Osteoporosis Therapies: Evidence from Health-Care Databases and Observational Population Studies
Osteoporosis is a well-recognized disease with severe consequences if left untreated. Randomized controlled trials are the most rigorous method for determining the efficacy and safety of therapies. Nevertheless, randomized controlled trials underrepresent the real-world patient population and are costly in both time and money. Modern technology has enabled researchers to use information gathered from large health-care or medical-claims databases to assess the practical utilization of available therapies in appropriate patients. Observational database studies lack randomization but, if carefully designed and successfully completed, can provide valuable information that complements results obtained from randomized controlled trials and extends our knowledge to real-world clinical patients. Randomized controlled trials comparing fracture outcomes among osteoporosis therapies are difficult to perform. In this regard, large observational database studies could be useful in identifying clinically important differences among therapeutic options. Database studies can also provide important information with regard to osteoporosis prevalence, health economics, and compliance and persistence with treatment. This article describes the strengths and limitations of both randomized controlled trials and observational database studies, discusses considerations for observational study design, and reviews a wealth of information generated by database studies in the field of osteoporosis
Comparison of FRAX Scores of Southern California Females of Mexican Descent Using US Hispanic and Mexico Database
Introduction. Regional differences for fracture risk of US Hispanics may vary by national origin. In California, where a majority of the Hispanic population is of Mexican descent, it is of interest to compare the FRAX absolute risk using the US Hispanic and Mexico databases.
Methods. We collected FRAX risk factor data from 134 women of Mexican descent in southern California. The FRAX risk score was calculated using the US Hispanic and Mexican databases, using the NOF guidelines for osteoporosis to compare the number of patients that would be selected for treatment.
Results. The 10-year absolute risk of major osteoporotic fracture among women of Mexican descent using the US Hispanic database was 4.82 ± 5.03 (95% CI 3.97–5.67) compared to 4.86 ± 4.72 (CI 3.98–5.44) using the Mexico database (P = .94). The 10-year risk for hip fracture was 0.86 ± 1.78 (CI .56–1.16) compared to 1.12 ± 1.97 (CI .79–1.45, P = .26). The mean conformity for meeting the interventional threshold by either risk score was 94.8%. Conclusion. The comparison between the FRAX databases demonstrates a similarity in the absolute risk of major osteoporotic fracture. Differences are noted in the absolute number of hip fracture subjects at risk, but there is a high rate of conformity
Identifying Individuals at Risk for Fracture in Guatemala
INTRODUCTION: The FRAX calculator combines a set of clinical risk factors with country-specific incidence rates to determine the ten-year absolute risk of major osteoporotic fracture. However, regional or country-specific databases from Central American countries are not available. We compared the use of various FRAX databases and the Pluijm algorithm in determining risk of fracture. METHODS: We collected clinical risk factor data needed for the FRAX calculator and Pluijm algorithm of Hispanic women in Guatemala and calculated the FRAX absolute risk measures of major osteoporotic fracture and hip fracture. Subjects were postmenopausal women greater than age 40 with no history of using medication that affect bone. A random sample of 204 women in 34 different regions women in Guatemala City was visited in their homes to complete the surveys. The Pluijm risk score and FRAX risk score using the US Hispanic, Spain, and Mexican databases were calculated. RESULTS: We used the US NOF guidelines for treatment which suggest a treatment threshold for patients with a 10-year hip fracture probability ≥ 3% or a 10-year major osteoporotic fracture risk ≥ 20%. The number of patients meeting the suggested threshold limits for treatment using the Spain and Mexico calculators were identical. There was 100% conformity in threshold limits for both hip and major osteoporotic fracture risk. The mean conformity for any fracture risk between US Hispanic and the other two databases was 97.5%. Conformity was 99.0% based on major osteoporotic fracture and 97.5% based on risk of hip fracture. The Pluijm evaluation shows conformity of 87.2% and 83.3%, respectively, when compared to the US Hispanic and Spain/Mexico FRAX thresholds for risk of fracture. DISCUSSION: Although the different FRAX databases provide variations in the absolute risk of fracture, the overall conformity to treatment thresholds amongst the US Hispanic, Spain, and Mexico databases show the database used would have little effect as to the decision to treat. The Pluijm tool conforms to the FRAX thresholds and can be used as well. It does not matter which country-specific calculator or assessment tool is used, as there are a similar number of patients that would meet the intervention threshold
