Sudden bilateral loss of vision in a 19-year-old man
Introduction: Posterior Reversible
Leukoencephalopathy Syndrome (PRES) is caused by
ischaemia commonly affecting the posterior cerebral
vasculature. It presents with sudden decreased vision,
headaches, nausea, vomiting, seizures, and altered
mental status.
Case presentation: A 19-year-old male presented to
the ophthalmic emergency complaining of sudden
bilateral loss of vision, which was down to light
perception He reported headaches, nausea, and
drowsiness since the previous day. He was a known case
of hypertension secondary to IgA nephropathy.
Magnetic resonance imaging (MRI) with STIR and
FLAIR sequences showed foci of hyperintensity within
the occipital lobes bilaterally. This confirmed the
suspected diagnosis of PRES. Discussion: Aetiological factors of PRES include
sudden increase in blood pressure, eclampsia, porphyria,
renal disease, and Cushing syndrome. These lead to
blood-brain barrier injury either by hyper- or hypoperfusion,
endothelial dysfunction, changes in blood
vessel morphology, hypocapnea, or immune system
activation. Histopathological changes in PRES include
activated astrocytes, scattered macrophages and
lymphocytes, often in the absence of inflammation or
neuronal damage.
Conclusion: PRES is usually a reversible neuroophthalmological
condition, however prompt
recognition and appropriate management is important to
prevent permanent brain injury or even death.peer-reviewe
Exoplanet Transmission Spectroscopy using KMOS
KMOS (K-Band Multi Object Spectrograph) is a novel integral field
spectrograph installed in the VLT's ANTU unit. The instrument offers an ability
to observe 24 2.8"2.8" sub-fields positionable within a 7.2' patrol
field, each sub-field producing a spectrum with a 1414-pixel spatial
resolution. The main science drivers for KMOS are the study of galaxies, star
formation, and molecular clouds, but its ability to simultaneously measure
spectra of multiple stars makes KMOS an interesting instrument for exoplanet
atmosphere characterization via transmission spectroscopy. We set to test
whether transmission spectroscopy is practical with KMOS, and what are the
conditions required to achieve the photometric precision needed, based on
observations of a partial transit of WASP-19b, and full transits of GJ 1214b
and HD 209458b. Our analysis uses the simultaneously observed comparison stars
to reduce the effects from instrumental and atmospheric sources, and Gaussian
processes to model the residual systematics. We show that KMOS can, in theory,
deliver the photometric precision required for transmission spectroscopy.
However, this is shown to require a) pre-imaging to ensure accurate centering
and b) a very stable night with optimal observing conditions (seeing
0.8"). Combining these two factors with the need to observe several
transits, each with a sufficient out-of-transit baseline (and with the fact
that similar or better precision can be reached with telescopes and instruments
with smaller pressure,) we conclude that transmission spectroscopy is not the
optimal science case to take advantage of the abilities offered by KMOS and

A uniform analysis of HD209458b Spitzer/IRAC lightcurves with Gaussian process models
We present an analysis of Spitzer/IRAC primary transit and secondary eclipse
lightcurves measured for HD209458b, using Gaussian process models to
marginalise over the intrapixel sensitivity variations in the 3.6 micron and
4.5 micron channels and the ramp effect in the 5.8 micron and 8.0 micron
channels. The main advantage of this approach is that we can account for a
broad range of degeneracies between the planet signal and systematics without
actually having to specify a deterministic functional form for the latter. Our
results do not confirm a previous claim of water absorption in transmission.
Instead, our results are more consistent with a featureless transmission
spectrum, possibly due to a cloud deck obscuring molecular absorption bands.
For the emission data, our values are not consistent with the thermal inversion
in the dayside atmosphere that was originally inferred from these data.
Instead, we agree with another re-analysis of these same data, which concluded
a non-inverted atmosphere provides a better fit. We find that a solar-abundance
clear-atmosphere model without a thermal inversion underpredicts the measured
emission in the 4.5 micron channel, which may suggest the atmosphere is
depleted in carbon monoxide. An acceptable fit to the emission data can be
achieved by assuming that the planet radiates as an isothermal blackbody with a

Bi-Directional Learning: Identifying Contaminants on the Yurok Indian Reservation.
