    New prognostic markers of hearing impairment in children: gene-gene interaction and approximation models

    Objective: to evaluate the gene-gene interaction, assess the risks and develop some approximation models of hearing loss / deafness occurrence in children, depending on the genes polymorphism gab junction B2 (GJB2, rs80338939), and interleukin-4 (IL-4, rs 2243250) and other risk factors. Materials and methods: Study included 102 children with hearing impairment: 68 with sensorineural (SNHL) and 34 with conductive hearing loss (CHL), among them 36 (35.29%) girls and 66 (64.71%) boys. The patients' age vary from 8 to 18 yo (on the average 13.90±3.11 yo). Diagnosis set by otorhinolaryngology (ENT) methods: ENT examination, computer audiometry, impedancemetry, tympanometry. The control group included 60 practically healthy children: 22 girls (36.67%), 38 boys (63.33%). Polymorphism of GJB2 (rs80338939) and IL-4 (rs 2243250) genes was studied by polymerase chain reaction method. Risk assessed by Relative Risk, Odds Ratio and 95% Confidential intervals.Results. The combination of 35delG / TT, as well as Non-Del / TT and 35delG/ TC genotypes in the genome is associated with a high risk of hearing loss in general children population (from 0.932 to 1.432; OR=19.5; p=0.003), as well as the appearance SNHL (from 0.765 to 1.765), stronger than the combination of unfavorable homozygotes TT / 35DelG - 1.765. The combination of homozygotes for the wild allele of both genes (especially CC / Non-Del) is associated with a low risk of deafness: hearing loss in general -1,068, for SNHL -0,908, for CHL -0,750 (p<0,01), for CC / 35delG combination, or TC / Non-Del: in general -0.068 -, and for SNHL -0.235 and -0.11 respectively, (p>0.05).Infectious diseases in anamnesis (meningitis, measles, mumps, or rubella) increases the likelihood of CHL by 9.41 times (OR=12.0; p=0.007). Concomitant chronic non-obstructive and obstructive upper and lower respiratory tract diseases increase the risk of both SNHL and CHL in children regardless of age: for SNHL 3.75-7.81 times (OR=6.50-10.9; p≤0.028-0.01), for CHL – 4.29-8.75 times (OR=6.19-12.9; p≤0.03-0.009). The revealed dependence of the indicators is best described by the logit-regression approximating models with high multiple correlation coefficient (R2=0.9761); low standard error of the model estimation (ε=0.1114); connection criterion F=124.2; degrees of freedom df=7.43 (p<0.001).Conclusions: Genes polymorphism's GJB2 (rs80338939) and IL-4 (rs 2243250) and their interactions are new prognostic markers of hearing impairment in children. Approximating models describe the likelihood of SNHL and CHL in observed population

    Побудовано математичну модель двовимірного нестаціонарного процесу конвективно-дифузійного поширення тепла в трубці ґрунтового теплообмінника за умови превалювання конвективних його складових над дифузійними, яка дозволяє отримати розподіл температури теплоносія вздовж трубки теплообмінника та всередині самої трубки. Розвинуто числово-асимптотичний метод для розв’язання відповідної сингулярно збуреної задачі з умовою третього роду на бічній границі. Наведено результати комп’ютерних розрахунків, здійснено їх аналіз.In this paper we considered mathematical modeling of groundwater heat selection using the ground heat exchanger tubes, which has important applications for the solution of design problems of modern heating systems using renewable energy sources. We built a mathematical model of the two-dimensional unsteady convection - diffusion process of heat in a heat exchanger tube soil, under conditions of the prevalence of convective its components over diffusive ones. This let us get the temperature distribution of the coolant along the heat exchanger tubes and within the tube. Thus we assumed homogeneity of the soil structure, the lack of thermal resistance of the wall and ground heat exchanger pressure loss in the tube bends. This model allows to describe processes of heat transfer in both horizontal and vertical heat exchanger. We built asymptotic expansion of the solution of the corresponding singularly perturbed problem with the condition of the third kind on the side of the boundary (surface of the tube), in which the tube was describes the interaction of the heat exchanger with an array of the ground, which laid up. Application of the asymptotic method allowed to split a complex process into its component parts and stand-alone supplement to the convective component solution amendments on the exit from the tube and side adjustments that take into account the influence of ambient temperature. We performed a series of computer experiments on the results of which we can conclude that the effectiveness of screening low potential ground heat using heat exchanger through the soil for its further use for heating. The fact that the water content of the soil yield a greater increase in heat energy compared with dry sand and clay using horizontal heat exchangers was confirmed. Besides, it was confirmed that vertical heat exchanger is effective for all types of soil, but significant problem is the loss of heat accumulated in depth during the rise of the coolant in the heat exchanger tube through the upper cool layers of the soil

