The Uncertainty Room: Strategies for Managing Uncertainty in a Surgical Waiting Room
Objective: To describe experiences of uncertainty and management strategies for staff working with families in a hospital waiting room. Setting: A 288-bed nonprofit community hospital in a Midwestern city. Methods: Data were collected during individual, semi structured interviews with 3 volunteers, 3 technical staff members, and 1 circulating nurse (n=7), and during 40 hours of observation in a surgical waiting room. Interview transcripts were analyzed using constant comparative techniques. Results: The surgical waiting room represents the intersection of several sources of uncertainty that families experience. Findings also illustrate the ways in which staff manage the uncertainty of families in the waiting room by communicating support. Conclusions: Staff in surgical waiting rooms are responsible for managing family members uncertainty related to insufficient information. Practically this study provided some evidence that staff are expected to help manage the uncertainty that is typical in a surgical waiting room, further highlighting the important role of communicating in improving family members experiences
Theories of Communication and Uncertainty as a Foundation for Future Research on Nursing Practice
As we enter the age of “precision medicine,” we will need “a greater tolerance of uncertainty and greater facility for calculating and interpreting probabilities than” (Hunter, 2016, p. 711) ever before. Nursing scholarship has produced the most widely known theory of uncertainty in illness (Mishel, 1988, 1990), but it emphasizes the psychological state of and deemphasizes communication. Communication scholars have attempted to overcome this deficit, but two of the most prominent of these perspectives, uncertainty management theory (Brashers, 2001) and the theory of motivated information management (Afifi & Morse, 2004), emphasize processes related to information seeking or avoidance in the service of uncertainty reduction, creation, or maintenance; in so doing, they tend to neglect important variations in the meanings of uncertainty. The article reviews these theories and also problematic integration theory, which centers the task of differentiating forms of uncertainty and other problematic meanings and the importance of form-specific adaptation of communication. The paper concludes with an agenda for collaborations between nursing and communication researchers aimed at advancing theory and practice
Using Social Network Analysis Methods to Assess the Impact of Community Engagement Projects on Classroom Dynamics
This case study used social network analysis methods to examine the evolution of friendship and academic collaboration networks among students in first-year seminar courses. Specifically, our research compared friendship and academic collaboration networks among students in courses with a significant focus on community engagement with networks among students in courses that did not require community engagement. We analyzed these networks using UCINET (Borgatti et al., 2002), a social network analysis software package. We first studied network cohesion measures—density, diameter, and average path length—to understand how easily information spread among classmates. Secondly, we studied network centralization measures—degree, closeness, and betweenness—which help to identify power inequalities in social groups (Hanneman, 2001). Results of our study suggest that integrating community engagement projects into curricula helps reduce power inequalities. In other words, community engagement projects appear to encourage the creation of connected friendships among first-year students
Rapid Evolution of BRCA1 and BRCA2 in Humans and Other Primates
The maintenance of chromosomal integrity is an essential task of every living organism and cellular repair mechanisms exist to guard against insults to DNA. Given the importance of this process, it is expected that DNA repair proteins would be evolutionarily conserved, exhibiting very minimal sequence change over time. However, BRCA1, an essential gene involved in DNA repair, has been reported to be evolving rapidly despite the fact that many protein-altering mutations within this gene convey a significantly elevated risk for breast and ovarian cancers. Results: To obtain a deeper understanding of the evolutionary trajectory of BRCA1, we analyzed complete BRCA1 gene sequences from 23 primate species. We show that specific amino acid sites have experienced repeated selection for amino acid replacement over primate evolution. This selection has been focused specifically on humans and our closest living relatives, chimpanzees (Pan troglodytes) and bonobos (Pan paniscus). After examining BRCA1 polymorphisms in 7 bonobo, 44 chimpanzee, and 44 rhesus macaque (Macaca mulatta) individuals, we find considerable variation within each of these species and evidence for recent selection in chimpanzee populations. Finally, we also sequenced and analyzed BRCA2 from 24 primate species and find that this gene has also evolved under positive selection. Conclusions: While mutations leading to truncated forms of BRCA1 are clearly linked to cancer phenotypes in humans, there is also an underlying selective pressure in favor of amino acid-altering substitutions in this gene. Predictors of anemia in preschool children: Biomarkers Reflecting Inflammation and Nutritional Determinants of Anemia (BRINDA) project.