The Yurok Tribe partnered with the University of California Davis (UC Davis) Superfund Research Program to identify and address contaminants in the Klamath watershed that may be impairing human and ecosystem health. We draw on a community-based participatory research approach that begins with community concerns, includes shared duties across the research process, and collaborative interpretation of results. A primary challenge facing University and Tribal researchers on this project is the complexity of the relationship(s) between the identity and concentrations of contaminants and the diversity of illnesses plaguing community members. The framework of bi-directional learning includes Yurok-led river sampling, Yurok traditional ecological knowledge, University lab analysis, and collaborative interpretation of results. Yurok staff and community members share their unique exposure pathways, their knowledge of the landscape, their past scientific studies, and the history of landscape management, and University researchers use both specific and broad scope chemical screening techniques to attempt to identify contaminants and their sources. Both university and tribal knowledge are crucial to understanding the relationship between human and environmental health. This paper examines University and Tribal researchers' shared learning, progress, and challenges at the end of the second year of a five-year Superfund Research Program (SRP) grant to identify and remediate toxins in the lower Klamath River watershed. Our water quality research is framed within a larger question of how to best build university-Tribal collaboration to address contamination and associated human health impacts
Effects on Breathing of Agonists to μ-opioid or GABA\u3csub\u3eA\u3c/sub\u3e Receptors Dialyzed into the Ventral Respiratory Column of Awake and Sleeping Goats
Pulmonary ventilation (V̇I) in awake and sleeping goats does not change when antagonists to several excitatory G protein-coupled receptors are dialyzed unilaterally into the ventral respiratory column (VRC). Concomitant changes in excitatory neuromodulators in the effluent mock cerebral spinal fluid (mCSF) suggest neuromodulatory compensation. Herein, we studied neuromodulatory compensation during dialysis of agonists to inhibitory G protein-coupled or ionotropic receptors into the VRC. Microtubules were implanted into the VRC of goats for dialysis of mCSF mixed with agonists to either μ-opioid (DAMGO) or GABAA (muscimol) receptors. We found: (1) V̇I decreased during unilateral but increased during bilateral dialysis of DAMGO, (2) dialyses of DAMGO destabilized breathing, (3) unilateral dialysis of muscimol increased V̇I, and (4) dialysis of DAMGO decreased GABA in the effluent mCSF. We conclude: (1) neuromodulatory compensation can occur during altered inhibitory neuromodulator receptor activity, and (2) the mechanism of compensation differs between G protein-coupled excitatory and inhibitory receptors and between G protein-coupled and inotropic inhibitory receptors
Exposure to models’ negative facial expressions whilst eating a vegetable decreases women’s liking of the modelled vegetable, but not their desire to eat
Introduction: Food enjoyment can be conveyed through facial expressions. Observing others’ enjoyment of food has been found to influence adults’ desirability of liked and disliked food. Exposing adults to other eaters enjoying nutritious foods that are typically disliked (e.g., vegetables) could enhance the consumption of vegetables by young adults. However, this remains to be examined in young adult populations. This study examined the effect of models’ facial expressions towards raw broccoli on young adult women’s change in liking and change in desire to eat a modelled vegetable (raw broccoli) and a non-modelled vegetable (cucumber).Methods: Young adult women (N = 205) were randomised to watch a video of unfamiliar adult models eating raw broccoli with a positive, negative, or neutral facial expression. Participants’ change in liking and change in desire to eat the modelled and non-modelled vegetable was examined.Results: Observing models conveying negative facial expressions whilst eating raw broccoli resulted in a statistically significant reduction in liking ratings of broccoli, but not cucumber. There was no effect of models’ facial expressions on the change in desire to eat foods.Discussion: These findings suggest that watching others express a negative facial expression whilst eating a raw vegetable reduces women’s liking of the modelled vegetable, in the absence of a significant change to their desire to consume these foods. This highlights the power of others’ negative facial expressions on food liking. Further work is needed to establish the effect of others’ facial expressions on vegetable intake
