    SODAS: Surveillance of Drugs of Abuse Study

    Objective: Novel Psychoactive Substance (NPS) as a form of recreational drug use has become increasingly popular. There is a paucity of information with regards the prevalence and clinical sequalae of these drugs. The aim of this study was to detect NPS in patients presenting to the Emergency Department (ED) with suspected toxicological ingestion. Methods: The prospective study was performed in a large Emergency Department (ED) in the UK. During a three month period eighty patients were identified by clinicians as having potentially ingested a toxicological agent. Urine sample were analysed using liquid chromatography-high resolution mass spectrometry and basic clinical data was gathered. Results: 80 patients with a history of illicit or recreational drug consumption had urine screenings performed. 49% (39) of patients undergoing a screen had more than one illicit substance detected. 20% (16) of patients tested positive for at least one NPS. Conclusions: Almost half of patients presenting had ingestion of multiple substances which correlated poorly with self reporting of patients. Developing enhanced strategies to monitor evolving drug trends is crucial to the ability of clinicians to deliver care to this challenging group of patients

    Analysis and clinical findings of cases positive for the novel synthetic cannabinoid receptor agonist MDMB-CHMICA

    Context: MDMB-CHMICA is a synthetic cannabinoid receptor agonist which has caused concern due to its presence in cases of adverse reaction and death. Method: 43 cases of suspected synthetic cannabinoid ingestion were identified from patients presenting at an Emergency Department and from post-mortem casework. These were subjected to liquid-liquid extraction using tertiary-butyl methyl ether and quantitatively analysed by Electospray Ionisation Liquid Chromatography – tandem Mass Spectrometry. For positive samples, case and clinical details were sought and interrogated. Results: 11 samples were found positive for MDMB-CHMICA. Concentrations found ranged from <1 – 22 ng/mL (mean: 6 ng/mL, median: 3 ng/mL). The age range was 15 – 44 years (mean: 26 years, median: 21 years), with the majority (82%) of positive results found in males. Clinical presentations included hypothermia, hypoglycaemia, syncope, recurrent vomiting, altered mental state and serotonin toxicity, with corresponding concentrations of MDMB-CHMICA as low as <1 ng/mL. Duration of hospitalisation ranged from 3 – 24 hours (mean: 12 hours, median: 8 hours). Discussion: The concentration range presented in this case series is indicative of MDMB-CHMICA having a high potency, as is known to be the case for other synthetic cannabinoid receptor agonists. The age range and gender representation were consistent with that reported for users of other drugs of this type. The clinical presentations observed were typical of synthetic cannabinoid receptor agonists and show the difficulties in identifying reactions potentially associated with drugs of this type. Conclusion: The range of MDMB-CHMICA concentrations in Emergency Department presentations (n=9) and post-mortem cases (n=2) was reported. No correlation between the concentration of this drug and clinical presentation or cause of death was reported in this sample. However, the potential for harm associated with low concentrations of MDMB-CHMICA and the symptoms of toxicity being non-specific was highlighted

    The Aetiology of Olfactory Dysfunction and Its Relationship to Diet Quality

    People with olfactory loss may choose foods rich in sugar, salt and fat to compensate their loss—foods that constitute a Western-style diet (WSD). However, olfactory dysfunction has not been consistently linked to any particular type of dietary change. Here we considered whether the aetiology of olfactory dysfunction may affect consumption of a WSD. Two-hundred and twenty-two people with olfactory dysfunction of varying cause, were tested for chemosensory performance and their frequency of consumption of a WSD. There was no evidence of a link between a WSD and olfactory dysfunction at the aggregate level, but an aetiology-based approach revealed various patterns, showing both positive and negative associations between olfactory performance and consumption of a WSD. We suggest a number of reasons why, in certain cases, greater olfactory dysfunction may be linked to lower intakes of a WSD, and the role that different aetiologies may have in affecting choices for foods that may appeal following olfactory impairment

    Human olfactory consciousness and cognition:Its unusual features may not result from unusual functions but from limited neocortical processing resources

    Human and animal olfactory perception is shaped both by functional demands and by various environmental constraints seemingly peculiar to chemical stimuli. These demands and constraints may have generated a sensory system that is cognitively distinct from the major senses. In this article we identify these various functional demands and constraints, and examine whether they can be used to account for olfaction's unique cognitive features on a case-by-case basis. We then use this as grounds to argue that specific conscious processes do have functional value, a finding that naturally emerges when a comparative approach to consciousness across the senses is adopted. More generally, we conclude that certain peculiar features of olfactory cognition may owe more to limited neocortical processing resources, than they do to the challenges faced by perceiving chemical stimuli

