuters in their daily life at railway stations. The overriding aim of this study is to examine how male and female commuters use and experience railway stations as gendered physical places and social spaces, during their daily travels. The empirical material is based on travel diaries, interviews with male and female commuters, participant observations inspired by auto-ethnography and interviews with planners and managers of the railway stations. Stations included in the study are Kimstad commuter rail station, Norrköping railway station and Stockholm Central station (all in Sweden). In this thesis this combination of theories makes it possible to define gender as rhythm. Through this theoretical frame the thesis analyses gendered power relations of bodies in time, space and mobility. Results from the study show that individuals are affected by gendered power relations that dwell in collective believes and in shape of materialized objects that encounter the commuter when visiting the railway station. In conclusion the study shows that both female and male commuters are affected by gender power relations while visiting railway stations. Even though they affect all, women seem to be most negatively affected by these power relations, since women by gender as rhythms are considered as either objects of decency or as sexually available to heterosexual men. Further, female bodies negotiate the risk of encountering sexual violence therefore gender as rythm tend to decrease the extent of women’s everyday living spaces, regardless of age and place
Vesetranszplantáció utáni szénhidrátanyagcsere-változások és annak hatásai a cardiovascularis rizikóra | Changes in carbohydrate metabolism after kidney transplantation and their effects on cardiovascular risk
Absztrakt:
Bevezetés: A transzplantáció utáni halálozások legfőbb okai a
cardiovascularis betegségek, amelyeknek egyik legfőbb rizikófaktora a diabetes
mellitus. Célkitűzés: Vizsgálatunk célja volt, hogy felmérjük
az újonnan kialakult diabetes mellitus előfordulási gyakoriságát és hatását az
allograft funkciójára, valamint a HEART-pont alapján a cardiovascularis rizikó
kockázatát. Módszer: Összesen 44 beteget vontunk be a
vizsgálatba, akiknél értékeltük az alapadatokat, majd OGTT-vizsgálatot követően
a betegeket három csoportba soroltuk, normál, emelkedett éhomi
vércukorszint/csökkent glükóztolerancia és (az) újonnan kialakult diabetes
mellitus. Az inzulinrezisztenciát HOMA-IR alapján értékeltük. Vizsgáltuk a
vesefunkciót, valamint a HEART-pont alapján a cardiovascularis rizikót.
Eredmények: Az alapadatok tekintetében a hideg ischaemiás
idő (p = 0,016), a testsúly (p = 0,035), a BMI (p = 0,025) és a HbA1c
(p = 0,0024) szignifikánsan különböztek a normál és a diabeteses betegek között.
A vesefunkció tekintetében a szérumkreatinin- (p = 0,0013), eGFR- (p = 0,0026),
valamint karbamid- (p = 0,0157) értékek szignifikánsan különböztek a normál és a
diabeteses betegek között. A HOMA-IR tekintetében a normál és a diabeteses
betegek között szignifikáns különbség volt (1,69 ± 0,51 vs. 6,46 ± 1,42; p =
0,0017). Következtetés: A cardiovascularis kockázat
csökkenthető, az allograft túlélése növelhető a diabetes mellitus időben történő

Abstract:
Introduction: Cardiovascular disease is the major cause of
deaths after transplantation, with diabetes mellitus being the main risk factor
in development. Aim: The aim of our study was to assess the
prevalence of new onset diabetes mellitus in connection with the cardiovascular
risk predicted by the HEART Score. Method: 44 patients were
involved in our study; after overview of baseline data, OGTT was performed,
followed by patient classification into the following groups: normal, impaired
fasting glucose/impaired glucose tolerance, and new onset diabetes mellitus.
Insulin resistance and kidney function were also assessed.
Results: Concerning baseline data, cold ischemic time (p =
0.016), body weight (p = 0.035), BMI (p = 0.025), and HbA1C (p =
0.0024) proved to be significantly different between normal and diabetic
patients. Significant difference was found based on HOMA IR between the two
groups 1.69±0.51 vs 6.46±1.42; p = 0.0017). Based on the HEART Score, patients
with new onset diabetes mellitus were put into Group 3, which also reflects the
risk which diabetes carries for the development of cardiovascular diseases.
Conclusion: Cardiovascular risk can be decreased with
increased allograft survival by early diagnosis and management of diabetes. Orv