State-Dependent and -Independent Effects of Dialyzing Excitatory Neuromodulator Receptor Antagonists into the Ventral Respiratory Column
Unilateral dialysis of the broad-spectrum muscarinic receptor antagonist atropine (50 mM) into the ventral respiratory column [(VRC) including the pre-Bötzinger complex region] of awake goats increased pulmonary ventilation (V̇i) and breathing frequency (f), conceivably due to local compensatory increases in serotonin (5-HT) and substance P (SP) measured in effluent mock cerebral spinal fluid (mCSF). In contrast, unilateral dialysis of a triple cocktail of antagonists to muscarinic (atropine; 5 mM), neurokinin-1, and 5-HT receptors does not alter V̇i or f, but increases local SP. Herein, we tested hypotheses that 1) local compensatory 5-HT and SP responses to 50 mM atropine dialyzed into the VRC of goats will not differ between anesthetized and awake states; and 2) bilateral dialysis of the triple cocktail of antagonists into the VRC of awake goats will not alter V̇i or f, but will increase local excitatory neuromodulators. Through microtubules implanted into the VRC of goats, probes were inserted to dialyze mCSF alone (time control), 50 mM atropine, or the triple cocktail of antagonists. We found 1) equivalent increases in local 5-HT and SP with 50 mM atropine dialysis during wakefulness compared with isoflurane anesthesia, but V̇i and f only increased while awake; and 2) dialyses of the triple cocktail of antagonists increased V̇i, f, 5-HT, and SP
Cytotoxic clinical isolates of Pseudomonas aeruginosa identified during the Steroids for Corneal Ulcers Trial show elevated resistance to fluoroquinolones.
BackgroundTo determine the relationship between type three secretion genotype and fluoroquinolone resistance for P. aeruginosa strains isolated from microbial keratitis during the Steroids for Corneal Ulcers Trial (SCUT) and for two laboratory strains, PA103 and PAO1.MethodsConfirmed P. aeruginosa isolates from the SCUT were divided into exoU(+) or exoU(-). The exoU(+) strains contained the gene encoding ExoU, a powerful phospholipase toxin delivered into host cells by the type three secretion system. Isolates were then assessed for susceptibility to fluoroquinolone, cephalosporin, and aminoglycoside antibiotics using disk diffusion assays. Etest was used to determine the MIC of moxifloxacin and other fluoroquinolones. Laboratory isolates in which the exoU gene was added or deleted were also tested.ResultsA significantly higher proportion of exoU(+) strains were resistant to ciprofloxacin (p = 0.001), gatifloxacin (p = 0.003), and ofloxacin (p = 0.002) compared to exoU(-) isolates. There was no significant difference between exoU(+) or exoU(-) negative isolates with respect to susceptibility to other antibiotics except gentamicin. Infections involving resistant exoU(+) strains trended towards worse clinical outcome. Deletion or acquisition of exoU in laboratory isolates did not affect fluoroquinolone susceptibility.ConclusionsFluoroquinolone susceptibility of P. aeruginosa isolated from the SCUT is consistent with previous studies showing elevated resistance involving exoU encoding (cytotoxic) strains, and suggest worse clinical outcome from infections involving resistant isolates. Determination of exoU expression in clinical isolates of P. aeruginosa may be helpful in directing clinical management of patients with microbial keratitis
Gender and PTSD: What Can We Learn from Female Police Officers?
Studies of civilians typically find that female gender is a risk factor for posttraumatic stress disorder (PTSD). Police and military studies often find no gender differences in PTSD. We compared 157 female police officers and 124 female civilians on several variables including trauma exposure, peritraumatic emotional distress, current somatization, and cumulative PTSD symptoms. We found that despite greater exposure to assaultive violence in the officer group, female civilians reported significantly more severe PTSD symptoms. Elevated PTSD symptoms in female civilians were explained by significantly more intense peritraumatic emotional distress among female civilians. We also found that female officers showed a stronger direct relationship between peritraumatic emotional distress and current somatization. Our findings suggest that apparent gender differences in PTSD may result from differences in peritraumatic emotionality, which influence subsequent PTSD and somatization symptoms. Emotionality may be more important than biological sex in understanding gender differences in PTSD