New Selective Estrogen Receptor Modulators (SERMs) in Development
Selective estrogen receptor modulators (SERMs) or estrogen agonists/antagonists have shown promise in osteoporosis in that they have the potential to reduce the risk of fracture, and also reduce the risk of breast cancer. SERMs maybe classified according to their core structure, which is typically a variation of the 17 beta-estradiol template and subclassified according to the side chain at the helix 12 affector region. The best known are the triphenylethylenes such as tamoxifen, used in the management of breast cancer. However, the clinical application of this class of SERMs has been limited due to endometrial stimulation. A second class is the benzothiophenes such as raloxifene and arzoxifene, which have skeletal benefit with little, if any, uterine stimulation. Indole-based SERMs such as bazedoxifene have a 2-phenyl ring system that serves as a core binding unit. Other classes include benzopyrans and naphthalenes (eg, lasofoxifene). In this review article, I will discuss raloxifene and three new SERMs—arzoxifene, bazedoxifene, and lasofoxifene—that have been recently studied. PRC2 loss induces chemoresistance by repressing apoptosis in T cell acute lymphoblastic leukemia
The tendency of mitochondria to undergo or resist BCL2-controlled apoptosis (so-called mitochondrial priming) is a powerful predictor of response to cytotoxic chemotherapy. Fully exploiting this finding will require unraveling the molecular genetics underlying phenotypic variability in mitochondrial priming. Here, we report that mitochondria) apoptosis resistance in T cell acute lymphoblastic leukemia (T-ALL) is mediated by inactivation of polycomb repressive complex 2 (PRC2). In T-ALL clinical specimens, loss-of-function mutations of PRC2 core components (EZH2, FED, or SUZ12) were associated with mitochondrial apoptosis resistance. In T-ALL cells, PRC2 depletion induced resistance to apoptosis induction by multiple chemotherapeutics with distinct mechanisms of action. PRC2 loss induced apoptosis resistance via transcriptional up-regulation of the LIM domain transcription factor CRIP2 and downstream up-regulation of the mitochondrial chaperone TRAP1. These findings demonstrate the importance of mitochondrial apoptotic priming as a prognostic factor in T-ALL and implicate mitochondrial chaperone function as a molecular determinant of chemotherapy response
Motif Minang Kaluak Paku Kacang Balimbiang pada Busana Kasual
Minangkabau sebagai salah satu suku bangsa yang mengisi kekhasan
budaya Indonesia memiliki warisan budaya yang terpencar dalam berbagai aspek
kehidupannya. Salah satu warisan budaya adalah seni ukir. Seni ukir yang
dikembangkan dengan mengambil ide dari alam memiliki makna-makna filosofi
bagi kehidupan masyarakat Minangkabau. Semua jenis ukiran yang dipahatkan di
Rumah Gadang menunjukkan unsur penting pembentuk budaya Minangkabau
bercerminkan kepada apa yang ada di alam. Salah satu ukiran pada rumah gadang
yaitu kaluak paku. Kaluak paku adalah nama salah satu motif ukiran dalam adat
Minangkabau. Berasal dari motif gulungan (kelukan/kaluak) pada ujung tanaman
pakis (paku) yang masih muda. Ukiran kaluak paku rumah gadang melambangkan
tanggung jawab seorang lelaki dalam adat Minangkabau kepada generasi penerus,
sebagai ayah dari anak-anaknya dan sebagai mamak dari kemenakan (keponakan).
Ukiran rumah gadang kaluak paku minangkabau inilah yang menjadi sumber ide
penciptaan busana pada tugas akhir ini.
Pada Penciptaan karya ini menggunakan beberapa metode, yaitu metode
pendekatan estetis dan ergonomis, metode pengumpulan data dengan studi
pustaka, dan motode penciptaan dengan teori Gustami Sp 3 tahap 6 Langkah.
Dalam proses pembuatan karya dibutuhkan beberapa data, cara pengumpulan data
acuan berdasarkan pengumpulan data pustaka yaitu berupa buku, jurnal pada
media sosial, serta aplikasi pada smartphone seperti pinterest. Data yang
dikumpulkan yang paling utama adalah gambar bentuk visual dari ukiran tanaman
kaluak paku minangkabau dan busana kasual.
Penciptaan karya yang dihasilkan yaitu berupa 8 busana kasual. Siluet pada
kesuluruhan hasil karya yaitu memiliki siluet A yang mengembang pada bagian
bawah. Pada penciptaan karya ini menggunakan bahan utama primisima.
Perpaduan warna yang diterapkan menggunakan warna khas minangkabau yang
diambil dari warna bendera adatnya “marawa” yaitu merah, hitam, dan kuning.
Karya- karya yang dihasilkan dengan penggunaan warna tersebut sangat sesuai
dengan tema yang mengangkat ukiran rumah gadang kaluak paku minangkabau.