Background: A lack of information on the etiology of anemia has hampered the design and monitoring of anemia-control efforts.Objective: We aimed to evaluate predictors of anemia in preschool children (PSC) (age range: 6-59 mo) by country and infection-burden category.Design: Cross-sectional data from 16 surveys (n = 29,293) from the Biomarkers Reflecting Inflammation and Nutritional Determinants of Anemia (BRINDA) project were analyzed separately and pooled by category of infection burden. We assessed relations between anemia (hemoglobin concentration <110 g/L) and severe anemia (hemoglobin concentration <70 g/L) and individual-level (age, anthropometric measures, micronutrient deficiencies, malaria, and inflammation) and household-level predictors; we also examined the proportion of anemia with concomitant iron deficiency (defined as an inflammation-adjusted ferritin concentration <12 μg/L). Countries were grouped into 4 categories on the basis of risk and burden of infectious disease, and a pooled multivariable logistic regression analysis was conducted for each group.Results: Iron deficiency, malaria, breastfeeding, stunting, underweight, inflammation, low socioeconomic status, and poor sanitation were each associated with anemia in >50% of surveys. Associations between breastfeeding and anemia were attenuated by controlling for child age, which was negatively associated with anemia. The most consistent predictors of severe anemia were malaria, poor sanitation, and underweight. In multivariable pooled models, child age, iron deficiency, and stunting independently predicted anemia and severe anemia. Inflammation was generally associated with anemia in the high- and very high-infection groups but not in the low- and medium-infection groups. In PSC with anemia, 50%, 30%, 55%, and 58% of children had concomitant iron deficiency in low-, medium-, high-, and very high-infection categories, respectively.Conclusions: Although causal inference is limited by cross-sectional survey data, results suggest anemia-control programs should address both iron deficiency and infections. The relative importance of factors that are associated with anemia varies by setting, and thus, country-specific data are needed to guide programs
"Women's rights, the European Court and Supranational Constitutionalism"
This analysis examines supranational constitutionalism in the European Union. In particular, the study focuses on the role of the European Court of Justice in the creation of women’s rights. I examine the interaction between the Court and member state governments in legal integration, and also the integral role that women’s advocates – both individual activists and groups – have played in the development of EU social provisions. The findings suggest that this litigation dynamic can have the effect of fueling the integration process by creating new rights that may empower social actors and EU organizations, with the ultimate effect of diminishing member state government control over the scope and direction of EU law. This study focuses specifically on gender equality law, yet provides a general framework for examining the case law in subsequent legal domains, with the purpose of providing a more nuanced understanding of supranational governance and constitutionalism

Intraindividual double burden of overweight and micronutrient deficiencies or anemia among preschool children
Background: Child overweight prevalence is increasing globally, but micronutrient deficiencies persist.
Objectives: We aimed to 1) describe the prevalence and distribution of intraindividual double burden of malnutrition (DBM), defined as coexistence of overweight or obesity (OWOB) and either micronutrient deficiencies or anemia, among preschool children; 2) assess the independence of DBM components, e.g., whether the prevalence of DBM is greater than what would be expected by chance; and 3) identify predictors of intraindividual DBM, to guide intervention targeting.
Methods: We analyzed data from 24 population-based surveys from the Biomarkers Reflecting Inflammation and Nutritional Determinants of Anemia project (separately by survey; n = 226 to n = 7166). We defined intraindividual DBM as coexisting OWOB and ≥1 micronutrient deficiency [e.g., Micronutrient Deficiency Index (MDI) \u3e 0; DBM-MDI] or anemia (DBM-Anemia). We assessed independence of DBM components with the Rao–Scott chi-square test and examined predictors of DBM and its components with logistic regression.
Results: DBM prevalence ranged from 0% to 9.7% (median: 2.5%, DBM-MDI; 1.4%, DBM-Anemia), reflecting a lower prevalence of OWOB (range: 0%–19.5%) than of micronutrient deficiencies and anemia, which exceeded 20% in most surveys. OWOB was generally not significantly associated with micronutrient deficiencies or anemia. In more than half of surveys, children 6–23 mo of age, compared with ≥24 mo, had greater adjusted odds of DBM-Anemia, anemia, and micronutrient deficiencies. Child sex and household socioeconomic status, urban location, and caregiver education did not consistently predict DBM or its components.
Conclusions: Intraindividual DBM among preschool children was low but might increase as child OWOB increases. The analysis does not support the hypothesis that DBM components cluster within individuals, suggesting that population-level DBM may be addressed by programs to reduce DBM components without targeting individuals with DBM
The LEECH Exoplanet Imaging Survey: Limits on Planet Occurrence Rates Under Conservative Assumptions
We present the results of the largest (m) direct
imaging survey for exoplanets to date, the Large Binocular Telescope
Interferometer (LBTI) Exozodi Exoplanet Common Hunt (LEECH). We observed 98
stars with spectral types from B to M. Cool planets emit a larger share of
their flux in compared to shorter wavelengths, affording LEECH an
advantage in detecting low-mass, old, and cold-start giant planets. We
emphasize proximity over youth in our target selection, probing physical
separations smaller than other direct imaging surveys. For FGK stars, LEECH
outperforms many previous studies, placing tighter constraints on the hot-start
planet occurrence frequency interior to au. For less luminous,
cold-start planets, LEECH provides the best constraints on giant-planet
frequency interior to au around FGK stars. Direct imaging survey
results depend sensitively on both the choice of evolutionary model (e.g., hot-
or cold-start) and assumptions (explicit or implicit) about the shape of the
underlying planet distribution, in particular its radial extent. Artificially
low limits on the planet occurrence frequency can be derived when the shape of
the planet distribution is assumed to extend to very large separations, well
beyond typical protoplanetary dust-disk radii ( au), and when
hot-start models are used exclusively. We place a conservative upper limit on
the planet occurrence frequency using cold-start models and planetary
population distributions that do not extend beyond typical protoplanetary
dust-disk radii. We find that of FGK systems can host a 7 to 10
planet from 5 to 50 au. This limit leaves open the
