    Inhaled Insulin: Intrapulmonary or Intranasal?

    Initial attempts delivered the insulin hormone intramuscularly, intravenously, and eventually subcutaneously. Other routes of administration of the drug were explored. These included oral, rectal, sublingual, buccal, transdermal, vaginal, intramuscular, intrapulmonary, and intranasal delivery systems. The purpose of these latter studies was to determine a noninjectable method to deliver insulin to patients with type 1 and 2 diabetes that would effectively lower blood sugar, control hemoglobin A1c (in much later studies), and allow patients a simpler, less invasive, and more direct control oftheir underlying disease process.  In January 2006 the United States Food and Drug Administration approved Exubera (Pfizer Pharmaceuticals, New York, NY) as the first pulmonary inhaled insulin.     Reducing User Perceived Latency in Smart Phones Exploiting IP Network Diversity

    The Fifth Generation (5G) wireless networks set its standard to provide very high data rates, Ultra-Reliable Low Latency Communications (URLLC), and significantly improved Quality of Service (QoS). 5G networks and beyond will power up billions of connected devices as it expands wireless services to edge computing and the Internet of Things (IoT). The Internet protocol suite continues its evolution from IPv4 addresses to IPv6 addresses by increasing the adoption rate and prioritizing IPv6. Hence, Internet Service Providers (ISP's) are using the address transition method called dual-stack to prioritize the IPv6 while supporting the existing IPv4. But this causes more connectivity overhead in dual-stack as compared to the single-stack network due to its preference schema towards the IPv6. The dual-stack network increases the Domain Name System (DNS) resolution and Transmission Control Protocol (TCP) connection time that results in higher page loading time, thereby significantly impacting the user experience. Hence, we propose a novel connectivity mechanism, called NexGen Connectivity Optimizer (NexGenCO), which redesigns the DNS resolution and TCP connection phases to reduce the user-perceived latency in the dual-stack network for mobile devices. Our solution utilizes the IP network diversity to improve connectivity through concurrency and intelligent caching. NexGenCO is successfully implemented in Samsung flagship devices with Android Pie and further evaluated using both simulated and live-air networks. It significantly reduces connectivity overhead and improves page loading time up to 18%

    Characterization of maize genotypes using microsatellite markers associated with QTLs for kernel iron and zinc

    224-234Crop genetic resources rich in Fe and Zn provide sustainable and cost-effective solution to alleviate micronutrient malnutrition. Maize being the leading staple crop assumes great significance as a target crop for biofortification. We report here wide genetic variation for kernel Fe and Zn among 20 diverse maize inbreds lines, majority of which were bred for quality protein maize (QPM) and provitamin-A. Kernel Fe ranged from 30.0 - 46.13 mg/kg, while kernel Zn ranged from 8.68-39.56 mg/kg. Moderate but positive correlation was observed between the micronutrients. Characterization using 25 Single sequence repeats (SSRs) linked to QTLs for kernel Fe produced 58 alleles. Similarly, 86 alleles were identified from 35 SSRs linked to QTLs for kernel Zn. One unique allele for kernel Fe and three unique alleles for kernel Zn were identified. The mean polymorphic information content (PIC) was 0.40 for both kernel Fe and  Zn. Jaccard’s dissimilarity coefficients varied from 0.25 - 0.91 with a mean of 0.58 for kernel-Fe while 0.27- 0.88 with a mean of 0.57 for kernel Zn. Principal coordinate analysis depicted diversity of inbreds. Cluster analysis grouped the inbreds into three major clusters for both kernel Fe and Zn. Potential cross combinations have been proposed to develop micronutrient rich hybrids and novel inbreds with higher Fe and Zn. The information generated here would help the maize biofortification programme to develop nutritionally enriched hybrids

Gut microbiota is a key regulator of gut-brain axis and may shape our brain physiology; thus, any change in its composition may change our behaviour due to altered psychiatric conditions.     Acute Thumb Reconstruction with Iliac Crest Bone Graft and Groin Flap: A Case Report

    Crush injury with bone loss results in shortening the length of the thumb. Most of the immediate intervention is amputation and stump closure. Revision amputation with stump closure gives loss of functional length and the patient is unable to do daily activities as before. In most of all hand functions, the opposition of the thumb plays an important role. Reconstruction of the thumb with iliac crest bone graft with its functional length is one of the major achievements for the patient. As in our case, the patient is right-hand dominant he is more concerned about the functional length of the thumb, whatever the aesthetic appearance. Here we present a case of a 24-year-old male with thumb reconstruction in a severely injured thumb with loss of bone and soft tissue just distal to the base of the proximal phalanx. Nine months postoperatively, the patient showed a great outcome with restored thumb length, function, grip strength, and a good range of motion

    Comparative study on the outcome of primary skin closure versus delayed primary skin closure in case of emergency laparotomies

    Introduction: One of the most common complications that one can encounter is the surgical site infection (SSI) after any surgical procedures especially after exploratory laparotomy for perforative peritonitis. A surgical wound is considered as infected when there is purulent drainage from the wound. The purpose of the present study was to compare the primary wound closure technique and delayed primary wound closure technique with regard to the rate of wound infection and other complications which are usually associated with the infection like wound dehiscence, stitch sinuses, incisional hernias and duration of hospital stay. Materials and Methodology: 120 participants were randomly divided into two groups with 60 subjects each for primary wound closure and delayed primary wound closure. Data was coded and entered in Microsoft Excel sheet and the data analysis was done using SPSS-17. Association between qualitative variables will be analysed using Chi-square test. Associations between quantitative variables were analysed using independent sample t-test. Non parametric tests were if whenever necessary. When a p value of less than 0.05 is considered as statistically significant.&nbsp

    Background: Measles is the leading cause of child mortality in India. MR vaccine provides immunity against measles & rubella and was offered in 2 doses – 9 completed months and 16-24 months of age. This study aims to assess the awareness among the parents of children (aged 9 months – 2 years) in Mangalore about the presence and the risks associated with measles and rubella and to determine how the parents perceive the MR vaccination drive launched by the Government of India in terms of its necessity and safety of their children. Methodology: This cross-sectional study was conducted among 188 parents of children between the age of 9 months – 2 years from health centers in Mangalore. The data was collected using a semi-structured questionnaire- consisting of 3 sections: 1) Socio-demographic information of study participants 2) Knowledge-based questions on measles and rubella 3) Perception of MR vaccination drive. Data was analyzed using SPSS version 21. Appropriate statistical tests were applied, and results were interpreted. Results: The study population mainly comprised individuals aged 23-30 years (37.8%), predominantly females (65.3%), they prefer government hospitals for child vaccination (71.5%), resulting in 73.1% of children being meeting national immunization targets by age 2 years. Only 33,4% of the parents have understanding of Measles and Rubella diseases and associated risks. Parents exhibit awareness of the MR vaccine drive (Overall 86.7%) through newspapers (46.0%) and schools (39.0%), supporting its mandatory status (76.0%). Doctors play a crucial role in addressing parental queries (59.5%) regarding the MR vaccination drive. Conclusions: Parents show awareness of the MR Vaccination Drive but lack understanding of Measles and Rubella diseases and associated risks. Despite limited knowledge, parent’s express receptiveness to the MR Vaccination, drive for their children’s safety, with some showing interest in learning more about the vaccine and diseases