Banff-score-változások a marginális donorokból származó veséknél
Absztrakt
Bevezetés: A transzplantációs várólistán szereplő betegek
számának folyamatos növekedése miatt az ideális donor kritériumainak
kibővítésére van szükség. Célkitűzés: A szerzők azt a kérdést
vizsgálták, hogy vesetranszplantáció után egy és öt évvel van-e szignifikáns
különbség a vesefunkcióban és/vagy -morfológiában a marginális és ideális
donorból származó vesével rendelkező betegek között. Módszer: A
vizsgálatba 275 beteget vontak be, közülük 97 marginális és 178 „ideális”
veserecipiens volt. A marginális és az „ideális” veserecipiensek körében
vizsgálták a donoralapadatokat és elemezték a transzplantáció után egy és öt
évvel a funkcionális és hisztopatológiai változásokat.
Eredmények: A graft funkcióját vizsgálva a transzplantáció
után egy évvel nem volt különbség a két betegcsoport között, míg az ötödik évben
a szérumkreatinin szignifikánsan magasabb (p = 0,0001) és a glomeruláris
filtrációs ráta szignifikánsan alacsonyabb volt (p = 0,003) a marginális
veserecipiensek csoportjában az ideális veserecipiensek csoportjához képest. A
vese morfológiai elváltozásait vizsgálva már egy évvel a transzplantáció után a
tubulitis (p = 0,014), valamint az interstitialis gyulladás (p = 0,025)
szignifikánsan gyakoribb volt a marginális veserecipiensek csoportjában.
Következtetés: A vesetranszplantáció után egy évvel a vese
funkciójában még nincs szignifikáns különbség a marginális és „ideális”
veserecipiensek között, de a szövettani elváltozásokban már szignifikáns

A veseátültetés első 50 éve Magyarországon
The first Hungarian kidney transplantation was performed by Andras Nemeth in Szeged in 1962, approximately 50 years ago. A preliminary agreement with Eurotransplant was signed in 2011, and special patient groups gained benefit from this cooperation in 2012, wnich lead to a full membership to Eurotransplant. This event inspired the authors to review the history of Hungarian kidney transplantation of the past 50 years, from the first operation to recent via the specific cornerstones of the transplant program. The donor of the first Hungarian kidney transplantation was the brother of the recipient. The operation itself was technically successful, but the lack of immunosuppression caused graft rejection, and the patient died after 79 days. His brother, the donor, is still healthy, after 50 years, and he encourages everybody to donate organs. Organized kidney transplant program started more than 10 years later, such as 1973, in Budapest. The program was supported by the Ministry of Health. New centers joined the program later, Szeged in 1979, Debrecen in 1991 and Pecs in 1993. These four transplant centers work currently in Hungary, and 6611 kidney transplantation has been performed up to the end of year 2012. Orv. Hetil., 2013, 154, 846-849
Immunszupprimált betegek multiplex bőrdaganatainak elektrokemoterápiás kezelése [Electrochemotherapy for multiple cutaneous tumors in immunosuppressed patients]
Introduction: The risk of cutaneous malignancies is significantly higher in immunosuppressed patients compared to the general population. These high-risk skin tumors tend to be aggressive, multiplex, rapidly growing lesions. It is common to see local recurrence after surgical excision. Multiplex tumors are difficult to treat, especially in the head/ neck region.Objective: Amongst the standard treatment options, electrochemotherapy can be a suitable option. Our aim was to evaluate the efficacy of electrochemotherapy in immunocompromised patients.Method: In 9 immunosuppressed patients, 118 (average: 13, n = 5-27) non-melanoma skin tumors were treated with electrochemotherapy with intravenous administration of bleomycin, according to the ESOPE criteria.Results: The median follow-up was 15 months. 6 months after the treatment, the objective response rate was 96%. We observed complete response in 88%, partial response in 8% and progressive disease in 2% of the treated lesions. In 2%, the response was not evaluable.Conclusion: In immunocompromised patients, electrochemotherapy is an effective and safe therapeutic option for non-melanoma skin tumors. In order to provide more ideal management for this special sub-group, prevention, multidisciplinary approach and optimized immunosuppressive therapy is essential
