Chronic obstructive pulmonary disease (COPD) is the third leading cause of morbidity and mortality globally, contributing to a substantial use of resources. According to World Health Organization estimates, 65 million people have moderate to severe COPD. The condition is also recognised as a systemic disease with  extrapulmonary manifestations, such as peripheral muscle dysfunction, malnutrition and depression, which further contribute to disability, poor quality of life, exacerbations and mortality. Optimum management requires nonpharmacological interventions combined with pharmacological treatment. However, the former is often neglected and not widely used in daily practice, with the focus mainly on the latter
Ten Years in the Academic Integrity Trenches: Experiences and Issues
In 2016, our university launched its Academic Integrity Program (AIP) in order to promote and protect academic integrity. All commencing students must complete this online AIP within 14 days of starting their course. Satisfactory completion of this module with a test score of 80% is required before students can access their course materials. Interestingly, this university program mirrors a decade long effort in our IS discipline area to educate students regarding the importance of academic integrity and values through improvements in education, detection of misconduct, and procedures that deal with this. In this paper, we analyze four descriptive cases of academic misconduct involving plagiarism, collusion, and contract cheating. We then describe the role of academic culture and English language proficiency, noting how these contribute to academic integrity. Additionally, we analyze quantitative data from the faculty Plagiarism Recording System that reflect a decade of front-line experiences of IS academics in upholding and reinforcing academic integrity in both large (1,000 student) undergraduate and small (20 student) postgraduate courses. We describe the AIP that educated 15,000 new students commencing courses in 2016. Finally, we raise issues and make recommendations to deal with the next decade of efforts to improve academic integrity
C1 inhibitor deficiency: 2014 United Kingdom consensus document
C1 inhibitor deficiency is a rare disorder manifesting with recurrent attacks of disabling and potentially life-threatening angioedema. Here we present an updated 2014 United Kingdom consensus document for the management of C1 inhibitor-deficient patients, representing a joint venture between the United Kingdom Primary Immunodeficiency Network and Hereditary Angioedema UK. To develop the consensus, we assembled a multi-disciplinary steering group of clinicians, nurses and a patient representative. This steering group first met in 2012, developing a total of 48 recommendations across 11 themes. The statements were distributed to relevant clinicians and a representative group of patients to be scored for agreement on a Likert scale. All 48 statements achieved a high degree of consensus, indicating strong alignment of opinion. The recommendations have evolved significantly since the 2005 document, with particularly notable developments including an improved evidence base to guide dosing and indications for acute treatment, greater emphasis on home therapy for acute attacks and a strong focus on service organisation. Examining the response programming function of the Quiet Eye: Do tougher shots need a quieter eye?

The final version is available from Springer Verlag via the DOI in this record.Support for the proposition that the Quiet Eye (QE) duration reflects a period of response programming (including task parameterisation) has come from research showing that an increase in task difficulty is associated with increases in QE duration. Here, we build on previous research by manipulating three elements of task difficulty that correspond with different parameters of golf-putting performance; force production, impact quality and target line. Longer QE durations were found for more complex iterations of the task and furthermore, more sensitive analyses of the QE duration suggest that the early QE proportion (prior to movement initiation) is closely related to force production and impact quality. However, these increases in QE do not seem functional in terms of supporting improved performance. Further research is needed to explore QE's relationship with performance under conditions of increased difficulty

A new grapevine yellows phytoplasma from the Buckland Valley of Victoria, Australia
A new phytoplasma detected in grapevines with grapevine yellows disease from the Buckland Valley of Victoria, Australia was characterized. Buckland Valley grapevine yellows phytoplasma (BVGYp) could not be amplified by PCR using primers specific for the stolbur (STOL, 16SrXII) group of phytoplasmas indicating that it was unlikely to be a STOL group phytoplasma. BVGYp was amplified by PCR using primers specific for both the aster yellows (AV, 16Sr I) and STOL phytoplasma groups, indicating that it may be more closely related to the AY group phytoplasmas. Sequence analysis of 16SrRNA gene sequences showed that BVGYp clustered with AY and STOL groups of phytoplasmas. Sequence similarities were determined by pairwise comparisons of the 16S rDNA sequence of BVGYp WAY and STOL group phytoplasmas and BVGYp was more closely related to the AY group phytoplasmas. Although the data indicate BVGYp may form a newAY subgroup, the similarity coefficients between BVGYp and phytoplasmas from the AY, STOL and Mexican periwinkle virescence groups, derived from putative RFLP patterns, were less than 90%, so BVGYp may actually form a new phytoplasma group.
Metal-Insulator oscillations in a Two-dimensional Electron-Hole system
The electrical transport properties of a bipolar InAs/GaSb system have been
studied in magnetic field. The resistivity oscillates between insulating and
metallic behaviour while the quantum Hall effect shows a digital character
oscillating from 0 to 1 conducatance quantum e^2/h. The insulating behaviour is
attributed to the formation of a total energy gap in the system. A novel looped
edge state picture is proposed associated with the appearance of a voltage

The First Very Long Baseline Interferometric SETI Experiment
The first Search for Extra-Terrestrial Intelligence (SETI) conducted with
Very Long Baseline Interferometry (VLBI) is presented. By consideration of the
basic principles of interferometry, we show that VLBI is efficient at
discriminating between SETI signals and human generated radio frequency
interference (RFI). The target for this study was the star Gliese 581, thought
to have two planets within its habitable zone. On 2007 June 19, Gliese 581 was
observed for 8 hours at 1230-1544 with the Australian Long Baseline Array. The
dataset was searched for signals appearing on all interferometer baselines
above five times the noise limit. A total of 222 potential SETI signals were
detected and by using automated data analysis techniques, were ruled out as
originating from the Gliese 581 system. From our results we place an upper
limit of 7 MW/Hz on the power output of any isotropic emitter located in the
Gliese 581 system, within this frequency range. This study shows that VLBI is
ideal for targeted SETI, including follow-up observations. The techniques
presented are equally applicable to next-generation interferometers, such as
