Length of maternity leave and health of mother and child - a review
Summary: Objectives:: Assessment of the literature on the length of maternity leaves and health of mothers and children; evaluation of the Swiss situation in view of the maternity leave policy implemented in 2005. Methods:: Review of thirteen original studies identified by PubMed using topic-related terms. Results:: A positive association was shown between the length of maternity leave and mother's mental health and duration of breastfeeding. Extended maternity leaves were also associated with lower perinatal, neonatal and post-neonatal mortality rates as well as lower child mortality; however, results are obtained in ecological studies. There is less evidence regarding other health outcomes. The new policy in Switzerland extends maternity leave for a considerable number of women to 14 weeks. With this prolongation, fewer depressive symptoms and longer breastfeeding duration can be expected, while benefits regarding other health outcomes would warrant longer leaves. Conclusions:: Longer maternity leaves are likely to produce health benefits. The new policy in Switzerland will probably improve the situation of those women, who previously were granted only minimal leave and/or mothers with additional social risk factor
Tumour-targeting properties of antibodies specific to MMP-1A, MMP-2 and MMP-3
Purpose: Matrix metalloproteinases (MMPs), a group of more than 20 zinc-containing endopeptidases, are upregulated in many diseases, but several attempts to use radiolabelled MMP inhibitors for imaging tumours have proved unsuccessful in mouse models, possibly due to the limited specificity of these agents or their unfavourable pharmacokinetic profiles. In principle, radiolabelled monoclonal antibodies could be considered for the selective targeting and imaging of individual MMPs. Methods: We cloned, produced and characterized high-affinity monoclonal antibodies specific to murine MMP-1A, MMP-2 and MMP-3 in SIP (small immunoprotein) miniantibody format using biochemical and immunochemical methods. We also performed comparative biodistribution analysis of their tumour-targeting properties at three time points (3h, 24h, 48h) in mice bearing subcutaneous F9 tumours using radioiodinated protein preparations. The clinical stage L19 antibody, specific to the alternatively spliced EDB domain of fibronectin, was used as reference tumour-targeting agent for in vivo studies. Results: All anti-MMP antibodies and SIP(L19) strongly stained sections of F9 tumours when assessed by immunofluorescence methods. In biodistribution experiments, SIP(SP3), specific to MMP-3, selectively accumulated at the tumour site 24 and 48h after intravenous injection, but was rapidly cleared from other organs. By contrast, SIP(SP1) and SIP(SP2), specific to MMP-1A and MMP-2, showed no preferential accumulation at the tumour site. Female gender is associated with dental care and dental hygiene, but not with complete dentition in the Swiss adult population
Aim: Little is known about the impact of gender on oral health, besides the influence of reproductive factors on female dentition. The aim of this study was to analyse gender differences with regard to oral health and oral health behaviour in the Swiss adult population, using data from the Swiss Health Survey of 2002. Subjects and methods: The Swiss Health Survey regularly collects detailed information on health and health determinants of a random sample of the Swiss population aged 15years and older. A written questionnaire including items on oral health was filled in by 7,115 men and 9,026 women in 2002. Weighted prevalence rates were calculated, and multivariate logistic regression analyses used to evaluate the role of gender for complete and functional dentition and for oral health behaviour. Finally, the multivariate model was stratified for men and women and run for the outcome "complete dentitionâ. Results: Women had a higher prevalence of visits to the dentist in the last 12 months (64% versus 60% men), of visits to dental hygienists (39% versus 34%), and of frequent tooth brushing (several times a day) (82% versus 66%). Adjusting for preventive behaviour, socio-demographic and socio-economic variables in multivariate analyses, female gender was positively associated with the prevalence of visits to the dentist (OR 1.33, 95% CI 1.24-1.43) and of frequent tooth brushing several times a day (OR 2.57, 95% CI 2.36-2.79), but not with complete dentition (OR 1.01, 95% CI 0.93-1.09). In the stratified analysis, associations of complete dentition with socio-economic status, oral hygiene and preventive behaviour were stronger in men than in women. Conclusion: Gender plays an important role in oral health. Adult women report a more pronounced preventive oral health behaviour than men; their dentition, however, is not more often complete. Mechanisms involved in women's oral health are probably more complex than those underlying men's oral health. An in-depth gender analysis is needed to shed more light upon this issu
Cloud Structure of Galactic OB Cluster Forming Regions from Combining Ground and Space Based Bolometric Observations
We have developed an iterative procedure to systematically combine the millimeter and submillimeter images of OB cluster-forming molecular clouds, which were taken by ground based (CSO, JCMT, APEX, IRAM-30m) and space telescopes (Herschel, Planck). For the seven luminous (10 ) Galactic OB cluster-forming molecular clouds selected for our analyses, namely W49A, W43-Main, W43-South, W33, G10.6-0.4, G10.2-0.3, G10.3-0.1, we have performed single-component, modified black-body fits to each pixel of the combined (sub)millimeter images, and the Herschel PACS and SPIRE images at shorter wavelengths. The 10 resolution dust column density and temperature maps of these sources revealed dramatically different morphologies, indicating very different modes of OB cluster-formation, or parent molecular cloud structures in different evolutionary stages. The molecular clouds W49A, W33, and G10.6-0.4 show centrally concentrated massive molecular clumps that are connected with approximately radially orientated molecular gas filaments. The W43-Main and W43-South molecular cloud complexes, which are located at the intersection of the Galactic near 3-kpc (or Scutum) arm and the Galactic bar, show a widely scattered distribution of dense molecular clumps/cores over the observed 10 pc spatial scale. The relatively evolved sources G10.2-0.3 and G10.3-0.1 appear to be affected by stellar feedback, and show a complicated cloud morphology embedded with abundant dense molecular clumps/cores. We find that with the high angular resolution we achieved, our visual classification of cloud morphology can be linked to the systematically derived statistical quantities (i.e., the enclosed mass profile, the column density probability distribution function, the two-point correlation function of column density, and the probability distribution function of clump/core separations)
Further evidence that the neurodevelopmental gene FBXW7 predisposes to Wilms tumor
Somatic variants in the NOTCH pathway regulator FBXW7 are frequently seen in a variety of malignancies. Heterozygous loss-of-function germline variants in FBXW7 have recently been described as causative for a neurodevelopmental syndrome. Independently, FBXW7 was also considered as a susceptibility gene for Wilms tumor due to a few observations of heterozygous germline variants in patients with Wilms tumor. Whether the same FBXW7 variants are implicated in both, neurodevelopmental delay and Wilms tumor formation, remained unclear. By clinical testing, we now observed a patient with neurodevelopmental delay due to a de novo constitutional mosaic FBXW7 splice site pathogenic variant who developed Wilms tumor. In the tumor, we identified a second hit frameshift variant in FBXW7. Immunohistochemical staining was consistent with mosaic loss of FBXW7 protein expression in the tumor. Our data support the role of constitutional FBXW7 pathogenic variants in both, neurodevelopmental disorder and the etiology of Wilms tumor. Therefore, Wilms tumor screening should be considered in individuals with constitutional or germline pathogenic variants in FBXW7 and associated neurodevelopmental syndrome