    Twenty-First Century Climate Change and Submerged Aquatic Vegetation in a Temperate Estuary: The Case of Chesapeake Bay

    Introduction: The Chesapeake Bay was once renowned for expansive meadows of submerged aquatic vegetation (SAV). However, only 10% of the original meadows survive. Future restoration effortswill be complicated by accelerating climate change, including physiological stressors such as a predicted mean temperature increase of 2-6°C and a 50-160% increase in CO2 concentrations. Outcomes: As the Chesapeake Bay begins to exhibit characteristics of a subtropical estuary, summer heat waves will become more frequent and severe. Warming alone would eventually eliminate eelgrass (Zostera marina) from the region. It will favor native heat-tolerant species such as widgeon grass (Ruppia maritima) while facilitating colonization by non-native seagrasses (e.g., Halodule spp.). Intensifying human activity will also fuel coastal zone acidification and the resulting high CO2/low pH conditions may benefit SAV via a CO2 fertilization effect. . Discussion: Acidification is known to offset the effects of thermal stress and may have similar effects in estuaries, assuming water clarity is sufficient to support CO2-stimulated photosynthesis and plants are not overgrown by epiphytes. However, coastal zone acidification is variable, driven mostly by local biological processes that may or may not always counterbalance the effects of regional warming. This precarious equipoise between two forces - thermal stress and acidification - will be critically important because it may ultimately determine the fate of cool-water plants such as Zostera marina in the Chesapeake Bay. Conclusion: The combined impacts of warming, coastal zone acidification, water clarity, and overgrowth of competing algae will determine the fate of SAV communities in rapidly changing temperate estuaries

    Do Single Men Smell and Look Different to Partnered Men?

    Previous research indicates human body odor (BO) can signal kinship, sickness and genetic compatibility. Based on research indicating single males have higher testosterone levels than partnered males and that higher testosterone levels are associated with stronger smelling BO, the current study aimed to determine if, by extension of previous findings, single males’ BO smells stronger than partnered males’ BO. Eighty-two heterosexual women aged 18–35 years rated the BO and faces of six different males also aged 18–35 years. Consistent with the hypothesis, single men’s BO smelled stronger than partnered men’s BO and single men’s faces were rated as more masculine than partnered men’s faces.     Ballistic research techniques: visualizing gunshot wounding patterns

    There are difficulties associated with mapping gunshot wound (GSW) patterns within opaque models. Depending on the damage measurement parameters required, there are multiple techniques that can provide methods of “seeing” the GSW pattern within an opaque model. The aim of this paper was to test several of these techniques within a cadaveric animal limb model to determine the most effective. The techniques of interest were flash X-ray, ultrasound, physical dissection, and computed-tomography (CT). Fallow deer hind limbs were chosen for the model with four limbs used for each technique tested. Quarantined 7.62 × 39 mm ammunition was used for each shot, and each limb was only shot once, on an outdoor range with shots impacting at muzzle velocity. Flash X-ray provided evidence of yaw within the limb during the projectile’s flight; ultrasound though able to visualise the GSW track, was too subjective and was abandoned; dissection proved too unreliable due to the tissue being cadaveric so also too subjective; and lastly, CT with contrast provided excellent imaging in multiple viewing planes and 3D image reconstruction; this allowed versatile measurement of the GSW pattern to collect dimensions of damage as required. Of the different techniques examined in this study, CT with contrast proved the most effective to allow precise GSW pattern analysis within a cadaveric animal limb model. These findings may be beneficial to others wishing to undertake further ballistic study both within clinical and forensic fields

    Stagnation temperature measurement using thin-film platinum resistance sensors

    The measurement of stagnation temperature in high-speed flows is an important aspect of gas turbine engine testing. The ongoing requirement to improve the accuracy of such measurements has led to the development of probe systems that use a thin-film platinum resistance thermometer (PRT) as the sensing element. For certain aspects of engine testing this type of sensing device potentially offers superior measurement performance to the thermocouple, the temperature sensor of choice in most gas turbine applications. This paper considers the measurement performance of prototype PRT-based stagnation temperature probes, up to high-subsonic flow conditions, using passively aspirated probe heads. The relatively poor temperature recovery performance of a simply constructed probe has led to the development of a new design that is intended to reduce the impact of thermal conduction within the probe assembly. The performance of this so-called dual-skin probe has been measured through a series of tests at a range of Mach numbers, incidence angles and Reynolds numbers. The data reveal that a high probe recovery factor has been achieved with this device, and that the application of this design to engine tests would yield the measurement performance benefits of the PRT whilst requiring small levels of temperature recovery compensation