    The clinically relevant diabetic foot syndrome morphology

    Objective: to evaluate the morphological changes in diabetic foot syndrome emphasizing the ultrastructural cellular changes.Material and methods. Biopsies of the chronic lesions in 120 diabetic foot patients following necessary preparatory stages were studied by histological, histochemical and electron microscopy methods.Results. The inflammatory infiltration and destruction and involving all layers of skin and soft tissue, the phenomena of vacuolation, apoptosis or karyolysis; tissue edema, diffuse infiltration by neutrophils, vascular plethora with stasis, thrombosis, and necrosis were observed. Microcirculatory tract thrombosis exacerbated tissue ischemia, contributing to the progression of pathology. The macrophages' cytoplasm characterized by the presence of phagocytosed and digested microorganisms often intact inside them. The changes in the status of nuclear and perinuclear material were accompanied by compression and dehydration of the cell, which ended in its fragmentation and the formation of tightly contacting bodies of various shapesConclusions. Morphological changes in diabetic foot syndrome are diverse and uneven, occurring not only on tissue and cellular level but involves subcellular and molecular structures, encouraging further research in this field. Foci of destruction and lysis of the newly formed epithelium are characteristic for diabetic foot, which is not observed in the normal wound process

    The interaction between silver nitrate and niobium pentoxide has been studied under conventional thermal treatment (TT) as well as mechanochemical and sonochemical treatment (MChT and UST, respec-tively). The products of reaction have been investigated using XRD, DTA-TG, Raman spectroscopy, adsorp-tion of nitrogen. MChT and UST promote lowering the temperature of silver niobate (SN) formation.     Associations of BCL-2 (RS17759659), CTLA-4 (RS231775), APO-1/FAS (RS2234767) genes polymorphisms with activity of proliferation and apoptosis in thyroid tissue of patients with nodular forms of goiter combined with autoimmune thyroiditis and thyroid adenoma

    The study of apoptosis and proliferative activity in the thyroid gland (TG) tissue of patients with nodular goiter and autoimmune thyroiditis (NGAIT) and thyroid adenoma (TA) is based on the expression/density of Fas/FasL, BCL-2, p53, and Ki-67 markers assessment depending on the genetic polymorphisms of BCL-2 (rs17759659), CTLA-4 (rs231775) and APO-1/Fas (rs2234767) genes.Several mechanisms of thyroid cells' programmed killing are activated in NGAIT and TA with domination of Fas-induced apoptosis, which strongly associates with the BCL-2 gene's (rs17759659) promoter (F=25.33; p<0.001) and almost six fold weaker associates with the CTLA-4 gene's (rs231775) promoter (F=4.23, p=0.017). Factors that decrease the likelihood of NGAIT and TA regardless of the CTLA-4 (rs231775) and APO-1/Fas (rs2234767) genes' genotypes are the high Ki-67 density and reduction of cells containing p53 or BCL-2 proteins (OR=0.07-0.17; 95% CI OR: 0.03-0.36; p<0.001, and OR=0.08-0.11; 95% CI OR: 0.02-0.31; p<0.001, re­spectively). High expression of surface Fas and FasL in lymphoid infiltration and de­struction of thyroid cells (stronger in GG-genotype carriers of the BCL-2 gene by 18.54% (pAA=0.043) and 36.18% (pAG=0.018), respectively) indicates the initiation of the external pathway of apoptosis through the caspase mechanism (effector caspase- 8)
